Background: Presently, only few tests have been published for evaluating the efficacy of skin cleansers, and they are neither standardized nor widely-accepted. Objectives: We developed a new in-vivo test method for the semi-quantitative evaluation of the efficacy of cleansing products and evaluated its validity and reliability. Methods: Five different skin cleansers were tested on the forearms of 25 healthy volunteers according to the washing procedure using standard dirt. We evaluated the residual amount of standard dirt on the cyanoacrylate skin surface stripping sample using a dermoscopy and an image analysis program. We also graded the cleansing power through a pattern analysis of dermoscopic images. For the comparison regarding the validity, we obtained data according to other preexisting methods using a Chromameter, Mexameter and Glossymeter. Results: The cleansing efficacy of five skin cleansers which was assessed by our new method showed significant correlations with the results from other preexisting methods using a Chromameter and Mexameter (Pearson correlation coefficient 0.826 and 0.738, respectively, P<0.001). This new method also showed a high internal consistency reliability (Cronbach`s alpha=0.997). Conclusion: We presented a new in-vivo method for assessing the cleansing efficacy of skin cleansers which was shown to be reliable and valid. Further studies are required to establish a standardized method to objectify cleansing effectiveness of skin cleansers.

Background: Microneedle fractional radiofrequency (RF) induces deep dermal heating and leaves the epidermis less affected, which showed therapeutic effects for wrinkle, pore and various scars. Sublative fractional RF induces not only superficial fractional ablation of upper epidermis but also upper dermal remodeling, which had been reported to show improvement of skin aging. Objectives: We evaluated efficacy and safety of combined microneedle fractional RF and sublative fractional RF for acne scars in Asian skin. Methods: 20 patients (mean age 23.65±2.94, skin phototype III-IV) received three consecutive combined microneedle fractional RF and sublative fractional RF at 4-week intervals over 12 weeks. Investigator`s global assessment and complications were assessed up to 2 months after last treatment by standardized photographs. Subjective improvement and pain score were also assessed. Results: Follow-up results revealed that 4 patients had clinical improvement of 76% to 100%, 10 had improvements of 51% to 75%, 6 had improvements of 26% to 50%. The mean duration of post-therapy crusting or scaling was 5.21 days and post-therapy erythema lasted 2.52 days. Conclusion: We suggest that combined microneedle fractional RF and sublative fractional RF may provide a new treatment algorithm for acne scars in Asians. The limitations are the small number of patients.

Background: Postherpetic neuralgia (PHN) is the most common complication of herpes zoster (HZ). Recently, there was an open label pilot study which showed that gabapentin administered in acute herpes zoster was effective in reducing the incidence of PHN. However, high-dose gabapentin may cause adverse effects in older patients. Objectives: A prospective study was performed to evaluate the efficacy of treatment with low-dose gabapentin plus valacyclovir for the management of acute pain and the prevention of PHN in patients with acute HZ. Methods: From March, 2012 to July, 2013, 64 patients (age, ≥50 years) who presented with HZ within 3 days of rash onset with moderate to severe pain (≥4 on the 10-point Likert scale) were recruited. The patients received 1000 mg of valacyclovir 3 times a day for 7 days plus gabapentin at an initial dose of 300mg/d, titrated up to a maximum of 900 mg/d. The severity of pain, quality-of-life measures (determined by the Dermatology Life Quality Index, DLQI), and adverse events were assessed at recruitment and 1, 3, 5, 8, and 12 weeks later. Results: Among the total of 64 patients (mean age, 63.4 years), the overall incidence of zoster pain (≥4 on the Likert scale) at 3 months was 3.13%. The DLQI improved significantly at 3 months and patients experienced minimal adverse events from the medication. Conclusion: The combination of low-dose gabapentin and valacyclovir administered acutely in patients with HZ is well-tolerated and reduces the incidence of PHN.

Background: The use of growth factors in skin rejuvenation is emerging as a novel anti-aging treatment. While the role of growth factors in wound healing is well established, their use in skin rejuvenation has only recently been to be studied and no controlled trials have been performed. Objective: We evaluated the anti-aging effects of secretory factors of endothelial precursor cells differentiated from human embryonic stem cells (hESC-EPC) in Asian skin. Methods: A total of 25 women were included in this randomized, controlled split-face study. The right and left sides of each participant’s face were randomly allocated to hESC-EPC conditioned medium (CM) or saline. To enhance epidermal penetration, a 0.25-mm microneedle roller was used. Five treatment sessions were repeated at 2-week intervals. Results: Physician’s global assessment of pigmentation and wrinkles after treatment revealed statistically significant effects of microneedling plus hESC-EPC CM compared to microneedling alone (p<0.05). Skin measurements by Mexameter and Visiometer also revealed statistically significant effects of microneedling plus hESC-EPC CM on both pigmentation and wrinkles (p<0.05). The only minimal adverse event was mild desquamation in one participant. Conclusion: Secretory factors of hESC-EPC improve the signs of skin aging and could be a potential option for skin rejuvenation. (Ann Dermatol 26(5) 584∼591, 2014)
