Osteogenesis imperfecta (OI) is a group of genetic disorders characterized by bone fragilityand connective tissue manifestations. We report a successful liver transplantation (LT) in an8-month-old boy with OI and cholestatic biliary cirrhosis. After 4 cycles of intravenouspamidronate, LT was performed under intravenous anesthesia using a left lateral sectionfrom his mother without mechanical retractors. The operation time was 420 min andestimated blood loss was 520 mL requiring one unit of RBC transfusion. He was dischargedwithout surgical complications. Therefore, LT should be considered for patients with endstage liver disease and OI under organic multidisciplinary cooperation.
Single Incision Laparoscopic Cholecystectomy without a Camera Operator

Purpose: This study aimed to evaluate the implementation of solo surgery using a laparoscopic scope holder for single incision laparoscopic cholecystectomy (SILC). Methods: With a glove port and a flexible high-definition scope, SILC was performed through a single trans-umbilical incisional site with CO2 pneumoperitoneum at a pressure of 12 mmHg. Fiftyeight patients who underwent solo SILC using a scope holder (Solo-SILC) were compared to 15 patients who underwent camera operator-assisted SILC (Ca-SILC) in terms of intraoperative and postoperative outcomes. Results: The mean BMI and operation time were 23.0±3.6 kg/m2 and 64.4±16.6 min in Ca-SILC and 25.0±3.8 kg/m2 and 58.2±27.1 min in Solo-SILC, respectively (p=0.067 and p=0.410). Estimated blood loss was negligible and an additional assistant port was not required in either groups. A case of gallbladder perforation and bile leak was noted in the Ca-SILC group, and 13 cases of bile leak in the Solo-SILC group, with no significant differences (p=0.167) during the surgery. Postoperative outcomes including surgical complications, diet restriction, diarrhea and hospital stay were not significantly different except for shoulder pain (p<0.001). Conclusion: Even with the limitations of a small number of patients, Solo-SILC proved to be a feasible technique. To confirm the safety of solo-SILC, further studies with a larger sample size are required.

With increasing demand for liver transplantation, liver grafts are used from marginal donors who were not considered suitable. This trend toward the use of marginal donors reinforces the need to balance the recipient’s condition and graft quality and to perform a liver retrieval process with a proper method for preservation of liver function. Although the issues in organ procurement are debatable, rapid and high pressure perfusion for the aorta only with a cold dissection method can be an optimal method for the successful recipient outcome in liver transplantation using a marginal graft. 2007년 이후 한국의 생체간이식 수는 큰 변화가 없지만, 뇌사자 장기이식 수는 급격히 늘어나 현재 연 1,500여건에이르고 있다. 그럼에도 불구하고 전체 이식 대기자 수도꾸준히 늘어가고 있어 전체 대기자의 15% 정도만이 이식을 받고 있는 실정이다. 그 중 뇌사자 간이식 대기자 수는2014년 기준으로 6,000명 가량으로 추정되며, 매년 생체간이식은 연 800∼900건, 뇌사자 간이식은 연 350∼400건이시행되고 있다. 따라서 전체 간이식 대기자의 20%만이 간이식을 받고 있는 실정이다(1).

NIST PQC 표준화 작업 Round 4에 올라 있던 유일한 아이소제니 기반 KEM 알고리즘인 SIKE에 대한 효율적인 키 복구 공격이 발표됨에 따라, 이를 대체할 수 있는 키 교환 알고리즘인 CSIDH가 다시 주목받고 있다. CSIDH는 현재까지 알려진 공격에 안전한 아이소제니 기반 키 교환 알고리즘으로, CRS 스킴을 현대화하여 효율적인 NIKE를 제공한다. 본 논문에서는 CSIDH-512를 ARM Cortex-M7에서 구현하고 three-level 하이브리드 몽고메리 감산을 적용하여 최적화한 성능을 측정한 뒤 그 결과 및 한계에 대해 서술하고 향후 연구 방향을 제시한다. 이는 기존에 제시되지 않았던 32-bit 임베디드 기기에서의 CSIDH 구현으로, 향후 다양한 임베디드 환경에서 CSIDH 및 파생 암호 알고리즘을 구현하는데 본 논문의 결과를 이용할 수 있을 것으로 기대한다. As an efficient key recovery attack on SIDH/SIKE was proposed, CSIDH is drawing attention again. CSIDH is an isogeny-based key exchange algorithm that is safe against known attacks to date, and provide efficient NIKE by modernizing CRS scheme. In this paper, we firstly present the optimized implementation of CSIDH-512 on ARM Cortex-M7. We use three-level hybrid Montgomery reduction and present the results of our implementation, limitations, and future research directions. This is a CSIDH implementation in 32-bit embedded devices that has not been previously presented, and it is expected that the results of this paper will be available to implement CSIDH and derived cryptographic algorithms in various embedded environments in the future.
