        업무 관련성 견갑상 포착 신경증이 의심되는 3례

        Objectives : Entrapment of the suprascapular nerve is frequently overlooked in the differential diagnosis of shoulder pain. Methods : Suprascapular entrapment neuropathy is a well-defined clinical entity and EMG and NCV is used to confirm a diagnosis. But the diagnosis typically not considered until patients develop severe weakness secondary to atrophy of the spinous musculature that the nerve supplies. Results : A narrow suprascapular notch has rarely been reported as a work-related factor of this entrapment neuropathy. Diagnosis of suprascapular entrapment neuropathy is based on the patients' clinical course, neurologic, radiologic, and electrophysiologic findings. One of the most helpful evaluations was anteriorposterior projection with the X-ray tube angled 15-30 degree caudally. The suprascapular entrapment neuropathy is relatively uncommon entity of shoulder discomfort(pain, weakness, and atrophy). Conclusions : If the worker who used his shoulder joint repetitiously having the shoulder pain and muscle weakness, we must rule out the suprascapular entrapment neuropathy. And it is needed to evaluate the motions which cause suprascapular entrapment neuropathy as the ergonomic factor.

      • TNF-α 유전자형과 방광암과의 관계

        연구목적: TNF-α는 일부 종양의 종양화 과정과 관련이 있는 것으로 알려져 있다. 본 연구는 TNF-α 발현에 영향을 미치는 TNF-α 촉진자 -308 부위의 유전적 다형성이 방광암과 관련이 있는지 유무를 알고자 시행하였다. 대상 및 방법: 유전자 분석을 위하여 환자 113명 및 대조군 109명으로부터 혈액을 채취하여 genomic DNA를 분리한 후 PCR-RFLP 및 direct DNA sequencing을 통하여 TNF-α유전자의 다형성을 조사하여 방광암의 발생, 병기 및 분화도와 비교 검토하였다. 결과: TNF-α 촉진자 -308 부위의 유전형은 대조군에서는 GG형이 83.5%(90례 및 GA형이 16.5%(19례)로 관찰되었으며 AA형은 없었다. 환자군에서는 GG 형이 85.4%(97례), GA형 및 AA형은 각각 13.1%(15례)및 0.8%(1례)에서 관찰되었다. 두 군 모두에서 GG형이 가장 많이 나타났으며 다음으로 GA형을 보이고 AA형은 1례의 방광암 환자에서만 관찰되었다. -308부위의 경우도 두 군 사이에 유전자형의 차이는 없었다(p=0.259) 분화도별 분포를 보면 grade I이 20례, grade II가 49례, grade Ⅲ은 34례였고 병기별로 표재성인 경우가 90례였으며 침윤성은 14례였다. 분화도가 나빠질수록 GA형이 증가하였다(p=0.04). 그러나 병기와 TNF-α promoter -308부위의 유전자형 사이에는 유의한 상관 관계가 없었다(p=0.123). 결론: 방광암 환자의 혈액에서 GA genotype이 관찰되는 경우, 분화도가 나쁠 가능성이 매우 높기 때문에 좀 더 적극적인 치료와 세밀한 추적관찰을 함으로써 방광암으로 인한 사망과 암의 진행을 예방할 수 있을 것으로 기대한다. Purpose : Tumor necrosis factor-alpha (TNF-α) is involved in tumorigenesis of several cancers as an endogenous tumor promoter. The purpose of this study was to investigate whether genetic polymorphism of TNF-α promoter region (-308) was associated with human bladder tumor. Materials and Methods: The DNA from 113 and 109 respective blood samples of bladder tumor Patients and control group was analyzed by PCR-based restriction fragment length polymorphism (RFLP) and direct DNA sequencing methods to characterize the genetic polymorphism of -308 promoter region of the TNF-α gene in bladder tumor patients. We compared the association of bladder tumor with genetic Polymorphism of TNF-α promoter region(-308) in relation to the stage, grade, recurrence and progressio. Results : Eighty-six percents(97/113) of bladder tumor patients and 83.5% (90/109) of control group showed genotype GG at -308 region of TNF-α. Difference in genetic variations of TNF-α promoter (-308) did not exist between bladder tumor patients and control group(p=0.259). Tumor grade was significantly related to the GA genotype (p=0.04). The higher is the grade in bladder tumor, the more frequent was the GA genotype. Tumor stage, recurrence and progression were not significantly associated with genetic polymorphism of TNF-α promoter region (-308). Conclusion: The GA genotype of TNF-a promoter region (-308) had a significant impact on TNF-α production and was related to higher grade tumor compared to GG genotype. TNF-α serum levels in bladder tumor patients were significantly higher than controls. These data suggested that TNF-α might involve the tumorigenesis of the bladder rather than treatment or prevention of bladder tumor.

      • 알코올 금단 발작 환자에서 진전 섬망 예방에 대한 Nimodipine과 Chlordiazepoxide의 효과 비교

        Background & objectives : An increase of calcium influx during alcohol withdrawal state may contribute to neuronal hyperexitability, which cause delirium tremens(DT). Some investigators have found calcium channel blockers effects in prevention of DT in experimental animals. Therefore we evaluated the preventive effects of calcium channel blocker(nimodipine) and benzodiazepine(chlordiazepoxide) on the development of DT in alcohol withdrawal seizure(AWS) patients. Method : A total 59 patients with AWS were divided into three groups according to the management nimodipine-treated, chlordiazepoxide-treated, and control(no specific medication) groups. We compared the incidence rates of DT in the three groups. Results : Total incidence rate of DT was 30.5%(18 of 59 patients). There were 6 DTs of 18 patients(33.5%) in nimodipine-treated group, 4 of 21 patients(22.5%) in chlordiazpoxide-treated group, and 8 of 20 patients(44.0%) in control. Conclusion : The control group(44.0%) showed the highest incidence rate of DT. And nimodipine-treated(33.5%) and chlordiazepoxide-treated group(22.5%) were followed. However, this result failed to demonstrated statistically significant differences due to small numbers size.

      • 인체 치은섬유모세포에 대한 니켈의 세포독성과 Ferulic Acid 및 Vitamin E의 독성경감효과

        Nickel is major metal used in the nickel-chromium alloys of most orthodontic appliances and partial denture. But this metal is known to cause hypersensitivity, dermatitis and asthma. In addition, significant carcinogenic and mutagenic potentials has been demonstrated for compounds containing nickel. The purpose of this study was to develop the compounds repairing on the cytotoxicity of nickel in cultured human gingival fibroblasts. The cytotoxicity of nickel sulfide and nickel oxide in human gingival fibrobalsts treated with dose dependent nickel was measured by MTT assay for cell viability and XTT assay for cell adhesion activity. The cytotoxicity of ferulic acid itself also was investigated by the same cells and methods. The procedure of repairment in human gingival fibroblasts injuried by NiS_50 (nickel sulfide IC_50) and Ni0_50 (nickel oxide IC_50) was carried out to observe after treatment of ferulic acid by dose dependent manner. The 5×10⁴ cells/㎖ of human gingival fibroblasts in each well of 24 multidish were cultured. After 24 hrs, the cells were treated with solution of all groups. After the human gingival fibroblasts were cultured in same condition for 48 hrs, the colormetric methods were performed to evaluate the cytotoxicity. The light microscopic study was carried out to observe morphological changes of cultured human gingival fibroblasts. The results were as follows : 1. According to the MTT absorbance, nickel sulfide and nickel oxide were significantly decreased in 25μM and 50μM, respectively. IC_50 of nickel sulfide and nickel oxide were 138.1μM and 311.7μM, respectively. 2. According to the XlT absorbance, nickel sulfide was significantly decreased in 50 μM and nickel oxide was significantly decreased in 100μM, IC_50 of nickel sulfide and nickel oxide were 104.4μlM and 367.8μM, respectively. 3. MTT_50 and XTT_50 of ferulic acid were 2,130.3μM and 1,773.7μM, respectively. These results were determind to nontoxic by Borenfreund at., al. 4. The inhibitory effects of ferulic acid against nickel-induced cytotoxicity were significantly increased from 25μM ferulic acid, but inhibitory effects of 100μM ferulic acid was more decreased than that of 50μM ferulic acid. 5. Treatment of ferulic acid and vitamin E combination was more assumed an upward curve than that of ferulic acid only. 6. The cell numbers and cell shapes were repaired from 1μM ferulic acid. However 100μM ferulic acid showed degeneration again. These results suggest that ferulic acid and vitamin E may be decrease the cytotoxicity of nickel sulfide and nickel oxide in cultured human gingival fibroblasts.

      • 제 6번 뇌신경 마비 증상으로 발현된 비소세포성 폐암 : 이차성 사대 전이 골수 변화 Secondary to metastatic clivus bone marrow change

        Isolated abducens nerve palsy usually occurs in the vasculopathic age group. It is often associated with diabetes mellitus, hypertension, and atherosclerosis. But also it occurred in the head trauma and skull base tumor. A 71-year-old man with diplopia was admitted. He had suffered from right temporo-orbital sided mild headache without nausea and vomiting. Physical and neurological examination revealed a right lateral rectus paralysis. Simple X-ray skull series showed unusual opacitic lesion on right side(Fig 1.). Brain MRI and temporal bone CT showed diffuse bone marrow change in right clivus(Fig 2, 3). He was discharged and transferred the Korea Cancer Center Hospital(KCCH). He was evaluated in department of oncology, KCCH. Chest CT finding was ill-defined margined lung cancer. The pathologic findings were non-small cell lung cancer. Ten months after follow-up, he was expired.

        By the spectral analysis of many heavy rains' recorded data, at Han river basin, an emprical spectral density function model is presented for the spatial distribution of storm. Using the storm data which was augmented by the Stochastic correlation with it's neighbors, the multiquadric random ndom surface of total storm depth is constructed. And to separate the local components from it's regionals and find the regional charcteristics, a double Fourier analysis was applied to the total depths. The local components, storm residuals of each storm was assumed to be an homogeneous random field and investigated with it's autocorrelation function. For the practical application, isotropic was assumed which was identifyed with emprical data. Coefficients of normalized autocorrelation for all storms were shown similar appearance. Using this emprical result, an example of the radial spectral density function is presented which represents the spatial characteristics of heavyrains over Han-river basin during 1975~1983.

      • 흰쥐의 감각신경절세포에 대한 Methylmercuric chloride의 독성효과

        흰쥐의 배양 척수감각신경절세포에 대한 methylmercuric chloride(MMC)의 세포독성을 조사하기 위하여 여러 농도의 MMC가 포함된 배앙액에서 24시간 동안 배양한 후 MTI assay법에 의하여 세포의 생존율을 조사하였다. MMC는 농도에 비례하여 세포의 생존율을 감소시켰다. 또한 여러 농도의 MMC가 포함된 배앙액에서 흰쥐의 척수 감각신경절(dorsal root ganglion, DRG) 세포를 24시간 동안 처리한 결과 MTI_(50) 값이 25μM MMC에서 나타났다. MMC는 Borenfreund등(1988)의 독성판정기준에 의하여 흰쥐의 배앙 DRG 세포에 고독성인 것으로 나타났다. 이상의 결과는 MMC가 흰쥐의 배양 척수감각신경절세포에 강한 신경독성 효과가 있음을 제시하였으며 또한 배양신경세포는 약제의 효과를 검색하는데 효과적이었다. To examine the cytotoxic effect of methyl mercuric chloride(MMC) on the cultured rat spinal dorsal root ganglion(ORG) neurons, cell viability was measured by MTT assay after rat spinal ORG neurons were incubated with media containing various concentrations of MMC for 24 hours The results were as follows ; 1. MMC decresed cell viability of rat spinal ORG neurons in a dose-dependent manner. 2. MIT_(950) value was a 25uM MMC after spinal ORG neurons were cultured for 24 hours at various concentrations of MMC. 3. MMC was heighly toxic on the cultured ORG neurons by the toxic criteria of Borenfreund et al.(1988). From above the results, it is suggested that methylmercury involves in neurotoxicity by the decrease of cell viability in cultured rat spinal DRG neurons, and cultured neurons are effective in screening the putative agents.
