        신발유형별 보행동작의 운동역학적 동작분석

        The purpose of this study was to prepare some useful data for the prevention of injury caused by shoes when female was walking. Kinematic variables were analyzed with the aid of the visual materials filmed with a 16mm high speed camera, such as angular changes of joints, displacement, velocity, and momentum of the center of body gravity. The subjects for the research were normal 4 female high school students: The analytic results of the mechanism of each locomotion pattern were as follows: 1. It is desirable Kinematically that the dispersion of power should not be caused in the movement of lower-legs by keeping natural flexion and extension of joint. 2. Too much vertical displacement of the center of body gravity was thought undesirable because it caused the dispersion of power against the moving direction. To get some propulsion in the moving direction, it was efficient to use as much energy as possible from 8 F-F support phase to the toe-off. And also the center of body gravity was moved by the inertia just after toe-off. 3. The correct walking motion occurred when the walker moved their hip joint or knee joint gently with their upper body upright and their eyes looking around. 4. In order to lengthen the stride it was essential to practice intensively the flexion of the hip joint, extension of knee and plantar flexion of ankle joint. 5. It was proved that in race walking, they could get much greater propulsion from the flexion of the ankle joint than from the hip joint or the knee joint. It was, therefore, very important to train walkers so that the initial planter flexion of the ankle joint might occur from 6 F-F of the supporting phase. 6. The flexion of the ankle joint produced the added propulsion in the moving direction but the excessive flexion increased energy consumption and caused strain on the ankle joint because it dispersed power in moving the center of body gravity. 7. It was desirable to narrow the ankle of the flexion of the hip and knee joints because it could speed up revolutions per minute of the lower limb through lessening the inertia of the revolutions. The angle made by knee joint was desirable for getting the greatest propulsion in the moving direction. 8. High heel was lower than barefoot or jogging shoes in the displacement of the center of body gravity. This result showed that the displacement of the center of body gravity was made safe by lowering the flexion and extension of knee joint.

        수완진동 증후군에서 신경장해의 조기진단을 위한 객관화된 방법

        목적: 통각과 진동감각 역치검사 및 수부와 수지의 악력, 그리고 수지의 운동기능검사(태평) 등과 근로자들이 호소했던 증상에 대한 HAVS의 스톡홀름 워크샵 분류와의 관련성을 통해 좀 더 객관화된 신경학적 조기검사 방법을 찾고 타당성을 검증하고자 하였다. 방법: 2000년 1월부터 2007년 12월까지 수부진동에 폭로되고 수부의 불편함을 호소하여 진동 장해에 대한 검사를 시행한 497명의 근로자를 대상으로 하였다. 일차적으로 문진을 통해 직업력에 대해 알아보고 과거력을 조사했다. 대상자들의 스톡홀름 워크샵 분류에 따른 소견을 확인하고 수부의 말초 순환기능 검사(손톱압박, 수지피부 온도), 신경기능 검사(통각과 진동각 역치), 운동기능 검사(악력, 태평) 등을 시행하였다. 다른 질환의 감별을 위한 몇 가지 이학적 검사를 실시하고 스톡홀름 워크샵 분류에 따른 검사 결과들을 분석하였다. 결과: 사용 도구는 그라인더 단독 사용이 265명 (67.3%)으로 가장 많았으며 평균 노출 연수는 14.8년이었다. 통각과 진동감각에서 스톡홀름 워크샵 분류 단계에 따른 역치가 증가하는 경향을 보였으나 통각은 양측 모두 통계적인 유의성이 없었고, 진동감각은 125와 250 Hz에서 양측 모두 유의했다(p=0.006-0.038). 수부와 수지의 악력은 단계가 올라갈수록 감소하는 경향을 보였지만 수부와 수지 모두 우측에서만 통계적으로 유의했다 (p=0.041, p<0.01, p=0.034). 태평 첫수는 단계가 올라 가면서 전반적으로 태핑 첫수가 감소하였으며 좌측에서만 통계적으로 유의했다(p=0.002-0.019). 결론: 본 연구는 수부 진동 노출자들의 수부 불편함에 대한 신경계통을 초기에 정량적으로 객관화하여 진단할 수 있는 단일 방법은 아직 없으며 일상생활의 불편함과 스톡홀름 워크샵 분류 등을 통한 주관적인 증상에 더하여 수지 통각과 진동감각 역치검사, 수부와 수지 악력검사, 운동기능검사(태핑) 등을 통합하면 초기에 객관적으로 진단하는 것이 가능할 것으로 판단된다. Objectives: In order to determine find out the best methods for a more objective detection of neurologic abnormality in early hand ann vibration syndrome(HAVS), early with analyzing the validity of each of the detection methods was analyzed. We evaluated the relationships between the sensorineural stage of Stockholm-revised vibration syndrome classification and the results of several tests. Methods: 497 workers were investigated for symptom, exposure duration, the types of tools used, and medical history from January 2000 to December 2007. Pain sense threshold, vibrotactile threshold, hand grasp force, finger grasp force, and a finger tapping frequency test were performed by the workers. Results: The grinder(67.3%) was the most commonly used tool and the mean exposure duration was 14.8 years. Although the pain sense and vibrotactile threshold level tended to increase according to sensorineural stage of the Stockholm classification, there was statistically significant difference in the vibrotactile threshold of 125, 250 Hz (P=0.006~0.038) but not in the pain sense threshold. Hand and finger grasp force tended to decrease according to the sensorineural stage of Stockholm classification and there was statistically significant difference(P=0.041, <0.001, 0.034) only on the right hand side. The tapping frequency also generally decreased according to the sensorineural stage of the Stockholm classification and there was statistically significant difference (P=0.002~0.019) only on the left hand side. Conclusions: Although there is no single standardized method that can objectively diagnose the Sensorineural component of early HAVS early, the combination of subjective symptoms, the sensorineural stage of Stockholm classification, the pain and vibrotactile threshold test, the hand and finger grasp force, and the finger agility (tapping) test can objectively detect sensorineural component of HAVS early.

      • 효과적인 하중운반체계의 동역학적 연구

        The purpose of this study was to get the quantitative data of effect of two different load carrying system on Kinematics and Kinetics. The subjects for the research were 2-adults. The mechanism of each load carrying pattern was measured by a motion analylizer, a forceplatform and EMG system. The conclusions were get from Kinematics variables and Kinetics variables. 1. It was not effected at the vertical reaction force(Fz) of left, right foot according to the big value the 1st peack of 10kg, 15kg fore, back-pack carrying system, 20 kgw, 30 kgw back-pack carrying system in walking. But it was effected at the vertical reaction force(Fz) on 30 kgw back-pack carrying system in walking. 2. It was not effected at the Anterior-Posterior reaction force(Fx) of left, right foot according to high value of the 1st peack on 10 kgw, 15 kgw fore, back-pack carrying in walking. But, it was effected on the 20 kgw, 30 kgw only back-pack carrying in walking. 3. It was effected at the left-right reaction force(Fy) of left-right foot according to the big value of the 2nd peak on 10 kgw, 15 kgw fore-backpack carrying system in walking. It was more effective than 20 kgw, 30 kgw back-pack carrying system in walking. There was not disorder force(Fx) on 10 kgw, 15 kgw fore-backpack carrying system in walking. 4. Analysis of computer graphic, on the pack carrying system in walking. It was efficient in 10 kgw, 15 kgw fore-backpack carrying system in walking. But, it was efficient on 20 kgw, 30 kgw backpack carrying in walking. Castrocnemius, Tibialis anterior, Vastus lateralis, Vastus medialis, are stressed to bend their body foreward. So it occurred fatigue fast, injured muscles and joints. 5. Extension anyles and flection angles of the knee joint and the hip joint on 10 kgw, 15 kgw fore-backpack carrying system in walking showed less than those on 20 kgw, 30 kgw backpack carrying system in walking. It was efficient on the 10 kgw, 15 kgw fore-backpack carrying system in walking. 6. Magnitudes of EMG of Castrocnemius, Tibialis anterior, Vastus lateralis, Vastus medialis, were increased according to increasing weight road and were decreased on the 20 kgw, 30 kgw back-pack carrying system in walking.

        Fabrication and superconducting property of $MgB_2$ tape with Al metal powder addition

        The sub micron sized spherical $MgB_2$ powders were synthesized by spray reaction method. $MgB_2$ tapes with Al addition were fabricated by Powder in Tube (PIT) method. The superconducting property and microstructure of Al doped $MgB_2$ tapes were characterized by X-ray diffraction, optical microscopy and transport measurement under magnetic field. The $J_c$ value of $MgB_2$ tapes was increased with 10 vol. % Al addition. The $J_c$ value of 5,500 A/$cm^2$ and 11,000 A/$cm^2$ at 4.2 K and 5 T were obtained for the $MgB_2$ tape and 10 vol. % of Al added $MgB_2$ tape without heat treatment, respectively. The $J_c$ value of 8,000 A/$cm^2$ and 33,000 A/$cm^2$ at 4.2 K and 5 T were obtained for the $MgB_2$ tape and 10 vol. % of Al added $MgB_2$ tape with heat treatment, respectively. The $J_c$-B curves show enhancement in $J_c$ (B), which suggests that the microstructure and transport properties of $MgB_2$ tapes have been improved with Al addition.

        Background/Aim: Transjugular Intrahepatic Portosystemic Shunt (TIPS) has been undergone for control of gastric variceal bleeding. But, TIPS has some limitations as hepatic encephalopathy or shunt occlusion, etc. Balloon Occluded Retrograde Transvenous Obliteration (BRTO) also has been undergone for these patients. So, we compared the effectiveness and safety of TIPS and BRTO for control of gastric variceal bleeding. Methods: Between January 2003 and February 2012, TIPS or BRTO have been undergone for gastric variceal bleeding. All patients had patent portal veins on preprocedural computed tomography. The patient who failed BRTO underwent TIPS for rescue therapy. Data regarding technical success, rebleeding, hepatic encephalopathy and survival were analyzed by Chisquare test and Kaplan-Meier method with Log-rank test. Results: 1. Among 100 patients, 32 patients underwent TIPS and 68 underwent BRTO. Technical success was achieved in 96.9% (31/32) in TIPS group and 86.8% (59/68) in BRTO group. There was no statistical difference of age, sex distribution, and Child-Pugh class in each group. 2. Early rebleeding within 5 days after procedure were 18.8% (6/32), 4.4%(3/68) (p=0.019) and late rebleeding after 5 days were 18.8% (6/32), 1.5% (1/68) in each group (p=0.002). 3. Hepatic encephalopathy was developed or aggravated in 53.1% (17/32), 4.4%(3/68) (p<0.001). 4. After procedure, Child-Pugh class was aggravated in 50% (16/32) and 19.1% (13/68) (p=0.004). 5. The survival time were 45.4 in TIPS and 62.3 months in BRTO group (p=0.04). 6. Procedure related complications did not have statistical difference in each group. Conclusion: BRTO can more effectively control of gastric variceal bleeding with less worsening of Child-Pugh class, hepatic encephalopathy, relatively decreased rebleeding rate and longer survival period than TIPS. So, if the patients who have ceased gastric variceal bleeding in the presence of gastrorenal shunt, BRTO may be considered as initial therapeutic procedure for control of gastric variceal bleeding.

      • Synthesis and characterization of fine and homogeneous BSCCO-2223 precursor powder by spray pyrolysis process for PIT process

        Homogeneous and fine BSCCO precursor powders were prepared by spray pyrolysis (SP) method starting from an aqueous solution of metal nitrates. The influence of solution concentration, furnace reaction temperature and types of spray on microstructure and average particle size were investigated. The synthesized precursor powders had a narrow particle size distribution and low carbon content. The reactivity of precursor powder by SP method is very high, attributed to the fine and narrow particle size distribution of precursor powders. The precursor powder by SP method promoted a very quick formation of the 2223 phase for short sintering time. The XRD result shows high possibility of reducing secondary phases such as large AEC and almost phase pure microstructure through more homogeneous reaction.

        MOD 공정에 의해 제조된 YBCO 박막의 압력 의존성

        The growth properties of MOD-YBCO films were investigated. To enhance the growth rate of YBCO layer and inhibit the build-up of HF gas during the annealing process in TFA-MOD for YBCO coated conductors the method of low pressure annealing was employed. Total pressure was changed from 700Torr to 1Torr and its effect on growth of YBCO films was compared with atmospheric one. The lower Pressure was effective to control of the pore size in MOD method . Surface morphology of YBCO films processed at low total pres sure was rough and composed of random YBCO (103) grains. But large pores, usually observed at atmospheric process in MOD disappeared and also the number of pores was reduced at low pressure annealing. Also discussed ate the effects of Fluorine-free Y and Cu precursor solution on the development of microstructure. Dense surface me phology and with less and small pores can be provided through controlling Fluorine content.

      • HCC : PE-075 ; Clinical courses of primary hepatic angiosarcoma

        Background/Aim: Angiosarcoma is a rare and aggressive liver malignancy originated from endothelial cells. Due to non specific symptoms and signs, it is difficult to diagnose at the early stage, radiologic findings of hepatic angiosarcoma are non specific, and it is not easy to differentiate with hemangioma or other liver malignancy. We reviewed the clinical courses of hepatic angiosarcoma those histologically confirmed. Patients and Methods: Patients who have been diagnosed at Keimyung University Dongsan Hospital between 2001 and 2011 were enrolled. we retrospectively reviewed the pattern of disease detection, the sites of metastasis , treatment responses, and prognosis. Results: 1.8 patients were histologically confirmed as primary hepatic angiosarcoma. Mean age was 66.3 year-old (41-80 yo). 4 patients were male. All patients’ serum HBsAg and anti-HCV were negative and five patients were heavy alcoholics. 2. Only two patients (25%) were diagnosed by radiology findings, and then definitive diagnosis was made on the basis of percutaneous needle biopsy in seven (87.5%) and operation in one (12.5%). 3. At initial disgnosis, extrahepatic metastasis were detected in three patients (37.5%), and metastatic site were spleen and lung (n=1), pericardium (n=1), and bone (n=1). 4. Three patients (37.5%) received conservative treatment only, one (12.5%) under went surgical resection and four (50%) received chemotherap. Median survival time in treatment group were 427days and only 36days in conservative group (p value = 0.022). Conclusion: Only few patients of hepatic angiosarcoma can be diagnosed by radiologic findings, we have to actively consider liver biopsy to confirm. More than one third of patients already had metastatic lesion at initial diagnosis, and the clinical courses were very poor. So, we have to consider aggressive treatment for better survival.
