        두경부암에서 방사면역치료의 역할

        Radioimmunotherapy (RIT) is a therapy that takes advantage of the “cross-fire” effect ofemitted radiation by radionuclides conjugated to tumor-directed monoclonal antibodies (mAb)(including those fragments) or peptides. While RIT has been successfully employed for thetreatment of lymphoma, mostly with radiolabeled antibodies against CD20 [90yttrium (90Y)-ibritumomab tiuxetan; Zevalin® and 131iodine (131I)-tositumomab; Bexxar®], its use in solid tumorsis more challenging, so far. Immuno-PET, a tool for tracking and quantification of mAbswith PET in vivo, is an exciting novel option to improve diagnostic imaging and guide mAbbasedtherapy. RIT in solid tumors including head and neck cancer may be an alternative treatmentwith advances in various biological, chemical, and treatment procedures, and it may helpto reduce unnecessary exposure and enhance the therapeutic efficacy. Also, immuno-PET basedon RIT might play an important role in cancer staging, in patients or targets selection of targetedtherapeutics and in monitoring the response of targeted therapeutics as precision medicine. In this review, fundamentals of RIT/immune-PET and current knowledge of the preclinical/clinical trials in RIT for solid tumor including head and neck cancer are reviewed.

      • VLSI 인터커넥션에 대한 풀-웨이브 방법을 이용한 신호 왜곡 해석에 관한 연구

        본 논문에서는 3차원 인터커넥트(3D interconnect) 구조를 해석하기 위하여 ADI-유한차분시간영역(ADI-FDTD: Alternating Direction Implicit Finite Difference Time Domain)법으로 맥스웰 회전 방정식(Maxwell's curl equation)을 계산하는 수치 해석 모델을 개발하였고, 개발한 ADI-유한차분시간영역법을 이용하여 3.3 V CMOS 기술로 설계된 샘플러 회로의 일부의 영역에 대해 컴퓨터 모의 실험 결과하여 입력된 구형 전압 신호가 금속 배선을 거치면서 5∼10 ps의 신호 지연과 0.1∼0.2 V의 신호 왜곡이 발생되는 것을 확인하였다. 결론적으로 ADI-유한차분시간영역법을 이용한 풀-웨이브 해석을 통하여 고속의 VLSI 인터커넥트에서의 전자기 현상을 정확하게 분석할 수 있음을 제시하였다. As demonstrated in this paper, the full-wave analysis using ADI-FDTD exhibits a promise for accurate modeling of electromagnetic phenomena in high-speed VLSI interconnect.

        ADI-FDTD 방법을 이용한 3차원 인터커넥트 모델링

        본 논문에서는 3차원 인터커넥트(3-D interconnect) 구조를 해석하기 위하여 ADI-유한차분시간영역(ADI-FDTD, Alternating Direction Implicit Finite Difference Time Domain)방법으로 맥스웰 회전 방정식(Maxwell's curl equation)을 계산하는 수치 해석 모델을 개발하였다. 3차원 인터커넥트 모델내의 전자기파 문제를 해석하기 위하여 맥스웰 회전 방정식을 ADI-유한차분시간영역방법으로 이산화 하였으며, ADI-유한차분시간영역의 경계에서 발생하는 반사파를 해결하기 위하여 흡수 경계 조건인 완전 정합 층 방법(PML, Perfectly Matched Layer)을 도입하였다. 개발한 ADI-유한차분시간영역방법 및 완전 정합 층의 수치 모델을 검증하기 위하여 3차원 마이크로스트립 전송선(microstrip transmission line) 구조를 3차원 그리드(grid) 구조로 모델링한 후, 시간영역에서 전계 분포를 컴퓨터로 모의 실험하였다. 그리고 본 논문에서 제안한 ADI-유한차분시간영역방법과 종래의 스탠다드 유한차분시간영역방법의 수치적 성능을 정량적으로 비교, 분석하였다. We introduced PML(Perfectly Matched Layer) absorbing boundary condition to solve the effect of the reflected wave at the interface. Evaluting the numerical model of PML and ADI-FDTD, we simulated the electric field distribution in time domain. We compare standard FDTD with ADI-FDTD, and analysis the result.

        동적 외비밸브 허탈의 진단과 치료: 비익강화이식과 Alar Flaring Suture의 활용

        In contrast to Caucasians, dynamic collapse of external nasal valve is not a common cause of nasal obstruction in Asians. Nevertheless, Asians with thin skin and weak nasal cartilages can have such problems. To diagnose these patients, visual inspection of external nose during forced inspiration is most important, which physicians frequently neglect. If dynamic collapse is suspected, physicians should locate the maximal site of collapse through modified cottle maneuver. The mainstay of treatment is proper reinforcement of main site of collapse with cartilage graft or various suture techniques. This article presents diagnosis and successful surgical treatments of two cases of dynamic external valve collapse with alar batten graft and alar flaring suture technique.         가와사끼병에 합병된 관상동맥 질환의 혈관조영술적 분석

        Purpose : To evaluate the coronary angiographic findings of patients with Kawasaki disease and to investigatethe natural course of aneurysms of the coronary artery. Materials and Methods : Between June 1989 and January1996, we evaluated the coronary angiographic findings of 12 consecutive children with Kawasaki disease whosecoronary artery was abnormal. On initial study, we retrospectively analysed the size, configuration, and locationof 35 coronary aneurysms, and in five children, follow-up coronary angiography was performed at intervals of 17 to28 (mean, 23) months. Seventeen aneurysms detected on initial study were evaluated for subsequent change. Results: Initial coronary angiography showed the aneurysms to be diffuse in 7 cases(20%), saccular in 7(20%), fusiform in17(49%), and tubular in 4(11%). They were large in 10 cases (29%), medium in 22(63%), and small in 3(9%) ; theirlocation in the coronary artery was proximal(71%), middle in 6(17%), and distal in 4(11%). The right coronaryartery was involved in 18 case(51%), and the left coronary artery in 17(49%). Follow-up study showed that theaneurysm had regressed in 11 cases (65%), persisted in 2(12%), and progressed to stenosis in 1(6%) and occlusionin 3(18%). Two(50%) of the four large aneurysms showed complete occlusion. On the other hand, medium and smallaneurysms showed regression in 9(82%) and in all cases, respectively. Diffuse-type aneurysms were complicated byocclusion in 2 cases (50%) and stenosis in 1(25%). On the other hand, six fusiform aneurysms (75%) and allsaccular and tubular aneurysms had regressed. Conclusion : In patients with kawasaki disease, informationregarding the size and configuration of coronary aneurysms may be useful for predicting the natural course andprognosis of coronary artery disease.

        양측 액와유방 접근법과 개방성 접근법과의 비교를 통한 일측 액와유방 접근 내시경 갑상선 전 절제술과 중심 경부 절제술의 수술적 완전성에 대한 평가

        Background and Objectives The aim of this study was to evaluate surgical completeness inendoscopic total thyroidectomy with central neck dissection via unilateral axillo-breast approach(UABA) compared with bilateral axillo-breast (BABA) and open approach (OA) by means ofthe radioactive iodine uptake (RAIU) ratio and thyroglobulin (Tg) of remnant thyroid. Subjects and Method From July 2010 to March 2013, 82 patients who had underwent totalthyroidectomy with central neck dissection and postoperative radioactive iodine (RAI) ablationfor papillary thyroid carcinoma were enrolled. Of these patients, 27 patients underwent UABA,24 patients BABA, and 31 patients OA. Clinicopathologic data, surgical outcome, stimulated Tgand RAIU ratio on the first postoperative RAI ablation scan were compared among 3 groups. Results Patients in the endoscopic surgery groups (UABA, BABA) were younger than thosein the OA groups. Invasiveness such as operation time, postoperative pain, and drain amount inUABA was less than that in BABA and severer than that in OA. Other variables regarding clinicopathologicand surgical data were not significantly different. Stimulated Tg and RAIU ratiodid not show significant differences among 3 groups (p=0.659 and p=0.664). Conclusion The completeness of UABA was comparable with that of BABA and OA. TheUABA may be a safe option for patients who need endoscopic thyroidectomy for papillarythyroid carcinoma.
