        신세포암의 CT 소견 : 핵등급 및 세포유형의 연관

        Purpose : To evaluate the CT appearance of renal cell carcinoma and to correlate it with nuclear grading andcell type. Materials and Methods : The size, outer margin and heterogeneity of inner texture of renal cellcarcinomas in 86 patients were evaluated on CT scan and were correlated with nuclear grade and cell type. Results: Tumors less than 5cm were of low grade in 28 of 31 patients, while those larger than 5cm were of low grade in 28of 55 patients. The lesions which showed no or round protrusion with a sharp margin were of low grade in 26 of 28patients, while those which showed an undulated or indistinct margin were of low grade in 30 of 58 patients.Lesions which showed a homogeneous solid appearance, focal low densities, or mostly cystic changes were of lowgrade in 23 of 26 patients, while those which showed multiple or confluent low densites were low grade in 33 of 60patients. There was a significant difference in the distribution of nuclear grading between the groups.Differences in CT apperance according to cell type were not found. Conclusion : Differences in less than 5cmshowing no or round protrusion with a sharp margin, homogeneous inner texture with solid appearance, andheterogeneous inner texture with cystic appearance or focal low densities on CT scan were thought to be thefindings suggesting low nuclear grading.

        Purpose : To evaluate the CT appearances of renal cell carcinoma and correlate them with the nuclear grading and cell type which had been reported as important long term prognosticators. Materials and Methods : Retrospective review of contrast enhanced CT scans was performed in 86 cases of palhologically confirmed renal cell carcinoma. The size, outer margin, and heterogeneity of inner texture of the tumor were evaluated by CT and the distribution of those patterns according to nuclear grade and cell type was compared. The size of the neoplasm was determined by measuring the maximum axial diameter. The shape of the outer margin was classified as sharp or indistinct. The sharply marginated tumors were subclassified as no protrusion, round protrusion, and undulated protrusion. The inner texture was classified as homogeneous solid and heterogeneous containing low density areas. Lesions of the latter type were subclassified as having focal low densities, scattered or confluent low densities, and nearly cystic appearance. Results : The size and outer margin of the tumor were closely correlated with nuclear grade(p<0.01). The tumors less than 5 ㎝ in diameter were of low nuclear grade in 28 of 31 patients. However, the tumors more than 5 ㎝ in diameter were low grade in 28 and high grade in 27 patients. There was a statistically significant difference in distribution of nuclear grading between the two groups (p=0.0002). The lesions which showed no protrusion or round protrusion with sharp margin were low grade in 26 of 28 patients. The lesions which showed undulated or indistinct margin were low grade in 30 and high grade in 28 patients. There was a statistically significant difference in distribution of nuclear grading between the two groups (p=0.0008). The extent of low densities and nuclear grade were not closely correlated(p>0.05). However, the lesions which showed homogeneous solid appearance, focal low densities, or almostly cystic appearances were low grade in 23 of 26 patients while the lesions which showed multiple or confluent low densities were low grade in 33 and high grade in 27 patients. There was a statistically significant difference in distribution of nuclear grading between the two groups(p=0.002). No difference in CT apperances of renal cell carcinoma according to cell type was found. In two patients with sarcomatoid-type lesions, the tumors were more than 5 cm in size with indistinct margin and multiple low densities, and the nuclear grading of lesions was nuclear grade 4. Conclusion : The size and outer margination of renal cell carcinoma correlated most closely with nuclear grading; heterogeneity of inner texture was also correlated. Tumor size of less than 5 cm, no protrusion or round protrusion with sharp margin, homogeneous inner texture with solid appearance, and heterogeneous inner texture with cystic appearance or focal low densities appear to be CT findings that predict low nuclear grading.

        소규모 전원장치를 이용한 다채널 자기 신경자극 시스템

        자기 신경자극기는 신경의학, 재활의학을 포함한 많은 임상의학 분야에서 널리 쓰이고 있다. 자기 신경자극기는 전력소모가 크고 고압 전원이 필요한 이유 때문에 단채널 시스템만이 임상환경이나 연구환경에서 쓰여 왔었다. 본 논문에서는 4채널 자기 신경자극기 개발을 소개하였다. 고압 커패시터를 충전하는데 있어서 단계적 충전방식을 사용하여 작은 크기의 직류 전원장치로 다채널 시스템을 구현하였고, 채널별로 서로 다른 자극 프로토콜로 자극 코일을 구동할 수 있는 다채널 제어시스템을 구성하였다. 개발된 시스템은 신경계통 장애가 있는 환자들의 재활이나 자기적인 침술 등 다양한 기능적 자기 자극에 활용될 수 있을 것으로 기대된다.

        자계 거울 효과를 이용한 신경 자극 코일

        시변 자계를 이용하여 비접촉적인 방법으로 신경을 자극하기 위해서는 신경자극코일에 대용량 전류 펄스를 순시적으로 인가해야만 한다. 신경자극코일에 인가하는 전류 펄스의 주파수를 증가할수록 신경자극코일에 전력을 공급하는 전력공급기의 용량이 커지게 되고 신경자극코일에 발생하는 열량도 커지는 문제가 발생한다. 본 논문에서는 이러한 문제를 완화하기 위해 새로운 형태의 신경자극코일을 제안하였다. 신경자극코일에 강자성체 구조물을 부착하여 자계 거울 효과를 내게 하면 신경자극코일의 효율이 증가하게 된다. 다양한 형태의 신경자극코일에 대해 자계 거울 효과를 유한요소법으로 분석하였고 몇 종류의 신경자극코일에 대해 실험한 결과를 제시하였다. For non-contact nerve stimulations using time varying magnetic field, high amplitude current pulses have to be applied to a magnetic nerve stimulation coil. To increase the magnetic stimulation frequency we have to increase both power supply capacity and cooling capacity of the magnetic nerve stimulator. To alleviate these problems. we propose a new magnetic nerve stimulation coil design methods. Utilizing magnetic mirror effect of a ferro-magnetic plate attached to a magnetic stimulation coil. we have improved efficiency of the stimulation coil. We have analyzed magnetic mirror effect for various kinds of stimulation coils using the finite element method, and we present experimental results obtained with several kinds of stimulation coils.
