        디지털 화상 병력 시스템과 디지털 방사선치료 기록 시스템의 개발과 사용 경험

        목적 : 방사선치료 기록의 전산화를 통하여 "paperiess" 환경을 이룩하고 원활한 방사선치료의 흐름에 도움을 주기 위 함. 대상 및 방법 : 삼성서울병원의 병원 정보 시스템과 네트워크를 통하여 통합 방사선 관리 시스템(Comprehensive Radiation Oncology Management System:C-ROMS)을 구축하였다. C-ROMS의 한 부분인 디지털 화상 병력 시스템(digital image chart: DIC)과 디지털 방사선치료 기록 시스템(digital radiotherapy record system: DRRS)을 개발하였다. DIC와 DRRS는 Deiphi 프로그래밍 언어에 의해서 개발되었고 windows 95 환경에서 구동이 되며 과내 23대의 개인용 컴퓨터를 근거리 통신망으로 연결하여 사용하고 있으며 전송 속도는 10 Mbps (megabit per second)이다. DIC와DRRS은 1998년 12월부터 1999년 12월까지 2,556명의 환자에서 사용하였다. 결과 : lU년 2월부터 1999년 12월까지 2,556명의 환자에서 IS,732개의 이미지를 파일로 저장하여 DIC에 이용하였다. 1998년 12월부터 DRRS를 이용하여 120명의 환자에서 종래의 치료 기록지를 대체해서 DRRS에 의존하여 방사선치료 및 치료 중 환자 관리를 이룩할 수 있었다. 치료 완료 후에는 규격화된 치료 기록지를 인쇄해서 보관하고 방사선치료 중 "Paperless" 환경을 이룩함으로써 환자의 원활한 치료 및 자료의 보관이 용이하게 되었다. 결론: DIC와 DRRS를 이용하여 치료기록의 전산화와 데이터베이스 구축하였으며 기존의 방사선치료 기록지를 디지털화 하여서 "paperless" 환경의 치료에 기여하였다. Background :The authors have developed a Digital image chart(DIC) and digital Radiotherapy Record System (DRRS). We have evaluated the DIC and DRRS for reliability, usefulness, ease of use, and efficiency. Materials and Methods :The basic design of the DIC and DRRS was to build an digital image database of radiation therapy Patient records for a more efficient and timely flow of critical image information throughout the department. This system is a submit of comprehensive radiation oncology management system (C-ROMS) and composed of a picture archiving and communication system (PACS), a radiotherapy information database, and a radiotherapy imaging database. The DIC and DRRS were programmed using Delphi under a Windows 95 environment and is capable of displaying the digital images of patients identification photos, simulation films, radiotherapy setup, diagnostic radiology images, gross lesion Photos, and radiotherapy Planning isodose charts with beam arrangements. Twenty-three clients in the department are connected by Ethernet (10 Mbps) to the central image server (Sun Ultra-sparc 1 workstation). Results :From the introduction of this system in February 1998 through December 1999, we have accumulated a total of 15,732 individual images for 2,556 patients. We can organize radiation therapy in a 'paperless' environment in 120 patients with breast cancer. Using this system, we have succeeded in the prompt, accurate, and simultaneous access to patient care information from multiple locations throughout the department. This coordination has resulted in improved operational efficiency within the department. Conclusion :The authors believe that the DIC and DRRS has contributed to the improvement of radiation oncology department efficacy as well as to time and resource savings by providing necessary visual information throughout the department conveniently and simultaneously. As a result, we can also achieve the 'paperless' and 'filmless' practice of radiation oncology with this system.

        불완전 방사선치료 환자의 분석

        목적 : 본 연구는 계획된 방사선치료의 과정 도중에 방사선치료를 완료하지 못하는 환자들의 빈도를 조사하고 그 이유를 분석하고자 하였다. 대상 및 방법 :삼성서울병원 치료방사선과에서 방사선치료를 받은 환자 중에서 1996년 4월부터 1997년 5월까지 방사선치료를 시행 받은 1,100명의 환자들을 대상으로 하였다. 대상 환자들 중에서 최초 계획 방사선량의 95$\%$ 이하를 받은 경우를 불완전 방사선치료로 정의하여 그 빈도와 이유를 조사하였다. 결과 : 불완전 방사선치료의 빈도는 1,100명 중 128명으로 12$\%$였다. 불완전 방사선치료 환자들의 운동수행능력은 전체 환자들에 비하여 불량하였으며, 방사선치료의 목적은 고식적 목적이 많았다. 불완전 방사선치료의 가장 흔한 이유는 환자의 거부였으며 이는 주로 환자의 방사선치료에 대한 불신과 경제적 부담 때문이었다. 결론 : 불완전 방사선치료의 빈도를 줄이기 위해서는 방사선치료의 임상적 적응 여부는 물론 환자의 사회 경제적인 여건을 잘 고려하여 방사선치료의 대상 환자를 선정하는 것이 요망되며 이는 특히 고식적 방사선치료의 경우에 더욱 중요하다고 할 수 있겠다. Purpose : This study is to see how much proportion of the patients receiving radiation therapy drop out during radiation therapy and to analyze the reason for the incomplete treatment. Materials and Methods : The base population of this study was 1,100 patients with registration numbers 901 through 2,000 at Department of Radiation Oncology, Samsung Medical Center, Seoul, Korea. Authors investigated the incidence of incomplete radiation therapy, which was defined as less than 95$\%$ of initially planned radiation dose, and the reasons for incomplete radiation therapy. Results : One hundred and twenty eight patients (12$\%$) did not complete the planned radiation therapy. The performance status of the incompletely treated patients was generally Poorer than that of the base population, and the aim of radiation therapy was more commonly palliative. The most common reason for not completing the planned treatment was the patients' refusal of further radiation therapy because of the distrust of radiation therapy and/or the poor economic status. Conclusion : Careful case selection for radiation therapy with consideration of the socioeconomic status of the patients in addition to the clinical indication would be necessary for the reduction of incomplete treatment, especially in the palliative setting.

        자궁 경부암 고선량율 강내조사 치료의 국내 현황과 적정 치료방법

        Brachytherapy is an essential part of radiotherapy for uterine cervical cancer. The low dose rate (LDR) regimen has been the major technique of intracavitary therapy for cervical cancer. However, there has been an expansion in the last 20 years of high dose rate (HDR) machines using Ir-192 sources. Since 1979, HDR brachytherapy has been used for the treatment of uterine cervical cancer in Korea. The number of institutions employing HDR has been increasing, while the number of low dose rate system has been constant. In 1995, there was a total 27 HDR brachytherapy units installed and 1258 cases of patients with cervical cancer were treated with HDR Most common regimens of HDR brachytherapy are total dose of 30-39 Gy at point A with 10-13 fractions in three fractions per week. 24-32 Gy with 6-8 fractions in two fractions per week, and 30-35 Gy with 6-7 fractions in two fractions per week. The average fractionation regimen of HDR brachytherapy is about 8 fractions of 4.1 Gy each to Point A. In Korea, treatment results for HDR brachytherapy are comparable with the LDR series and appears to be a safe and effective alternative to LDR therapy for the treatment of cervical carcinoma. Studies from the major centers report the five-year survival rate of cervical cancer as. 78-86$\%$ for Stage 1, 68-85$\%$ for stage 11, and 38-56$\%$ for Stage III. World-wide questionnaire study and Japanese questionnaire survey of multiple institutions showed no survival difference in any stages and dose-rate effect ratio (HDR/LDR) was calculated to be 0.54 to 0.58. However the optimum treatment doses and fractionation schemes appropriate to generate clinical results comparable to conventional LDR schemes have yet to be standardized. In conclusion, HDR intracavitary radiotherapy is increasingly practiced in Korea and an effective treatment modality for cervical cancer. To determine the optimum radiotherapy dose and fractionation schedule, a nation-wide prospective study is necessary in Korea. In addition, standardization of HDR application (clinical, computer algorithms, and dosimetric aspects) is necessary.

        자궁경부암 IIIB 기의 방사선치료 성적

        From September 1985 through September 1989,56 patients with stage IIIB carcinoma of the cervix were treated with radiation therapy with curative aim. The overall survival at 5 year was $38{\%}$. The survival rate was better for patients treated with combined external radiotherapy and high dose rate intracavitary radiotherapy than with external radiotherapy alone. No significant survival difference was observed between the unilateral and bilateral parametrial extension of the tumor Seventeen patients experienced recurrence within the irradiated field with a loco-regional recurrence rate of $30{\%}$. Ten patients had complications ($18{\%}$). The complications were mild in three, moderate in four, and severe in three patients. A study was made on the relationship between the fraction numbers of intracavitary radiotherapy, vaginal packing and the complication rate, respectively. In this analysis author observed that the significant treatment factor influencing the survival of cervical cancer was the use of intracavitary radiation, and meticulous vaginal packing could decrease the late complication rate of radiotherapy of cervical cancer.

        Accuracy of Dose Estimation in High Dose Rate Intracavitary Radiotherapy of Carcinoma of the Uterine Cervix

        자궁경부암의 고선량율 강내치료의 매 분할 치료시마다 강내 applicator의 위치 차이로 인하여 조사 선량이 치료전 계획된 선량과 차이가 있을 수 있다. 저자는 자궁경부암 환자의 강내치료시 치료전 계획된 A점 선량 500 cGy와 치료직후에 촬영한 전후 및 측면 선 사진을 이용하여 계산된 A점 선량을 비교한 결과, 82예의 강내치료에서 치료 후 계산된 조사 선량과 계획 선량은 $500\pm18 cGy$이었으며 $84\%$에서 $500\pm25 cGy$의 범위에 포함되었다. 이러한 결과에서 계획 선량과 치료 후 계산된 조사 선량사이에 비교적 높은 일치율을 확인할 수 있었다. In brachytherapy of uterine conical cancer using a high dose rate remote afterloading system, it is of prime importance to deliver a accurate dose in each fractionated treatment by minimizing the difference between the pre-treatment planned and post-treatment calculated doses. The post-treatment calculated point A dose was not much different from the pretreatment planned dose (500 cGy). The $average{\pm}standard$ deviation was $500\pm18cGy$ and 84 percent of 82 intracavitary radiotherapy was within the range of $500\pm25cGy$.

        골반 방사선 치료후의 골 변화와 손상

        방사선치료후 발생할 수 있는 골 손상은 골반 부전골절, 방사선 골 괴사, 무혈성 괴사, 골 육종 등이다, 특히 최근 방사선치료후의 골탄성도의 변화로 발생하는 골반 부전골절에 관하여 많은 보고가 있었다. 과거 방사선치료 후의 골 손상 특히 골절의 빈도는 선형가속기를 이용한 메가볼트 방사선치료 바뀌면서 현저히 줄어 들어서 매우 낮은 것 으로 보고 되었으나 최근에는 8∼20%로 보고가 되고 있다. 따라서 골반 부전골절의 임상적 의미와 중요성과 예방 및 치료에 대하여 관심이 증가되고 있다. 저자는 자궁경부암 치료 후 관찰한 골반 부전골절, 골 괴사, 무혈성 괴사, 및 2차성 골육종등 골반골 변화와 손상에 관하여 저자의 경험과 문헌고찰을 통하여 보고 하고자 한다. There has been recent interest in radiation-induced bone injury in clinical conditions, especially for pelvic insufficiency fracture (PIF). A PIF is caused by the effect of normal or physiological stress on bone with demineralization and decreased elastic resistance. Pelvic radiotherapy (RT) can also contribute to the development of a PIF. A PIF has been regarded as a rare complication with the use of megavoltage equipment. However, recent studies have reported the incidence of PIFs as 8.2∼20% after pelvic RT in gynecological patients, an incidence that was higher than previously believed. The importance of understanding a PIF lies in the potential for misdiagnosis as a bony metastasis. If patients complain of pelvic pain after whole-pelvis radiation therapy, the presence of a PIF must be considered in the differential diagnosis. The use of multibeam arrangements and conformal RT to reduce the volume and dose of irradiated pelvic bone can be helpful to minimize the risk of fracture. In addition to a PIF, osteonecrosis and avascular necrosis of the femoral head can develop after radiation therapy. Osteoradionecrosis of the pelvic bone is a clinical diagnostic challenge that must be differentiated from an osseous metastasis. A post-radiation bone sarcoma can result as a long-term sequela of pelvic irradiation for uterine cervical cancer.

        21세기 방사선종양학의 전망 : 최근의 진보와 한국에서의 발전

        최근의 방사선치료는 치료의 질을 향상시켜서 치료 후의 삶의 질을 높이는 것을 목표로 하고 있다. 국 내에서도 방사선치료 환자와 치료 시설의 빠른 증가로 방사선종양학 분야는 많은 발전이 되고 있으며, 치료기술 또한 3-dimensional conformal radiotherapy의 보편화, linac based stereotactic radiosurgery의 활 발한 적용과 intensity modulated radiation therapy (IMRT)의 도입 준비 등 고난도의 치료 기술도입이 시 도되고 있다. 저자는 최근 20년간 한국에서의 방사선 종양학의 발전을 조망하고 최근 발전되는 4차원 적 방사선치료, IMRT의 현황, biological conformality치료의 개념, 항암제와 방사선치료의 병용에 대하여 살펴보고 최근 정보기술 혁명에 따른 인터넷과 방사선종양학 분야의 정보관리 시스템의 중요성 및 원 격진료의 세계적 현황 등에 대해서 알아보고, 21세기 한국에서의 방사선치료의 질을 올리기 위한 방법 들을 제시하였다. 이들은 1) QA (quality assurance) 향상, 2) 공동 프로토콜에 의한 3상 임상 연구의 필 요성, 3) 특정 암에 대한 통일된 치료 프로토콜 또는 가이드라인, 4) 전국적인 방사선종양학 관련 자료의 광역 data base구축과 중요 암에 대한 patterns of care study 등 시행의 필요성이다 The objective of recent radiation therapy is to improve the quality of treatment and the after- treatment quality of life. In Korea, sharing the same objective, significant advancement was made due to the gradual increase of patient number and rapid increase of treatment facilities. The advancement includes generalization of three-dimensional conformal radiotherapy (3D-CRT), application of linac-based stereotactic radiosurgery (SRS), and furthermore, the introduction of intensity modulated radiation therapy (IMRT). Authors in this paper prospectively review the followings: the advancement of radiation oncology in Korea, the recent status of four-dimensional radiation therapy, IMRT, the concept of the treatment with biological conformity, the trend of combined chemoradiotherapy, the importance of internet and radiation oncology information management system as influenced by the revolution of information technology, and finally the global trend of telemedicine in radiation oncology. Additionally, we suggest the methods to improve radiotherapy treatment, which include improvement of quality assurance (QA) measures by developing Koreanized QA protocol and system, regional study about clinical protocol development for phase three clinical trial, suggestion of unified treatment protocol and guideline by academic or research societies, domestic generation of treatment equipment's or system, establishment of nationwide data base of radiation-oncology-related information, and finally patterns-of-care study about major cancers.

        자궁경부암 환자의 근치적 방사선치료성적

        목적 : 삼성서울병원 치료방사선과에서 근치적 목적으로 외부조사와 고선량율 강내치료를 이용하여 치료한 자궁경부암 환자의 치료 성적을 분석하고자 하였다. 대상 및 방법 : 1994년 9월부터 1998년 7월까지 근치적 목적으로 방사선치료를 시행한 106명의 환자를 대상으로 분석하였으며, 환자의 연령분포는 22~89세(중앙값, 61세)이었다. 98명의 환자에서 편평상피암이었다. 환자의 FIGO 병기는 CIS 4명, IA 4명, IB 17명, IIA 15명, IIB 33명, IIIA 2명, IIIB 27명, IVA가 4명이었다. ECOG 활동도는 88명에서 1이하였다. 11명의 환자에서 전보조화학요법이 시행되었다. 방사선치료는 상피내암인 4명을 제외한 102명에서 30.6-50.4 Gy를 외부 조사하였으며 고선량율 강내치료를 모든 환자에서 A점 기준으로 24 Gy/6회 시행하였다. 치료 예후 인자는 연령(≤60세 vs >60세), 병리조직학적 소견(편평상피암 vs 기타 병리), FIGO 병기(IIA 이하 vs IIB vs IIIA 이상), ECOG 활동도(ECOG 0, 1 vs 2), 항암화학요법의 시행여부, 그리고 방사선치료 후 반응정도(완전관해 vs 부분관해), 방사선치료기간(≤55일 vs >55일)에 따라 비교하였다. 환자의 추적관찰기간은 6~66개월(중앙값, 28개월)이었다. 결과 : 전체 3년 및 5년 생존율은 각각 82%, 73%이었으며, 무병생존율은 각각 72%, 69%이었다. FIGO 병기별 생존율은 병기 IB, IIA, IIB, 그리고 III에서 3년 생존율이 각각 100%, 83%, 87%, 62%이었고 5년 생존율은 IB 100%, IIA 69%, IIB 80%, III에서는 62%이었다. 단변량분석에 따른 예후인자를 살펴보면 전체생존율에서는 FIGO 병기와 방사선치료의 반응이 무병생존율과 골반부 조절율에는 나이와 병기, 방사선치료의 반응, 방사선치료기간이 의미 있는 인자로 확인되었다. 방사선치료 부작용으로 직장 출혈은 모두 14명(13%)의 환자에서 나타났다. 결론 : 자궁경부암 환자의 고선량율 강내치료와 외부 방사선치료는 효과적인 치료방법임을 확인하였으며, 치료기간의 단축이 중요한 예후 결정 요인임을 확인할 수 있었다. Purpose : This study was conducted to evaluate the treatment results, prognostic factors, and complication rates after high dose rate (HDR) brachytherapy in patients with uterine cervix cancer who were treated with curative aim. Materials and Methods : Of 269 cervix cancer patients treated at the department of radiation oncology, Samsung Medical Center from September 1994 to July 1998, the 106 who were treated with radical radio-therapy were analyzed. The median age was 61 years (range 22 to 89). All patients except 4 with carcinoma in situ (CIS) were given external beam radiotherapy (range 30.6~50.4Gy to whole pelvis) and HDR brachytherapy. The common regimens of HDR brachytherapy were a total dose of 24~28Gy with 6~7 fractions to point A at two fractions per week. The median overall treatment time was 55 days (range 44 to 104) in patients given both external beam radiotherapy and HDR brachytherapy. Results : Early responses of radiotherapy were evaluated by gynecologic examination and follow-up MRI 1 month after radiotherapy. Treatment responses were complete remission in 72 patients, partial response in 33 and no response in 1. The overall survival (OS) rate of all patients was 82%, and 73%, and the disease free survival (DFS) rate was 72%, and 69%, at 3, and 5 years, respectively. The pelvic control rate (PCR) was 79% at both 3 and 5 years. According to the FIGO stage,3 and 5 year OS were 100% and 50% in CIS/IA, 100% and 100% in IB, 83% and 69% in IIA, 87% and 80% in IIB, and 62% and 62% in III, respectively. The 3 year OS in 4 patients with stage IVA was 100%. Three-year DFS were 80% in CIS/IA, 88% in IB, 100% in IIA, 64% in IIB, 58% in III, and 75% in IVA. Three-year PCR were 100% in CIS/IA, 94% in IB, 100% in IIA, 84% in IIB, 69% in III, and 50% in IVA. By univariate analysis, FIGO stage and treatment response were significant factors for OS. The significant factors for DFS were age, FIGO stage, treatment response and overall treatment time (OTT). For pelvic control rate, treatment response and OTT were significant factors. By multivariate analysis, FIGO stage had a borderline significance for OS (p=0.0825) and treatment response had a borderline significance for DFS (0=0.0872). A total of 14 patients (13%) experienced rectal bleeding, which occurred from 3 to 44 months (median, 13 months) after the completion of radiotherapy. Conclusion : HDR brachytherapy protocol of Samsung Medical Center combined with properly optimal external beam pelvic irradiation is a safe and effective treatment for patients with uterine cervix cancer. The authors found that OTT of less than 55 days had a positive impact on pelvic control and survival rate.

        마우스공장 소낭선세포의 방사선 감수성에 관한 실험적 연구

        To determine the radiosensitivity and dose-survival characteristics of jejunal crypt cells, experimental study was done using total 40 mice. Single irradiation of 1,000 rad to 1,600rad was delivered to whole bodies of mice, using a cesium 137 animal irradiator. The number of regenerating crypts per jejunal circumference was counted, by using a jejunal crypt cell assay technique, and dose response curve was measured. The average number of jejunal crypt Per circumference in control group was 140±10. Mean lethal dose(D0) of moose jejunal crypt cell was 135rad.

        원발성 질암의 방사선치료

        A retrospective study was carried out of 20 patients with histologically proven invasivs carcinoma of the vagina from 1979 to 1993; 17 cases with curative primary radiotherapy and three cases with postoperative radiotherapy. The five and ten year overall survival rates were 79% and 53%, respectively. Survival was strongly correlated with stage. Five year survival rates of stage Ⅰ, stage Ⅱ, and stage Ⅲ, Ⅳ were 100%, 78%, and 0%, respectively. Eight patients experienced recurrences; five within the irradiated volume, two distant metastasis, and one combined both local and distant metastasis. there was no significant late complication. From these data, radiation is effective in the management of the vaginal cancer patients and optimum treatment modality and total dose recommendation are made.
