      • Segmental juvenile xanthogranuloma

        A 14-year-old boy presented with multiple, 0.3 to 0.5 cm-sized, brown nodules along the left side of the waist. The lesions first appeared 6 months ago and have increased in size and number. The clinical differential diagnoses included prurigo nodularis, steatocystoma multiplex, and juvenile xanthogranuloma (JXG). Histopathologic examination showed dense lymphohistiocytic infiltration in the dermis. Touton-type giant cells were also present. The presence of histiocytes were clearly visible with the CD68 stain. From the clinical and histological findings, the diagnosis of segmental JXG was made. JXG is the most common form of the non-langerhans cell histiocytosis. It is a self-limiting disorder which typically occurs during infancy or childhood. JXG usually presents as a solitary, yellow-brown papule and commonly affects the head, neck, and trunk. Atypical variants of JXG include generalized, mixed, plaque, and clustered forms. Among them, segmental JXG is an extremely rare subtype. The segmental distribution of JXG is believed to be attributed to somatic skin cell mosaicism where the mutated cells of the lesion are derived from the bone marrow. Homing and local expansion of the mutated predominant cell species of JXG may lead to segmental distribution. Dermatologists should be aware of the wide morphological spectrum of JXG.

        The Linguistic Context of Relics

        The purpose of this paper is to describe and classify various relics found in English and to account for linguistic motivations behind the existence of relics by looking at their linguistic contexts. Although there are previous studies of relics, most of them focused on either a single particular form or a mere taxonomy of some relics, even the latter being rare. No systematic studies have been done about the linguistic context retaining relics. The examination of phoneme, morpheme, meaning, word, construction, and word order relics shows that compounds, affixation, and idiomatic phrases tend to retain relics. It is argued that the formulaic property of those linguistic expressions motivates the occurrence of a relic, whereby the syntactic and semantic structures of the expressions become so opaque as to be unfeasible for a linguistic change.

        The Beneficial Effect of Korean Red Ginseng Extract on Atopic Dermatitis Patients: An 8 Weeks Open, Noncomparative Clinical Study

        Background: Atopic dermatitis (AD) is a chronic or chronically relapsing, eczematous, severely pruritic inflammatory skin disorder. Korean red ginseng (KRG) has been shown previously to exhibit diverse biological effects including anti-inflammatory and antipruritic effects in a murine model. Objective: We aimed to investigate the beneficial effects of KRG on AD patients, to determine whether there was improvement in disease severity, skin barrier function, pruritus and sleep disturbance relief. Methods: An open, noncomparative clinical study that utilized KRG tablets (500 mg/tablet) was conducted. This study included 41 patients with mild to moderate AD diagnosed by the Korean atopic dermatitis guidelines. Three visits to the hospital at days 1, 28±7, and 56±7 for evaluation were made. The effects of KRG were assessed by measuring eczema area and severity index (EASI) score, transepidermal water loss (TEWL), the visual analogue scale (VAS), total amount of topical agents used in recent 8 weeks and investigator global assessment (IGA). Results: Patients taking KRG tablets showed significant decreases in EASI score and TEWL, and the VAS of pruritus and sleep disturbance were significantly reduced. The amount of topical agents used was reduced but not by a statistically significant amount. IGA at the third visit showed improvement of AD compared to the second visit, but the difference was not statistically significant. Conclusion: KRG can be safely used as a health food to achieve clinical improvement of AD as well as improving overall quality of life, and has potential for further development.       • Eosinophilic fasciitis

        A 52-year-old man presented with diffuse stiffness and immobility of the right forearm for 6 months. Physical examination showed diffuse contraction and induration with linear depressions on the Rt. arm. The MRI scan showed increased signal intensity in the fasciae. Histopathologic findings revealed thickened fascia with dense mononuclear infiltrate. From the findings, the diagnosis of eosinophilic fasciitis was made. The patient showed dramatic improvement after treatment with prednisolone (30mg/day) and methotrexate (12.5mg/week). Eosinophilic fasciitis (EF) is an uncommon connective tissue disease characterized initially by edematous and erythematous extremities, followed by peau d’orange pigmentation and induration. Linear depressions known as the “groove sign” often occur within the thickened skin following the course of the underlying vessels. The differential diagnosis of EF include systemic sclerosis, scleredema, and scleromyxedema. Although the etiology of EF remains unclear, it is known to be associated with vigorous exercise, drugs, Borrelia infection, and various hematological and autoimmune diseases. Laboratory examinations may reveal peripheral eosinophilia, hypergammaglobulinemia, and elevated erythrocyte sedimentation rate (ESR). The gold standard of EF treatment is high-dose corticosteroid. Other effective treatments include hydroxychloroquine and methotrexate. We herein report a rare case of eosinophilic fasciitis.

      • P075 : Beneficial effect on dry skin by using neutral soap

        Background: Patients with xerosis may present rough skin with red spots and cracks and may experience pruritus. Cleaning with cleansers not only removes oils, Soil, dirt, and bacteria of skin but also moisturizes the skin. However, the interaction of cleanser with skin can cause tightness, as well as dryness, irritation and erythema due to it’s pH and damage of skin proteins and lipids. Objectives: The purpose of this study is to find out the beneficial effect on xerosis and the influence on skin barrier impairment after using the neutral soap. Methods: A randomized, comparative clinical trial for effectiveness and recovery of skin barrier function of neutral pH bar soap (Dove®) compared with control bar soap (Ivory®) on the 60 patients with xerosis. The patients were analyzed by PGA (physician``s global assessment of clinical response), clinical grading scores for visible dryness and tactile roughness evaluations (5-point scale), transepidermal water loss (TEWL), and self-examination of itching (10㎝ VAS). Results: PGA of the neutral soap group was improved more than PGA of the control group (Dove; 45.8%, Ivory; 29.2%). Visible skin dryness (5-point scale) was improved in bothstudy groups. TEWL was not significantly different between the natural soap group and the control group, that is, 0.99±0.7 in the natural soap group and 1.6±2.4 in the control group (P> 0.05). Conclusion: Based on these results, the neutral pH of the mild soap are predicted to improve skin barrier function in xerosis patients.

      • Cutaneous pili migrans

        An otherwise healthy 44-year-old man presented with a 4-month history of asymptomatic, slightly elevated serpiginous skin eruption on the chin. On manual compression, the lesion showed a moving tendency. Differential diagnoses include hypertrophic scar and cutaneous pili migrans. Dermoscopic examination showed a linear, dark-blue material embedded within an erythematous background. A punch biopsy was performed from the edge of the lesion. During the procedure, a black, linear material was found and it was easily extracted. Histopathologic examination revealed dense lymphocytic infiltration around the hollow structure where we presume the hair shaft had been placed. A diagnosis of cutaneous pili migrans was made. The lesion completely resolved after the punch biopsy. Cutaneous pili migrans is a rare skin condition in which a hair shaft is embedded in the superficial skin. The eruption is usually asymptomatic and tends to move with pressure. The diagnosis can be made after the extraction of the bare hair shaft through biopsy. The etiology of the cutaneous pili migrans is suggested that the hair shaft found within the epidermis is from an outside source. After the burrowing of the foreign hair, its subsequent movement is likely the result of muscular contraction. It is demonstrated that Asian hair has the largest cross-sectional area and highest tensile strength, which explains the high incidence of cutaneous pili migrans in Asians.

      • Repigmentation in Vitiligo Universalis associated with Chemotherapy

        A 50-year-old woman with vitiligo universalis of 20 years presented with multiple hyperpigmented patches on the face for 5 months. The pigmented lesions developed after starting FOLFOX chemotherapy consisting of oxaliplatin, fluorouracil, and leucovorin for sigmoid colon cancer. Histopathology of the hypopigmented lesion revealed loss of melanin pigment and melanocytes. Histopathology of the pigmented lesion revealed the presence of basal layer melanin and melanocytes, suggesting repigmentation of vitiligo universalis. A month after stopping chemotherapy, the pigmented lesions gradually lightened. Vitiligo is a common depigmenting disorder caused by the loss of melanocytes. Vitiligo universalis is complete or near complete loss of pigmentation of the whole body. Recent studies have shown that in vitiligo, melanocytes may not be totally absent and can be reactivated under appropriate stimulus such as UV radiation and systemic immunosuppressive drugs. In the FOLFOX regimen, fluorouracil can also cause variable hyperpigmentation by means of melanocyte-stimulating hormone or by direct stimulation of melanocytes. We suggest that chemotherapy may have initiated the reactivation of melanocyte activity in the vitiligo lesions. When pigmented lesions occur in vitiligo universalis patients, it is easy to misdiagnose them as pigmented skin disorders such as melasma. Therefore, physicians must perform careful examinations to avoid misdiagnosis.
