        Comparison of Scapular Kinematics During Active Shoulder Horizontal Adduction Between Subjects With and Without Limited Range of Motion of Shoulder Horizontal Adduction

        Background: Shoulder horizontal adduction (HA) is performed in many activities of daily living. The limited range of motion (LROM) of HA is affected by the tightness of the posterior deltoid, infraspinatus, teres major, and posterior capsule of glenohumeral joint. The LROM of shoulder HA contributes to excessive scapular abduction. Objects: The aim of this study is to compare the scapular abduction distance and three dimensional displacement of the scapula during shoulder horizontal adduction between subjects with and without the LROM of shoulder HA. Methods: 24 subjects (12 people in LROM group and 12 people in normal ROM group) participated. Subjects with less than 115° of HA ROM were included in LROM group. Shoulder HA was performed 3 times for measuring scapular abduction distance and three-dimensional displacement of the scapula. Tape measure was used for measuring scapular abduction distance. Scapular abduction distance was normalized by dividing the scapular size. Polhemus Liberty was used for measuring the three dimensional displacement of the scapula. Results: Normalized scapular abduction distance was significantly greater in LROM group than normal ROM group (p<.001). Three-dimensional displacement of the scapula during shoulder HA was greater in LROM group than normal ROM group (p<.05). Conclusion: LROM group had a greater scapular abduction and three-dimensional displacement of the scapula during shoulder HA compared to normal ROM group.

        논문 : 강원도 양양지역 소나무림의 지상부 바이오매스와 수관층 연료특성에 관한 연구

        본 연구의 목적은 강원도 양양지역에 분포하는 소나무림을 대상으로 지상부 바이오매스와 수관층 연료특성에 대하여 분석하고자 하였다. 연구대상지는 강원도 양양군 현남면 하월천리 일대 소나무림으로 선정하였으며, 총 13본의 표본목을 벌채하여 분석하였다. 본 연구 결과에 의하면, 양양지역 소나무림의 줄기밀도(g·cm-3)는 0.347~0.409, 지상부 바이오매스 확장계수는 1.251~1.419의 범위로 나타났다. 지상부 연료량은 총 161.6 Mg·ha-1로 나타났으며, 줄기 126.4 Mg·ha-1, 가지 29.3 Mg·ha-1, 잎 5.9 Mg·ha-1로 나타났다. 수관층 연료특성 인자에 관한 연구에서는 수관화 확산 시 연소될 수 있는 연소가능 연료(잎~1 cm이하 가지)의 비율은 45.2%로 나타났고, 연료량을 수관체적으로 나눈 연소가능 연료밀도(kg·m-3)는 평균 0.178로 나타났다. 본 연구의 결과는 소나무림의 바이오매스 및 탄소 흡수량 추정과 수관화 위험성을 연료적 관점에서 평가하는데 있어 유용한 기초자료가 될 것으로 사료된다. The objective of this study was to analyze the above-ground biomass and crown fuel characteristics of Pinus densiflora stands in Yangyang, Gangwon province. A total of thirteen representative trees were destructively sampled in Yangyang region. The results showed that the stem density (g·cm-3) was 0.347~0.409, whereas the above-ground biomass expansion factors ranged from 1.251~1.419. In terms of crown fuel biomass, the above-ground biomass was 161.6 Mg·ha-1 while the stem biomass, branch biomass and needle biomass were 126.4 Mg·ha-1, 29.3 Mg·ha-1 and 5.9 Mg·ha-1, respectively. Needles and twigs with less than 1 cm diameter accounted 45.2% of the total crown fuel load. The available crown bulk density, which was calculated by dividing the crown fuel load to the crown volume, was 0.178 kg·m-3. The results of this study on the biomass and carbon stocks estimation of the Pinus densiflora together with the crown fire hazard assessment based on crown fuel loads are very significant information for the forest managers.

        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

      • Glyoxal-induced exacerbation of pruritus and dermatitis is associated with <i>staphylococcus aureus</i> colonization in the skin of a rat model of atopic dermatitis

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Atopic dermatitis (AD) is a highly pruritic, chronic inflammatory skin disease associated with hyperreactivity to environmental triggers. Among those, outdoor air pollutants such as particulate matter (PM) have been reported to aggravate pre-existing AD. However, underlying mechanisms of air pollution-induced aggravation of AD have hardly been studied.</P> <P><B>Objective</B></P> <P>To investigate the molecular mechanisms by which glyoxal, a PM-forming organic compound, exacerbates the symptoms of AD induced by neonatal capsaicin treatment.</P> <P><B>Methods</B></P> <P>Naïve and AD rats had been exposed to either fresh air or vaporized glyoxal for 5 weeks (2 h/day and 5 days/week) since one week of age. Pruritus and dermatitis were measured every week. The skin and blood were collected and immunological traits such as Staphylococcus aureus skin colonization, production of antimicrobial peptides and immunoglobulin, and mRNA expression of inflammatory cytokines were analyzed.</P> <P><B>Results</B></P> <P>Exposure to glyoxal aggravated pruritus and dermatitis in AD rats, but did not induce any symptoms in naïve rats. Staphylococcus aureus skin colonization was increased in the skin of both naïve and AD rats. Expression of antimicrobial peptides such as LL-37 and β-defensin-2 was also increased by exposure to glyoxal in the skin of both naïve and AD rats. The mRNA expression of Th1-related cytokines was elevated on exposure to glyoxal. However, serum immunoglobulin production was not significantly changed by exposure to glyoxal.</P> <P><B>Conclusion</B></P> <P>In AD rats, exposure to glyoxal exacerbated pruritus and cutaneous inflammation, which was associated with increased colonization of <I>S. aureus</I> and subsequent immunological alterations in the skin.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Exposure to glyoxal aggravated the symptoms in AD rats, but did not induce AD in naïve rats. </LI> <LI> <I>S. aureus</I> skin colonization and subsequent expression of antimicrobial peptides were increased after exposure to glyoxal. </LI> <LI> Exposure to glyoxal elevated the production of Th1-related cytokines such as TNF-α and IFN-γ in the AD lesional skin. </LI> </UL> </P>

        〈목주잡가〉의 구조적 성격과 내적 의미

        “Mokchu chapka” is a series of Korean sijo poems composed by Hwang Yun-sǒk (1729-1791), a Confucian scholar and poet during the 18th century. This particular poetic work was created when the author was appointed as the magistrate of the district of Mokchǒn, relatively a marginal position in the country, and the poems became an important source to understand the author’s consciousness and emotional experience reflected in the lyrics. Hwang Yunsǒk employed the extended form of sijo, called yǒn-sijo, which enabled him to convey the details of his feelings. However, he detach himself from the convention of ordinary sijo and created a unique form and the content, in which he deals with his loyal attitude toward the ruler on the one hand and expresses his affection for his family on the other. His poems also illuminate his selfhood in reality. In general, Hwang’s poems are characterized by two major subjects: the embodiment of the Confucian ideology and the author’s own individual aspect of life. In other words, Hwang’s poems can be divided between the autobiographic writing and the expression of the Confucian ideology. Particularly, he portrayed the reality of his life as a local official in the autobiographic writing, which captures his private feelings and the condition of life. On the other hand, the poems show that the internalization of the Confucian norms and ideology propelled him to focus on study and self-cultivation. In this context, “Mokchu chapka” has interwoven various elements of Hwang’s consciousness of reality and his feelings. In conclusion, “Mokchu chapka” reflects the literati’s consciousness of an elite man in the local area during the 18th Century and also shows the interiority of the poet. While he served as the magistrate of Mokch’ǒn, he struggled to overcome his frustration with the reality and his poem-writing likewise provided him with a way to overcome his frustration through the realization of the Neo-Confucian ideology in the literary world.

        영구치의 선천적 결손을 동반한 복합 치아종

        치아종은 치아조직의 비정상적인 과성장으로인해 형성되는 외배엽성 상피세포와 중배엽세포로 구성된 혼합종양이다. 양성 치성 종양 중에서 가장 흔하며 신생물이라기보다 과오종으로 보고 있다. 치아종은 악골내 모든 부위에서 발생할 수 있지만 치아와 유사한 복합 치아종은 비교적 상악 전치부에 호발하고,불규칙한 형태를 나타내는 복잡 치아종은 하악 구치부에 호발한다. 일반적인 증상이 없어 일상적인 방사선 검사에서 주로 발견되며 영구치의 맹출지연이나 매복,유치의 만기잔존 등의 원인이 될 수 있지만 영구치의 선천적 결손을 동반한 증례는 매우 드물다 치료는 외과적 제거가 추천되며 완전한 적출시 재발은 드물다. 본 증례는 영구치의 선천적 결손을 동반한 복합 치아종 환자에 대하여 보고하는 바이다. Odontoma is the most common benign odontogenic tumors, and have been defined as mixed odontogenic tumor composed of epithelial and mesenchymal cells. Odontoma is believed to be hamartomatous rather than neoplastic in nature. The classification by WHO divides odontoma into 2 groups such as complex odontoma and compound odontoma. Compound odontoma comprises dental tissues, resembling the morphology of a tooth and has predilection for the anterior maxilla, In contrast, complex odontoma has unorganized mass, not resembling the normal tooth and has predilection for the posterior mandible. Odontoma is almost asymptomatic, so it is usually found on routine radiographic examination. Common presenting symptom is impacted or unerupted permanent teeth and retained primary teeth, but coexistent odontoma and congenital missing of permanent teeth is a very rare condition. The recommended treatment for an odontoma is conservative surgical excision, with care taken to remove the surrounding soft tissue. This report presents 2 patients with compound odontoma of the mandible who have congenital missing of the permanent teeth.

      • 소나무(Pinus densiflora) 목질을 이용한 수용액 중의 Cu(II)흡착

        Milled Korean pine (Pinus densiflora) wood was used to evaluate its adsorption capacity of Cu(Ⅱ) ions from aqueous solution by running a series of batch experiments. Prior to the tests, the milled woods were pretreated with 1N NaOH, 1N HNO_(3), and distilled water, respectively, to examine the effect of pretreatment. Within the tested pH range between 3 and 6, copper adsorption efficiency of NaOH-treated wood(96~99%) was superior to the HNO_(3)-treated wood(19~31%) and distilled water-treated wood(18~35%). The efficiency of copper removal by wood enhanced with increasing solution pH and reached a maximum copper ion uptake at pH 5~6. Adsorption behavior of copper onto both raw and HNO_(3)-treated woods was mainly attributed to interaction with carboxylic acid group. For NaOH-treated wood, carboxylate ion produced by hydrolysis or saponification was a major functional group responsible for Cu sorption. NaOH treatment of wood changed the ester and carboxylic acid groups into carboxylate group, whereas HNO_(3) treatment did not affect the production of functional groups which could bind copper. A pseudo second-order kinetic model fitted well for the sorption of copper ion onto NaOH-treated wood. A batch isotherm test using NaOH-treated wood showed that equilibrium sorption data were better represented by the Langmuir model than the Freundlich model.
