두개골 외판을 이용한 안와저파렬골절(眼窩底破裂骨折)의 재건

Orbital implants used in repairing the blowout fracture must have some physical properties to restore the contour and dimensions of the orbit, and to provide some indirect support for the glove. Also, they should provide a physiologically accepttable and physically inert smooth surface which will not form adhesions. Oculoplastic surgerns have been used many materials for the reconstruction of the orbital floor including alloplastic implants, autogenous bone and autogenous cartilage, but any of them was not choosen as the most proper implant in repairing the blowout frature owing to various disadvantages. Authors have used outer-table calvarial bone segments preserving the periosteum in the reconstruction of blowout fracture. We have implanted this material in 13 patients for last 2 years. Satisfactory functional and cosmetic results were achieved in 12 patients without noticeable complications. One patient showed slight undercorrection of the preoperative enopthalmos, but the patient satisfied after second operation for the reimplantion of another outer-table calvarial bone segment. No patient complained remnant diplopia or other major complications including infection and displacement of implant. Authors believe that the outer-tabe calvarial bone segment is the ideal implant for use in repairing the blowout fracture. In this paper we would like to present the operative technique, the advantages of the implant and related bibliographical review .

Lymphangioma is a benign, growth of lymphatic tissue that is present at birth or develops in early childhood, which may cause serious alterations in growth and developmemt. The problems with facial lymphangioma is usually releated directly to their size and to the area of the face which is involved. The lesions themselves may range from small, localized blemishes to huge facial masses involving both soft tissue and underlying bone and causing great distortion and asymmetry. The facial bones are seldom involved, but the natutal evolution of an individual lesion often cannot be accurately predicted when the child is first seen. Any changes in the underlying facial bone could be due either to a direct growth of the lesion into the bone, or secondary to pressure of the lesion growing outside the bone itself. A case of cystic lymphangioma extending from the neck to the tongue is reported. A six-year-old female was admitted because of swelling of the tongue. At that time, the tongue reportedly reached the extraoral size of 7x5x2.5cm and a soft, diffuse swelling of left anterior neck was revealed. The removal of cystic mass including left neck dissection and partial glossectomy were undertaken. The another case of lymphangioma is located on mandibular cheek. A twenty nine-year-old male was admitted because of palpable mass of the left mandibular area and fissure of palate. The radical excision of mass with mandibulectomy of body were undertuken. Thus we reported such a rare case and reviewed the lymphangioma.

Since the frontal area is the most cosmetically obvious, a slight distortion of symmetry or deperssion renders gross changes in appearance and is a significant handicap for the pat-ients. Frontal depressions or bons defects afther trauma or operation are not only a cos-metically significance, but also aprotective role for brain. We have experienced 5cases of the reconstructions for frontal depressions with the ope-rations using autogenous bone grafts, dermofat grafts, methyl methacryate contouring or parascapular free flap.The results were satisfactory escept one, absorption of grafted bon-e. We also preferrde methyl methacrylate in the cases of no cranial bone defect or large cranial bone defect and autogenous bone grafts in small cranial bone defects.

Neurofibromatosis is an autosomal deminant neurocutaneous systemic disease characterized by multiple Cafe-au-lait spots. generalized cutaneous neurofibromatosis. central nervous system tumors. skeletal defects and a myriad of somatic and endocrinologic abnormalities. It occurs in about 1:3000 live births and both sexes are equally affected. From 1989 to 1992, 21 patients who admitted to our department were evaluated in clinical aspects. Then we conclude as follows: 1) The onset was usually before 10 years of age. and the incidence between the sexes was approximately the same. 2) The familial tendency was approximately 48%. 3) The head and neck were the usual sites of the disease. 4) Bone changes occurred in about 24%. 5) Most of the tumor could be removed through early diagnosis to prevent recurrence. bone changes and malignant changes. And at the same time, trough reconstructive surgery, results both functionally as well as aesthetically satisfactory could be achieved.

N/A Background: Human adjuvat disease means the autoimmune disease or autoimmune disease like syndrome developed after plastic surgery using foreign body implantation. After 1st report of HAD by Miyoshi at 1964, a number of cases have been reported especially from Japan. However, there has been no report regarding HAD in Korea. Authors report 12cases of HAD which were experienced recently, Methods: 12patients who represented autoimmune disease or autoimmune disease-like syndrome after plastic surgery using foreign body implantation were evaluated clinically. Results: 1) The mean interval from plastic surgery to onset of HAD was 14.9years. 2) Eight of tweleve patients had undergone rhinoplasty and paraffin was most frequently used. 3) Seven of our patients were given the diagnosis of rheumatoid arthritis and the others were unclassified HAD. 4) Five patients underwent the operation to remove foreign material, Only one was improved. Conclusion: In korea, there has been done a number of plastic surgery using paraffin or silicone, So we have to concern about HAD after plastic surgery using foreign body implantation.

The face lift can be divided into two categories : gravitational and intrinsic changes. Against these changes, face lift surgery has been enhanced by several modifications and innovative techniques. On the one hand, there is a requirement to make the surgery of the aging face more effective and improve the durability. We experienced 152 rhytidectomy cases, from March 1,1980 to February 28, 1992. Among these, recent 41 cases were operated by using authors modified method. 1. Short vertical incision including temporal back-cut. 2. Retrotragal or marginal tragal incision at the preauricular area. 3. Incision just inside the hair line at the posterior neck. 4. Tight gauze packing for the bleeding control and immediate tissue expansion effect. 5. Subcutaneous undermining. 6. Negative pressure suction drain, and simple dressing without compression. In this paper, we would like to report the advantages of our modified method with brief review of the related references.
