        This study was performed to investigate the effect of intraoral snoring device on the electromyographic(EMG) activity of several masticatory muscles. For the study, 40 dental students were selected. They had no visible signs and symptoms except mild joint sound. Two types of intraoral appliance were fabricated : anterior positioning splint(APS), commonly used for patients with temporomandibular disorders, and snoring appliance(SA), mandibular advancement type device. The mean distance of mandibular advancement of each appliance was 3㎜ and 5㎜, respectively. The BioEMG^ (Bioresearch Inc., USA) was used to measure of the EMG activity of three pairs of muscles, which were anterior temporalis(TA), superficial masseter(MM), and anterior digastricus(DA), with regard to change of mandibular positions. Three sessions of measurement were carried out. First session was baseline measurement, to record the EMG activity before insertion of intraoral device. The mandibular positions observed were rest position, protrusive position, protrusive-clenching position, and clenching position. Second session was the measurement of the EMG activity related to APS, which was performed two times, namely, with APS and without APS. Third session was also measured two times, with SA and without SA. The data were processed with SPSS windows^ program for the maximum and the mean value of the EMG activity in each recording. The maximum EMG activities with adaptation of SA were higher than those with APS or with natural occlusion in general, but the EMG activity in protrusive-clenching position were higher with APS. The correlation of the mean EMG activity between each session in the same mandibular position showed two patterns. That was, without appliance, correlation among three sessions were highly significant in general, but with appliance, there were no consistent pattern of correlation. In summary, adaptation of intraoral snoring device increased masticatory muscle activity, especially in the mandibular rest and the protrusive positions. This results also suggest, however, that the muscle activity might be lowered if baseline activity is lowered with many pre-treatment modalities for mandibular relaxation.

        This study was performed to investigate the chewing movements in subjects with unilateral chewing habit. 42 patients with temporomandibular disorders and 44 dental students without any signs and symptoms of the temporomandibular disorders were selected as the patients group and the control group, respectively. BioEGN of the Biopak^ (BioResearch Inc. USA) was used to record chewing movements pattern as to chewing velocity, range of movements and symmetry of the path to midline. The data were analyzed by the preferred chewing side and/or the affected side by SAS/stat program. The results obtained were as follows 1. In control group, right side chewing subjects showed more deflected path to preferred chewing side than left side chewing subjects have done. But in patients group, there was no significant difference in chewing pattern between the two subjects. 2. In unilaterally affected patients. right side affected patients showed slight difference between the movements to both sides, but left side affected patients showed significant deflection to the affected side. 3. There were some difference in preferred chewing side movement pattern between the patients group and the control group in right chewing subjects whereas no difference was seen between the two groups in left chewing subjects. 4. There was no difference in chewing velocity and range of movements between right and left side movement in all cases compared by preferred chewing side or affected side.

        후두하 신장운동이 두경부자세와 흉쇄유돌근 및 승모근 활성에 미치는 영향

        This study was performed to investigate the effect of suboccipital stretch on the head and neck posture and the electromyographic(EMG) activity of some cervical muscles. For this study, 39 patients with temporomandibular disorders(TMD) and 34 dental students without any signs and symptoms in the masticatory system were selected as the patients group and as the normal group, respectively. Head position by goniometer CROM$^{(R)}$(Performance attainment, St. Paul, USA), EMG activity by BioEMG$^{(R)}$(Bioresearch Inc., Milwaukee, USA), and craniocervical posture by cephaloradiography were observed in both natural head posture(NHP) and head posture with suboccipital stretch(tuck posture) abtained from slight posteroinferior finger pressure on the chin. Variables measured on the cephaloradiograph were SN angle, atlas angle, CVT angle, occiput-atlas and atlas-axis distance, and pharyngeal width. The data obtained were analysed by SPSS windows program and the results of this study were as follows : 1. In the sagittal plane, degree of anterior rotation of the head by suboccipital stretch was 6.3 in the patients group, and 6.2 in the normal group, respectively. So there was no significant difference between the two groups for degree of anterior rotation, but the position of the head in the patients group were more posteriorly extended than in the normal group in both NHP and tuck posture. 2. EMG activity of the stemocleidomastoideus in the patients group, and that of the upper trapezius and the sternocleidomastoideus in the normal group were increased by suboccipital stretch. The range of EMG activity, however, in these cervical muscles were 1.6 -2.3)u.V. 3. Cephalometric variables such as SN angle, atlas angle, CVT angle, occiput-atlas and atlas-axis distance except pharyngeal width were generally increased by suboccipital stretch. There was some difference, however, in results between the two groups. Atlas angle was not changed in the patients group whereas CVT angle was not changed in the normal group. 4. The distance from subocciput to spinous process of axis was significantly increased as much as 3.0mm in the patients group, and 3.7mm in the normal group by suboccipital stretch.

        하악 정중선의 편위와 하악골의 높이차가 저작근 활성에 미치는 영향

        This study was performed to investigate the effect of mandibular midline shift and difference of mandibular height between both sides on the electromyo- graphic(EMG) activity of the masticatory muscles on clenching or gum chewing movement. For this study, 105 patients with temporomandibular disorders(TMD) were selected and panoramic radiograph were taken. Amount and side of the midline shift and height of the mandible from antegonial notch to the top of the condylar head were measured on panoramic view. $BioEMG^{(R)}$ (Bioresearch Inc., Milwaukee, USA) was used for recording of EMG activity(${\mu}V$) of the anterior temporalis and the superficial masseter on clenching or gum chewing movement. EMG activity on clenching during 533msec period were measured for activity of the starting point and the one second-after activity as the early EMG and the maximum EMG, respectively. EMG activity on gum chewing movement were measured for activity of the first and the second chewing stroke. The data collected were analysed by SPSS windows program, and the results of this study were as follows : 1. Height of the mandible was 8.06cm on right side and 8.03cm on left side, and showed no difference by age, but significantly differed by sex with higher in male subjects. 2. Mean value of the midline shift was 0.1mm with range of 0~5mm on both sides. The amount and side of the midline shift did not related with height difference of the mandible and/or the EMG activity of the masticatory muscles on clenching. 3. Prevalence of higher right side and higher left side of the mandible were almost same, and the EMG activity of higher side was not higher than that of the other side. 4. In the subjects with height difference of more than 5mm between both sides of the mandible, the early EMG activity on clenching were differed for the anterior temporalis, but the maximum activity were differed for the superficial masseter. 5. In the subjects with height difference of more than 5mm between both sides of the mandible, EMG activity of the anterior temporalis of the gum chewing side was not higher than that of the other side when chewing on the side of lower height, but in the subjects with height difference of less than 5mm, the EMG activity was higher than that of the other side.
