        한국판 프렌차이 실어증선별검사의 개발을 위한 정상성인의 표준화 연구

        Objective: To develop Korean version of Frenchay Aphasia Screening Test (K-FAST) and to standardize K-FAST in normal adult population in Korea. Method: The ‘river scene’ of stimulus set in original version of FAST was adapted for K-FAST. English version of the test instructions and scoring methods were translated into Korean and reverse-translation was performed by English- Korean bilinguals. The test structures and language domains (comprehension, expression, reading, and writing) were unchanged and possible maximum score was 30 points. We standardized K-FAST in 240 normal adult populations (male 102, female 138) whose ages were above 44. Basic personal information was collected through an interview and we performed Edinburgh handedness inventory (EHI), K- MMSE and K-FAST. Subjects who had history of brain disease, cognitive communicative disorders, or K-MMSE scores less than 2 percentile of same age group were excluded. K-FAST scores were analyzed according to the age and education groups. Results: Mean EHI, K-MMSE, and K-FAST scores in total subjects were 9.4±1.2, 25.9±2.8, 25.4±3.3 points, respectively. Post-hoc analysis of K-FAST scores according to age groups classified into 3 age groups, 45∼64, 65∼74, and ≥74 years and education groups into 0, 1∼9, ≥10 years of total education. K-FAST scores decreased significantly as increase of age (r=−0.441, p=0.000) and decrease of total years of education (r=0.580, p=0.000). Conclusion: Newly adapted K-FAST can be used for screening of aphasia in Korea and the standardized data according to age and education levels may provide useful reference values for interpretation of the results of K-FAST.       • 명칭성 실어증( Anomia ) 환자에서 보이는 단어 산출 과정의 단원적 구조

        본 연구는 명칭 실어증 환자의 단어 산출 과정을 통해 단어 산출과 관련된 다음과 같은 문제를 조사하기 위해 실시되었다: (1) 개념 정보 처리 구조와 어휘 정보 처리 구조가 분리되어 있는가? (2) 개념 구조는 단원적인가? (3) 외국어인 영어 어휘 구조는 한글의 어휘 구조와 어떤 관련성을 가지고 있는가? 본 실험은 좌측 뇌경색으로 명칭 실어증이 발생한 남자 환자를 대상으로 시행하였다. 실험은 그림 명명 과제(picture naming task), 단어 명명 과제(word naming task), 의미범주 판단과제(semantic categorization task), 어휘 판단 과제(lexical decision task) 등의 방법으로 진행하였고 한글로 검사한 후 일정한 시간이 경과한 뒤 영어로 검사를 시행하였다. 그림 명명 과제에 사용된 그림은 모두 98개였는데, 환자는 이 중 30개(30.6%)를 명명할 수 있었다. 명명에 실패한 68개의 그림 중에서 그림과 관련된 단어의 정의, 유의어 및 반의어, 단어의 첫 음절을 단서로 제공한 후 검사하였는데 다른 단서를 제공한 경우에는 그림 명명에 도움이 되지 않았고 첫 음절을 단서로 제공했을 경우에 ???????? 실수를 보여 의미 정보처리에는 큰 문제가 없는 것으로 나타났으며, 단어 명명 과제에서도 모두 정확한 반응을 보여 소리내어 읽기에도 문제가 없는 것으로 나타났다. 또한, 어휘 판단 과제에서도 11개 단어에서만 실수를 보여 어휘 시스템에도 큰 문제가 없는 것으로 해석된다. 따라서, 명칭 실어증 환자 SDK에서 나타나는 그림 명명의 어려움이 의미 시스템, 어휘 시스템, 단어 음운 부호 생성 및 발성 등의 문제보다는 의미 시스템과 어휘 시스템간의 연결과정의 손상에 의한 것으로 추정된다. 또한 의미 범주에 따라 개념구조가 단원적(modular structure)으로 구성되어 있는가를 조사한 실험에서는 실수 비율 분석과 의미 범주화 판단 시간에서 인공물과 자연물이 단원적으로 구성되어 있는 것으로 나타났다. 모국어인 한국어의 어휘 시스템과 외국어인 영어 어휘 시스템간의 분리 가능성에 대한 조사에서는 실수의 형태로 보아서 두 언어 시스템이 일부는 중첩되고 나머지는 독립적인 형태로 존재한다는 가설이 지지되는 것으로 나타났다. This study was performed to investigate the problems associated with word production in an anomic aphasic. The questions were as follows: 1) Are the structure of the semantic information system separated from the lexical system? 2) Does the semantic system have a modular structure? 3) What's the difference and relationship between Korean and English in word production process? We examined a right-handed male patient who had been diagnosed as an anomic aphasia by Western Aphasia Battery (WAB) after cerebral infarction on the left hemisphere. Four tasks, such as picture naming, word naming, semantic categorization, and lexical decision task were performed. We used ninety eight pictures which were consisted of forty eight natural things and fifty artificials. After completion of four tasks with Korean, same tasks excluding the semantic categorization were performed through English. And the results were compared between two languages. In picture naming task, the patient named correctly in thirty (30.6%) out of the ninety eight pictures. After given verbal cues, the patient answered correctly in forty five more pictures (45.9%). Most effective cue was initial phonemes or syllables of the target name. In word naming task, the patient read all the names correctly. In case of semantic categorization and lexical decision task, the error rate was only 5.1% and 11.2%, respectively. And picture naming performance differed in the natural and artificial category members. Furthermore, some naming errors in English were overlapped with the naming errors in Korean, but the other naming errors in English occurred only in English naming. In conclusion, these results were suggestive of the possibility of disruption at the connection between lexical and semantic systems in anomic aphasia. And the semantic system may have modular structure, such as artificial versus natural things as shown in this study. The test results performed by English revealed that the word production system of the two languages had both the common and independent areas.

        Anatomical and Electrophysiological Myotomes Corresponding to the Flexor Carpi Ulnaris Muscle

        This study was designed to investigate the incidence of lateral root of the ulnar nerve through cadaveric dissection and to analyze its impact on myotomes corresponding to the flexor carpi ulnaris (FCU) assessed by electrodiagnostic study. Dissection of the brachial plexus (BP) was performed in 38 arms from 19 cadavers, and the connecting branches between the lateral cord and medial cord (or between lateral cord and ulnar nerve) were investigated. We also reviewed electrodiagnostic reports from January 2006 to May 2008 and selected 106 cases of single-level radiculopathy at C6, C7, and C8. The proportion of abnormal needle electromyographic findings in the FCU was analyzed in these patients. In the cadaver study, branches from the lateral cord to the ulnar nerve or to the medial cord were observed in 5 (13.1%)of 38 arms. The incidences of abnormal electromyographic findings in the FCU were 46.2% (36/78) in C7 radiculopathy, 76.5% (13/17) in C8 radiculopathy and 0% (0/11)in C6 radiculopathy. In conclusion, the lateral root of the ulnar nerve is not an uncommon anatomical variation of the BP and the FCU commonly has the C7 myotome. Needle EMG of the FCU may provide more information for the electrodiagnosis of cervical radiculopathy and brachial plexopathy.
