The incidence of coexisting hyperparathyroidism and empty sella syndrome is rare and the etiology and incidence of their coexistence is not known. The association of hyperparathyroidism and the empty sella syndrome may be related to multiple endocrine neoplasia (MEN) syndrome due to a genetic disorder. We experienced a rare case of hyperparathyroidism presenting as acute pancreatitis combined with empty sella. We report here a 37-year old female who manifested epigastric pain because of acute pancreatitis. She had hypercalcemia due to parathyroid adenoma. A pituitary gland was not visible in the sella turcica on MRI scans. On genetic analysis, she did not show a mutation of the MENIN gene. Empty sella is thought to be a coincidental finding with hyperparathyroidism.

Objectives: The objectives of this study were to evaluate the effect of smoking on utilization of and expenses in ambulatory dental care. Methods: The subjects of this study were 5,751 men aged over 20 years selected from the Korea Health Panel data collected during 2010-2012. Pack-years of smoking were calculated based on the survey data. Dental utilization was defined as dental outpatient clinic use least once in three years. The expenses in ambulatory dental care were summed to determine total dental health care spending for three years. To detect the effect of smoking, we used multiple logistic regression analysis for dental utilization and multiple linear regression for expenses in ambulatory dental care after changing log. Results: The pack-years had a significant effect on dental utilization and expenses in participants aged 40-59 years. After controlling for income level, bedtime brushing, and the presence of chronic disease, dental care utilization rates in the over 10 to less than 25 pack-year and the over 25 pack-year groups were about 1.4 (OR=1.37, (P =0.007)) and 1.6 times (OR=1.59, (P =0.001)) those in the 0 pack-year group. In comparison with the 0 pack-year group, the over 10 to less than 25 pack-year group spent 50,000 won more over dental expenses, while the corresponding number for the over 25 pack-year group was about 100,000 won. Thus, the ambulatory dental care expenses increased with the packyears of smoking. Conclusions: Smoking significantly influenced the utilization and expense in ambulatory dental care in men aged 40-59 years.
Hepatic Iron, Serum Ferritin, <i>HFE</i> Mutation, and Hepatic Fibrosis in Chronic Hepatitis C

<P><I>Objectives:</I> We studied the status of hepatic iron deposition and its relationship with blood iron indices, liver histology, and <I>HFE </I>gene mutations in Korean patients with chronic hepatitis C (CH-C). <I>Methods:</I> 105 patients with CH-C who underwent pretreatment liver biopsy were consecutively enrolled. The hepatic iron deposition, histological activity and fibrosis were assessed by appropriate pathological scoring systems, clinical data including serum iron indices, and <I>HFE</I> gene mutation. <I>Results:</I> Hepatic iron deposition was found in 37 patients (35%), which was not significantly associated with degree of hepatic fibrosis or steatosis. The serum ferritin level was elevated in 27% of the patients and was an independent factor associated with hepatic iron deposition by logistic regression; however, it was not significantly associated with hepatic fibrosis either. Only H63D heterozygote was found in 6 out of 48 patients (12.5%), which was not different from the prevalence of H63D mutation in the Korean population (8.5%). <I>Conclusions:</I> Hepatic iron deposition was uncommon and mild in Korean CH-C. Neither hepatic iron deposition nor serum ferritin were significantly related to the severity of hepatic fibrosis, which does not support the significant role of iron in the progression of hepatic fibrosis.</P><P>Copyright © 2009 S. <P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>To investigate the effect of body mass index (BMI) on survival in patients with breast cancer according to tumor subtype, metabolic syndrome, and systemic treatment.</P> <P><B>Patients and Methods</B></P> <P>We identified 5668 patients who underwent curative surgery for breast cancer between 1996 and 2013 from the clinical data of a single institution. Disease-free survival (DFS) and overall survival (OS) were calculated and compared between the patients with BMI ≥ 25 kg/m<SUP>2</SUP> and < 25 kg/m<SUP>2</SUP> in all patients and in specific subgroups, including tumor subtype, metabolic syndrome, and systemic treatment.</P> <P><B>Results</B></P> <P>In all patients, BMI ≥ 25 kg/m<SUP>2</SUP> was an unfavorable factor for OS (<I>P</I> = .030) but not for DFS. In the HR<SUP>+</SUP>/HER2− subgroup, DFS and OS were longer in patients with BMI < 25 kg/m<SUP>2</SUP> than ≥ 25 kg/m<SUP>2</SUP> (<I>P</I> = .012 and .005, respectively). In patients with more than one metabolic syndrome, BMI was an unfavorable factor for OS (hazard ratio, 2.669; <I>P</I> < .001)</P> <P><B>Conclusion</B></P> <P>BMI ≥ 25 kg/m<SUP>2</SUP> was an unfavorable survival factor, particularly in patients with HR<SUP>+</SUP>/HER2− breast cancer.</P>
Removal of Kirschner Wire That Migrated from the Pelvic Bone into the Right Ventricle of the Heart

A sixty-year-old man was admitted due to chest pain. He had a history of pelvic bone fracture fixation with Kirschner wire about 20 years earlier. On examination, we detected a Kirschner wire that had migrated into the right ventricle. Without cardiopulmonary bypass, we removed the migrating Kirschner wire via median sternotomy. The patient recovered without complications and was discharged on the 5th postoperative day.

Bi2O3 buffer layers were deposited on Pt/Ti/SiO2/Si substrates by an R.F. magnetron sputtering method in order to improve the ferroelectric properties of SBT (SrBi2Ta2O9) thin films. The volatility of bismuth brings about an obvious non-stoichiometry of the SBT thin films and causes secondary phases to appear. The Bi2O3 buffer layers were found effective in achieving a lower crystallization temperature and in interrupting the diffusion of Pt towards SBT thin films. In this experiment, we have found that the presence of Bi2O3 buffer layers was responsible for the enhanced ferroelectric properties and crytallinities of SBT thin films.
