        혈소판 풍부혈장의 임상적 응용

        Platelet-rich plasma is an autologous source of platelet-derived growth factor and transforming growth factor beta that is obtained by sequestering and concentrating platelets by gradient density centrifugation. We have used platelet-rich plasma for bone graft, especially allobone graft, at implant surgery, sinus lift procedure, and cyst enucleation. This article is retrospective study from October 1999 to November 2000. All cases were good healing and clinical success.

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

      • A Phase 1 Study Using Autologous Natural Killer Cells in Patients with HAIC-Hepatocellular Carcinoma

        Aims: Natural killer (NK) cell-based immunotherapy has recently been tried with advances of understanding the role of immune defense against hepatocellular carcinoma (HCC). To improve NK cells therapy, we focused to increasing delivery of NK cells and synergic effect combined with hepatic arterial infusion chemotherapy (HAIC). Methods: We did a prospective, open label, phase 1 trial of the safety and efficacy of autologous NK cells through hepatic arterial infusion as sequential therapy after HAIC in advanced HCC patients. Between March 2016 and July 2017, 11 patients were included who showed favorable response more than stable disease (SD) after 2 sessions of HAIC in advanced HCC patients with child A. A total 4 sessions of HAIC were performed the protocols of infusion of cisplatin (25/m2) and 5-fluorouracil (750/m<sup>2</sup>) for 4 days every 3-4 weeks interval. The peripheral blood mononuclear cells of patients by leukapheresis were ob-tained after 3<sup>rd</sup> HAIC and NK cells were expanded for 2 weeks under Current Good Manufacturing Practices (cGMP). Patients received planned dosage of NK cells through chemoport into hepatic artery for 5 days after 4<sup>th</sup> HAIC (3 patients; 2.5x108, 3 patients; 5x10<sup>8</sup>, 5 patients; injection of 10x10<sup>8</sup> NK cells). The primary end point was safety of NK cell injection; secondary endpoint included objective response rate (modified Response Evaluation Criteria In Solid Tumors), time to progression, dura-tion of response and immunologic efficacy. Results: Any adverse events of NK cells injection were none according to dosage. An objective response was observed in 7 patients (63.6%) included three complete responses and four partial responses. Stable disease was observed in 2 patients and progressive disease was in 2 patients and thus disease control rate was 81.8%. The mean duration of time to progression was 9.7±5.3 month and duration of response without chemotherapy was 6.1±5.2 month. The newly metastatic lesion was occurred in 3 patents (27.2%; lymph node 1 patients, Lung 2 patients). Two patients were died by tumor progression and others were still alive. The increasing immunologic response was observed in 5 patients (55 %) to evaluate cytotoxicity and NK cell proportion of peripheral mononuclear cells after NK cell injection. Conclusions: The HAIC and NK cells immunotherapy is safe and effective treatment in the advance HCC patient with favorable liver function. The additional studies are urgently required to establish the new novel treatment.

        Peptoniphilus mikwangii sp. nov., Isolated from a Clinical Specimen of Human Origin

        <P>A novel Gram-positive, anaerobic, and coccus-shaped bacterium, designated as strain ChDC B134(T), was isolated from a human postoperative infectious lesion in the right maxilla between the lateral incisor and canine and was characterized by polyphasic taxonomic analysis. 16S rRNA gene sequence analysis revealed that the strain ChDC B134(T) belonged to the genus Peptoniphilus, as it showed sequence similarities to Peptoniphilus indolicus KCTC 15023(T) (94.0 %) and Peptoniphilus asaccharolyticus KCTC 3321(T) (93.8 %). The prevalent fatty acids of of strain ChDC B134(T) were C16:0 (20.3 %), C18:1 cis 9 (34.3 %), and C18:0 (13.2 %). The DNA G+C content was 30.9 mol%. The cell wall diamino acid was D-ornithine, which is a property shared by other reference type strains of the genus Peptoniphilus. Based on the results of phenotypic, chemotaxonomic, and phylogenetic analysis, strain ChDC B134(T) (=KCOM 1628(T) = KCTC 15227(T) = JCM 30223(T)) should be classified as the type strain of a novel species of genus Peptoniphilus, for which the name Peptoniphilus mikwangii sp. nov. is proposed.</P>

        비대칭적 경골 치환물을 사용한 슬관절 전치환술의 임상적, 방사선학적 결과

        Purpose : Purpose of this study was to evaluate the clinical and radiological results and complications of total knee arthroplasty(TKA) using the systems including an asymmetric tibial tray and a deep trochlear-grooved femoral component. Materials and Methods: Ninety-five total knee replacement arthroplasties(68 patients) with the implant system, which has the asymmetric tibial tray and a deep trochlear grooved femoral component were analyzed clinically and radiologically. Retrograde analysis with the Knee Society clinical rating system, roentgenographic evaluation and scoring system and the Hospital for Special Surgery(HSS) score were done. The patients were average 66 years old and the follow-up period was 23month(range, 12 month to 49 month) Results : Knee score was improved from 43 points to 90 points. Function score was improved from 47 points to 85 points. HSS score was improved 57 points to 89 points. Tibio-femoral angle was improved from varus 4o to valgus 5o. In last clinic visit, there was no case which had progressive radiolucent line or above 4 points in radiolucent score except one septic loosening case. There was no case of patellofemoral complications, including patellofemoral instability, patellar fracture, patellar component failure, patellar component loosening, and patellar clunk syndrome. Conclusion: The TKA in this series showed excellent clinical and radiological results and low incidence of patellar complication.

        Case Report : Lambert-Eaton myasthenic syndrome as a cause of persistent neuromuscular weakness after a mediastinoscopic biopsy -A case report-

        There are many causes of prolonged postoperative muscle weakness, including drugs, residual anesthetics, cerebrovascular events, electrolyte imbalance, hypothermia, and neuromuscular disease. Neuromuscular diseases are relatively rare, with the most common being myasthenia gravis and Lambert-Eaton myasthenic syndrome (LEMS). We report an unusual case in which a patient who was given a muscle relaxant during mediastinoscopy developed postoperative muscle weakness that was ultimately diagnosed as secondary to LEMS.       • 심장 이식 2예 보고

        97년 9월 이후 2예의 동소 심장이식수술을 시행하였는데 대상 환자들은 남녀 각 1명, 나이는 여자는 55세, 남자는 23세이고, 체표면적은 각각 1.54m2, 1.62m2였다. 대상환자들은 확장성 심근증과 제한성 심근증 환자로 NYHA IV 정도의 심한 심부전증 증상이 있었다. 수술 후 면역억제제로는 사이클로스포린, 아자티오프린, 스테로이드를 사용하였다. 첫 번째 환자에서는 수술 10일 후 심근조직검사에서 ISHLT 기준의 I 단계의 거부반응을 보였고, 이후의 조직검사에서는 거부반응이 없었으며 술후 2개월째에 대상포진, 8개월째에는 구강 칸디다증으로 입원 치료를 받았다. 두 번째 학자에서는 술후 2일째 사이클로스포린에 의한 신독증을 보였으나, 순조롭게 회복되어 퇴원하였다. Since September 1997, we have performed two cardiac transplantations in one dilated cardiomyopathy and one restrictive cardiomyopathy. The patients were a 55-year-old female and a 23-year-old male, and their body surface area(m2) were 1.51 and 1.62 respectively. Both patients were suffered from dyspnea as NYHA functional class IV. Postoperative immunosuppression was accomplished with cyclosporine, azathioprine and predinisolone. In case 1, there was an ISHLT grade IA rejection in endomyocardial biopsy at postoperative 9th day which disappeared at the tome of next biopsy 20 days later. The patients were treated for herpes zoster and oral candidiasis at postoperaive 2 months and 8 months, respectively. In case 2, nephrotoxicity related to cyclosporine developed 2 days after the surgery, but was soon resolved and the patient was discharged in good condition.

        Fabrication of Low Carbon Steel Coated with 18%Cr-2.5%Ni-Fe Powder by Laser Cladding and Its Application on Plastic Injection Mold for Aluminum Diecasting

        Laser cladding a surface treatment process that grants superior characteristics such as toughness, hardness, and corrosion resistance to the surface, and rebuilds cracked molds; as such, it can be a strong tool to prolong service life of mold steel. Furthermore, compared with the other similar coating processes - thermal spray, etc., laser cladding provides superior bonding strength and precision coating on a local area. In this study, surface characteristics are studied after laser cladding of low carbon steel using 18%Cr-2.5%Ni-Fe powder (Rockit404), known for its high hardness and excellent corrosion resistance. A diode laser with wavelength of 900-1070 nm is adopted as laser source under argon atmosphere; electrical power for the laser cladding process is 5, 6, and 10 kW. Fundamental surface characteristics such as crossectional microstructure and hardness profile are observed and measured, and special evaluation, such as a soldering test with molten ALDC12 alloy, is conducted to investigate the corrosion resistance characteristics. As a result of the die-soldering test by immersion of low carbon alloy steel in ALDC12 molten metal, the clad layer's soldering thickness decreases.

      • Highly Repeatable and Recoverable Phototransistors Based on Multifunctional Channels of Photoactive CdS, Fast Charge Transporting ZnO, and Chemically Durable Al<sub>2</sub>O<sub>3</sub> Layers

        <P>Highly repeatable and recoverable phototransistors were explored using a 'multifunctional channels' structure with multistacked chalcogenide and oxide semiconductors. These devices were made of (i) photoactive CdS (with a visible band gap), (ii) fast charge transporting ZnO (with a high field-effect mobility), and (iii) a protection layer of Al2O3 (with high chemical durability). The CdS TFT without the Al2O3 protection layer did not show a transfer curve due to the chemical damage that occurred on the ZnO layer during the chemical bath deposition (CBD) process used for CdS deposition. Alternatively, compared to CdS photo transistors with long recovery time and high hysteresis (del V-th = 19.5 V), our 'multi-functional channels' phototransistors showed an extremely low hysteresis loop (del V-th = 0.5V) and superior photosensitivity with repeatable high photoresponsivity (52.9 A/W at 400 nm). These improvements are likely caused by the physical isolation of the sensing region and charge transport region by the insertion of the ultrathin Al2O3 layer. This approach successfully addresses some of the existing problems in CdS phototransistors, such as the high gate-interface trap site density and high absorption of molecular oxygen, which originate from the polycrystalline CdS.</P>
