        일부 조선업 근로자의 직업성 요통 발생실태 및 위험요인에 관한 연구

        목적 : 이 연구에서는 한 사업장의 동일한 코호트내에서 직업성 요통의 발생 및 발생원인에 대해 조사하고자 하였다. 방법 : 연구대상은 1995년 12월 현재 한 조선업체에 종사하는 근로자 중에서 1995년 12월 31일 까지 요통이 이미 발생한 근로자와 1995년 당시 검진 미수검자를 제외한 9,784명을 연구대상으로 하였다. 요통 발생군은 1996년부터 1998년까지 3년간 추적 관찰하여 요통으로 4일 이상 통원 또는 입원치료를 받은 근로자로 하였다. 발생률은 요통발생 당시 연도를 기준으로 하여 해당 연도의 4일 이상 신규치료를 받은 중등도 이상의 요통근로자를 분자로 하였고, 해당년도 사업장의 종사하는 근로자 총수(과거 요통자 제외)를 분모로 간주하였다. 요통 발생군은 외상성 요통과 직업관련성 요통으로 대별하였으며, 위험요인과 질병 발생간의 관련성을 보기 위한 Cox모형에서 그 위험도를 파악하고자 할 때에는 직업성 요통만을 환자군으로 선정하였다. 결과 및 결론 . 요통 발생률은 근로자 천명당 1998년 3.1명, 1997년 8.8명, 1996년 7.8명이었다. 요통점유율의 범위는 10.99∼25.66이었다. 연령별 요통 발생률은 30대, 40대에서 높은 발생률을 보였다. 진단명에 따른 발생률은 추간판 탈출증, 염좌, 척추골절, 척추분리증, 척추협착증 순이었다 직업성 요통 발생자들의 직업적 요인을 알아보기 위하여 요통 발생 당시 작업형태를 알아본 결과, 물건을 들다가 66명(35.3%)이 발생하여 가장 큰 빈도를 차지하였고, 물건을 옮기는 과정에서 19명(10.2%)이 발생하여 2위를 차지하였고, 넘어지거나(8.6%), 물건을 당기는 자세(8.0%), 값자기 일어서다(7.5%) 순으로 요통이 발생하였다. 직업성 요통에 영향을 주는 일반적 특성 및 직업적 특성에 판한 상대위험도 및 95% 신뢰구간를 구한 결과, 요통발생에 영향을 주는 유의한 변수로는 체질량 지수(비만도), 교대여부, 중량물 취급여부였다. Objectives : This study was conducted to estimate the incidence rate, and to identify the risk factors for the occupational low back pain among shipyard workers. Methods : The study subjects consisted of 9,784 workers who were employed in a ship-building industry (excluded workers who had a history of low pack pain before 1995 or did not take periodic health examination in 1995) . The cases were 220 people who experienced back pain from January 1, 1996 to December 31, 1998. To assess risk factors for occupational low back pain, Cox propotional hazard model was used. Results : During the recent three years from 1996 to 1998, the incidence rate per 1,000 persons was 7.8 in 1996, 8.8 in 1997, and 3.1 in 1998. The main causes of work-related low back pawn were sprain, strain and disc herniation. Lifting was the most common cause of back pain (35.3%) , and carrying (10.2%) and pulling (8.0%) were followed. In Cox proportional harzard model, independent risk factors for back pain were body mass index(R.R. ; 1.54, 95% C.1. : 1.06-2.25), shift work(R.R. ; 1.65, 95% C.1. : 1.19-2.28), and lifting heavy material(R. R., 3.95, 95% C.1. : 2.29-6.82). Conclusions . This study suggests that the risk factors of back pawn In shipyard work-ers were body mass index, shift work and lifting.

      • 젖소 足皮膚炎에 대한 오존수의 治療 效果

        This study was carried out to determine therapeutic the effect of ozonated water therapy on bovine pododermatitis. In addition, bactericidal effect of ozonated water on etiological agent of bovine pododermatitis was examined. The pathohistological examination for the pododermatitis, according to application with ozonated water and ozone ointment was investigated. Thirty healthy cattle were divided two groups(each of 15) : control group (povidone group), treatment group (ozone solution group). Various parameters were evaluated in terms of the lameness score, swelling score, lesion score, WBC count, neutrophil count, pathohistological finding, and antimicrobial action. The decrease of lameness and lesion score were shown in hoof lesions on 14 days after application of ozonated water. Significant decrease of swelling was shown in hoof lesions on 14 days 1 Corresponding author after application of ozonated water(p < 0.01). In hematological findings, WBC count revealed values within normal range. The number of neutrophils was slightly higher than that of normal, however, this was improved on 14 days after application of ozonated water. In pathohistological findings, recovery was rapid macroscopically and microscopically in the treatment with ozonated water on the hoof lesions and ozonated water was effective. In antimicobial action, bactericidal effect was observed in treatment with ozonated water on the hoof lesions and ozonated water was effective.

        젖소 말초혈액 림프구로부터 소백혈병 바이러스 배양 및 전자현미경적 관찰

        국내 젖소의 54.2%가 BLV에 감염되어 있지만 현재까지 국내에서는 소백혈병 바이러스 (BLV) 입자를 확인한 연구 보고가 없기 때문에 BLV 항체 양성 소의 말초혈액림프구를 배양, BLV를 발현시켜 전자현미경으로 바이러스 입자를 검출하였고 배양조건에 따른 바이러스 발현율 및 발현 시간을 비교하였다. 검사 결과 전형적인 C-형 바이러스를 확인할 수 있었으며 BLV 단크론 항체를 이용한 면역염색결과 BLV 항원 양성으로 확인되었다. BLV 는 대부분 세포 외부에 분포하고 있었으며 세포질 막에서 생성, 발아되어 나오는 것도 관찰되었다. 전체 바이러스의 크기는 90~100 nm였으며 nucleocapsid는 40~60nm였다. 소태아혈청 (FBS)과 T- 및 B-림프구 분열촉진물질(mitogen)을 각각 첨가하여 배양한 결과 두 군 모두에서 BLV 발현이 확인되었다. Lipopolysaccharide 첨가군은 배양 12시간, Conconavalin A 첨가군은 배양 24시간에 각각 림프구의 10%에서 바이러스가 관찰되었다. 또한 FBS만 첨가한 군과 FBS와 mitogen을 모두 첨가하지 않은 군에서도 관찰되었으나 바이러스의 수는 적었다. 본 연구에서 확립된 BLV 배양 기법을 활용하면 BLV에 감염된 소 중 바이러스를 발현하는 소, 즉 전파능이 있는 개체를 찾아내어 우선적으로 도태할 수 있기 때문에 BLV 감염으로 인한 피해를 막는데 효과적으로 이용될 수 있을 것으로 사료된다. Many studies have been performed on the bovine leukemia virus (BLV) since bovine leukosis had been reported in 1968 in Korea. However, there was no report on the ultrastructural examination of BLV. An attempt to detect C-type viral particles in the cultured peripheral blood lymphocytes of Holstein-Friesian dairy cattle, was made to determine whether in vitro viral expression might be used as a reliable method to identify the cow which is likely to transmit BLV. In transmission electron microscopic (TEM) examination, the virus particles were found predominantly outside of the lymphocytes even though a few particles were also observed within the membrane bound cytoplasmic vacuoles. All of them were C-type particles consisting of a central, electron-dense core separated by a clear area from a limiting envelope with a unit membrane structure. Virus particles were easily detected in the lymphocyte which was cultured with medium supplemented with either T-lymphocyte mitogen (conconavalin A) or B-lymphocyte mitogen (lipopolysaccharide). Identical viral particles, although fewer, were also consistently present in the lymphocytes cultured with medium which was containing foetal bovine serum (FBS) only and which was containing neither FBS or mitogen. By contrast, no virus particle was detected in extensive examination of lymphocytes before culture. In conclusion, the BLV cultivation and detection methods established in this study could be used as a tool to identify and eliminate the cattle which can transmit the BLV.

        Canine juvenile cellulites의 진단과 치료 증례

        Canine Juvenile cellulitis was diagnosed in 2 puppies hospitalized in Veterinary Medical Teaching Hospital of Seoul National University. Characterized dermatological problems were presents of scale, crust, purlent exudation and alopecia limited on their face. On cytologic examinations of direct impression smears for the lesions, numerous neutrophils and macrophagies were observed. No bacteria and fungus were noted. Treatments with administration of predinisolone and antibiotic orally combined with topical shampoo treatment had good results for the two puppies.

      • Brucella abortus 국내 분리주의 Heat Shock Protein 암호 groE 유전자의 염기서열 분석과 발현

        GroE that is a heat shock protein composed of GroEL and GroES is known as an immunodominant target of both the humoral and cellular immune responses in bovine brucellosis. This study was carried out to characterize groE gene encoding heat shock proteins of B. abortus isolated in Korea and to evaluate the immunogenicity of the GroE protein expressed in E. coli system. In PCR the specific signals with the size of 2,077 bp were detected in five strains isolated from the mammary lymphnodes of the dairy cattle that were serologically positive and the reference strains. In comparison of the sequences of nucleotides and amino acids among the strains, GroES showed 100% identity in both sequences. GroEL was evaluated 99.0∼99.9% in nucleotides and 98.0∼100% homology in amino acids. The groE gene including groES and groEL was inserted into pET29a vector and constructed pET29a-GroE recombinant plasmids. The inserted groE was confirmed by digestion with Ncol and EcoRl endonucleases and nucleotide sequencing. E. coli BL (DE3) was transformed with pET29a-GroE, named as E. coli BL (DE3)/pET29a-GroE. In SDS-PAGE, it was evident that the recombinant plasmid effectively expressed the polypeptides for GroES (10 kDa) and GroEL (60 kDa) in 0.5, and 2 hours after IPTG induction. The immuno-reactivity of the expressed proteins were proved in mouse inoculation and Western blot analysis.

      • Expression Profile of Three Splicing Factors in Pleural Cells Based on the Underlying Etiology and Its Clinical Values in Patients with Pleural Effusion

        <P>Splicing factors (SFs) are involved in oncogenesis or immune modulation, the common underlying processes giving rise to pleural effusion (PE). The expression profiles of three SFs (HNRNPA1, SRSF1, and SRSF3) and their clinical values have never been assessed in PE. The three SFs (in pellets of PE) and conventional tumor markers were analyzed using PE samples in patients with PE (<I>N</I> = 336). The sum of higher–molecular weight (Mw) forms of HNRNPA1 (Sum-HMws-HNRNPA1) and SRSF1 (Sum-HMws-SRSF1) and SRSF3 levels were upregulated in malignant PE (MPE) compared to benign PE (BPE); they were highest in cytology-positive MPE, followed by tuberculous PE and parapneumonic PE. Meanwhile, the lowest-Mw HNRNPA1 (LMw-HNRNPA1) and SRSF1 (LMw-SRSF1) levels were not upregulated in MPE. Sum-HMws-HNRNPA1, Sum-HMws-SRSF1, and SRSF3, but neither LMw-HNRNPA1 nor LMw-SRSF1, showed positive correlations with cancer cell percentages in MPE. The detection accuracy for MPE was high in the order of carcinoembryonic antigen (CEA, 85%), Sum-HMws-HNRNPA1 (76%), Sum-HMws-SRSF1 (68%), SRSF3, cytokeratin-19 fragments (CYFRA 21-1), LMw-HNRNPA1, and LMw-SRSF1. Sum-HMws-HNRNPA1 detected more than half of the MPE cases that were undetected by cytology and CEA. Sum-HMws-HNRNPA1, but not other SFs or conventional tumor markers, showed an association with longer overall survival among patients with MPE receiving chemotherapy. Our results demonstrated different levels of the three SFs with their Mw-specific profiles depending on the etiology of PE. We suggest that Sum-HMws-HNRNPA1 is a supplementary diagnostic marker for MPE and a favorable prognostic indicator for patients with MPE receiving chemotherapy.</P>
