Iodinated Contrast Media Can Induce Long-Lasting Oxidative Stress in Hemodialysis Patients

Purpose: Due to their comorbidities, dialysis patients have many chances to undergo radiologic procedures using iodinated contrast media. We aimed to assess time-sequenced blood oxidative stress level after contrast exposure in hemodialysis (HD) patients compared to those in the non-dialysis population. Materials and Methods:We included 21 anuric HD patients [HD-coronary angiography (CAG) group] and 23 persons with normal renal function (nonHD-CAG group) scheduled for CAG, and assessed 4 oxidative stress markers [advanced oxidation protein products (AOPP); catalase; 8-hydroxydeoxyguanosine; and malondialdehyde] before and after CAG, and subsequently up to 28 days. Results: In the nonHD-CAG group, only AOPP increased immediately after CAG and returned to baseline within one day. However, in the HD-CAG group, all four oxidative stress markers were significantly increased starting one day after CAG, and remained elevated longer than those in the nonHD-CAG group. Especially, AOPP level remained elevated for a month after contrast exposure. Conclusion: Our study showed that iodinated contrast media induces severe and prolonged oxidative stress in HD patients.
공장 이전적지 개발사업의 리스크 요인 중요도에 관한 실증분석

본 연구는 공장 이전적지 개발사업에 내재되어 있는 리스크 요인을 도출하고 중요도를 평가하는데 목적이 있다. 이를 위해 개발사업 단계를 개발사업 전 평가단계, 개발사업 준비 및 인·허가 단계, 개발단계의 3단계로 구분하고 각 사업의 리스크를 단계별·속성별·요인별 로 구분하여 체계를 구축하였다. 분석 결과에 따르면 사업단계별 중요도는 개발사업 전 평가단계, 개발사업 준비 및 인· 허가 단계, 개발단계 순으로 개발사업 전 평가단계가 가장 중요하다고 할 수 있다. Fuzzy – AHP 보정 종합 중요도의 결과에 따르면 공법적 사법적 규제 미확인(용도지역 변경 불가), 사업기간의 적합성 미흡(수요 및 이전재원을 고려한 단계별 개발), 도시계획시 설 변경·결정, 공장 이전부지 확보 실패, 개발규모 설정의 오류(수요를 고려한 적정 개발규 모) 순으로 산정되어 AHP 종합 순위도와 유사한 것을 알 수 있다. The purpose of this study is to draw risk factors and evaluate the importance of each factor of the development business of factory site relocation. Phases of development consists of (1)Pre-development business assessment phase, (2) Development business preparation and licensing phase, and (3) Development phase and risk factors for each phase were examined by stage, attribute, and factors. According to the analysis result, important phases are in order of Pre-development business assessment phase, Development business preparation and licensing phase, Development phase. Therefore, pre-development business assessment phase is the most important phase. According to the analysis result of the Fuzzy-AHP adjusted important factors, important phases are in order of unidentified judicial regulation (restricted change of zoning regulation), unsuitable business period (the phasing of land developing with consideration of demand and financial resource), and alteration or confirmation of urban planning facilities, failure to secure factory relocation site, and misspecification of development scale (setting optimal development scale through the consideration for demand). Its importance order appears to be similar to the importance order of AHP relative importance factors.
주민제안 방식의 지구단위계획 수립에 관한 연구 -군산시 도시재생 선도지역 사례를 중심으로-

This study analyzed the problem occurred on “Determination of urban management planning of a factory site of PaperCorea in Gunsan” which is a part of residents proposed method, and proposes improvement plan that aims for efficient maintenance. The conclusion drawn from this study is as follows. As Gunsan government does not have an operation guideline for District Unit Planning, PaperCorea prepared objective indicator by reviewing factory relocation case, facility relocation/ development case, and other significant development in the jurisdiction of Gunsan based on such data as its population, scale of commercial area, and infrastructure. Therefore, Local government should prepare operation guideline of the residents’ proposal for its vitalization. Information sharing procedure should be included in operation guideline to promote participation of the residents. Also, time limit of the residents’ participation should be specified in order to proceed District Unit Planning regardless of civil complaints. Though the newspaper notice and briefing session for hearing of resident comments are held while at a preparation of District Unit Planning stage, it cannot deliver enough information to the residents. In order to promote participation of the residents, information should be delivered through various types of media such as local government website, and detailed business plan should be fully explained by a local council. 급성신우신염 환자에서 균혈증의 위험 인자 분석 -단일기관 분석-

Purpose: Acute pyelonephritis (APN) is among the most common infectious diseases. Most APN occurs in young women and easily treated. Bacteremia has been associated in approximately 20-30% of those with APN. But recent documents demonstrated that blood cultures provide no useful information toward the clinical management of acute pyelonephritis. Thus we compared demographic and clinical characteristics as related to the bacteremic status, and investigated the risk factors for bacteremia. Methods: One hundred sixty five patients, who visited myongji hospital for APN from January, 2004 to December, 2006 were included. Retrospective data were analyzed by medical record review. Results: Bacteremic patients (N=51, 30.9%) were significantly older than those in nonbacteremic group (p<0.0001), had elevated serum creatinine (p=0.008), decreased platelet counts (p=0.029), lower serum protein (p=0.010), and lower serum albumin (p=0.011) than those without bacteremia. Hematuria was more severe in bacteremic patients (p<0.0001). The bacteremic cases were observed more frequently in patients with complicated APN patients than uncomplicated patients (46.7% vs. 21.4%, p=0.001). No significant difference existed between the bacteremic and non-bacteremic patients in the prevalence of resistance to quinolone of E. coli. In multivariate logistic regression analysis, serum albumin (p=0.023), hematuria (p=0.003), and age (p=0.003) at presentation were found to be independent risk factors for bacteremia in acute pyelonephritis. Conclusion: Our study reveals that patients with bacteremia have different clinical characteristics compared to those without bacteremia. It is recommended to concern about the presence of bacteremia in the treatment of APN.
