      • A Simulation Study to Reduce the Crowdedness of Emergency Department in a Hospital by Short-Stay Unit

        Purpose: Our society is rapidly ageing and there are many related issues which are challenging us. One issue is the over-crowdedness in the Emergency Departments (ED) of many hospitals. The ED is under constant pressure to increase the capacity due to high medical care demands. How to better allocate resources to maintain the medical quality in ED concerns the hospital management. Methodology/Approach: The study focuses on improving the overcrowding emergency department of a case hospital. The hospitalist system proposes Short Stay Unit (SSU) as a measure to improve ED quality. However, to what scale of this SSU and to what degree of resource allocation will be adequate? The process simulation technique is implemented to address this. We build two simulation models: one to represent the current condition and for checking the validity; another to study the impact of SSU. Findings: Overcrowding emergency department deteriorate medical quality and often times sparks on conflicts among patient, the family and the medical staff. This study uses process simulation to evaluate the current patient flows of ED and to provide better route arrangement and resource allocation to reduce patient``s waiting and holding time in ED. The NEDOCS (National ED Over Crowding Study) is as key performance indicator to evaluate the medical quality of ED. Research Implication: The quantitative effects of the hospitalist system can be addressed through simulation analysis with many flexible configurations and can deal with randomness with ease. The results also provide the management better decision aids. Originality/Value: It is an original study and the first of its kind in using process simulation to analyze hospitalist system through process simulation study.
