        무증상 갑상선기능저하증 환자에서 Thyroxine 치료에 따른 혈청 지질 및 아포지단백의 변화

        Background: Subclinical hypothyroidism(SCH) is a common biochemical abnormality which can be found in routine screening tests of thyroid function. We are increasingly faced with the question of whether its an indication for thyroxine replacement therapy. The effect of thyroxine replacement on lipid profile in SCH has aroused a great interest because of an association of overt hypothyroidism(OVH) with hyperlipidemia and increased risk of coronary artery disease. Method: We prospectively evaluated the changes in lipids and apoproteins before and after thyroxine replacement therapy in 23 patients with SCH and in 37 patients with OVH. We measured serum total cholesterol and triglyceride using autoanalyzer, high density lipoprotein(HDL) chole-sterol by dextran sulfate method, Apo A1 and Apo B by immunonephelometric assay. Results: Thyroxine replacement therapy significantly decreased total cholesterol, low density lipoprotein(LDL) cholesterol and apo B levels, but did not affect the level of triglyceride, HDL cholesterol or apo AI in patients with OVH. In SCH, thyroxine replacement therapy with the doses to normalize serum TSH concentrations also decreased significantly the level of cholesterol and LDL cholesterol albeit apo B levels did not change. Moreover, in most of patients with OVH (11 of 12) and in all of patients with SCH(5 of 5) who had had hyperchlesterolemia before treatment, thyroxine replament normalized their cholesterol and LDL cholesterol levels. Conclusion: In regard to the beneficial changes in blood lipid levels, patients with SCH should be treated, especially in cases who have other risk factors for the development of atherosclerosis. If thyroxine replacement only will reduce the incidence of coronary artery disease in SCH remains to be elucidated by long-term prospective studies(J Kor Soc Endocrinol 11:41-51, 1996).

        기관내 삽관을 위한 Atracurium 의 적정량에 대한 임상적 고찰

        The usefulness of a nondepolarizing muscle relaxant for endotracheal intubation is limited by its relatively slow onset of neuromuscular block compared to that achieved with succinylcholine. Several attempts have been made to produce a more rapid onset of muscle relaxation for endotracheal intubation. A large dose of nondepoarizing muscle relaxant may produce rapid onset but cauae undesirable side effects and a prolonged duration of neuromuscular block. The authors observed the degree of vocal cord relaxation and intubation condition 2 minutes after administration of atracurium and measured changes in mean arterial blood pressure and heart rate at the time of arrival in the operating room, just before abministration of atracurium, 2minutes after administration, and 5mins after intubation. The 40 of patients in this observation were divided into four groups. Group 1; 10patients, received 0.3 mg/kg of atracurium Group 2; 10patients, received 0.4 mg/kg of atracurium Group 3; 10patients, received 0.5 mg/kg of atracurium Group 4; 10patients, received 0.6 mg/kg of atracurium The results were as follows; 1) There were better intubating conditions in Group 3 and 4 than in Group 1 and 2(p$lt;0.001). 2) Mean arterial blood pressure and heart rate did not change significantly after administration of atracurium in all four groups. From the above results we conclude that 0.5~0.6 mg/kg of atracurium provides satisfactory intubation condition 2 minutes after administration without hemodynamic changes.

        예비 수요자 주택선호특성을 고려한 유형별 고령자주택 개발방안에 관한 연구

        산업화와 도시화를 경험한 사회는 인구통계학적변화를 경험하고 있다.이러한 변화는 세계적으로 고령화 인구를 점차 증가시키고 있으며,이러한 현상이 세계적으로 확산되고 있다.한국도 2000년에 고령화 사회에 접어 들어들었으며,2018 년에는 고령 사회가 직면할 것으로 예상된다.따라서 이러한 현상에 대비한 고령자주택의 개발 등의 노인의 라이프 생활을 고려한 주택개발이 강조되고 있다.본 연구의 목적은 고령자 주택에 입주하기 원하는 예비 수요자의 특성에 따른 결정요인을 분석하는 것을 목적으로 한다.이를 바탕으로 본 연구에서는 유형별로 보다 세분화하여 이항 로지스틱 회귀분석을 통한 고령 자 주택의 선택의사에 미치는 영향요인을 세분화 하였다.이를 통해 개선방안 및 시사점을 도출하였는데 이에 대한 시사점으 로,첫째,고령자주택은 개발 유형별로 선호하는 특성이 상이하게 도출됨을 알 수 있었다.둘째,개인특성에 따른 지표가 가장 많은 부분을 차지하고 있어 고령자주택의 수요조사가 우선적으로 시도되어야 함을 알 수 있었다.셋째,도심형 고령자주택 개발 시 원활한 개발을 위해 지자체 차원의 지가를 고려한 토지확보 방안이 필요하다.넷째,교외형 고령자주택 개발 시 경기 도권에 수요층이 자연스럽게 고령자주택으로 입주하고 생활할 수 있도록 근교에 생활환경기반을 마련해야 할 것이다.마지 막으로 전원형 고령자주택 개발 시 다양한 생산,여가 등의 프로그램의 도입되어야 할 것으로 판단된다. The society experiencing the industrialization and urbanization has got over the socio-demographic change. these changes make the number of the population around the world, and this phenomenon is flowing into the whole country. Korea has became a Aging Society since 2000 and will be turned into the aged society by the 2018. therefore, the importance of preparing elderly living life such as silver town is getting emphasized. the purpose of this study is aimed at analyzing the decision elements of the preliminary demanders’ intention who selecting Elderly Housing. Based on this study, it was broken down by the type much more. Binary Logistic Regression Analysis of Factors affecting the Elderly housing choices were subdivided. Through this process, improvement and the implications of this study was derived. this study deducts 3 kinds of implications. First, the preference for the development of elderly housing are different with each type of characteristics. Second, the indicators along with the individual characteristics account for the most part of the surface. so the specific investigation for the demand must be required to check the indicators. Third, when it comes to development of urban elderly housing, it requires to have a part of a local government plans securing the land. Fourth, when it comes to development of suburb elderly housing, it is required to arrange the living environment around the suburbs to let user classes living in Gyeonggi-do flow into elderly housing and live their new-life in the suburbs. Finally, when it comes to development of rural elderly housing, a variety of production, leisure and other programs should be made and put into there.

        조경식물의 식재와 이용 : 원주시와 횡성군을 중심으로

        본 연구에서는 강원도 원주시와 횡성군애에 식재 활용 되고 있는 조경 식물의 이용 상황을 조사 분석하여 현재 어떠한 식물이 어떻게 이용되고 있는가를 파악함으로서 그 이용방법에 대한 개선 방안을 모색하고 아울러 원주시와 횡성군의 아름다운 경관 형성을 위한 관리 방법 등을 제시하였다. This study was executed to find out how to improve the planting and use of landscaping plants in Weonju and Hoengseong. 1. The number of street trees were 22.068 and the species number were 10 species in Weonju in 2004. The major species of street trees were Ginkgo biloba(58%), Prunus sargentii(15%), Zelkova serrata(9%), Prunus armeniaca var, ansu(8%), and Acer palmatum(6%). The ratio of native species versus exotic were 50:50. In Hoengseong, the number of street trees was 13,500 and the specise number were 15 species. The major species of street trees were Prunus sargentii(42%), Ginkgo biloba(23%), Acer triflorum(12%) Prunus armeniaca var, ansu(6%), and Prunus mume(4%). The ratio of native species versus exotic were 67:33. The species of which planting frequency within two areas was very high were Ginkgo biloba and Prunus sargentii. 2. It is necessary to select tree species suitable for the characteristics of the locality and to raise distinctive street trees that contribute to the tourist industry. For the purpose, the appropriate street trees in two areas are Cornus controversa, Quercus aliena, Zelkova serrata. Prunus padus, Sorbus alnifolia, Sorbus comixta, Albizzia julibrissin, Acer triflorum. Styrax japonica, Chionanthus retususm Celtis sinensism, Prunus yedoensis, Malus sieboldii, Crataegus Pinnatifida, Prunus armeniaca var. ansu and Pyrus pyrifolia etc.. 3. Appropriate pruning adds to the aesthetic and prolongs the useful life, it also requires less managing of insects and diseases to maintain good healthy of street tees. Street trees were not properly pruned due to electric lines and shortage of pruning information. The pruning was controlled by Korea Electric Power Co, which has no pruning information. Pruning must be maintained by a professional landscape company to maintain good shape such as with Bonsai. The shrubs planting zone between street trees and other trees, and preservation plates were established for healthy of street trees. They have to be repaired and maintained well to keep better environmental conditions. The proper fertilization, the control of pests and diseases, the installation of drainpipe and the use of soil brought from another place were needed to improve the planting, use and maintenance of landscape plants. 4. The species number of school trees and flowers of 102 schools in Weonju and Hoengseong were 17species, 16species respectively. The major species of school trees in Weonju were Juniperus chinensis(24%), Ginkgo biloba(17%), Pinus densiflora(14%), Zelkova serrata(14%), and Pinus koraiensis(9&), and those of school trees in Hoengseong were Pinus koraiensis(44%), Abies holophylla(25%), Juniperus chinensis(8%), and Ginkgo binloba(8%). The major species of school flowers in Weonju were Rosa centifolia(47%), Forsythia koreana(24%), Magnolia kobus(12%), and Rhododendron schlippenbachii(6%), and those of school flowers in Hoengseong were Forsythia koreana(36%), Rhododendron schlippenbachii(33%), Magnolia kobus(6%) and Dicentra spectabilis(6%). 5. The species number of the protection trees designated by Woenju and Hoengseong were 15 species. The major species of protection trees were Zelkova serrata(100 trees), Ginkgo biloba(18) Pinus densiflora(7), Quercus spp.(5), Juniperus chinensis(4) and Alnus japonica(4). 6. The landscape plants planted around 2004 in weonju were Prunus yedoensis(2,563trees), Betula platyphylla var, japonica(2,000), Abies holophylla(1,785), Diospyros kaki(1,100), Prunus sargentii(880) and Prurus armeniaca var. arsu(708) etc.. The shrubs planted were Rhododendron obutusum(21,559 plants), Rosa certjfolia(7,150), Rhododendron yedoense var. poukhanense(5,950), Forsythia koreana(3,000) and Ligustrum obtusifolium(2,500) etc.. The landscape plants planted in Hoengseong Acer triflorum(928trees), Prunus yedoensis(455), Zelkova serrata(327), Thuja orientalis(261), Prunus sargentii(257), Pinus koraiensis(200), Prunus persica for, rubro-plena(200) and Pyrus pyrifolia(200) etc.. The shrubs planted were Rhododendron yedoense var, poukhanense(15,936). Syringa dilatata(10,090). Forsythia koreana(9,660). Cercis chinensis(3,200). Buuxus microphylla var, koreana(2,600) and Rosa centifolia(1,868) etc.. 7. The species numbers of the herbaceous planted in 2004 in Weonju were 24 species and the ratio of native species versus exotic were 7:17. The major species of perennial plants were Aster koraiensis(30,656 plants), Coreopsis drummondii(7,656), Rudbeckia bicolor(6,000), Chrysanthemum morifolium(4,850) and Chrysanthemum zawadskii var, latilobum(4,312). The major species of annuals and biennials were Cosmos bipinnatus(672,000plants), Zinnia elegans(35,600), Petunia hybrida(26,920), Viola tricolor(23,000), Helianthus annuus(17,000), and Geranium cinereum var, pubcaulescens(5,200). In Hoengseong, the numbers of herbaceous plants were 906,310 plants and the species numbers were 15 species. The major species of perennials plants were Aster koraiensis(70,480 plants), Hemerocallis fulva(20,070), and Phlox drummodii(18,000). The major species of annuals and biennials were Phlox hybrida(174,000 plants), Cosmos bipinnatus(125,000), Zinnia elegans(109,000), Tagetes patula(96,700), Vinca rosea(89,000) and Calendula officinalis(70,000). 8. Through these result, it was thought that the diversification of planting species, the selection of plants suitable to each space and the generalization of use of native species were needed.

      • Arthroscope 시스템 설계에 관한 연구

        본 연구에서는 관절 내시경 시스템 개발을 목표로 임상 교수들의 조언과 사용자의 편의를 우선으로 하는 PC기반 GUI 형태의 관절 내시경 시스템 설계에 초점을 맞추어 연구하였다. 관절 내시경 시스템의 성능 향상을 위한 모듈 설계하여 검증을 통한 시험 및 시뮬레이션 분석을 하였다. 또한 임상 교수의 관절 내시경 시스템 개발 참여로 사용자 중심의 편리한 시스템 구성 및 관련 H/W 및 S/W 기술 축적뿐만 아니라 PC기반 GUI 형태의 윈도우 S/W 개발로 해외 시장개척을 위한 경쟁력이 강화될 것으로 기대된다.

        응급실로 내원한 장중첩증의증 환아의 진단방법에 대한 임상적 고찰

        The purpose of this study is to evaluate the appropriate diagnostic methods for the patients with suspected intussusception. Intrssusception is a state that a portion of alimentary tract is telescoped into a segment just caudad to it, and has a characteristic symptoms of cyclic irritability, vomiting, bloody stools and palpable mass, It is the most common cause of acute or acquired intestinal obstruction in infants and children, and needs early diagnosis and immediate treatment. Authors reviewed retrospectively medical records of 411 infants & children with suspected intussusception from January 1, 1992 to December 31, 1994 and report the results with literature reviews. The results were as followings; 1) 263 cases were definite intussusception(DI) out of 411 suspected intussusception(SI). 2) The male to female ratio was 2.3 : 1 in SI and 2.8 : 1 in DI. 3) The distribution of age revealed that 85% of SI and 84% of DI were under 24 months old, and the most common age was 8 months in SI and 10 months in DI. 4) There were upper respiratory infection in 84 cases(2)%), gastroenteritis in 32 cases(8%) and bronchiolitis in 8 cases(2%) as preceding diseases. 5) The simple X-ray showed gaseous bowel distension in 212 cases(52%). 6) The most common symptoms and signs were cyclic irritability and abdominal pain(94%), bloody stools(72%), palpable abdominal mass(47%), in order. 7) Of 411 patients, 252 cases(61%) were diagnosed and treated by air enema(228 cases) and/or exploration(24 cases). Among 296 cases(72%) who showed bloody stools in digital rectal examination and glycerin enema, 245 cases(83%) were confirmed as intussusception through air enema and/or operative intervention, and that corresponds to 93% of DI. 8) Intestinal perforation as complication occurred in 3 of 393 cases who underwent air and barium enema, but all of them recovered without any sequelae after operation. Conclusively, it is thought that air enema preceded by digital rectal examination and glycerin enema is safe and useful method in diagnosis and treatment of infants and children with SI.

      • 유화제 첨가량이 MMA/BA 합성 라텍스 폴리머 시멘트 모르타르의 특성에 미치는 영향

        The purpose of this study is to clarify the effect of the emulsifier ratio on properties of the polymer modified mortar using methylmethacrylate-buthyl acrylate(MMA/BA) latexes, and to obtain basic data necessary to develope appropriate latexes for cement modifiers. Polymer modified mortars using MMA/BA latexes are prepared with various monomer ratios, and tested for air content, flexural and compressive strengths. From the test results. we knew that the water-cement ratio is decreased with an increase in the amount of emulsifier. And the air content is increase with an increase in the polymer cement ratios. In general, the superior compressive and flexural strength of polymer-modified mortars using MMIA/GA latexes is obtained ant a bound MMA content of 80 percent and a emulsifier ratio of 6 percent.

      • 스파르가눔(sparganum) 표피의 미세구조

        An electron microscopic study was performed in order to know the basic tegumental structure of sparganum, which was obtained from the subcutaneous tissue of naturally infected snakes (Matrix tigrina lateralis). Two anterior and middle portions of worm tissuses were cut, and prepared for transmission electron microscopy by conventional methods. Then the sections were examined by means of A.E.I. Corinth 500 electron microscope. General structure of the tegument of sparganum resembled that of the body wall of other cestodes on electron micrograph. The teguments both of the anterior and middle portion revealed such components, i. e., an outer vesicular layer with numerous microtriches, and inner fibrous layer. Below the fibrous layer, there were layers of muscle bundles, and tegumental cells. The surface of sparganum was covered by intermingling microtriches of two distinctly different types. But the characteristic elongated 'pyramid-shaped' microtriches were observed mainly on the surface of anterior portion, whereas those of slender cylindrical 'filamentous form' were distributed mainly on the surface of middle portion of the worm. The tegumental cells, which were often filled with many secretary granules, as well as the flame cell-like structures having characteristic cilia were found in the subtegumental tissue. And possible function of this larval tegument were discussed.
