        두개강내 "De Novo" 동맥류 : 증례 보고 Case Report

        There are numerous theories of pathophysiology in intracranial aneurysm, but currently accepted hypothesis was congenital defect of medial layer of vascular wall and the major developing factor was hemodynamics. Almost physcians were belived that treatment of intracranial aneurysm was completely through clipping of aneurysmal neck But many reports were emphasized newly developed intracranial(De Novo) aneurysm after clipping of initially identitied intracranial aneurysm. The authors have same experienced of 2 cases intracranial 'Do Novo" aneurysm. We are stress there were never completed treatment of aneurysm that cliiping of initially identified intracranial aneurysm and should be attention to 'De Novo" aneurysm.

        소아 두부외상의 임상적 분석

        Objectives : With the advancement of a social life, the pediatric head injuries(PHI) occur greater than ever. Since the PHI differs from adult head injury with regards to mechanism of trauma, prognosis, and mortality, it is important to identify the characteristics of the PHI for its proper treatments and prognosis. Methods : For this study, a series of 365 PHI patients under 15 years of age who were admitted to our hospital, were evaluated from January 1991 to December 1996. The clinical variable studied were age, sex, Glasgow coma score(GCS), causes of trauma, diagnosis, symptoms, associated injuries and Glasgow outcome score (GOS). The characteristics of PHI were evaluated according to presentations of skull fractures, intracranial hemorrhages, associated injuries, GCS at admission and GOS. Results : Mean age of the studied patients was 6.51 years of age. The majority of PHI patients were under the 7 years of age(66.7%). The ratio of male to female was 2.2:1. Seasonally, PHI occurred more frequently during March to August(61.6%). The main causes of the injuries were accidental falls and traffic accidents(47.1% and 46.3%). One hundred ninety seven(54%) patients suffered from skull fractures and 110(30.1%) patients were developed intracranial hemorrhages and acute epidural hematomas(17.8%) which were the most common intracranial hemorrhages. There was statistical significance between skull fractures and intracranial hemorrhage (p=0.032) and between GCS and GOS(p=0.001). However, there was no statistical significance between skull fractures and intracranial hemorrhage(epidural hematomas, subdural hematomas, and intracerebral, intraventricular and subarachnoid hemorrhage)(p=0.061, 0.251 and 0.880). Also there were no significance of prognosis between under the seven and over the 8 years of age(p=0.349). Conclusions : The core management for PHI is prevention from its occurrences. However, when unexpected accident occurs, early diagnosis and treatment for PHI by through examination for associated injuries and other damages even if there is no skull fracture are essential in managing patient's outcome.

        시차주사열량계를 이용한 진공백 성형 프리프레그의 경화 거동 연구

        The cure kinetics of carbon fiber-reinforced prepreg for Vacuum Bag Only(VBO) process was studied by differential scanning calorimetry (DSC). The total heat of reaction (ΔHtotal = 537.1 J/g) was defined by the dynamic scanning test using prepregs and isothermal scanning tests were performed at 130oC~180oC. The test results of isothermal scanning were observed that the heat of reaction was increased as the temperature elevated. The Kratz model was applied to analyze the cure kinetics of resin based on the test results. To verify the simulation model, the degree of cure from panels using different cure cycles were compared with the measurement. The simulation model showed that the error against the experimental value was less than 3.4%. 본 연구에서는 시차주사 열량계(differential scanning calorimetry, DSC)를 이용하여 진공백 성형 공정에 사용되는 탄소 섬유 프리프레그의 경화 거동을 연구하였다. 프리프레그를 이용한 동적주사 시험을 통해 전체 발열량(ΔHtotal = 537.1 J/g)을 측정하였고, 130oC~180oC에서 등온주사 시험을 실시하였다. 등온주사 시험의 결과는 온도가 올라감에 따라 발열 반응이 증가되는 것을 확인하였다. 시험 결과를 토대로 수지의 경화 거동을 분석하기 위하여 Kratz 모델을 적용하였다. 서로 다른 경화 사이클로 제작된 평판의 경화도를 경화 예측 모델과 실험값을 비교하였다. 경화 예측 모델은 실험값 대비 3.4% 이하의 오차율을 보여 결과가 잘 부합하는 것을 확인하였다.

        경화공정에 따른 복합재 부품 외부와 내부의 경화 거동 차이에 대한 연구

        We measured the thermal conductivity of composite materials manufactured by the autoclave and vacuum bag only processes and predicted the cure behavior of the external and internal of composite parts with a cure kinetics model. The temperature difference between the external and internal depends on the processes because of the change of thermal conductivity. In the autoclave process, the temperature and cure behavior of the internal were similar to those of the external because of the high thermal conductivity. However, the temperature of the internal of the vacuum bag only process was different from that of the external. The difference can influence the part quality and evacuation of air. Compression tests were performed to find the mechanical property using 0o unidirectional specimens. The composite of the vacuum bag only process was found to have a lower compressive strength than that of the autoclave process. 본 연구는 탄소 복합소재를 이용하여 오토클레이브와 진공백(Vacuum Bag Only) 공정으로 소재의 열전도계수를구하고 경화 거동 모델을 통해 표면부와 내부의 경화 거동을 예측 및 비교하였다. 공정에 따른 열전도 계수의 변화로 인해 내부 온도가 표면부와 차이를 보였다. 오토클레이브 공정의 경우 높은 열전도 계수를 통하여 표면부와 내부의 온도는 거의 일치를 하였으며, 경화 거동 역시 유사하게 진행이 되었다. 하지만, 진공백 공정의 경우 표면부와 내부의 경화 거동이 많은 차이를 보였다. 이 차이는 부품 내부의 품질 및 공기 배출에 영향을 준다. 기계적 물성 차이를 확인하고자 0도 단방향 시편으로 압축시험을 수행하였고, 그 결과 진공백 성형이 조금 낮은 물성을 갖는다는 것을 확인하였다.

      • 소아 천식환자에서의 에어로졸제제의 사용방법에 대한 평가

        Recently, the effect of aerosol therapy is wananted in the prevention and treatment of bronchial asthma and the aerosols that had not been interested for a long period, are widely used and recommanded by many doctors. Because the aerosols act locally in the airway, they are more rapid action with smaller dosage and less systemic side effects than the oral regimens. But the usage of these aerosol inhaler had been limited in the children because of the technical difficulties. This study was designed to evaluate whether the patients use metered-dose inhaler(MDI) correctly and make them more effect by correcting errors if present. Of 115 asthmatic children, we applied ordinary method of MDI to 106 patients and rebreathing method to 9 patients and after the instruction of correct inhalation technique,we evaluated the adequacy of inhalation of aerosol from MDI. The results were as follows.' 1) According to the distribution of age, 97 patients (91.5%) of which ordianary method was applied, were over 5 years of age and all patients applied rebreathing method were less than 7 years of age. 2) 71.7% of patients applied ordinary method, have used MDI incorrectly, but, it was reduced to59.6% & 42.1 in the 2nd a 3rd test by continuous, repetitive instruction. And the most common error was the actuation of the canister after a period of inspiration. 3) 29.2% of patients applied ordinary method, had no therapeutic effect owing to the inadequate use of MDI and the most common error in these patients was the actuation of canister after full inspiration. 4) On the ordinary method, although the success rate was ll.l% in the patients less than 5 years of age, it was increased according to the age. 5) According to the number of errors which the most patients had taken, three was 36.8% in the 1st test, but one was 46.5% & 37.5% in the 2nd& 3rd test. 6) The most patients who had no therapeutic effect from MDI took an error in the 1st a 2nd test and each rate was 74.2% & 90.9%, respectively. 7) In spite of the age younger than the former, the patients applied rebreathing method had the relatively high success rate. But the errors in the inadequate patients were too large or small amount of air in the vinyl bag, too rapid breathing and nasal breathing on biting the mouthpiece. The authors found that all physicians who recommand the aerosol medication to asthmatic children should teach each patient the correct technique when aerosol therapy is started and should check again of follow-up examination to assure that the patient obtains maximum effect from aerosol therapy.

        저압범위 확대에 따른 태양광발전시스템 직류배선 설계방법

        The Korea Electro-technical Code (KEC) established in 2018 extended the DC low-voltage range under 750 V to 1,500 V. Accordingly, it is expected that there will be some changes in the domestic photovoltaic industry. In Korea, most of PV generation systems have been operated under 750 V for cost and regulatory reasons. If KEC is implemented from 2021, PV generation system is expected to improve power generation efficiency by constructing under 1,500 V. However, in order to apply the new low- voltage range in PV generation system, it seems to be prepared more concretely. Especially, KEC currently deals with regulations for low-voltage cables and PV generation system, but the impact on PV generation system and existing cables by applying the expansion of DC 1,500 V is not considered. And the rated voltage 1 kV cable currently used is not appropriate to PV generation system expected to be constructed under 1,500 V from 2021. In addition, due to the characteristics of PV generation system, it is necessary to regulate the use of cable for PV generation system rather than using general cables at 1,500 V. Therefore, this paper proposes a revision plan of KEC applied in 2021 and a design method to prepare DC wiring for PV generation system applicable to DC 1,500 V.

        다낭종신환자에서 뇌동맥류 파열 : 2례 보고 Report of Two Cases

        The authors present two cases of ruptured intracranial aneurysms associated with nearly total destruction of kidney function due to polycystic kindney in one aged 35 years and the other 53 years. One patient had a aneurysm on anterior communicating artery and the other had three aneurysms located in cavenous and bifurcated portions of internal carotid artery, and anterior communicating artery. They were successfully treated by a direct surgical approach. We stress surgical management in a state of good consciousness regardless of poor kidney function, because the risk of early by rebleeding is seemed to be higher than that by renal failure or others.
