택시운전자 애로사항에 관한 연구 : 광주 전남 운수연수원 교육생을 중심으로

The taxi drivers have heard the complaints against their services from the customers, the money which taxi drivers have to take over to the company out of their daily earning from the company owners, and the low income from home. These complex problems are a burden to taxi drivers. According to an empirical analysis about the various situation of the taxi drivers, the taxi drivers' bottleneckes and their causes are as follows: 1. The strong desire for changing their driving jobs resulted frora the dissatisfaction with their jobs. 2. An obsession with the money which they have to take over to the company. 3. Thge chronic disease from the accumulated fatigue. And the comparative situation about the duty hour of the taxi drivers and their income are as follows: 1. the gooney which taxi drivers have to take over to the company out of their daily earning within the limited duty hour is deficient about 20%. 2. Their wages are very low. (small-sized taxis ; 450 000 Won, Medium-sized ; 550 000 Won) The following will be some proposals to improve taxi drivers' bottlencks. First, inducement of a full monthly wage system to be able to guarantee the living cost, making card system for the taxi fare and the exemption from a tax on value added. Second, the improvement of taxi drivers' quality. 1. The guatantee of the refreshment training hour.(calculating the refreshment training hours into duty hours) and the inducement of the full monthly wage system. 2. The inducement of the system of the re-contract for the employment in order to prevent taxi drivers' indolence according to the full monthly wage system. 3. The inducement of the panalty system against the drivers who supply bad services and have a high traffic accidents. This study has its own limit and was not able to suggest the measures of improvement on the complicated problems. Judging from the present situation of taxi drivers' problems, the taxi drivers' bottlenecks will be likely to be more serious than we have thought until now.

상아질지각과민증에 대한 펄스형 Nd: YAG 레이저의 치료효과에 관한 임상적 연구

저자는 치과용 레이저를 이용한 상아질지각과민증 치료효과를 임상적으로 평가하고자 상아질지각과민증환자 24명을 대상으로 각 환자마다 상아질 지각과민 치아 중 레이저로 치료 할 치아1개(실험군)와 치료하지 않을 치아 1개(대조군)를 선정하여, 치과용 air-syringe에 의한 냉자극과 치과용 탐침자에 의한 기계적 자극을 가한 후 Visual Analogue Scale(VAS)을 통해 지각과민증 정도를 평가하였으며, 이후 실험군 치아의 치경부 마모면에 광섬유 레이저에너지 전달방식의 펄스형 Nd:YAG 레이저의 일종인 Sunlase Master(Sunrise Technologies Inc., U.S.A., 파장 1064nm, 펄스지속시간 120(sec, 광섬유직경 320(m)를 사용하여, 0.3-1.0W, 10Hz 조건으로 환자의 반응에 따라 1회2분을 초과하지 않는 범위 내에서 비접촉식과 접촉식을 병행하여 3회에 걸쳐(초진 당일, 1일후, 3일후) 레이저를 조사한 후 처음조사 직후, 그리고 초진 1일후, 3일후, 7일후, 14일후 VAS를 채득하여 조사전과 비교, 분석, 평가하였던 바, 상아질 지각과민증이 각각 약 50%, 약36%, 약45%, 약 53% 감소하였으나 (p(0.05) 경시적으로는 유의한 차이를 보이지 않았다. 그리고 24명중 9명에 대해서는 초진 30일 후 까지 레이저조사치료효과를 평가하였던 바, 상아질지각과민증이 처음조사 직후, 감소하였으며 (p(0.05)역시 경시적으로는 유의한 차이를 보이지 않았다. 초진 30일 후 상아질지각과민증이 초진 14일 후에 비해 약간 증가되는 경향을 보여, 레이저조사 치료효과가 약화되는 경향인지 여부에 대해 향후 추가적인 연구가 필요할 것으로 사료된다.안되었다.기란 현실적으로 불가능하므로, 본 연구에서는 이러한 제약점을 극복할 수 있는 근사적인 지체시간을 계산하는 방법을 제시한 점에서 의미를 갖을 수 있다.수들은 직업의 선택이나 소득을 예측하기 위한 요소들로 포함될 수 없었다. 따라서 후속연구에서는 이를 보완해야 할 것이며, 최근 들어 우리 나라에서도 재택근무에 대한 관심이 대두되고 있으나 아직 개념정의나 그 중요성과 가치, 그리고 실태 파악과 같은 연구가 활발히 이루어지지 못하고 있으므로 이에 대한 심층적인 연구가 행해져야 할 것이다.d similar flower proceeding dates in all branches. but "Daepung" showed similar flower proceeding dates in all branches.est in HB. Mean period of wetting duration was in the order of DS>HB>MB, while the dew point depression was greatest in DS.ANCOVA, Pearson correlation을 이용하여 분석하였으며, 그 결과는 다음과 같다. 캠프 프로그램은 소아 당뇨병 환자의 자기 효능을 증진시키고 환자 역할 행위 이행을 높여주는데 효과적 이었다. 소아 당뇨병 환자의 자기 효능은 환자 역할 행위 이행과 순 상관 관계가 있어, 자기 효능이 증진될수록 환자 역할 행위 이행 정도가 높아졌다. 무조건 사주지 않는다(8.0%), 무조건 사준다(3.1%)로 식품광고에 나오는 식품 요구시 부모의 70.3%가 거절하는 것으로 나타났다. 거절 이유는 건강에 나쁘다는 것이 가장 큰 이유였으며 강남과 강북 어린이간에 유의적인 차이가 있었다(p<0.001).

마우스 삼차신경 척수로핵 미측소핵 아교질 신경세포에서 Bicuculline Methiodide (BMI)의 비 $GABA_A$ 수용체 매개성 반응

Bicuculline은 전기생리학 연구 분야에서 가장 일반적으로 사용되는 $GABA_A$ 수용체 길항제중 하나다. 물에 잘 녹지 않는 특성 때문에 실험적으로는 bicuculline methiodide (BMI)와 같은 4차암염(quaternary ammonium salts)의 형태로 주로 제작되어 이용되고 있다. 그러나 다수의 연구에서 BMI가 비 $GABA_A$ 수용체 매개 반응이 알려졌다. 삼차신경 척수로핵 미측소핵 아교질은 유해자극 과정과 연관이 있는 것으로 알려져 있으나, 아교질 신경세포에서 BMI의 비 GABAergic 활성에 대해서는 거의 알려지지 않았다. 본 연구는 whole cell patch clamp 방법을 이용하여 마우스 삼차신경 척수로핵 미측소핵의 아교질 신경세포에서 전류고정법(current clamp)과 전압고정법(voltage clamp)을 통해 BMI가 막전압 변화 및 세포막 전류의 변화에 미치는 영향을 조사하여 다음과 같은 결과를 얻었다. 첫째, 삼차신경 미측소핵 아교질 신경세포에서 BMI는 세포막 탈분극을 유발하였다. 둘째, BMI에 의한 효과가 $GABA_A$ 수용체에 매개하지 않는다는 결과들이 도출 되었다. 셋째, BMI에 의해 유발된 세포막 탈분극은 PIC와 CNQX, AP-5, strychinine과 TTX을 포함하고 있는 길항제 혼합물의 존재하는 조건에서도 지속적으로 유지되었다. 그리고, BMI에 의해 유도되는 탈분극의 반응이 fBIC의 다른 유도체이며 GABA 수용체 억제제로서 알려져 있는 BMB 또는 BMC에서도 나타나는지 확인해 보는 실험 결과 BMI는 막전류의 내향성 전류를 유도하지만, fBIC의 다른 유도체인, BMB와 BMC는 외향성 전류를 유도하였다. 이상의 결과들을 종합해 보면 마우스 삼차신경 척수로핵 미측소핵 아교질 신경세포에서 Bicuculline Methiodide (BMI)는 free base bicuculline (fBIC)과는 달리 $GABA_A$ 수용체를 매개하지 않고 막 탈분극을 유발한다는 확실한 결론에 도달했다. 또한, 이 결과는 BMI의 작용기전이 fBIC의 활동기전과 다를 수 있음을 의미하며 이와 같이 fBIC와 BMI의 다른 반응을 보이는 것은 다름 아닌 fBIC에 결합된 $I^-$ 에 의한 특이적 반응과 관련된 기전이 있을것이라는 제안을 해 볼 수 있다. 이러한 제안은 GABA 수용체의 길항제로 알려진 BMB나 BMC의 경우는 BMI와는 다르게 외향성 전류가 기록된 결과를 통해 BMB, BMC와 다르게 BMI에는 fBIC에 $I^-$를 붙여서 사용을 한 결과에 의해 뒷받침이 된다. 앞으로의 연구에서는 삼차신경 척수로핵 미측소핵 아교질 신경세포에서 BMI에 의해 유도된 세포막 탈분극의 세부 기전에 대한 증명이 필요할 것으로 보인다. 이 세부 기전이 밝혀지게 되면 작용기전과 관련해서 $I^-$의 역할이 밝혀지게 될 것이며, 이는 향후 $I^-$를 활용한 다양한 전기생리학적 연구와 임상적인 활용을 보다 확대시키는 결과로 이어 지리라 기대해 본다.

Osteomyelitis of Mandibular Condyle : A Case Report in 9-year-old Child

Osteomyelitis means inflammation of the bone marrow. It usually begins in the medullary cavity, involving the cancellous bone; then it extends and spreads to the cortical bone and eventually to the periosteum. The cause is usually thought to be microbiological. But there still are factors that predispose to produce a possible bone infection such as injuries, syphilis, actionomycosis, chronic kidney failure, alcoholism, malnutrition, radiotherapy, and chemotherapy. Treatment of modalities have been directed toward eradicating microbes and improving circulation in the early stage. In the case presented, surgical debridement and IV antibiotics were the treatment of choice. Osteomyelitis in children is mainly affected in the mandible. And in childhood, the mandibular condyle is regarded as an important center of mandibular growth. Therefore, in young patients, osteomyelitis involving this region may cause a restraint of mandibular development, resulting in facial asymmetry. So diagnosis in the early stage is important in child with osteomyelitis. Recently, we have encountered an interesting case of osteomyelitis of the mandibular condyle in 9-year-old boy. So we present the case and review the literature about osteomyelitis.

In the case of using surgical clips for division of the cystic duct during laparoscopiccholecystectomy, surgical clip migration into the common bile duct is known to be an extremely rare complication. However, its mechanism is currently unclear. The incidence of performing laparoscopic hepatectomy has significantly increased in the laparoscopic center of hospitals. Accordingly, the complications associated with using clips in laparoscopic cholecystectomy have increase as well. A 49 year-old female patient underwent laparoscopic left hepatectomy due to left intrahepatic duct stones with cholangitis. We report here on a case of a clip that migrated into the common bile duct and the duct had bile stones. We also review the relevant literature.
