치료 후 임상적으로 무병상태인 자궁경부암 환자에서 FDG-PET scan의 임상적 유용성에 관한 연구

목적: 자궁경부암 환자에서 재발의 조기 진단방법으로 FDG-PET scan의 임상적 유용성을 알아보기 위하여 수술 후 혹은 방사선치료 후 임상적으로 무병상태인 자궁경부암 환자에서 FDG-PET scan을 시행하여 재발의 조기 진단에 있어 FDG-PET scan의 임상적 유용성을 알아보고자 하였다. 연구방법: 자궁경부암으로 원자력 병원에서 수술 후 혹은 방사선치료 후 임상적 무병 상태로 추적 검사 중인 환자 중 전신 FDG-PET scan을 시행한 101명을 대상으로 이학적 검사, 종양표지자, 컴퓨터 단층촬영 등의 기존의 추적 관찰방법과 FDG-PET scan의 영상소견을 비교 분석하였다. 전신 FDG-PET scan의 촬영은 GE advance scanner를 이용하였고, 370-555MBq(10-15mCi)의 FDG를 주사한 후 50분 후부터 촬영을 시작하였으며, 전신 영상을 얻은 후 20분간 국소 영상을 얻었다. 국소 관심 영역의 표준 섭취 계수를 구하여 값이 3.0 이상이거나 주변조직에 비해 높은 섭취율을 보인 경우에 양성으로 판정하였으며 CT, MRI 및 가능한 경우에 경피적 천자술을 시행하여 재발여부를 확인하였다. 결과: PET scan에서 양성소견을 보인 17명(16.8%)중 8명(47.1%)에서 추후 임상적 소견 및 컴퓨터 단층촬영에서 재발이 확인되었으며 이중 2명에서 조직학적으로 재발이 확인되었다. PET scan에서 양성소견을 보인 부위는 종격동 림프절에서 5예, 폐실질에서 5예, 골반림프절에서 4예, 중앙병소에서 3예, 쇄골하 림프절에서 2예로 종격동 림프절과 폐실질에서 가장 많았다. 무병상태로 추적관찰중인 자궁경부암 환자에서 조기재발을 진단하는데 있어 PET scan의 민감도는 100%였으며, 특이도는 90.3%이었다. 양성 예측율(positive predictive value, PPV)은 47.1%(8/17), 음성 예측율(negative predictive value, NPV)은 100%(84/84), 위양성율(false positive rate)은 52.9%(9/17)였다. 결론: FDG-PET scan은 기존의 방사선 검사나 종양 표지자로서 발견하지 못한 전이 또는 재발을 임상적으로 무병상태인 환자의 7.9%(8/101)에서 발견 할 수 있었다. FDG-PET scan은 높은 위양성율에도 불구하고 전신 영상을 함께 얻을 수 있으며 작은병소 혹은 여러 부위에서 전이 또는 재발한 경우 진단이 쉽고 예상치 못한 부위의 원격전이를 찾아낼 수 있어 자궁경부암 환자의 재발의 조기진단에 유용하게 사용될 수 있을 것을 사료된다. Objective: The purpose of this study was to investigate the clinical feasibility of FDG-PET (Positron Emission Tomography) scan in patients with clinically no evidence of disease after treatment of cervical cancer. Methods: One hundred and one patients with clinically NED(no evidence of disease) state after treatment of cervical cancer underwent PET scan. FDG-PET scan was obtained with a GE Advance Scanner, beginning at 50 minutes after injection of 370-555 MBq(10-15 mCi) of 18F FDG. Regional scan was also obtained with emission image. Uptake exceeding 3.0 SUV was determined as a positive finding. Recurrence was confirmed by CT, MRI, and needle biopsy if possible. Results: Among 101 patients showing no evidence of disease, 17 patients(16.8%) showed abnormal PET scan findings. Clinically, 8 patients(7.9%) were confirmed to have recurrent lesion by CT, MRI or by needle biopsy. PET scan could detect recurrent lesions in the mediastinum or lung(10/17), pelvis(7/17), and supraclavicular lymph node(2/17). The sensitivity and specificity of PET scan in patients with cervical cancer showing no evidence of disease were 100% and 90.3%, respectively. The positive predictive value, negative predictive value and false positive rate were 47.1%, 100% and 52.9%. Conclusion: PET scan could detect 7.9% of early recurrence in patients with cervical cancer with NED status. FDG-PET scan may be a useful method in detecting metastases or recurrence of a cervical cancer showing no evidence of disease by routine conventional imaging studies.

We experienced 7 cases of patients who were performed 99mTc-methylene diphosphonate bone scintigraphy for the evaluation of diseases they had, Their bone scintigrams showed incidental radionuclide upiake in the gastrointestinal tracts and they had no special symptom or sign attributable to the findings, Case 1 showed radionuclide uptake in the stomach and both lung and the patient had suffered from hypercalcemia and azotemia. Case 3 and case 6 showed diffuse radionuclide uptake in the stomach and intestinal tract, Others shawed diffuse or regional radionuclide uptake in the intestinal tracts. Radionuclide uptake in tbe gastrointestinal tract by 99mTc-methylene diphosphonate is caused by a certain pathologic lesion but also can be seen in the normal gastrointestinal tract. So, one who reads bone scintigrams should be alert for the pathologic lesion in the gastrointestinal tract although one must interpretate with the concept of this normal variations.
