      • How Should We Assign Large Infiltrative Hepatocellular Carcinomas for Staging?

        Aims: Infiltrative gross morphology of hepatocellular carcinoma (HCC) is known to be associated with poor prognosis. To date, there has been no study that fully addressed the potential role of tumor morphology in staging HCC, although it requires further clarification. Therefore, we analyzed the prognostic impact of the infiltrative type HCC by evaluating patients who underwent liver resection for HCC, and attempted to clarify how to assign this HCC subtype in the current staging systems to increase their discriminatory ability. Methods: A total of 774 HCC patients who underwent curative liver resection were retrospectively reviewed and the prognostic significance of infiltrative type HCC was assessed using the Barcelona Clinic Liver Cancer (BCLC) and American Joint Committee on Cancer (AJCC) staging systems. The infiltrative type HCC is defined as a mass with foci varying in size which fuse to form a larger foci without a distinct margin or a mass with a permeative appearance which blends into the background of the cirrhotic liver with an indistinct margin. The cumulative incidence of OS was determined according to the AJCC T-stage and BCLC staging system and the impact of the infiltrative type HCC on each staging system was evaluated using Kaplan-Meier plots (log-rank test), censoring the patients who were lost to follow-up. The Akaike information criterion (AIC) and concordance index (c-index) were calculated to compare the prognostic powers of each staging systems. Results: Seventy-four patients (9.6%) had infiltrative HCCs with a higher proportion of multifocal tumors, larger tumors, vessel invasion, increased tumor marker levels, and advanced T-stages than those with nodular HCC (all, P<0.01). Infiltrative morphology was independently associated with lower overall survival (OS), but its impact was significant when the tumor size was ≥4cm (P<0.001). Under current AJCC and BCLC staging criteria, these large infiltrative HCCs were associated with significantly worse OS in early AJCC T-stages (T1b/T2, P<0.001) and BCLC stage A/B (P=0.01) but not in advanced AJCC (T3/T4) and BCLC C (Fig 1 & 2). The reassignment of this subtype to T3 and T4 increased the discriminatory ability of AJCC T-staging with lower AIC values (3086.9 and 3084 vs. 3103.6) and higher c-index (0.69 and 0.69 vs. 0.67), respectively (both, P<0.05) (Table 1). For BCLC staging sequential reassignment of large infiltrative HCC from BCLC A to BCLC B and from BCLC B to BCLC C also improved the prognostic performance. Conclusions: Large infiltrative type HCC should be assigned to the advanced stages beyond T1 or T2 of the AJCC staging or beyond BCLC stage A or B. We recommend assuming the large unifocal infiltrative type HCCs on surgical specimen as tumors with multiple foci and reassign them from AJCC-T1 and T2 to AJCC-T3, or assuming all large infiltrative HCCs staged AJCC-T1 to T3 as those with macrovascular invasion and reassigning them to AJCC-T4. Second, for BCLC staging, we recommend any large unifocal-looking infiltrative type HCCs staged BCLC-A on imaging studies to be reassigned to BCLC-B while definitely multifocal HCCs initially staged BCLC-B to BCLC-C. This enable finer stratification of HCC patients and provide more accurate prognostic competence.

        Adenomatoid odontogenic tumor (AOT) is a rare, benign odontogenic tumor that predominantly appears in the second decade of life in female patients. Most AOTs occur in the anterior part of the maxilla and are usually associated with impacted anterior teeth. There are three types of AOT, follicular, extrafollicular, and peripheral, which are classified based on the location of the lesion and its association with the impacted tooth. We report a rare case of AOT associated with an impacted right mandibular lateral incisor in an 11-year-old female patient.

      • Predictors of HCC Recurrence beyond MILAN Criteria after Primary Liver Resection

        Aims: There is a controversy whether to undergo liver resection or transplantation at early stage HCC. Thus, we aimed to find factors that reliably predict HCC recurrence beyond MILAN criteria which could help prioritizing transplantation as initial therapy. Methods: This is a retrospective study including patients who were diagnosed with HCC within MILAN criteria and had liver resection as initial treatment from year 2002 to 2016. In total, 152 patients were enrolled for analysis. The primary endpoint of this study is to find factors that enable prediction of HCC recurrence beyond MILAN criteria at the time of first recurrence. Secondary endpoints were factors associated with recurrence and recurrence free survival. Cox regression model was employed to select factors associated with recurrence. Results: Out of 152 patients, 56 patients developed HCC recurrence. Out of 56 patients 12 patients (21.5%) showed HCC recurrence beyond MILAN criteria at the initial time of recurrence. Time to recur was significantly shorter in patients out of MILAN criteria compared to those recur within MILAN (14±10 vs 28±23 months, P=0.005). HCC recurrence was independently associated with increased baseline tumor size, ALT levels (>35 IU/L) and degree of AFP decrease after resection (all, P<0.05). Factors associated with HCC recurrence beyond MILAN criteria were greater tumor number, higher child pugh score, and increased baseline serum ALT levels (all, P<0.05). Lastly, independent factors associated with recurrence free survival rate were recurrence beyond MILAN criteria, remote recurrence from resection site and degree of AFP decrease (all, P<0.05). Conclusions: HCC recurrence pattern (within versus beyond MILAN) has significant impact in the recurrence free survival even when patients are cured at early stage. Therefore, to prolong survival, prediction of patients who would lead to HCC recurrence beyond MILAN criteria is important and early liver transplantation could be an alternative treatment in this group.

        高壓 酸素 療法이 複合 租織 移植에 미치는 影響

        Even thought the composite graft has many advantage, its clinical use has been limited survival. Early vascularization from the recipient site is essential for the survival of the composite graft. We grafted 15mm-sized composite tissue on the rabbit ear. And the rabbit was treated with hyperaric oxygen or calcium antagonist. The survival area, survival rate, gross and histologic change were studied using the biopsy specimen which was taken on 2nd, 5th, 7th, and 14th postoperative day. The result are as follows; 1. The survival rate of hyperbaric oxygen treated group(89.7%) was significantly higher than that of the control group(65.3%).(p<0.001) 2. The survival rate of the calcium antagonist treated group(67.5%) was not significantly higher than the control group(65.3%).(P>0.1) 3. The color of the graf was monitored. At postoperative 24-48 hours, the color of the graft truned into pink-tined tone, and than changed into purple color at postoperative 4th-5th day. The color of the graft returned to nomal pinkish hue at 7th-10th postoperative day. We observed the same color change in each groups of survived composite graft. 4. The histologic finding of the hyperbaric oxygen treated group was compaired to those of the control or calcium antagonist treated group. The hyperbaric oxygen treated group showed less degeneration of epithelial cells and chondrocytes as well as less interstitial edema, but it showed increased chondroblasts and chondrocyte layers. In conclusion, we noticed that the hyperbaric oxygen treatment significantly increases the survival of the composite graft.

        Oral bacteria were isolated and identified from teeth, gums and tongues of patients who were visited at Dentistry of Yeungnam University Medical Center and the effect of chemical substances an the growth of these bacteria was investigated. In result, each isolates was identified as Bacillus pumilus, Bacillus coagulans and Bacillus subtilis var. globigii. It was showed that the MIC of sodium chloride against all the tested strains was M. These oral bacteria were sensitive to antibiotics such as ampicillin, kanamycin and penicillin, however, these organisms have a strong tolerance to the ripampicin. It was confirmed that the growth of oral bacteria was not restrained by use of 1.0% of sodium propionate and sorbic acid.

        This study was conducted to compare the antioxidant activity and properties of domestic and imported beers using malts made from domestic and imported organic barleys. The total polyphenol content in domestic beer and imported beer extracts were at 19.58 mg GAE/g and 18.83 mg GAE/g, respectively, and the flavonoid contents were approximately at 20.47 mg CE/g and 18.24 mg CE/g, respectively, which are higher than imported beer. Additionally, in domestic beer of DPPH and ABTS radical scavenging activity were higher than imported beer. The viscosity of domestic and Imported beers were determined at 6.30 cP and 7.40 cP respectively, indicating that domestic beer has a lower viscosity. The contents of acetic acid in both Domestic and Imported beers, were approximately 707 ppm and 701.67 ppm, respectively. Domestic and imported beers have been evaluated 4.6 pH and 4.5 pH value, respectively. The sample color of the domestic and imported beer have the degree of chromaticity at 23 European Brewery Convention(EBC). Therefore, domestic beer made from domestic organic barley can be used as a functional food more effectively compared with imported beer.

        The effect of oriental medicines an the growth of oral bacteria(B. pumilus JU4, B. coagulans JU6, B. subtilis var globigii JU7 and B. subtilis var. globigii JU9) which were isolated from teeth, gums and tongues of patients was investigated. lt was shown that the water extracts of Glycyrrhiza glabra L. var. glandulifera Regel et Zucc., Schizandrae Fructus and Coptidis Rhizoma have the strong antimicrobial activity against every test organism. Especially, the water extract of Schizandrae Fructus has the most antimicrobial activity against all test bacteria of which MIC were the same as 0.25%. The growth of oral bacteria B. pumilus JU4 was inhibited by addition of the water extracts of Glycyrrhizae Radix, Schizandrae Fructus and Coptidis Rhizoma in the concentration of 0.9%, 0.2% and 0.1% respectively. It was tested that specific growth rates of B. pumilus JU4 in each concentration of the water extracts of Glycyrrhizae Radix, Schizandrae Fructus and Coptidis Rhizoma. From the results, the test showed that mean of specific growth rates of B. pumilus JU4 decreased 102%, 186% and 210% in 3% concentration of the water extracts of Glycyrrhizae Radix, Schizandrae Fructus and Coptidis Rhizoma more than those in control respectively.

        산업재해에 의한 수부손상의 임상적 고찰 (Ⅱ)

        Industrial hand injury causes not only loss of hand function but also many socio-economic problem, because they are low socio-economic class, age of productivity and unemployed status due to functional disability of hand. Mainly, inexpert workers suffer from industrial hand injury (62%) probably due to absence of safe guard, safe-education and occupational training. Therefore, industrial hand injury may be diminished by preventive measure such as adequate safeguard, safe-education and occupational training. Statistical analysis was carried out retrospectively on 495 patients with industrial hand injury at Dept. of plastic and Reconstructive Surgery, Korea University College of Medicine, KURO hospital, from September 1986 to December 1988, and following items are studied. 1. Constitutions of industrial compartments and labors in Kuro and Youngdeungpo region. 2. Educational status of the patients. 3. Area distribution. 4. Monthly distribution. 5. Day of week distribution. 6. Onset time distribution. 7. Duration of occupation in the patients. 8. Causative factor. 9. Underlying factor. 10. Physical treatment and its duration. 11. postoperative grasping power. 12. Aesthetic result. We want this paper attribute to reduction of industrial hand injury.

        방사형 3차원 동탄성 무한요소

        본 연구에서는 적층지반상의 지반-구조물 상호작용 해석을 위한 새로운 3차원 무한요소를 제안하였다. 이 무한요소는 Cartesian 좌표계에서 정식화되었으며, 수평, 수직 그리고 모서리 무한요소로서 구성된다. 무한요소의 정식화를 위하여 유도한 변위형상함수는 무한 원역으로 방사되는 다중파동성분을 묘사하는 복소지수함수와 유한요소와 결합되는 단면의 거동을 나타내는 다항함수로 이루어졌다. 이 연구에서 제안하는 무한요소의 성능을 검증하기 위하여 주파수영역에서 여러 가지 예제해석을 수행하였다. 균질 및 적층지반상 정형 및 임의 형상의 강체기초와 묻힌 케이슨 기초의 무차원 동적 거동을 구하였으며, 다른 연구자들의 결과와 비교 검토하였다. This paper presents new three dimensional radial infinite elements for soil-structure-interaction problem in a multi-layered half-space. Three kinds of radial infinite elements, including horizontal, vertical and comer infinite elements, are developed in Cartesian coordinates using approximate wave propagation functions with multiple wave components. Numerical example analyses are presented for rigid arbitrarily shaped footings, and an embedded caisson on a homogeneous and layered half-space in frequency domain. The numerical results obtained show the effectiveness of the proposed infinite elements.
