        진달래 꽃에 의한 Grayanotoxin 중독 3 례

        Traditionally, the Rhododendron species has been used in gastrointestinal disorder or hypertension. Grayanotoxin exists in honey, flowers, pollen, and the nectar of the Rhododenron species. We experienced 3 cases of Grayanotoxin intoxication. The symptoms of intoxication were nausea, vomiting, hypotension, bradycardia, diplopia, dizziness, and chest discomfort. Generally, the treatment for Grayanotoxin intoxication is fluid resuscitation and injection of atropine sulfate. The patients who were intoxicated with Grayanotoxin were discharged without complication after supportive care.

        응급센터에 내원한 비-안와-뇌 털곰팡이증(Rhino-Orbital-Cerebral Mucormycosis)환자 2례

        Rhino-orbital-cerebral mucormycosis is a very rare, but mostly fatal infection and is usually found in diabetic or immunocompromised hostss. It has a characteristic rapid progression, so we pay close attention to patients who have symptoms associated with rhino-orbital-cerebral mucormycosis. The symptoms due to rhino-orbital-cerebral mucormycosis are fever, ulceration or necrosis in nasal cavity, edema of the periorbital area or face, decreased visual acuity, headache, decreased mentality, etc. Because of the rapid progression and high mortality rate, close attention is needed in patients who complained such symptoms and signs. We experienced two cases of rhino-orbital-cerebral mucormycosis which had a poor prognosis, and we report them here.

      • Inhibiting Invasion into Human Bladder Carcinoma 5637 Cells with Diallyl Trisulfide by Inhibiting Matrix Metalloproteinase Activities and Tightening Tight Junctions

        <P>Diallyl trisulfide (DATS), an organosulfur compound in garlic, possesses pronounced anti-cancer potential. However, the anti-invasive mechanism of this compound in human bladder carcinoma is not fully understood. In this study, we evaluated the anti-invasive effects of DATS on a human bladder carcinoma (5637) cell line and investigated the underlying mechanism. The results indicated that DATS suppressed migration and invasion of 5637 cells by reducing the activities and expression of matrix metalloproteinase (MMP)-2 and MMP-9 at both the protein and mRNA levels. DATS treatment up-regulated expression of tissue inhibitor of metalloproteinase (TIMP)-1 and TIMP-2 in 5637 cells. The inhibitory effects of DATS on invasiveness were associated with an increase in transepithelial electrical resistance and repression of the levels of claudin family members. Although further studies are needed, our data demonstrate that DATS exhibits anti-invasive effects in 5637 cells by down-regulating the activity of tight junctions and MMPs. DATS may have future utility in clinical applications for treating bladder cancer.</P>

        Anti-invasive effects of decitabine, a DNA methyltransferase inhibitor, through tightening of tight junctions and inhibition of matrix metalloproteinase activities in AGS human gastric carcinoma cells

        <P>The DNA methyltransferase inhibitor decitabine, 5-Aza-2'-deoxycytidine, possesses anti-metabolic and anticancer activities in various cancer cells. However, the biochemical mechanisms underlying decitabine-induced inhibition of invasiveness and metastasis have not been thoroughly studied. In this study, we investigated the effect of decitabine on the correlation between tightening of tight junctions (TJs) and anti-invasive activity in AGS human gastric cancer cells. Our data indicated that the inhibitory effects of decitabine on cell motility and invasiveness were associated with increased tightness of the TJ, which was demonstrated by an increase in transepithelial electrical resistance (TER). Immunoblotting results indicated that decitabine repressed the levels of the claudin proteins, major components of TJs that play a key role in the control and selectivity of paracellular transport. Furthermore, matrix metalloproteinase (MMP)-2 and -9 activity in the AGS cells was dose-dependently inhibited by treatment with decitabine, and this was correlated with a decrease in mRNA and protein expression. In addition, these effects were related to inactivation of the phosphoinositide 3-kinase (PI3K)/Akt pathway in AGS cells. In conclusion, this study suggests that TJs and MMPs are critical targets of decitabine-induced inhibition of invasiveness in AGS human gastric cancer cells.</P>

      • 저혈압환자의 손가락 간이혈당검사 결과에 대한 평가

        Purpose: Due to its rapidity and easy accessibility, the fingerstick blood glucometer has been used in almost all hospitals and private clinics, and even by patients themselves. We also have used it even in shock patient care, but shock shows global tissue hypoperfusion, especially in peripheral tissue. The changes of peripheral circulation have an influence on the results for fingerstick glucose. To evaluate the accuracy of the glucometer for patients with poor peripheral perfusion, we designed this study. Methods: A prospective, nonrandomized comparison group study was done. A hypotensive group and a normotensive group were compared. We obtained three data from each patient; venous blood glucose level (clinicopathologic laboratory), venous blood glucose level (by glucometer) and fingerstick glucose level (by glucometer). Results: We saw a significant difference between the fingerstick glucometer results and the laboratory glucose levels in hypotensive patients: 131.67±55.33 mg/dl vs. 147.23±62.06 mg/dl (paired t-test, p<0.05). There was no significant difference between fingerstick and laboratory glucose in normotensive patients; 101.75±20.14 mg/dl vs. 105.60±21.95 mg/dl (paired t-test, p>0.05). There was no significant difference between the results of venous glucometer and laboratory test in either group; 142.37±61.27 mg/dl vs. 147.23±62.06 mg/dl (paired t-test, p>0.05) and 102.98±17.02 mg/dl vs, 105.60±21.95 mg/dl (paired t-test, p>0.05). Although some statistical differences existed between the results, all of the error rates were in an acceptable range (within 15%, accepted by American Diabetes Association consensus). Conclusion: These results suggest that the blood glucose level of the glucometer with venous blood is more accurate than that with peripheral blood in patients with poor peripheral circulation.

        <P>The DNA methyltransferase inhibitor decitabine, 5-aza-2'-deoxycytidine, has been found to exert anti-metabolic and anticancer activities when tested against various cultured cancer cells. Furthermore, decitabine has been found to play critical roles in cell cycle arrest and apoptosis in various cancer cell lines; however, these roles are not well understood. In this study, we investigated decitabine for its potential anti-proliferative and apoptotic effects in human leukemia cell lines U937 and HL60. Our results indicated that treatment with decitabine resulted in significantly inhibited cell growth in a concentration- and time-dependent manner by the induction of apoptosis. Decitabine-induced apoptosis in U937 and HL60 cells was correlated with the downregulation of anti-apoptotic Bcl-2, XIAP, cIAP-1 and cIAP-2 protein levels, the cleavage of Bid proteins, the activation of caspases and the collapse of mitochondrial membrane potential (MMP). However, apoptosis induced by decitabine was attenuated by caspase inhibitors, indicating an important role for caspases in decitabine responses. The data further demonstrated that decitabine increased intracellular reactive oxygen species (ROS) generation. Moreover, N-acetyl-L-cysteine, a widely used ROS scavenger, effectively blocked the decitabine-induced apoptotic effects via inhibition of ROS production and MMP collapse. These observations clearly indicate that decitabine-induced ROS in human leukemia cells are key mediators of MMP collapse, which leads to apoptosis induction followed by caspase activation.</P>

        분류 기법을 이용한 방광암 재발 예측

        방광암은 소변에 저장하는 방광에 생기는 악성종양으로, 연령이 증가함에 따라 발생빈도가 점차 증가하게 된다. 본 연구에서는 기존에 잘 알려진 몇 가지 특징 기법과 분류 방법을 이용하여 방광암 재발 예측을 위한 다양한 예측 모형을 생성하였다. 그 다음으로 예측 분류 모형에 대한 분류 정확도를 측정하여 비교·분석함으로서 방광암 재발 위험을 예측하는 데 가장 적합한 모형을 선별하였다. 실험 결과, 특징 선택 기법은 Minimum Redundancy Maximum Relevance (MRMR)이 Conditional Mutual Information Maximization (CMIM)보다 상대적으로 더 높은 정확도를 보였으며, 특히 데이터 객체들로부터 가장 영향을 미치는 10개의 특징을 선택하여 베이지안 네트워크 모형에 적용하였을 때 예측 정확도가 가장 높게 나타났다. 이 연구를 통해 암 재발 위험을 정확히 예측함으로서 향후의 의료진들이 환자의 암 예방 및 예후를 추정하는 데 기여할 수 있을 것으로 기대된다. Bladder cancer is a malignant disease that occurs in the urinary bladder. It is the most common type of cancer after prostate cancer in the whole world. Bladder cancer can occur at any age, but it is more common among older people than the younger ones. In this paper, we built several classification models to predict the recurrence of bladder cancer with various well-known feature selection and classification techniques. In our experiment, we selected the most suitable data mining model to predict the risk of bladder cancer recurrence using several classification algorithms, such as artificial neural network, bayesian network and support vector machine. Our experiments proved that minimum redundancy maximum relevance criterion is better than conditional mutual information maximization, especially, the highest classification accuracy is obtained by bayesian network model, which contains only 10 important features. Therefore, we derived a conclusion that bayesian network model is the most appropriate for predicting the risk of bladder cancer recurrence, when comparing with other classification methods. We expect to give help in preventing cancer and estimating prognosis by discovering useful knowledge that is extracted from the model.

        모 자동차 도장공정에서 사용한 신너의 성분과 공기중 노출에 관한 연구 : Ethylene Glycol Ethers류를 중심으로

        This study was conducted to identify components of thinners and to evaluate worker´s exposure to them in an auto manufacturing plant. Main components including Ethylene Glycol Ether were analyzed quantitatively as well as qualitatively. More than 18 components were analyzed qualitatively in 12 thinner samples. Toluene was the most frequently detected, and its amount ranged from 8.5% to 62.5%. 11 of 12 thinner samples have EGEE, EGEEA, and EGBE named Ethylene Glycol Ether. 23.7% of EGEE was included in C sample and 3.3 % in G sample. EGEEA was detected to 7.3% in D sample, and lower than 2.6 % in other 6 samples. Amount of EGBE was as much as 25.9% in G sample, 19.6% in E sample, 14.6% in A sample, and 11.3% in C sample. In addition to these components, a little amount of several components such as ketone, alchol, ether, and ester were also detected. But all thinner samples have not included benzene. Mixing room shows the highest concentration. Of several material included in thinner, worker´s exposure to toluene was 12.9 ppm in mixing room, and 3.3 ppm in third room which were higher than other components. Ethylene Glycol Ether detected in air was only EGEEA. Frequency of EGEEA detected in the air was as much as 90% in the mixing room and 40% in the third room. Worker´s average exposure to EGEEA was 2.4 ppm lower than TLV(5 ppm). In order to evaluate worker´s exposure in process using thinner, air sample to analyze toluene, xylene, benzene, Ethylene Glycol Ether, MIBK must be taken

        Clinical Characteristics of Acute Aortic Syndrome in Korean Patients: From the Korean Multi-Center Registry of Acute Aortic Syndrome

        Background and Objectives: Acute aortic syndrome (AAS) is a heterogeneous group of disorders that often present with severe chest or back pain. It includes acute aortic dissection (AD), intramural hematoma (IMH), dissecting aneurysm, and penetrating aortic ulcer (PAU). The clinical picture of AAS and its prognosis have not been studied in a large number of Korean patients. Therefore, we organized a multi-cen-ter registry to identify the clinical characteristics and treatment patterns, as well as long-term outcomes in Korean patients with AAS. Subjects and Methods: Five-hundred twenty-eight patients, who had been diagnosed with AAS, were enrolled into this registry from 10centers. On a retrospective basis, we collected demographic, laboratory, imaging data, as well as follow-up clinical outcomes by reviewing medical records from individual centers. All the data were collected in core lab and analyzed in detail. Results: The mean patient age was 60.1±14.5 years; the male-to-female ratio was M : F=297 : 231. The prevalent risk factors for AAS in-cluded hypertension (361, 68.4%) and diabetes (52, 11.1%). The components of AAS that are included in this study are acute AD (446, 84.5%),IMH (57, 10.7%), and PAU (11, 2.1%). By type of AAS, patients diagnosed with Stanford A were 45.6% of enrolled patients, whereas those with Stanford B were 54.4% of enrolled patients. Among nearly half of the patients were treated with medicine (55.7%) alone, whereas 40.0%underwent surgery and 4.3% underwent endovascular treatment. Overall, the in-hospital event rate was 21.2% and the in-hospital death rate was 8.1%. The mean follow-up duration was 42.8 months and there showed 22.9% of total event and 10.1% of death during this period. Conclusion: By organizing a multi-center registry of AAS, we could identify the characteristics of AAS in real-world Korean patients. Further,prospective study is warranted with a larger number of patients.
