The association of classic Kaposi's sarcoma with other neoplasm(primary or secondary) may serve as a clinical indicator for altered immunologic surveillance mechanisms. The association have been discussed for several decades. But the association of Kaposi's sarcoma with pancreatic cancer has not been reported. We report a case of Kaposi's sarcoma associated with pancreatic cancer in a 79-year-old man, who had tender papules and plaques on the left hand. The histopathologic findings showed typical features of Kaposi's sarcoma as a dermal tumor composed of vascular proliferations, vascular slits, spindle cells, extravasated erythrocytes and deposits of hemosiderin. The skin functions to eliminate foreign substances through the epidermis to epidermal surface; this process is called transepidermal elimination(TEE). TEE has been observed in various skin tumors. A 71-year-old woman presented with a well-circumscribed, red-brownish, subcutaneous nodule, 1.3×1.0 cm in size, located on the left thigh. The histopathologic finding of the lesion shows trichilemmal cyst with transepidermal elimination. (Korean J Dermatol 2003;41(7) : 959~961)
Correlates of Bone Mineral Density and Sagittal Spinal Balance in the Aged

Objective To investigate the relationship between bone mineral density (BMD) and sagittal spinal balance in the Korean elderly population. Methods The retrospective study included subjects aged 60 years and above, who had whole-spine lateral radiography and dual-energy X-ray absorptiometry (DEXA) within a year’s gap between each other. Sagittal vertical axis (SVA) for evaluation of sagittal spinal balance and five spinopelvic parameters were measured through radiography. The presence of compression fracture was identified. Correlations of BMD T-scores with SVA and with the spinopelvic parameters were assessed using Pearson correlation coefficient (PCC). Linear regression analyses were performed between SVA and the clinical and radiologic variables. Results One hundred twenty-two subjects (42 males and 80 females; mean age, 69.93±5.5 years) were included in the study. BMD, femur or spine, was not correlated with SVA or any spinopelvic parameters in both genders (PCC<±0.2), except that spine BMD in men was associated with sacral slope. Univariate regression analysis revealed association between SVA and lumbar lordosis, pelvic tilt, and compression fractures in both genders; it was also associated with age and pelvic incidence in females and with sacral slope in males. Multivariate linear regression model showed lumbar lordosis and compression fracture as variables affecting SVA in both sexes; pelvic incidence was another factor affecting SVA in women only.Conclusion BMD was not associated with sagittal spinal balance in the aged. Sagittal spinal balance was explained partly by lumbar lordosis and compression fracture. Further study is warranted to understand progression of sagittal imbalance with age.

Both synovial cysts and synovial chondromatoses originate from synovial tissue, and only one case has been reported to occur simultaneously in a patient. Our patient presented with a swelling of the right preauricular area and pain on opening the mouth. Magnetic resonance images demonstrated two cystic structures in the superficial lobe of right parotid gland. Under the clinical impression of a cyst in the salivary gland, an enucleation was performed. However, the final diagnosis based on microscopic examination was a synovial cyst associated with the right temporomandibular joint and a synovial chondromatosis arising within the cystic wall. Awareness of this combined lesion will help make a diagnosis and establish a proper treatment plan.

Zygomycosis and aspergillosis are representative filamentous fungal infections in the sinonasal region. However, their combined form seldom occurs. This report describes one case of zygomycosis and aspergillosis occurring in a patient that did not have diabetes or any other immunocompromising conditions. The patient presented with a history of intermittent swelling and pus discharge from the gingiva of the right maxillary posterior teeth. The radiographs demonstrated a periapical radiolucency around the right maxillary first molar, together with mild haziness and mucosal thickening of the right maxillary sinus. U nder t he c linical impression o f odontogenic sinusitis, a Caldwell-Luc operation was performed. However, the final diagnosis based on microscopic examination was combined zygomycosis and aspergillosis. Previous studies of zygomycosis occurring alone describe a very aggressive clinical course, including necrosis of tissue,cranial nerve involvement and death. Our patient did not show any of these symptoms, and recovered after surgical debridement only. It can be concluded from the present case that the prognosis of fungal infections depends on the immune status of the host, rather than behavior of the fungi.
