Widespread use of BCG has not controlled tuberculosis, and more effective vaccines are clearly needed. Although chemotherapy will remain the mainstay of antituberculosis treatment, the use of adjunctive immunotherapeutic modalitites is attractive, particularly in persons with drug-resistant tuberculosis. Administration of IL-2 or IFN-γto tuberculosis patients enhance bacillary elimination. Cell-mediated immunity is the critical protective immune response in tuberculosis. Mycobacterial antigens are recognized by T cells and that elicit production of protective cytokines are potentially important vaccine antigens. The 30 kDa antigen is secreted in large quantities by growing mycobacteria. That antigen elicits greater proliferation in lymphocytes from healthy tuberculin reactors than healthy tuberculin nonreactors. In this study, the T lymphocyte proliferative responses to 30 kDa antigen from Mycobactrium tuberculosis H37Rv were examined by using tonsilar and peripheral blood lymphocytes from PPD(+) and PPD(-) tonsilectomized persons. When cultured with 30 kD antigen, tonsilar mononuclear leukocytes and T cells of PPD(+) demonstrated more ^3H-thymidine incorporation than PPD(-) persons (stimulation index was 2.5 and 1.9, 0.8 and 1.0, repectively). Peripheral blood mononuclear cells (PBMC) and peripheral blood T lymphocytes were shown the similar responses to this antigen. The combination of IL-2 and 30 kDa antigen elicited a significant proliferative responsiveness in tonsilar mononuclear leukocytes and T cells of PPD(+) persons (SI was 20 and 14.1). PBMC and peripheral blood T cells of PPD(+) persons were also shown a significant responsiveness, but PPD(-) persons did not show. These results demonstrate that the 30 kDa antigen and IL-2 have a synergistic stimulatory property in mycobacteria sensitizing lymphocytes.
Factors to consider when performing high-intensity focused ultrasound (HIFU) treatment

목적: To introduce factors to consider when performing high-intensity focused ultrasound(HIFU) treatment in symptomatic uterine myomas and adenomyosis. 방법: A total of 26 women with symptomatic myomas and adenomyosis underwent HIFU between September 2015 and April 2016. The inclusion criteria were women with symptomatic myomas sized from 4-10 cm and adenomyosis. The exclusion criteria were pedunculated myomas with narrow stalk, calcified or degenerated myomas, the presence of more than 4 myomas, pregnant women, patients with active pelvic infection, standard contraindications for MRI, known intolerance to MRI contrast agents, and extensive abdominal scars directly anterior to the area of treatment. MRI was performed several days before treatment and 1month after treatment. Scaled signal intensity (SSI) was calculated by T2-weighted MRI before treatment. The non-perfused volume ratio was calculated by T1-weighted MRI after treatment. 결과: A total number of patients was 26, a total number of treated myomas was 42, a total number of treated adenomyosis was 7. Mean sonication time was 2h 46min ± 1h 7min. Pretreatment mean SSI was 18.4±7.2. After 1month of HIFU treatment, non perfused volume ratio of treated myoma and treated adenomyosis was 57.2 ± 18.2%, 25.3 ± 7.6% respectively. 결론: The NPV ratio was mainly influenced by SSI. HIFU can be offered as an alternative nonsurgical option for symptomatic myomas or adenomyosis in selected women.

목적: To evaluate the effectiveness of uterine suspension via mesh anchored to both sides of inferior portion of pubic ramus, combined with levator ani and perineal myorrhaphy as the surgical treatment of pelvic organ prolapse by assessing the surgical outcomes. 방법: This investigation was designed as retrospective analysis. From August 2006 to March 2014, 61 women who underwent uterine suspension via mesh anchored to both sides of inferior portion of pubic ramus, combined with levator ani and perineal myorrhaphy were included. 결과: In 61 women, age (mean ± SD) was 62.3 ± 8.3 years and 88.5 % were postmenopausal. BMI (mean ± SD) was 24.1 ± 2.9 kg/m2. Hypertension and diamellitus were presnt in 47.5% and 16.4%, respectively. The distribution of parity was 8.2 % (parity 1), 29.5 % (parity2), 36.1 % (parity 3), and 26.2 % (parity≥4). The operative data were as follow (median (25% - 75%)); operation time, 115 (105-135) minutes, estimated blood loss, 200 (100-250) mL, hospital stay, 7 (6-9) days, follow-up time, 4.0 (2.0-11.0) months, respectively. Comparison between preand postoperative data were as follow (mean ± SD ``preoperative``, mean ± SD ``postoperative``, p-value of paired T-test); C value, 4.2 ± 1.4cm, -6.7 ± 0.8cm, p<0.05, total vaginal length, 7.3 ± 1.2cm, 9.8 ± 1.3 cm, p<0.05, respectively. 결론: Uterine suspension via mesh anchored to both sides of inferior portion of pubic ramus, combined with levator ani and perineal myorrhaphy is a safe, effective, and minimally invasive method for pelvic organ prolapse.

Objective : To evaluate the clinical outcome of coil embolization for unruptured intracranial aneurysm (UIA) with oculomotor nerve palsy (ONP) compared with surgical clipping. Methods : A total of 19 patients presented with ONP caused by UIAs between Jan 2004 and June 2008. Ten patients underwent coil embolization and nine patients surgical clipping. The following parameters were retrospectively analyzed to evaluate the differences in clinical outcome observed in both coil embolization and surgical clipping : 1) gender, 2) age, 3) location of the aneurysm, 4) duration of the symptom, and 5) degree of ONP. Results : Following treatment, complete symptomatic recovery or partial relief from ONP was observed in 15 patients. Seven of the ten patients were treated by coil embolization, compared to eight of the nine patients treated by surgical clipping (p = 0.582). Patient's gender, age, location of the aneurysm, size of the aneurysm, duration of symptom, and degree of the ONP did not statistically correlate with recovery of symptoms between the two groups. No significant differences were observed in mean improvement time in either group (55 days in coil embolization and 60 days in surgical clipping). Conclusion : This study indicates that no significant differences were observed in the clinical outcome between coil embolization and surgical clipping techniques in the treatment of aneurysms causing ONP. Coil embolization seems to be more feasible and safe treatment modality for the relief and recovery of oculomotor nerve palsy.

<P><B><I>Background:</I></B> The neutrophil-to-lymphocyte ratio (NLR) is an independent prognostic marker in solid and hematological cancers. While the derived NLR (dNLR) was shown to be non-inferior to the NLR in large cohorts of patients with different cancer types, it has not been validated as a prognostic marker for multiple myeloma (MM) to date. <B><I>Methods:</I></B> Between May 22, 2011 and May 29, 2014, 176 patients with MM from 38 centers who were ineligible for autologous stem cell transplantation were analyzed. The dNLR was calculated using complete blood count differential data. The optimal dNLR cut-off value according to receiver operating characteristic analysis of overall survival (OS) was 1.51. All patients were treated with melphalan and prednisone combined with bortezomib. <B><I>Results:</I></B> The complete response rate was lower in the high dNLR group compared to the low dNLR group (7 vs. 26.1%, respectively; <I>p</I> = 0.0148); the corresponding 2-year OS rates were 72.2 and 84.7%, respectively (<I>p</I> = 0.0354). A high dNLR was an independent poor prognostic factor for OS (hazard ratio 2.217, 95% CI 1.015–4.842; <I>p</I> = 0.0458). <B><I>Conclusion:</I></B> The dNLR is a readily available and cheaply obtained parameter in clinical studies, and shows considerable potential as a new prognostic marker for transplantation-ineligible patients with MM.</P>

Soil contamination with heavy metal(loid)s has increasingly become a serious global environmental issue in recent years with considerable efforts made to remediate the contaminated soils. Interest in using biochar as a soil amendment is increasing because of its capacity to adsorb metal(loid)s and organic pollutants. This study investigated the effect of hairy vetch (HV) and poultry manure biochars (PMBC) (2% w/w) at 450 and 700℃ on the availability of arsenic (As) and cadmium (Cd) to earthworms ( Eisenia fetida) in a highly contaminated As soil. The bioavailability of As and Cd to earthworms was determined bymortality and avoidance tests. Biochars reduced the extractable As and Cd by 45-89% in treated soils. Weight loss of worms was observed in all the treatments. The mortality percentage reduced by 70-80% in biochars compared to control. Biochars PMBC 700, and HVBC 450 greatly reduced the metal(loid) accumulation in worm tissues compared to control. In the avoidance test, earthworms preferred the low-temperature biochars compared to the high-temperature soils. The worms partially selected both the treatments when placed between the control and biochars which may be due to presence of metal(loid)s in biochars. The availability of metal(loid)s to earthworms varied between the biochars (feed stock and temperature), therefore, care must be taken during the selection of amendments to remediate contaminated soils.
