        Abstract - Palladium catalyzed cross-coupling reactions of aryl halides with organo tin reagents and π-sufficient aromatic heterocycles proceeded easily and efficiently during carbon-carbon bond formation. We used this method for the anti bacterial agents, and synthesis of new quinolone derivatives which have C-C bond at C-7 position of general quinolone moieties. Quinolone moieties, and aryl tin or aromatic heterocycles in the presence of palladium catalyst were coupled to afford new quinolone derivatives. The synthesis and structure-activity relationship are presented.

        Asymptomatic Gastric Band Erosion Detected during Routine Gastroduodenoscopy

        The incidence of gastric band erosion has decreased to 1%. Gastric band erosion can manifest with various clinical symptoms, although some patients remain asymptomatic. We present a case of a mostly asymptomatic patient who was diagnosed with gastric band erosion during a routine health check-up. A 32-year-old man without any underlying diseases except for non-alcoholic fatty liver underwent laparoscopic adjustable gastric band surgery in 2010. He had no significant complications postoperatively. He underwent routine health check-ups with near-normal gastroduodenoscopic findings through 2014. However, in 2015, routine gastroduodenoscopy showed that the gastric band had eroded into the stomach. His gastric band was removed laparoscopically, and the remaining gastric ulcer perforation was repaired using an omental patch. Due to the early diagnosis, the infection was not serious. The patient was discharged on postoperative day 3 with oral antibiotics. This patient was fortunately diagnosed early by virtue of a routine health check-up; thus, eliminating the possibility of serious complications.

      • S-17 Is Endoscopic Passage an Important Prognostic Factor in Locally Advanced Esophageal SqCC?

        Introduction: Surgery and concurrent chemoradiotherapy (CCRT) are widely accepted alternatives for the curative treatment of patients with early stage or locally advanced esophageal cancer. Impossible endoscopic passage due to resistance is intermittently experienced in esophageal cancer patients. The objective of this study was to evaluate the prognostic influence of endoscopic passage in early stage or locally advanced esophageal squamous cell carcinoma (SqCC) patients treated with surgery or CCRT. Aims & Methods: This retrospective case-control study was based on medical records from a single tertiary medical center located in Daejeon, Republic of Korea. The records of 317 patients with esophageal SqCC treated with surgery or CCRT between January 2009 and December 2015 were reviewed, and 160 patients were ultimately selected. These 160 patients were divided into two groups based on their endoscopic passage findings: group A (possible endoscopic passage group), and group B (impossible endoscopic passage group, including patients requiring a pediatric endoscope for passage due to resistance). We then compared the clinical and endoscopic characteristics of these two groups retrospectively. Results: Of the 160 enrolled patients, 92 (57.5%) patients were assigned to group A and 68 (42.5%) to group B. Early stage esophageal SqCC (stage I, II) was significantly more prevalent in group A than in group B patients (p<0.05), and endoscopic stents were less frequently required in group A than in group B patients (p<0.05). Overall survival was better in group A than in group B patients (85.5% vs 41.4%, p<0.05). Progression-free survival was better in group A than in group B patients (52.3% vs 20.7%, p<0.05). In Subgroup analysis, similar results came out, even if only patients who underwent CCRT (n=92) have analyzed. Of the 92 enrolled patients, 58 (63%) patients were assigned to group A and 34 (37%) to group B. Conclusions: Our data suggest that endoscopic passage was an important prognostic factor in terms of overall survival and progression-free survival in patients with early stage or locally advanced esophageal SqCC treated with surgery or CCRT.

      • Carbamazepine-Induced Hepatotoxicity

        Introduction: Carbamazepine is a potent anticonvulsant agent with proven efficacy in the treatment of partial and tonic-clonic seizures. An epileptic man diagnosed Lennox-Gastaut syndrome with intractable epilepsy, treated with therapeutic dosages of carbamazepine developed severe hepatitis and hepatic insufficiency. He had a positive response to withdrawal of the drug and supportive care with hepatotonics. The liver injury pattern was hepatocellular type, and the modified Roussel Uclaf Causality Assessment Method scale (RUCAM score) was 8, suggesting adverse drug reaction of carbamazepine. Herein, we report a case of hepatotoxicity secondary to carbamezepine use. Case-Report: A 43-year-old South Korean man with Lennox-Gastaut syndrome visited emergency room (ER) because of the general weakness. Blood work revealed acute liver dysfunction with aspartate aminotransferase, 1737U/L; alanine aminotransferase, 1392 U/L; and international normalized ratio, 1.83 The total bilirubin and alkaline phosphatase levels were 5.70 mg/dL, and 95U/L respectively. He had normal baseline laboratory results before increasing the dose of carbamazepine. Hepatitis A immunoglobulin M and hepatitis B surface antigen were negative, and hepatitis C RNA levels were undetectable. Hepatitis E immunoglobulin M, Cytomegalovirus immunoglobulin M, Ebstein-Barr virus viral-capsid antigen immunoglobulin M was also all negative. On the other hand, the hepatitis B surface antibody was positive. Other etiologies, including autoimmune disease, common toxins, drugs, and iron- or copper-induced insult were considered. However, anti-smooth muscle, anti-mitochondrial, and antinuclear antibodies were all negative, and the serum copper, ceruloplasmin, and 24-hour urine copper levels were in the normal ranges. He usually taked carbamazepine 600mg orally every day and increased the dose of carbamazepine to 1200mg orally every day for strict control of the epilepsy, 3 weeks ago before visiting ER. And the TDM level of the carbamazepine surpassed the normal limit. He did not report any use of alcohol or illicit drugs. The RUCAM score was 8. These findings strongly suggested drug-induced liver injury. Abdomino-Pelvic CT was performed, and it revealed that there was no biliary structure problem. As a result, carbamazepine was immediately discontinued and the patient was managed with supportive care via hepatotonics. He showed improvement of the clinical and laboratory abnormalities, with aspartate aminotransferase and alanine aminotransferase levels of 30 and 8, respectively, after 8 weeks. (Figure) Conclusion: This case report describes a 43-year-old man with epilepsy who experienced liver injury after carbamezepine administration. Usually, the mechanism of the carbamezepine-induced hepatotoxicity is explained by idiosyncratic reaction, irrelevant with dose-dependent effect. However, our case was related with the dose of the carbamazepine. Therefore, our case emphasizes that liver function tests and the TDM level of the carbamazepine should be monitored periodically after administration of carbamazepine.

      • Employee Health Behaviors, Self-Reported Health Status, and Association With Absenteeism: Comparison With the General Population

        <P>Objectives:To perform a comparison between health behaviors and health status of employees with those of the general population, to evaluate the association between employee health behaviors, health status, and absenteeism.Methods:Cross-sectional study enrolled 2433 employees from 16 Korean companies in 2014, and recruited 1000 general population randomly in 2012. The distribution of employee health behaviors, health status, and association with absenteeism were assessed.Results:Employees had significantly worse health status and low rates of health behaviors maintenance compared with the general population. Multiple logistic regression model revealed that regular exercise, smoking cessation, work life balance, proactive living, religious practice, and good physical health status were associated with lower absenteeism.Conclusions:Maintaining health behaviors and having good health status were associated with less absenteeism. This study suggests investment of multidimensional health approach in workplace health and wellness (WHW) programs.</P>

        가토 모델에서 복강경 수술 시 전기방사 나노섬유 유착방지제의 효과 및 안전성

        Purpose: Most recently developed anti-adhesive membranes are not suitable for laparoscopic surgery due to weak mechanical properties or adhesive characteristics. To overcome these problems, we prepared electrospun bioabsorbable nanofibrous poly (lactic-co-glycolic acid)-based membranes as an adhesion barrier. We evaluated the efficacy and safety of this material for laparoscopic surgery in a rabbit model. Methods: A standardized laparoscopic surgical trauma was made on the rabbit’s uterine horn and adjacent abdominal wall to induce adhesion formation. The injured uterus was covered by a nanofibrous barrier or it was left untreated (the negative control group) (each group: n=14). To evaluate acute toxicity of this material, blood sampling was made 3 and 7 days after laparoscopic surgery to check liver and renal function. Three weeks after laparoscopy, a second look laparoscopy was performed and the adhesions were scored according to Blauer"s scoring system. Tissue between abdominal wall and uterus was obtained to examine microscopically. Liver, kidney and uterus were harvested to examine chronic toxicity. Results: 36.4% of the nanofiber treatment group and 70% of the untreated control group showed severe adhesions (grade>3) after laparoscopic surgery but failed to get a statistical significance (P=0.198). Acute and chronic toxicity induced by this material were not noted in the blood and tissue exam. Conclusion: This study showed that nanofiber barrier seems to be a novel resorbable biomaterial for the reduction of postoperative adhesions. Easy placement and handling of this material make these membranes potentially successful candidates for laparoscopic surgery. But further study is needed to get a statistical significance.

        The identification and characterization of xenoantigenic nonhuman carbohydrate sequences in membrane proteins from porcine kidney

        <P>The immunogenic nonhuman carbohydrate sequences in membrane proteins from porcine kidney were identified and characterized using MALDI-TOF MS and ESI-QTOF-MS. The MALDI profile, investigated by incubation with exoglycosidases, showed a series of about 40 carbohydrates that were identified as high mannose glycans (Man<SUB>3–9</SUB>GlcNAc<SUB>2</SUB>) and complex bi-, tri-, and tetra-antennary glycans with and without core fucose. The antennae of many of the complex glycans were terminated with α-galactose residues, with the numbers of these residues ranging from one up to the number of antennae. Negative ion ESI-MS/MS spectra confirmed the location of the α-galactose residues on the ends of the antennae. This total glycan profile of the membrane proteins from porcine kidney will thus provide important information for the study of molecular interactions between antigenic carbohydrates and proteins in xenotransplantation.</P>
