        고유수용성 조절이 만성 편마비 환자의 균형에 미치는 영향

        Hemiplegic patients usually present with difficulty maintaining balance. Balance retraining is the major component of rehabilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of prorpioceptive exercise program on the improvement of balance in the patients with chronic hemiplegia. Thirty subjects (mean age 57.0±9.8) were recruited and the subjects were divided into a proprioceptive group and a visual group. The subjects for the proprioceptive group were participated in the proprioceptive exercise program for 4 weeks, and the visual group were treated visual feedback training using Balance Master. At 4 week follow-up test, Berg Balance Scale significantly improved 1.1 points (p<.01), Timed Up & Go test improved 4.2 second (p<.01), and weight distribution during sit to stand also improved 5.0% (p<.01). As a result of this study, the proprioceptive control approach improved dynamic balance in the patients with chronic hemiplegia. It is suggested that there was no benefit of visual feedback training like as Balance Master when administrated in combination with other physical therapy interventions, compared with physical therapy alone using proprioceptive control approach to hemiplegia.

        자세근 강화운동이 편마비환자의 보행에 미치는 영향

        The purpose of this study was to determine the effect of strengthening exercise on the postural muscles for patients with hemiplegia. Subjects and Methods Sixteen subjects with hemiplegia (mean 56.6±9.9 years) were recruited. Treatment outcomes were assessed with Timed Up and Go test ad 6MWT. Results After a 4-week training period, the mean time of TUG decreased significantly and 16% increased in 6MWT. Retraining gait in patients with hemiplegia while to promote the activity of anti-gravity muscles was supported resulted in better walking abilities. Conclusion This novel approach provides efficient approach for the therapy of gait disturbance in patients with hemiplegia.

      • 가정방문 물리치료가 뇌졸중 환자의 운동기능과 일상생활동작 수행에 미치는 영향

        The purpose of this study was to determine the effects of home visiting physical therapy on the motor function and ADL in the person with chronic stroke. In this study, 50 individuals, lining in Ui-wang city, were chosen by Ui-wang city public health center and the home visiting physical therapy was performed once a week for 8 weeks and scored the MAS, the FIM and the MBI before and after. The results of this study were as follows. (1) The items with the highest score among subscales of the MAS was 'Sitting', the FIM was 'Comprehension', and the MBI was 'Ambulation' and the lowest were the MAS was 'Hand activities', the FIM was 'Bathing', and the MBI was 'Bathing self' respectively. (2) After 8 weeks home visiting physical therapy, average total score of MAS, FIM and MBI were 23.32, 89.00, and 61.72 and MAS and MBI a significant increment(p<0.05) but not FIM. Results shown here indicates that the home visiting physical therapy can result in changed the motor function and the ADL in the person with chronic stroke. Also, the results of this study can provide a reference for the successful therapeutic program in the persons with chronic stroke.

      • 고유수용성 조절과 시각적 피드백이 만성 편마비 환자의 안정성 한계에 미치는 영향

        Purpose: Hemiplegic patients usually present with difficulties in maintaining their balance. This study compared the effects of prorpioceptive exercise and visual feedback program on the limits of stability (LOS) in chronic hemiplegia patients. Methods: Thirty subjects (mean age $57.0{\pm}9.8$) were recruited. The subjects were divided into a proprioceptive group and a visual group. The subjects for the proprioceptive group participated in the proprioceptive exercise program for 4 weeks, and the visual group were treated with visual feedback training using a Balance Master. Results: At the 4 week follow-up test, the LOS in the proprioceptive group improved significantly in all directions (p<0.05). However, improvement was only observed in the forward direction in the visual feedback group. Therefore, the proprioceptive control approach improves the LOS in chronic hemiplegia patients. Conclusion: These results suggest that compared with physical therapy alone using a proprioceptive control approach to hemiplegia, there was no additional benefit of visual feedback training, such as Balance Master, when administrated in combination with other physical therapy interventions.

        Constraint-Induced Movement Therapy가 편마비측 손기능 증진에 미치는 영향

        Purpose: This study examined the effect of constraint-induced movement therapy (CIMT) on improving the hand function in hemiplegic side. Methods: Ten subjects without a control group were given CIMT to the hemiplegic side for 3 weeks. The effects of their hand function and sensibility were examined using a MAL and two point discrimination test. Repeated ANOVA was carried out for an analysis of the effects of the application of CIMT before and after treatment. Results: The participants showed significant improvement in their functional aspect with CIMT while there were no significant changes in the time domain variables. There was significant improvement in the quantitative and qualitative aspect of MAL, as well as significant improvement in the two-point discrimination function in all fingers. Conclusion: CIMT can enhance the motor function and sensory function of the hand in hemiplegic patients.

      • 골반동작을 이용한 체간조절이 편마비 환자의 족저압에 미치는 영향

        Purpose: The purpose of this study was to examine effect of trunk control using pelvic movements upon the foot pressure in patients with hemiplegia. Methods: Twelve males with hemiplegia were the procedure executed turnk control using pelvic movements. The foot pressure were measured using Parotec-system. Results: The data were analysed with paired t-test. First, there was a significant increase in external and internal sensors of dynamic foot pressure change of the hindfoot before and after therapy. Also there was significant increase in hallux (p<0.05). Second, there was a significant increase of affected side in support phase(p<0.05) and decreased of affected side in overlapping phase(p<0.05). Third, there was a significant increase in foot floor contact time and impulse pressure rate between affected and non affected side(p<0.05). Conclusion: The trunk control with pelvic movement had an significant effect on the legs by increase activities of hip flexors and abductors. Also had an effect on ankle dorsiflexion and plantar flexor by biomechanical movement.

        뇌졸중 환자의 체간하부 안정성 강화운동이 균형과 보행에 미치는 영향

        Purpose: The goal of this study was to investigate the effect of core strength exercises on the balance control and walking ability of stroke patients. Methods: Twelve stroke patients participated in this study. These patients were divided into two groups (a core stability group and a control group). There were 6 subjects in each group. They participated in core strength exercises for 3 weeks. These exercises included The Timed Up and Go Test (TUG). Messen Trairuieren Dokumentieren (MTD) Systems for balance and walking were measured and compared before and after the treatment. Results: First, core strength exercises improved balance control of patients with stroke by increasing weight shifting to the affected side. Second, core strength exercises improved the walking ability of patients with stroke by reducing the TUG score. Third, there was a significant correlation between balance control and walking. Conclusion: Core strength exercises are effective for improving both balance control and walking.

      • 단하지 보조기가 편마비 환자의 골반경사각에 미치는 영향

        The use of AFO in the management of patients with hemiplegia has been somewhat controversial. The purpose of the study was to identify the influence of the plastic ankle foot orthoses(AFO) on the characteristics of pelvic tilt in the patients with hemiplegia. Sixteen hemiplegic patients participated. The angle of pelvic tilt were measured using the BROM II . Results showed that the hemiplegia who weared plastic AFO, the angle of pelvic tilt was significantly less than not weared subjects. However. duration of wearing a plastic AFO and the degree of spasticity were not affected to the angle of pelvic tilt. Therefore the patients with hemiplegia should consider using the brace for the right purpose, also need to maintain the mobility of ankle joint while wearing the plastic AFO.

      • 기립자세 증진 운동이 파킨슨병 환자의 제자리 돌기 동작에 미치는 영향

        Purpose: The aims of this study was to evaluate the effects of shoulder lateral rotation and trunk extension exercises on standing posture improvement exercises using turning round in the patients with Parkinson's disease. Methods: For twenty-one patients with Parkinson's disease, shoulder lateral rotation and trunk extension exercises were performed for a total of three sets, ten times a set, three days a week for twelve weeks. To measure turning round, ink foot-print method was used. The number of steps and time were measured every week for before and after study (twelve weeks). Results: The results are as follows: Changes in steps and time on each week during twelve weeks showed significant differences, compared with before test(p<0.05). Conclusion: Standing posture improvement exercises can help perform turning round in the patients with Parkinson's disease.

        수정된 골프스윙 훈련이 편마비 환자의 보행 특성에 미치는 영향 - 단일 사례 연구 -

        Purpose: The purpose of this study was to determine the effect of modified golf swing training on gait characteristic in hemiplegic patient through Kwon 3D motion analysis system. Methods: This study has performed single subject design from September to October 2008. The subject had left hemiplegia due to CVA in December 2003. He has treated Bobath approach twice a week. In order to increase ankle dorsiflexion and knee flexion, the subject has applied modified golf swing training on the basis of Bobath approach. The measurement of gait characteristic was taken by Kwon 3D motion analysis system. Results: The results were as follows : 1) Walking velocity was increased 0.62m/sec than before the training. 2) Step length was increased 0.09m than before the training. 3) Left ankle and hip angle were increased, but left knee angle was decreased. Conclusion: It could be concluded that the activity modified golf swing training in walking pattern contributed to improve the movement quality and speed of gait.
