        Myopic Shift after Implantation of a Novel Diffractive Trifocal Intraocular Lens in Korean Eyes

        To evaluate the clinical outcomes of cataract surgery using novel diffractive trifocal intraocular lenses(IOLs) in Koreans. Methods: This was a retrospective, consecutive, interventional study of cataract surgery using POD FineVisionIOL implantation. Complete ophthalmologic examinations were performed preoperatively and postoperatively. Defocus curves were assessed over a range of +1.5 to -4.0 diopters in 0.5-diopter steps at one month postoperatively. Uncorrected distant visual acuity, uncorrected intermediate visual acuity, uncorrected near visual acuity,corrected distant visual acuity, distant-corrected intermediate visual acuity, and distant-corrected near visual acuitywere assessed one month postoperatively, and manifest refraction was performed during every visit. Results: The study analyzed 31 eyes in 20 patients. There were statistically significant improvements in uncorrecteddistance visual acuity, uncorrected intermediate visual acuity, uncorrected near visual acuity, corrected distancevisual acuity, distance-corrected intermediate visual acuity, and distance-corrected near visual acuity (p <0.001). The final postoperative refractive outcomes showed statistically significant myopic shifts compared to thetarget refraction based on SRK/T, SRK-II, Haigis, and Hoffer Q formulas (p < 0.001).         Bile Acid Receptor Farnesoid X Receptor: A Novel Therapeutic Target for Metabolic Diseases

        Bile acid has been well known to serve as a hormone in regulating transcriptional activity of Farnesoid X receptor (FXR), an endogenous bile acid nuclear receptor. Moreover, bile acid regulates diverse biological processes, including cholesterol/bile acid metabolism, glucose/lipid metabolism and energy expenditure. Alteration of bile acid metabolism has been revealed in type II diabetic (T2D) patients. FXR-mediated bile acid signaling has been reported to play key roles in improving metabolic parameters in vertical sleeve gastrectomy surgery, implying that FXR is an essential modulator in the metabolic homeostasis. Using a genetic mouse model, intestinal specific FXR-null mice have been reported to be resistant to diet-induced obesity and insulin resistance. Moreover, intestinal specific FXR agonism using gut-specific FXR synthetic agonist has been shown to enhance thermogenesis in brown adipose tissue and browning in white adipose tissue to increase energy expenditure, leading to reduced body weight gain and improved insulin resistance. Altogether, FXR is a potent therapeutic target for the treatment of metabolic diseases.

        가원격에서 조정할 수 있도록 해 주는 원격 PC조정리모콘 등의 제품들이 개발되고 있고 이를 편리성의 측면으로 이용한 제품들이 생겨나고 있다. 본 논문은 PC를 하드웨어 없이 원격에서 조작하기 위하여 앱에서의 간단한 조작으로 PC를 제어할 수 있도록 하였다. 그리고 즐겨찾기라는 독특한 기능을 추가하여 스마트폰에서 즐겨찾기 항목을 선택하여 PC에서 프로그램을 실행시킬 수 있도록 하였다.

        선택적 레이저 섬유주성형술 후 발생한 전방출혈 1예

        목적: 거짓비늘녹내장 환자에서 선택적 레이저 섬유주성형술 후 발생한 전방출혈 1예를 보고하고자 한다. 증례요약: 79세 여자 환자가 우안의 안압상승을 주소로 내원하였다. 우안 거짓비늘녹내장으로 안압하강제를 점안하며 경과를 지켜보았으며, 2년 전 우안의 유리체황반견인 및 백내장으로 초음파수정체유화술 및 인공수정체 후방삽입술, 유리체절제술을 시행 받았다. 시술 전 골드만 압평 안압계를 이용한 검사상 우안 안압 32 mmHg, 좌안 안압 20 mmHg로 측정되었다. 우안의 안압 상승에 대한 치료로 선택적 레이저 섬유주성형술을 시행하였으며, 시술 중 합병증은 발생하지 않았다. 시술 2일 뒤, 갑자기 발생한 우안 시력저하 및 안통을 주소로 내원하였으며, 내원 당시 최대교정시력 우안 광각인지, 좌안 20/40으로 측정되었고, 안압은 골드만 압평 안압계를 이용한 검사에서 우안 34 mmHg, 좌안 15 mmHg로 측정되었다. 세극등 현미경검사상 우안에 1.1 mm 높이의 전방출혈이 관찰되었다. 출혈 발생 3주일 후 전방출혈은 모두 소실되었으나, 골드만안압계를 이용한 검사상 우안 안압 42 mmHg로 측정되어 우안 섬유주절제술을 시행하였다. 결론: 선택적 레이저 섬유주성형술은 개방각 녹내장에서 안전하고 효과적으로 사용될 수 있는 치료 방법이다. 하지만 드물게 전방출혈과 함께, 조절되지 않는 안압 상승을 동반할 수 있으며, 이로 인해 녹내장의 진행에 영향을 줄 수 있다는 점은 선택적 레이저 섬유주성형술 시행 후 임상적으로 유의해야 할 점으로 생각된다. Purpose: To report a case of hyphema after selective laser trabeculoplasty (SLT) in a patient with pseudoexfoliative glaucoma. Case summary: A 77-year-old female was referred for elevation of intraocular pressure (IOP). Previously, she had been diagnosed with pseudoexfoliative glaucoma in the right eye and was using topical IOP-lowering agents. The best corrected visual acuity was 20/100 in the right eye and 20/40 in the left eye. IOP, measured with Goldmann applanation tonometer, was 32 mm Hg in the right eye and 20 mm Hg in the left eye. Gonioscopy revealed open-angle glaucoma with +2 trabecular meshwork pigmentation but without peripheral anterior synechiae or neovascularization. SLT was performed in the right eye. Two days later, the patient had sudden onset of blurred vision and pain in the right eye. Visual acuity was limited to light perception, and IOP was 34 mm Hg in the right eye. Slit-lamp examination revealed 1.1 mm hyphema with 4+ red blood cell count in the anterior chamber. Three weeks after the SLT, hyphema in the right eye disappeared, but IOP was measured to be 42 mm Hg. The patient underwent trabeculectomy in the right eye. Conclusions: SLT is an effective means of lowering IOP with low risk of complications. However, hyphema can rarely occur after SLT and can affect the outcome of the treatment.

        Crosstalk between FXR and TGR5 controls glucagon-like peptide 1 secretion to maintain glycemic homeostasis

        Though bile acids have been well known as digestive juice, recent studies have demonstrated that bile acids bind to their endogenous receptors, including Farnesoid X receptor (FXR) and G protein-coupled bile acid receptor 1 (GPBAR1; TGR5) and serve as hormone to control various biological processes, including cholesterol/bile acid metabolism, glucose/lipid metabolism, immune responses, and energy metabolism. Deficiency of those bile acid receptors has been reported to induce diverse metabolic syndromes such as obesity, hyperlipidemia, hyperglycemia, and insulin resistance. As consistent, numerous studies have reported alteration of bile acid signaling pathways in type II diabetes patients. Interestingly, bile acids have shown to activate TGR5 in intestinal L cells and enhance secretion of glucagon-like peptide 1 (GLP-1) to potentiate insulin secretion in response to glucose. Moreover, FXR has been shown to crosstalk with TGR5 to control GLP-1 secretion. Altogether, bile acid receptors, FXR and TGR5 are potent therapeutic targets for the treatment of metabolic diseases, including type II diabetes. Keywords

        Fat Biology in Triple-Negative Breast Cancer: Immune Regulation, Fibrosis, and Senescence

        Obesity, now officially recognized as a disease requiring intervention, has emerged as a significant health concern due to its strong association with elevated susceptibility to diverse diseases and various types of cancer, including breast cancer. The link between obesity and cancer is intricate, with obesity exerting a significant impact on cancer recurrence and elevated mortality rates. Among the various subtypes of breast cancer, triple-negative breast cancer (TNBC) is the most aggressive, accounting for 15% to 20% of all cases. TNBC is characterized by low expression of estrogen receptors and progesterone receptors as well as the human epidermal growth factor 2 receptor protein. This subtype poses distinct challenges in terms of treatment response and exhibits strong invasiveness. Furthermore, TNBC has garnered attention because of its association with obesity, in which excess body fat and reduced physical activity have been identified as contributing factors to the increased incidence of this aggressive form of breast cancer. In this comprehensive review, the impact of obesity on TNBC was explored. Specifically, we focused on the three key mechanisms by which obesity affects TNBC development and progression: modification of the immune profile, facilitation of fibrosis, and initiation of senescence. By comprehensively examining these mechanisms, we illuminated the complex interplay between TNBC and obesity, facilitating the development of novel approaches for prevention, early detection, and effective management of this challenging disease.

        Clinical Features of Pediatric Choroidal Neovascularization and Outcomes of Treatment with Intravitreal Anti-VEGF in Korean Patients

        Purpose: To analyze the clinical features of pediatric choroidal neovascularization (CNV) and report the outcomes of treatment with intravitreal anti-vascular endothelial growth factor (VEGF) in Korean patients. Methods: Patients aged 18 years or younger and diagnosed with CNV were studied retrospectively. We assessed best corrected visual acuity (BCVA), type and activity of CNV, changes of central macular thickness (CMT), recurrences, and complications after injection. Results: In total, 15 CNV lesions were noted. At initial diagnosis, the average age of patients was 12.42 years (SD, 4.25; range, 4-18 years). The mean follow-up period was 34.6 months (range, 12-122 months). The causes of CNV were idiopathic (n = 7), inflammation (n = 2), tumor (n = 3), and trauma (n = 3). A total of 54 injections (average 4.15/eye) was administered for CNV. BCVA (logarithm of the minimum angle of resolution) improved significantly from 0.68 (SD, 0.3) before injection to 0.45 (SD, 0.4) after injection (Wilcoxon signed rank test, p < 0.05). CMT was 377.93 (SD, 132.2), 293.17 (SD, 109.8), and 315.20 (SD, 120.2) before the injection treatment, after one injection, and at last follow-up, respectively. There was leakage at the injection site due to a treatment complication in one eye (one of 54 injections). Conclusions: In pediatric patients, CNV had a variety of causes. Intravitreal anti-VEGF treatment showed relatively good outcomes such as improved vision, but repeated injection was frequently required.

        Essential roles of bile acid receptors FXR and TGR5 as metabolic regulators

        Alterations of bile acid (BA) metabolism in type II diabetes (T2D) have revealed a link between BAs and metabolichomeostasis. The BA receptors farnesoid X receptor (FXR) and G-protein coupled BA receptor 1 (TGR5) both have shownto regulate lipid, glucose, and energy metabolism, rendering them potential therapeutic targets for T2D therapy. Astonishingly, BA signaling is vital as it has known to be a positive factor for beneficial improvements in vertical sleevegastrectomy surgery, in turn making these BA receptors beneficial tools in therapeutic targets for T2D. These metabolicregulators: FXR and TGR5 are essential modulators in the metabolic system, which affect the human machinery and will bethe focus of this review.
