        Malignant peritoneal mesothelioma is a rare neoplasm that arises from the mesothelium of a serosal cavity and is a rapidly fatal disease with a median survival of 4 to 12 months for untreated cases. Recently, we experienced a case with malignant peritoneal mesothelioma who was suspected hepatocelluar carcioma by abdominal CT scan and was confirmed by biopsy including immunohistochemical stain(calretinin) after surgery. We performed tumor excisions and wedge resection of the liver(segment Ⅷ)and inserted Tencoff catheter in abdominal cavity at 25th day of post-operation. We treated with intraperitoneal paclitaxel(25mg/m^(2)/day for 5 days) six courses monthly. She was well tolerable and is still living without any evidence of recurrence for 14th month of post-operation.

        Disruption of rsmA Gene of Pectobacterium carotovorum subsp. carotovorum LY34 and Effect on Pathogenicity

        The rsmA gene was cloned from soft-rot bacterium Pectobacterium carotovorum subsp. carotovorum LY34 (Pcc LY34), and its role in pathogenicity was investigated by marker exchange mutagenesis. From a cosmid library of Pcc LY34 genomic DNA, a positive clone carrying the rsmA gene was selected, and the gene was cloned by polymerase chain reaction (PCR) amplification. The gene is 186 bp in size and encodes a protein of 62 amino acids with a predicted molecular mass of 6,839 Da. The calculated pI of the RsmA is 8.16. The phylogenetic tree showed that the RsmA of Pcc LY34 appeared genetically identical to the CsrA of Pectobacterium atrosepticum SCRI1043 (100% identity) and similar to the CsrA of Yersinia pestis KIM10+(98.3%). The gene was disrupted by the $Km^r$ gene, and the cells became mutated (i.e., $RsmA^-$ mutant). The pathogenicity test revealed that the disease rating of the $RsmA^-$ mutant only differed slightly from that of the wild type on a slice of potato tuber and a Chinese cabbage stalk. These results suggest that RsmA is not an essential factor for the pathogenicity of Pcc LY34 and that the rsmA gene of Pcc LY34 is not completely derepressed in the $RsmA^-$ mutant for virulence-related genes, contrary to the results of Erwinia carotovora subsp. carotovora $RsmA^-$ mutant, which proved hypervirulent for celery petioles. These results showed that the microenvironmental conditions of the host and/or strain of pathogen are important for the coordination of virulence gene expression.

      • 저체중 출생이 학동기 및 청소년기 소아 천식의 중증도에 미치는 영향에 대한 고찰

        목 적 : 본 연구는 저출생 체중아로 출생한 소아에 있어서 학동기 및 청소년기 소아 천식의 중증도에 미치는 영향에 관해서 후향적으로 검토하였다. 방 법 : 대동병원 소아과와 호흡기내과에 통원 중에 있거나 부산 동래구 및 연제구 소재 개인의원에서 대동병원으로 전원 된 천식 환자 중에서 8세 이상 18세 이하의 소아 및 청소년을 대상으로 출생체중을 기준으로 1,500 g 이상 2,500 g 미만인 15명을 추출하여 연구군으로 하고 출생체중 2,500 g 이상이며 연구군의 나이에 일치하는 순서대로 2명씩 추출하여 이 30명을 대조군으로 하였다. 결 과 : 양군을 비교한 결과 천식 중증도는 연구군에서 유의하게 높았다.(P=0.015) 또 흡입 스테로이드 약의 사용 빈도(증례군 13명/15명, 대조군 4명/30명 : P=0.0001)와 사용 기간에 있어서도 연구군에서 유의하게 높게 나왔다.(P= 0.009) 혈중 총 IgE치와 아토피피부염의 유병률은 대조군에서 높았다.(각각 P=0.007, P=0.0001) 폐기능 검사와 PC(20), 특이 IgE 항체는 두 군 사이에 유의한 결과를 볼 수가 없었다.(P>0.05) 결 론 : 천식의 중증도와 흡입 스테로이드 사용의 빈도와 기간 등으로 미루어 볼 때 저출생 체중은 소아 천식의 중증도를 증가시키는 요인 일 수 있음을 시사한다. Purpose : Over the past several decades, the survival rates of low birth weight(LBW; birth weight <2,500 g) infants have been improving. Many of these infants have had bronchial asthma(BA) at childhood and aldolescence. The aim of this study is to investigate whether LBW influenced the severity of BA in prealdolescent children and aldolescence by a retrospective case control study. Methods : 15 LBW children(1,500 g<birth weight<2,500 g) between 8 year-old and 18 year-old with BA visiting Dae-dong hospital as the case group were compared with 2 BA children to each case as the control group(total 30 children) on the basis of following criteria : the same age, he/she attended Dae-Dong hospital or local medical center after 8 years old, birth weight was more than 2,500 g. Asthma severity was modified from 2003 Guideline for management of childhood asthma at The Korean Academy of Pediatric Allergy and Respiratory Disease. PFT(Pulmonary function test) were measured using a portable spirometer. The value are expressed as percentage predicted using standard reference values. Total IgE and specific IgE Ab. were assessed by the PRIST method and MAST Allergy Test. Methacholine challenge test and PC20 was assessed by the Masterlab. Results : LBW group had : a significantly more severe symptoms between 8 year-old and 18 year-old than control group.(P=0.015) A significantly more used to ICSs(Inhaled corticosteroids) and add-on therapy than control group.(P=0.009)(P=0.0001) There were no significant differences in the PFT and PC(20) and specific IgE Ab. between LBW group and control group. But, total IgE level is higher in the control group than in the LBW group. Conclusion : These results suggest that LBW is a risk factor for the severity of BA. We suggest that infants with birth weight of less than 2,500 g have a more severe risk of asthma during prealdolescent children and aldolescence than who were birth weight of more than 2,500 g.         교과내용학 : 동학에 나타난 고유사상 연구

        이 논문은 동학(東學)의 사상적 기저(基底)가 유교, 불교, 도교가 아닌 고유사상(固有思想)에 있음을 고찰한 글이다. 유·불·도(儒·佛·道)가 삼국시대에 전래된 이후 도교는 민간신앙(民間信仰)과 혼합되어 그 정체성이 뚜렷하지 않지만, 유교와 불교는 전통사상(傳統思想)으로 자리매김 하였다. 동학은 우리 민족의 종교이자 학문사상이기 때문에, 우리의 문화적 전통과 역사적 맥락에서 탐구되어야 한다. 그러나 유·불·도는 동학의 본질이 될 수 없다. 유교의 경우는 최제우(崔濟愚, 1824-1864)가 비록 조선의 선비로 자랐지만, 주유팔로(周遊八路)의 과정에서 유교의 한계를 직시하고 그러한 전통을 거부한다. 불교는 최제우로 하여금 유교적 지식인의 삶에서 깨달음의 종교적 삶으로 전환시키는데 기여하지만 동학과는 직접적인 관련성이 적다. 도교는 동학의 이상(理想)을 효율적으로 드러내기 위해 당시 보편적으로 쓰이던 도교 관련 어휘들을 원용(援用)한 것이다. 우리민족의 고유사상은 단군신화(檀君神話)와 이를 계승한 신라의 풍류도(風流道)에서 그 근원과 특징을 엿볼 수 있는데, 『동경대전(東經大全)』과 『용담유사(龍潭遺詞)』의 고유사상적 내용을 검토하면 다음과 같은 결론이 도출됨을 개진하였다. 첫째, 동학의 경전에 제시된 ‘천황씨(天皇氏)’는 중국의 신화적 인물이 아니라 단군신화의 ‘환웅(桓雄)’이다. 둘째, 최제우가 실천목표로 삼은 ‘다시개벽’은 환웅의 개벽사상을 계승한 것이다. 셋째, 동학의 주문수련(呪文修鍊)과 검결(劍訣)은 고대로부터 우리 민족이 중시한 가무(歌舞)의 전통을 계승한 것이다. 넷째, 동학은 풍류도의 포함삼교(包含三敎)의 논리 및 접화군생(接化群生)과 친연성을 가진다. This study aims at researching that the ideological foundation of Donghak is not Confucianism, Buddhism and Taoism (Yubuldo), but the indigenous Korean thought. Since the introduction of Yubuldo into Korea during the Three Kingdom period, Confucianism and Buddhism have settled down as traditional Korean thoughts, while the identity of Taoism, mixed with folk-beliefs, was not clear. Since Donghak is Korean people’s religion and academic thoughts, it should be explored in the Korean cultural tradition and historical context. However, Yubuldo cannot be the essence of Donghak. It can be found that our indigenous thought has the origin in the Myth of Dangun and the Pungrudo which inherited it and also holds the features related with them. After reviewing the contents of the indigenous thoughts in Dong-Kyung Dae-jun and Yongdam-yusa, the conclusion I reached is as follows. First, ‘Cheonhwang-ssi’ presented in Donghak Scripture is not a mythical Chinese character but ‘Hwanung’ in the myth of Dangun. Second, ‘Dasigaebyeok the recreation of the world’ at which Choi Je-u aimed in practice succeeded Hwanung’s Gaebyeok thoughts. Third, Jumunsuryeon and Geomga in Donghak succeeded the traditions of song and dance to which Korean people attached importance since ancient times. Fourth, Donghak has an affinity with Pungryudo’s logic of Pohamsamgyo - It includes all three religions, Yubuldo, and Jeobhwagunsaeng - It aims to contact, reform and save all living beings.

        레이저와 2차원 배열의 광전검출기를 이용한 구조물의 변위측정 시스템의 개발에 관한 연구

        일반적으로 구조물은 외부의 정적 및 동적 하중과 외부환경으로 인하여 피로균열과 부식이 발생하며, 이것은 구조물의 변형을 유발하여 결국 파단으로 이어지기 때문에 균열과 부식의 검출 및 평가와 함께 구조물의 변형에 의한 진동, 변위 기울기와 같은 거동을 감시하는 것도 매우 중요하다. 이에 레이저 센서 시스템을 이용하여 이러한 거동을 측정하여 이상 유무를 모니터링할 수 있는 구조물 안전감시 시스템을 개발하였다. 본 시스템은 2차원으로 배열한 광전센서를 이용하여 구조물의 변형에 의해 유발된 광궤적의 변화를 감시하며 또한 데이터를 취득하고 신호처리 할 수 있는 운용 프로그램도 갖추고 있다. 본 연구에서는 개발한 안전 진단 시스템의 필드 적용에 앞서 실험실에서의 몇 가지 실험을 통하여 그 효용성을 검증하였다. A Safety Monitoring System using a laser and 2-D arrayed photo sensors is developed. To monitor of the deformation and small rotation of structure the developed optical system using 2-D photo sensor array was used to detect the variation of optical orbit of laser which was induced by deformation of the structure. Also, an operating program to manage the system and an algorithm for the data acquisition and the database are introduced. In this study, we demonstrated the capabilities of this system by laboratory experiments before applying the system to the field.

      • 위암환자의 복강내에 투여한 Mitomycin C-Carbon Particle의 Mitomycin 용출에 관한 연구

        Locoregional recurrence is the most common type of recurrence in surgical operation of gastric adenocarcinoma, and peritoneal dissemination is one of the most difficult problems in advanced gastric adenocarcinoma treatment. Because the peritoneal cavity is the most common site of the first recurrence after gastric cancer resection, intraperitoneal chemotherpy seems a logical choice for cancer chemotherapy. The Mitomycin C(MMC) adsorbed by the activated charcoal particles(CH) is relatively released when the drug concentration surrounding the carbon particles becomes low in the peritoneum of the peritoneal cavity. For the intraperitoneal chemotherapy on the advanced gastric adenocarcnoma, mitomycin C adsorbed on activated carbon particles was administered in the peritoneal cavity just before abdominal wall closure. The closed drainage tubes were inserted in the peritoneal cavity and clamped for tuo hours after completion of operation. MMC concentrations were serially measured in peritoneal fluid, plasma and urine at 2hour, 48 hour, 72 hour and 168 hour following its administration in order to study the efficacy of the MMC-CH as a drug delivery system. There were minimal toxicities in born marrow, liver, and gastrointestinal system after intraperitoneal MMC-CH administration. The data of this study suggested that MMC-CH may have a somewhat more beneficial effect than surgery alone when administered in optimal dose and schedules, but the MMC concentration of the peritoneal fluid was not sufficient to eradicate remnant cancer cells, and effective duration of maintenance was only below 24 hours in the peritoneal fluid and plasma.

        Protective effect of Korean Red Ginseng against FK506-induced damage in LLC-PK1 cells

        Background: Compound FK506 is an immunosuppressant agent that is frequently used to prevent rejection of solid organs upon transplant. However, nephrotoxicity due to apoptosis and inflammatory response mediated by FK506 limit its usefulness. In this study, the protective effect of Korean Red Ginseng (KRG) against FK506-induced damage in LLC-PK1 pig kidney epithelial cells was investigated. Methods: LLC-PK1 cells were exposed to FK506 with KRG and cell viability was measured. Western blotting and RT-PCR analyses evaluated protein expression of MAPKs, caspase-3, and KIM-1. TLR-4 gene expression was assessed. Caspase-3 activities were also determined. The number of apoptotic cells was measured using an image-based cytometric assay. Results: The reduction in LLC-PK1 cell viability by $60{\mu}M$ FK506 was recovered by KRG cotreatment in a dose-dependent manner. The phosphorylation of p38, p44/42 MAPKs (ERK), KIM-1, cleaved caspase-3, and TLR-4 mRNA expression was increased markedly in LLC-PK1 cells treated with $60{\mu}M$ FK506. However, with the exception of p-ERK, elevated levels of p-p38, KIM-1, cleaved caspase-3, and TLR-4 mRNA expression were significantly decreased after cotreatment with KRG. Activity level of caspase-3 was also attenuated by KRG cotreatment. Moreover, image-based cytometric assay showed that apoptotic cell death was increased by $60{\mu}M$ FK506 treatment, whereas it was decreased after cotreatment with KRG. Conclusion: Taken together, these results suggest that the molecular mechanism of KRG in the FK506-induced nephrotoxicity may lead to the development of an adjuvant for the inhibition of adverse effect FK506 in the kidney.

      • 음낭외상시 고환파열에 대한 초음파 검사의 진단적 유용성

        Purpose: We determined the use of scrotal ultrasonography in the initial diagnosis and management of testicular injuries due to blunt scrotal trauma. Mateiials and Methods: We reviewed 54 patients of scrotal trauma in the last 8 years. Forty patients were evaluated by ultrasonography preoperatively. A heterogeneous echo pattern of the testicular parenchyma with loss of oval shape, contour definition was the basis for diagnosis of testicular rupture. Among them, 33 cases underwent scrotal exploration and 7 cases were managed conservatively. The remaining 14 patients were evaluated only by physical examination, 8 patients of them were explored immediately because of suspicious testicular mpture. The accuracy of ultrasonography were determined by comparing radiographic interpretations with intraoperative findings. Results: Among the 33 patients who were evaluated by ultrasonography and explored immediately, there were 26 suspected testicular ruptures on ultrasonography. Surgical exploration revealed testicular rupture in 23 and simple hematoma in 10. The testicular rupture was correctly diagnosed by ultrasonography in the 22 cases, and there were 4 false-positive and 1 false-negative diagnoses of testicular rupture. The sensivity and specificity for the diagnosis of testicular rupture are 96% and 60%, and the positive and negative predictive values are 85% and 86%, respectively, Conclusions: Scrotal ultrasonography, with the main radiographic finding of a heterogeneous echo pattern of the testicular parenchyma with loss of oval shape, contour definition, is highly sensitive on the diagnosis of testicular rupture. And it increases the salvage rate for testicular rupture with preservation of the testicular function and prevents delayed complications of chronic pain, atrophy and orchiectomy associated with missed testicular rupture.

      • 施肥條件이 사탕무의 生育과 Betacyanin 含量에 미치는 影響

        사탕무의 生育과 betacyanin 含量의 變化에 대한 施肥條件의 影響을 알아보기 위하여 室素 0, 10, 15 및 20㎏/10a와 加里 0, 10, 15 및 20㎏/10a 處理하여 試驗한 結果는 다음과 같다. 草長과 葉數는 室素 15㎏/10a 處理區까지는 增加되었으나, 20㎏/10a 處理區에서는 오히려 減少되었으며, 根直徑과 根長도 室素 施用量의 增加에 따른 有意的인 差異가 認定되었다. 그리고 根重은 無處理區에 비해 室素 處理區에서 增加되었으나, 室素 處理區間에는 큰 差異를 나타내지 않았다. Betacyanin 含量은 室素 15㎏/10a와 加里 20㎏/10a 處理區에서 0.126%로 가장 높았고, 生育段階別로는 파종후 110日에 가장 높은 含量을 나타내었다. These experiments were carried out to investigate the effect of nitrogen and potassium application on growth and betacyanin content of sugar beet. The experiments were respectively treated with 0, 10, 15 or 20㎏/10a nitrogen and 0, 10, 15 or 20㎏/10a of postassium, and the results were summarized as fellows. Plant height and number of leaves were increased by 15㎏/10a of nitrogen, otherwise; it was decreased by 20㎏/10a of nitrogen. There were significant differences root diameter and root length by increament of nitrogen application. Root weight was progressively increased by nitrogen treatments compared to the control, although there was no significant differences among the amount of nitrogen treatments. Betacyanin content was high 0.126% in the plot treated with 15㎏/10a of nitrogen and 20㎏/10a of potassium respectively. For the growth stage the corresponding value showed the highest level at the 110th day after seeding.
