The objective of this study was to investigate the managing realities of EVD and provide a descriptive survey with the attempt to obtain basic data needed for the standardization of EVD management. For that purpose, 220 questionnaire copies were distributed to those nurses who were working with intensive care units at university hospitals, general hospitals, semi-general hospitals in Seoul, Kyongki-de and local areas. Of 158 returned questionnaire copies, 139 were used for analysis. Data were collected during the term of academic seminars of Neurosurgery Nursing Committee and KICA (Korea Intensive Care Association), ranging from Oct. 26 through 27. 2000. The tools utilized a structured questionnaire form consisting of 21 questions on general characteristics, the level of understanding, the level of work performance, standard management guidelines, etc. The data collected were analysed with frequency, percentage, mean vague, chi -square test using the SPSS 10.0 program Findings were revealed, as follows: For managing the prevention of EVD infection, aseptic technique was found to be of the most importance (48.9%),'the technique should be carried out at an operation room'(92.8%), but it was actually performed at intensive care units (over 51.5%). 'The term of antibiotics use should be less than 7 days' (81.8%), while it was actually executed for less than 7 days (49.5%). 'The proper time of quittance from it should be within less than 7 days'(74.1%) but actual quittance was executed only by 51.1%. 'Set exchange is needed' by 54.0% but actual exchange was implemented merely by 28.1%. 'The subject of management should be nurses' was reflected by 40% but actually 52.0% were carried out. As a result of hating analysed the understanding of the management of EVD by general characteristics, a significant difference was found in the necessity for set exchange by hospitals, in the time of quittance of a catheter, and in ablution items by work experience. The result of hating analysed the work performance levels of EVD management by general characteristics revealed that a significant difference in administration facilities of medicine by the classification of hospitals, especially the use of antibiotics and the quitting time of a catheter by the classification of intensive care units, and in the use of antibiotics and ablution items by the standard management guidelines.
Usefulness of Bronchoscopy in Patients with Microbiologically Negative Pulmonary Tuberculosis

Background Recently, bronchoscopy has been widely used to aid microbiological diagnosis in patients with little sputum. However, the usefulness of bronchoscopy and the patient groups that benefit from subsequent microbiological confirmation have not been established. Methods We retrospectively analyzed the data of the Korean tuberculosis (TB) cohort, a nationwide prospective observation cohort including patients with TB, from September 2018 to October 2019 to evaluate the usefulness of bronchoscopy in patients with microbiologically negative (based on initial sputum polymerase chain reaction and culture Results) pulmonary TB. The primary outcome was the proportion of microbiological diagnoses after bronchoscopy. The secondary outcomes were the predictors of microbiological confirmation and the percentage of additional resistance detection after bronchoscopy. Results A total of 5194 patients were diagnosed with pulmonary TB, and 937 patients were microbiologically negative based on initial sputum findings. Of them, 319 patients underwent bronchoscopy; further microbiological confirmation was checked in 157 (49.1%) patients. The predictors of microbiological confirmation after bronchoscopy were age >65 years, female sex, and low body mass index (BMI). The rate of additional resistance detection was 10.5% [Multidrug resistant (MDR)/Rifampin resistant (RR) 3.8%; Isoniazid resistant (Hr) 5.7%]. Conclusion Bronchoscopy was useful for detecting resistant pathogens. Bronchoscopy should be consider for microbiologically negative pulmonary TB with female, aged >65 years, and low BMI for subsequent microbiological confirmation.

Background/Aims: The purpose of this study was to examine the clinical course and longterm outcomes of children with Allagille syndrome in Korea, and to evaluate the prognostic potentials of identified variables. Methods: We reviewed the clinical manifestations and outcomes of 30 children with Alagille syndrome, investigated from 1984 to 2006 until the end of this study (defined as death or last visit, mean follow-up: 5 years). Results: Cholestasis occurred in all patients. Cardiovascular abnormalities was found in 83.3%. Peripheral pulmonary stenosis was most common (23 patients). Butterfly vertebrae was seen in 30.0%, posterior embryotoxon in 43.3%, and a characteristic facial appearance in all patients. Longterm follow up of the 30 patients revealed that 8 patients had died (26.7%); six related to Alagille syndrome. Five patients died of a liver disease complication. Liver transplantation was carried out in five of the 30 patients (16.7%) and one of these died due to hyperacute rejection. At age two, cholestasis improved in 17 of the 30 patients. Those who had severe cholestasis at 2 years of age were closely associated with high mobidity such as hepatocellular carcinoma or liver cirrhosis leading to liver transplantation and mortality (p=0.001). Conclusions: The prognosis for the patients with Alagille syndrome was not favorable compared with that of the previous studies. Hepatic complications account for the most mortalities in patients with Alagille syndrome. Careful and complete assessments should be made in children who have cholestasis at 2 years of age.

<P><B>Abstract</B></P><P>HGTD-P is a hypoxia-responsive pro-apoptotic protein that transmits hypoxic signals directly to mitochondria. When overexpressed, HGTD-P induces cell death via typical mitochondrial apoptotic cascades. However, much is unknown about post-transcriptional modification and signaling networks of HGTD-P in association with cell death-regulating proteins. We performed yeast two-hybrid screening to identify the molecules involved in HGTD-P-mediated cell death pathways. In this study, we show that heat shock protein 90 physically interacts with HGTD-P and that suppression of Hsp90 activity by low concentrations of geldanamycin reduced HGTD-P-induced mitochondrial catastrophe through inhibition of mitochondrial translocation of HGTD-P.</P>

In this study, the authors investigated changes in the non-specific immune states of 35 acute leukemia patients(19 AML; 16 ALL) according to the state of their disease (25 at diagnosis; 10 in complete remission with three off-treatments) and compared them to those of normal controls. The natural killer(NK) cell activity aganist K562 target cells and antibody-dependent cellular cytotoxicity(ADCC) against L1210 target cells were measured by four-hour chromium release cytotoxic assays. The number of NK cells was estimated by immunofluorescence techniques using monoclonal antibodies of Leu-7a, while Leu-11 and T-cell subpopulations were estimated by monoclonal antibodies ctf CD3, CD4, CD8. Interleukin-2 (IL2) productivity was measured with peripheral mononuclear cells from five patients. The results were as follows; 1) The NK and ADCC activities significantly decreased in the initial state, which might be due to a decreased number of NK cells and also to a diluting effect by the circulating blasts in ADCC. 2) In the complete remission state, patients who had maintenance chemotherapy showed a lower NK cell number, which resulted in decreased NK activity during this period without any effect on ADCC. 3) Compared to the controls, such depression of nonspecific immune state was more pronounced in the AML group than in the ALL group. In conclusion, the non-specific cellular immune states differed between the initial state and the complete remission state, to which maintenance chemotherapy might be attributed. The mechanism and the clinical significance of such an altered immune state according to the diseased state in relation to maintenance treatment remain to be elucidated.
