        개 비강종양의 자기공명영상학적 평가 1예

        10년령의 잡종견이 2개월 전부터 지속적인 기침, 혈액성 비루 및 개구호홉을 주증으로 내원하였으며 혈액검사, 두부 X-ray촬영 및 두부 MRI촬영을 실시한 결과 빈혈소견과 함께 MRI 촬영에서 두개내강으로 파급되지 않은 비강종양으로 추정되는 큰 mass가 비강쪽에서 발견되었다. 그러나 축주의 요청에 의해 이틀 후 안락사하여 부검 후, 병리조직 검사를 실시한 결과 비강 선편평세포암종임을 확인하였다. 본 증례를 통하여 비강종양과 같이 두경부 병변에 있어서 자기공명영상진단법(MRI)은 고전적 방사선학적 검사법에 비교하여 종괴의 침습성과 파급정도를 정확히 평가함으로써 치료방향의 설정과 그 예후를 평가함에 있어 뛰어난 검사법임을 확인할 수 있었다. Nasal tumors have been reported frequently in dogs and nasal discharge has been the primary presenting complaint in nasal tumor patients. A 10-year-old 7 kg male mixed breed dog was presented to the animal hospital for evaluation of severe bloody nasal discharge. After physical examination, blood examination and X-ray examination, magnetic resonance imaging (MRI) examination was also conducted. Based on MRI nasal tumor which does not extend cranial vault was diagnosed. The nasal tumor was confirmed as adenosquamous cell carcinoma by histopathological examination after necropsy. The invasive characteristics of the neoplasm were documented by MRI. MRI was very valuable diagnostic tool for identifying extracranial and intracranial tumor invasion in a small animal.

        고자장 14.1 T MRI/MRS 시스템에서 양성자 자기공명분광법을 이용한 생체 외 인체피부 보습효과에 대한 연구

        목적: 초고자장 MRI/MRS 장비를 이용하여 인체 피부조직의 고해상도 자기공명영상과 자기공명 스펙트럼을 얻고자 하였다. 또한 수분억제가 제외된 자기공명 스펙트럼을 이용하여 스킨로션(피부보습제)의 피부 보습효과가 측정가능한지 알아 보고자 하였다. 대상 및 방법: Bruker 14.1 T MRI/MRS 장비를 사용하였다. 자기공명영상을 얻기 위하여 Spin Echo 및 Fast Spin Echo 펄스시퀀스를 사용하였고 자기공명 스팩트럼을 얻기 위하여 PRESS 펄스시퀀스를 사용하였다. 결과: 초고자장 MRI/MRS 장비를 사용하여 인체 피부에 대한 고해상도의 자기공명영상과 자기공명 스펙트럼을 획득하였다. 수분 신호가 포함된 스펙트럼 분석을 통하여 스킨로션을 바르기 전에 비하여 스킨로션을 바른 후에 인체 피부조직의 수분 함유량이 상당히 증가함이 나타났다. 결론: 본 연구는 14.1 T 고자장 MRI/WRS 장비에서 얻은 피부 조직에 대한 고해상도 자기공명영상과 자기공명 스펙트럼이 보습효과를 측정하는 데 사용될 수 있음을 시사해 주고 있다. Purpose: We Investigated to achieve high resolution magnetic resonance (MR) Imaging and spectra of human skin in vitro with using a 14.1 T MRI/MRS system, and to evaluate the hydration effect of a moisturizer by measuring the skin's water concentration. Materials and Methods: We used the Brukrer 14.1 T MRI/MRS system with a vertical standard bore that was equipped with a DMX spectrometer gradient system (200 G/cm at a maximum 40 A), RF resonators (2, 5 and 10 mm) and Para Vision software. Spin echo and fast spin echo pulse sequences were employed for obtaining the high resolution MR images. The 3D-localized point resolved spectroscopy (PRESS) method was used to acquire the MR spectra. Results: The high resolution MR images and spectra of human skin in vitro were successfully obtained on a 14.IT system. The water concentration of human skin after applying a moisturizer was higher than that before applying a moisturizer. Conclusions: The present study demonstrated that the high-resolution MR images and spectra of human skin from a high field MRS instrument could be applicable to evaluating the hydration state of the stratum corneum.

        개의 경막외 혈종의 자기공명영상학적 진단

        A 3-year-old female, 5kg, Shih-tzu developed an acute onset of depression, disorientation, hypersalivation, nystagmus after falling down 2 meter height place. In plain skull radiography, there was fracture line in the frontal and parietal bones and next day magnetic resonance imaging examination was performed. Magnetic resonance imaging of the brain was performed with 3.0 Tesla unit. Under general anesthesia, the dog was placed in prone with its head positioned in a birdcage coil. Transverse, sagittal and coronal fast spin echo images of the brain were obtained with the following pulse sequences: T1 weighted images (TR = 560 ms and TE = 18.6 ms) and T2 weighted images (TR = 3500 ms and TE = 80 ms). Magnetic resonance imaging showed epidural hematoma in the left frontal area resulting in compression of the adjacent brain parenchyma. Left lateral ventricle was compressed secondarily and the longitudinal fissure shifted to the right, representing mass effect. The lesion was iso-to slightly hyperintense on T1 weighted image and iso-slightly hypointense signal on T2 weighted image. At necropsy, there was a skull fracture and epidural hematoma in the left frontal area. Magnetic resonance imaging of epidural hematoma is reviewed.

        MRI of Hydrosyringomyelia Combined to Hydrocephalus and Occipital Dysplasia in a Dog

        척수공동증(hydrosyringomyelia)은 척수내의 중심관(central canal)이 확장되는 질환으로 사람에 있어서는 Dandy-walker 증후군이나 Chiari 기형과 같은 선천적인 기형에 의해 발생되거나 감염, 외상 및 종양과 관련되어서도 발생된다. 뇌수두증(hydrocephalus)은 뇌척수액이 뇌실내에 과도하게 축적되는 것을 말하며 후두골 이형성증(occipital dysplasia)은 후두공이 과도하게 위쪽으로 연장되어 열린 질병이다. 척수공동증과 뇌수두증은 컴퓨터단층촬영이나 자기공명영상 검사에 의해 확진되어질 수 있다. 3세령의 수컷 말티즈가 장기간의 발작 증세를 주증으로 내원하였다. 혈액검사상 특이소견은 없었으나 rostrodorsal-caudoventral oblique의 두개골 방사선 검사에서 심한 두개골 이형성증 소견이 확인되었고 열려진 천문을 통한 초음파 검사상 양측 외측 뇌실이 모두 심하게 확장된 것이 확인되었다. 자기공명영상 검사에서 뇌수두증과 척수공동증이 진단되었다. 이뇨제를 중심으로 한 내과적 처치에 반응을 하지 않아 외과적 감압술(ventriculo-peritoneal shunting)을 시행하였고 현재까지 잘 유지되고 있다. 자기공명영상검사법은 뇌수두증과 척수공동증을 진단하고 그에 따른 치료계획을 수립하며 예후를 평가하는데 있어 가장 우수한 진단 기법이다. Hydrosyringomyelia is a dilation of the spinal cord central canal. In human it may be caused by congenital malformations such as Dandy-Walker syndrome and Chiari malformations or may be acquired as a result of infection, trauma or neoplasia. Hydrocephalus is an excessive accumulation of cerebrospinal fluid within the ventricles and occipital dysplasia is the dorsal extension of the foramen magnum. Hydrosyringomyelia and hydrocephalus can be confirmed by computed tomography or magnetic resonance imaging (MRI). A 3-year-old male maltese was presented with a history of long-term seizure. Blood examination was all unremarkable. On rostrodorsal-caudoventral oblique radiograph of the skull showed severe occipital dysplasia. On brain sonography through the persistent fontanelle, severe lateral ventriculomegaly was revealed. MRI examination revealed hydrocephalus and hydrosyringomyelia. Diuretic therapy didn't reduce clinical symptoms and surgical decompression was conducted. The dog responded well with ventriculo-peritoneal shunting. MRI is the most superior modality to diagnose hydrocephalus and hydrosyringomyelia, to plan therapy and to determine the prognosis.

        American cocker spaniel dog에서 발생한 삼첨판 이형성 증례

        A 2-year-old, female, American cocker spaniel dog presented for a 1-year history of severe ascites, exercise intolerance, tachypnea. At that time, she was in an emergency state. First, the dog was stabilized with oxygen therapy. A diagnosis of cardiac problem was made from history, auscultation, radiograph, ECG, and echocardiography. Jugular pulsation was palpated and a harsh, systolic murmur of tricuspid regurgitation was prominent at the right cardiac apex. Tricuspid valve dysplasia (TVD) was confirmed with echocardiography, accompanying enormous myocardial hypertrophy. The clinical signs had been improved for 8 months with careful therapy and periodic abdominocentesis, and ascites was well controlled. The situation, however, became worse quickly in a week because the client did not follow our management schedule. Finally, she died due to dyspnea and shock. After the spontaneous death, necropsy and histopathological examination were performed and when we opened the thorax, a significantly large heart was observed. On histopathological findings, grossly myocardium appeared pale initially, then progressed to yellow and white. Microscopically, there was an extensive hemorrhage along with loss of myocardial striations. Interstitial fibrosis and various degenerative alterations in myocytes were also present.

        양성자 자기공명분광법을 이용한 우울증 동물모델에서의 항우울제 약물 효능 평가

        본 기초연구에서는 강제수영검사(forced swimming test, FST)에 의해 우울증이 유발된 쥐의 해마(hippocampus) 부위에서 뇌 대사물질(brain metabolites)중에 choline 신호의 변화를 관찰하였다. 본 연구의 목적은 우울증이 유발된 쥐에 항우울제인 desipramine-HCl을 투여하여 정상 쥐와 약물을 투여한 쥐 사이의 뇌 대사물질의 차이를 자기공명분광법(magnetic resonance spectroscopy: MRS)을 통하여 알아보는 것이다. 실험대상으로는 6주령의 SD rat을 사용하였으며 우울증을 유발시키고 항우울제의 약물평가를 위한 모델링 방법으로 강제수영검사를 시행하였다. 자기공명분광법을 시행하기 위한 localization 방법으로는 PRESS 펄스시퀀스를 사용하였다(TR: 2,500 ms, TE: 144 ms, averaging: 512, complex data point: 2048, scan time: 25 min). 정상 쥐 그룹과 우울증을 유발시킨 뒤 항우울제를 투여한 쥐 그룹 모두 양쪽 해마부위에서의 NAA/Cr, Cho/Cr 비율(ratio)은 차이가 나지 않았다. 또한 왼쪽 해마에서 정상 쥐와 항우울제를 투여한 쥐 사이에서 NAA/Cr, Cho/Cr 비율도 차이가 나지 않았고, 오른쪽 해마부위에서도 통계적으로 유의한 차이가 없었다. 본 연구를 통하여 anti-immobility효과를 나타내는 항우울제를 투여함으로써 choline 신호가 정상 쥐와 비슷하게 회복되는 것을 관찰하였다. 이는 앞으로의 항우울제 약물 평가에 있어 자기공명분광법을 도입함으로써 기존의 주관적인 판단을 배제할 수 없었던 행동변화분석만으로 항우울제의 효과를 평가하는 방법에 비해 객관적이고 정량적인 분석을 할 수 있다는 점에서 아주 유용한 방법이라 사료된다. In this study, we observed the alteration of choline signal intensity in hippocampus region of the depressive rat model induced by forced swimming test (FST). The purpose of this study was to evaluate the antidepressant efficacy in the depressive animal model using MR spectroscopy. Fourteen experimentally naive male Sprague-Dawley rats weighting $160{\sim}180\;g$ were used as subjects. Drug injection group was exposed to the FST except for control group. The drugs were administered subcutaneously (SC) in a volume equivalent to 2ml/kg. And three injections were administered 23, 5, and 1h before beginning the given test. 1H MR spectra were obtained with use of a point resolved spectroscopy (PRESS) localization sequence performed according to the following parameters: repetition time, 2500 ms; echo time, 144 ms; 512 average; 2048 complex data points; voxel dimensions, $1.5{\times}2.5{\times}2.5\;mm^3$ ; acquisition time, 25min. There were no differences in NAA/Cr and Cho/Cr ratio between the right and the left hippocampus both normal control rats and antidepressant-injected rats. Also, no differences were observed in NAA/Cr and Cho/Cr ratio between the normal control rats and the antidepressant-injected rats both the right and the left hippocampus. In this study, we found the recovery of choline signals in the depressive animal model similar to normal control groups as injecting desipramine-HCl which was antidepressant causing anti-immobility effects. Thus, we demonstrated that MR spectroscopy was able to aid in evaluating the antidepressant effect of desipramine-HCl.

        A Case of Atrial Septal Defect (ASD) in a Miniature Schunauzer Dog

        An atrial septal defect (ASD) is congenital heart disease with a communication between the atria, which allows blood to shoot from the atrium with pressure. A 3-month-old female Miniature Schunauzer was referred to the Veterinary Teaching Hospital of Konkuk University for the evaluation of systolic heart murmur. At presentation, the mucous membrane was cyanotic. On physical examination, an ejection-type systolic murmur was auscultated at the pulmonic area. In addition, thoracic radiography showed enlargement of main pulmonary artery and right atrial/ventricular enlargement. Echocardiography revealed dilated right atrium and atrial septal defect. However, mitral and tricuspid valve were still intact and well tolerating. The presence of an ASD was confirmed by identifying flow across the defect with color Doppler imaging. Doppler echocardiography provides a means of non-invasive documentation and quantification of ASD. Complete blood count and serum chemistry were not remarkable. Although large defect was confirmed between the two atrium, the patient did not show any obvious clinical signs of heart failure at this time.

        Isofourane으로 마취된 개에 medetomidine, medetomidine-buprenorphine, medetomidine-fentanyl의 경막외 투여 시 심혈관계 반응과 진통효과의 비교

        The aim of this study was to compare the reaction of the cardiovascular system, and the anesthetic effect among 3 experimental groups, epidural administration of medetomidine as a single agent, the combination of buprenorphine and medetomidine, and the combination of fentanyl and medetomidine. Twenty one dogs were anesthetized with isoflurane and allowed to breathe spontaneously. Epidural, arterial, and venous catheters were inserted. The tip of epidural catheter was positioned at the level of the space between the sixth and seventh lumbar vertebra. After a stable plane of anesthesia was achieved, these dogs were each administered one of the following treatments epidurally : medetomidine $10{\mu}g/kg$ (Group M), a combination of medetomidine $5{\mu}g/kg$ and buprenorphine $10{\mu}g/kg$ (Group M/B), and a combination of medetomidine $5{\mu}g/kg$ and fentanyl $10{\mu}g/kg$ (Group M/F). Heart rate (HR), Respiratory rate (RR), End-tidal carbon dioxide (EtCO2), and arterial blood pressure were measured before drug administration (base line) and 5, 10, 15, 20, 25, 30, 35, 40, 45, 50, 55, and 60 min postinjection. Blood gas analysis was performed before injection and 5, 15, 25, 35, 45, 60 min postinjection. Isoflurane was discontinued 80 min postinjection and pain/motor function were evaluated up to 260 min postinjection every 15 min. At the early stage of drug introduction (until 5 min), the HR was decreased significantly in all 3 groups compared with base line. In Group M, HR was significantly decreased compared with the other 2 groups. With time (starting 20 min after drug introduction), the HR was decreased significantly in Group M/B in respect to base line. However, no significant difference was seen number-wise in all 3 groups. During 60 min after drug introduction, the systolic, diastolic and mean arterial pressures were highest in Group M and lowest in Group M/F. Among 3 groups, drug action and motor loss duration were longest in Group M/F. Analgesic effect observed in the M/F group was the most prominent and long-lasting, compared to those seen in the other 2 groups. Given the fact that the recovery of motor function takes place in a short period of time after analgesic effects disappeared, additional use of M/F depending on the patient's condition would be a good way to achieve effective pain management. However, proper care should be taken to ensure the function of cardiovascular system in the patient because the administration of M/F under isoflurane anesthesia results in a significant decline in arterial blood pressure ($65{\pm}10mmHg$).

        자기공명미세영상 및 분광법을 위한 나선형 RF 표면코일의 감는 횟수에 따른 민감도 평가

        본 연구의 목적은 고해상도 미세영상을 얻을 수 있는 기술을 확보하기 위하여 나선형 multi-turned RF 표면코일(surface coil)의 민감도(sensitivity)를 평가하고자 하였다. RF 코일의 자기장 분포를 시뮬레이션 함으로서 자기장의 불균질성 (inhomogeniety)을 조사하고, 그 결과를 토대로 개선된 RF 코일을 디자인할 수 있다. 1, 3, 5번씩 감은 나선형 코일을 조사 한 결과, 3번 감은 나선형 코일의 민감도가 가장 높은 것으로 나타났다. 이를 통하여 나선형 RF 코일의 감는 횟수를 조절함으로써 코일의 민감도를 향상시킬 수 있다는 것을 확인할 수 있었다. 그리고 지나치게 감는 횟수를 늘리면 오히려 코일의 impedance를 증가시키게 되어 민감도를 감소시키며, 동시에 matching이 어려워진다는 사실을 확인하였다. 본 연구의 결과로서 지름이 4 cm인 나선형 wire RF 표면코일의 최적의 감는 횟수를 발견하였다. 또한 나선형 표면코일에서 감는 횟수의 증가는 항상 민감도에 긍정적인 영향을 미치는 것이 아니며, 반드시 최적의 감는 횟수를 찾아야 한다는 것을 확인하였다. The purpose of this study was to improve the technique so as to develop an advanced sensitive RF surface coil for investigating the sensitivities of the multi-spiral surface coils, and we eventually wanted to achieve high resolution of the microscopic MR images and MR spectra. The magnetic field inhomogeneity and shape of a surface coil were statistically estimated by simulation of the magnetic field distribution. On the basis of the experimental results with single, 3 and S-turned spiral RF surface coils, we found that the 3-turned coil had the highest sensitivity. The present study showed that the sensitivity of the RF surface coil was improved by increasing the number of spiral coil turns, and also the SNR of the RF surface coil was dependent upon the number of spiral coil turns. However, we found, rather strikingly, that the sensitivity of excessive turns of the coils was decreased due to the rise of the coil's Impedance. Thus, the present results demonstrated that the sensitivity was not proportional to the number of a spiral RF coil's turns, and the number of spiral coil turns should be optimized for obtaining the highest sensitivity and SNR.
