        Microimplant Anchorage (MIA)를 이용한 Ⅱ급 2류 성인 환자의 비발치 치험례

        상악 측절치와 견치의 치근 사이에 식립한 microimplant를 이용하여 상악 전치의 함입과 설측으로의 치근 이동을 얻을 수가 있었으며, 상악 견치와 제1소구치의 치근 사이에 식립한 microimplant를 이용하여 과맹출된 상악 견치를 함입시켰다. 또한 상악 제1,2대구치의 치근 사이에 식립한 microimplant를 이용하여 상악 견치 및 구치의 후방 이동 및 상악 전치의 후방 견인을 시행하였다. Anterior bite plane과 intrusion arch, 그리고 Ⅱ급 고무 등과 같은 전통적인 방법을 사용하여 하악 전치의 함입 및 구치부의 정출을 얻얼 수가 잇었으며, 과개 교합 및 상악 전치의 설측 경사가 해소 되면서 하악골이 전방으로 약간 이동하였다. 이와 같이 MIA는 Ⅱ급 2류 부정교합 환자에 있어서 Ⅱ급 견치 및 구치 관계 그리고 과개교합을 동시에 해소 하는데 절대적인 고정원을 제공하였다. Maxillary anterior teeth were intruded and lingually root torqued with two maxillary anterior microimplants between the lateral incisors and canines. Overerupted maxillary canines were intruded with two other microimplants between the maxillary canines and first premolars. Maxillary posterior teeth and canines were distalized, then the maxillary incisors were retracted with two maxillary posterior microimplants between the first and second molars. The mandibular anterior teeth were intruded and the mandibular posterior teeth were extruded with conventional method such as anterior bite plane, intrustion arch and Class Ⅱ elastics. The mandible moved slightly forward after the correction of deep bite and retroclination of the upper incisors. Consequently, microimplant anchorage (MIA) provided absolute anchorage for simultaneous correction of Class Ⅱ canine and molar relationships and deep overbite.

        유성견에서 periodontal distraction에 의한 급속 치아견인 시 치수 및 치주조직의 변화에 관한 연구

        유성견에서 periodontal distraction 과정을 통해 급속 견인 후에 치수와 치주조직의 변화를 평가하였다. 유성견의 상악 제2소구치를 발거하고 제3소구치 근심의 치간골을 부분 제거하였다. 견인장치는 6일 동안 하루에 2번씩 견인하고, 강화기 동안 0주, 1주, 3주, 5주, 7주, 9주에 치수와 치주조직의 변화를 임상적, 방사선학적, 조직학적, 면역조직화화학적으로 관찰하였다. 신장된 치주인대 내에서 견인 직후부터 4주까지 활발한 골밀도 증가를 보였으며, 특히 2-3주 사이에서 가장 활발하였다. 강화기 0주에 압박측 치조골의 흡수 및 파골세포 출현, 염증세포 침윤이 가장 많이 관찰되었고 1주의 신장측 치주인대에서 특징적인 골형성을 보이기 시작하였다. 신생골 형성은 1주와 3주에 가장 많이 관찰되었으며 9주에는 성숙골로의 대치 및 치주인대의 재생으로 대조군과 차이가 없었다. 실험군의 calcitonin gene-related peptide의 발현은 치수, 치주인대 내의 염증 부위에서 나타났으며 0주, 1주에 압박측 치주인대에서 증가하였다가 5주 이후에는 감소하여 대조군과 차이가 없었다. 이상과 같은 소견은 periodontal distraction에 의한 급속 치아 이동이 정상적인 골 재형성 과정을 도모하는 새로운 치아이동 방법이 될 수 있음을 시사한다. Objective: The aim of this study was to evaluate pulp and periodontal changes following rapid tooth retraction by periodontal distraction after bone undermining surgery in young adult dogs.<br> Methods:Alter extraction of second premolars, the interseptal bone mesial to the upper 3rd premolar was undermined. After activating the distraction appliance at 0.5 mm/day for six days, the dogs were sacrificed at 0, 1, 3, 5, 7, and 9 weeks during the consolidation period. Tissue changes of periodontium and pulp were evaluated radiologically, histologically, and immunohistochemically.<br> Results:Digital subtraction radiography showed active bone formation in the stretched periodontal ligament from 0 - 4 weeks. Resorption of the alveolar bone, appearance of osteoclasts, and infiltration of inflammatory cells were observed just after the activation period at the pressure side, and distinctive bone formation was seen in the tension side of the periodontal ligament from 1 week. New bone formation was active at 1 - 3 weeks. The expression of calcitonin gene-related peptide in the experimental group was increased at the alveolar bone and pulp, and periodontal ligament at the pressure side from 0 - 1 week, and it decreased after 5 weeks to become similar to that of the control group.<br> Conclusions:The results showed that rapid tooth movement using periodontal distraction can be new form of orthodontic tooth movement for accelerating normal bone formation.

        과두흡수환자의 자기공명영상 사진 평가 및 악안면 골격형태에 대한 연구

        The aim of this study was to investigate clinical assessment, panorama & MRI findings and cephalometric characteristics in 42 patients with condylar resorption, who visited in the Department of Oral Medicine Kyungpook National University Hospital at 2006. The results were as follows; 1. Clinical assessment 1) Female was 34 and male was 8, females were predominant. Distribution of age showed as follows; 10s was 14, 20s was 13, 30s was 7, 40s was 3, 50s was 4 and 60s was 1 patient. 10s and 20s were predominant. 2) Most of the patients had parafunctional habit. 2. Findings of panorama & MRI 1) Most of the patients had degree of Grade II condylar resorption by panorama taking. 2) Most of the patients had disc dislocation and belonged to the degree of stage IV by MRI taking. 3. Cephalometric Characteristics 1) SN, SAr and saddle angle in female patients were significantly smaller and SN in male patients showed only significantly smaller than normal group. 2) SNA showed no difference from the normal group in both patients. SNB was smaller and ANB was lager in female patients than normal group. 3) SN-GoMe and FMA increased in patients. 4) Total posterior facial height & ramus height were significantly smaller. 5) Mandibular body length did not show any significant difference.

        10대 하악과두흡수 환자의 임상적 평가 및 악안면 골격형태에 대한 연구

        The aim of this study was to investigate clinical assessment and cephalometric characteristics in 10s patients with condylar resorption, who visited in the Department of Oral Medicine Kyungpook National University Hospital at 2006, by use of panorama, transcranial view and lateral cephalometric radiograph. The results were as follows; 1. Clinical assessment 1) Total number of patients were 59. Female was 47 and male was 12, Females were predominant and patient's age ranged from 12 to 19. 2) Most of the patients had Grade II condyle resorption. 3) The number of who had Class I occlusion was 27, which was the largest group. The number of patients with openbite was 20. Average overjet was 3.58mm and average overbite was 0.97mm. 4) Most of the patients had parafunctional habit. 5) The patients of showing the pain in condylar resortion was 41 and the case of not showing the pain was 18. 6) Treatment duration of 23 patients were less than 1 month, 28 patients were treated with supported therapy. 2. Cephalometric Characteristics 1) A 16-year-old female patients showed smaller SNA, SNB and larger articular angle significantly as compared with those of normal group. 2) A 17-year-old female patients showed smaller SN, SAr, TPFH, ramus height and larger SN-GoMe, FMA, articular angle significantly as compared with those of normal group. 3) Over 18 years old female patients showed smaller SN, SNB, TPFH, ramus height and larger ANB, FMA, SN-GoMe, LAFH, articular angle, gonial angle significantly as compared with those of normal group. 4) Over 18 years old male patients showed smaller SN, TPFH, ramus height and larger FMA, SN-GoMe, LAFH, articular angle, gonial angle significantly as compared with those of normal group. 5) There was no significant difference between 10s and normal group in mandibular body length.

        하악과두흡수 환자의 임상적 평가 및 악안면 골격형태에 대한 연구

        Condylar resorption, or condylysis can be defined as progressive alteration of condylar shape and decrease in mass. Condylar resorption is a poorly understood progressive disease that affects the TMJ and that can result in malocclusion, facial disfigurement, TMJ dysfunction, and pain. The aim of this study was to investigate clinical assessment and cephalometric characteristics in 224 patients with condylar resorption, who visited in the Department of Oral Medicine Kyungpook National University Hospital at 2006, by use of panorama, transcranial view and lateral cephalometric radiograph. The results were as follows; 1. Clinical assessment 1) Total number of patients who visited with chief complaints of TMD were 2419 and 224 (9.3%) among them revealed the condylar resorption, Among patients group with condylar resorption, female was 183 and male was 41, females were predominant. 2) Patient's age ranged from 12 to 70 and mean age was 30.6 years old with a strong predominance for 10s and 20s. Distribution of a showed as follows; 10s was 26.3%, 20s was 34,8%, 30s was 13.8%, 40s was 11.2%, 50s was 7.1%, 60s was 6.3% and 70s was 0.4%. 3) Most of the patients had parafunctional habit. 4) The case of showing the pain in condylar resorption was 145, the case of not showing the pain was 79. 5) Treatment duration of the patients was relatively short. 2. Cephalometric Characteristics 1) ANB which means the retruding of the mandible increased significantly than normal group. The ANB of female was lager than male group as the means of ANB were 5.05 in female and 3.57 in male, 2) SN-GoMe and FMA increased in resorption patients, but FH-PP did not show any significant difference. The FMA of female was lager than male group as the means were 31.69 in female and 30.44 in male. 3) Total posterior facial height was significantly smaller and total anterior facial height showed no significant increase as compared with those of the normal group. Condylar resorption was predominant in young female which was caused by more vertical facial pattern in female than male and increase of parafunctional habit in young age. It was thought that the patients who have a risk factor increasing the compressive stress at condyle caused by obliquely inclined masseter and medial pterygoid show high prevalence of condylar resorption.

        CBCT-재구성 파노라마영상의 근원심 치축에 관한 연구

        본 연구에서는 cone beam computed tomography (CBCT)를 이용한 삼차원 영상을 기준으로 CBCT-재구성 파노라마영상 형성 시의 신뢰성을 입증하고 CBCT 삼차원 영상과 비교하여 실제 근원심 치축을 가장 잘 나타낼 수 있는 이차원 파노라마영상을 찾고자 하였다. 연구 모형은 두부계측방사선사진의 평균치에 따라 제작되었고 CBCT와 파노라마방사선영상 촬영을 위해 촬영 시마다 연구 모형이 반복적으로 재위치되었다. 파노라마방사선영상은 -5°, 0°, +5°로 연구 모형의 교합평면을 변화시키면서 촬영되었다. 이후 CBCT 삼차원 영상과 CBCT-재구성 파노라마영상 및 파노라마방사선영상에서 측정된 계측값을 평가하였다. CBCT 삼차원 영상과 CBCT-재구성 파노라마영상의 전치 및 견치부에서 통계학적으로 유의성 있는 차이를 나타내지 않았으나 대부분의 소구치부에서는 통계학적으로 유의성 있는 차이를 나타내었다. 그리고 CBCT-재구성 파노라마영상과 파노라마방사선영상은 대부분의 치아에서 통계학적으로 유의성 있는 차이를 나타내었다. CBCT-재구성 파노라마영상이 근원심 치축 평가 시 임상적으로 사용 가능함을 보여주었다. Objective: The purpose of this study was to confirm the reliability of a cone beam computed tomography (CBCT)-generated panoramic view based on a CBCT 3D image and to find the most helpful 2D panoramic image compared with CBCT 3D image when examining the mesiodistal tooth axis. Methods: A test model was constructed according to cephalometric norms. The test model was repeatedly repositioned for CBCT and panoramic radiographic imaging. Panoramic radiographs were acquired at each of the following 3 occlusal plane positions: -5°, 0°, and +5°. Measurements of mesiodistal tooth axis in CBCT 3D image, CBCT-generated panoramic view, and panoramic radiographs were compared. Results: Compared with the CBCT-generated panoramic view, CBCT 3D image showed significant difference in the mesiodistal tooth axis in the premolars and no significant difference in the mesiodistal tooth axis in the incisors and canines. Mesiodistal tooth axis on the CBCT-generated panoramic view was significantly different from that on panoramic radiographs. Conclusions: CBCT-generated panoramic view can be a useful tool for evaluating mesiodistal tooth axis.

        상악 골격성 확장장치와 페이스 마스크를 사용한 골격성 III급 성장기 아동의 치험례

        Face mask treatment with a rapid palatal expander is common in children with Class III malocclusion and maxillary deficiency. However, it can cause anchorage loss and dental side effects. To overcome these disadvantages, the use of skeletal anchorage such as miniplates in the infrazygomatic crest area or palate has been reported. This clinical case report presents face mask treatment in skeletal Class III adolescent with maxillary skeletal expander (MSE). MSE has been developed for the maxillary expansion in adults, but it can be used as a skeletal anchorage for face mask treatment in children. The post-treatment profile was significantly improved without any dental side effects.

        교정용 브라켓의 종류와 각도, 호선의 코팅 여부에 따른 마찰력의 비교

        본 연구의 목적은 코팅된 호선, 다양한 브라켓, 그리고 브라켓-호선 각도가 교정용 호선이 브라켓을 활주 이동하는 동안 발생되는 마찰력에 어떠한 영향을 미치는지에 대하여 평가해보는 것이었다. 고정식 장치를 이용한 교정치료 시 발생할 수 있는 상황을 시뮬레이션하기 위하여 4종류의 브라켓(금속 브라켓인 Micro-arch, 단결정 세라믹 브라켓인 Perpect Clear2, active type의 자가결찰 브라켓인 Clippy-C, passive type의 자가결찰 브라켓인 Damon3)과 5종류의 교정용 호선(0.014", 0.016", 0.016" × 0.022" inch coated Ni-Ti 호선, 0.016", 0.016" × 0.022" inch Ni-Ti 호선)이 사용되었고 브라켓-호선 각도는 각각 0˚, 3˚, 6˚, 9˚로 조절되었다. 모든 실험군에서 자가결찰 브라켓군, Micro-arch군, Perpect Clear2이 순으로 정지, 운동 마찰력이 유의하게 높았다 (p < 0.001). 0˚와 3˚의 브라켓-호선 각도에서 같은 크기의 Ni-Ti 호선은 코팅 여부에 따른 정지, 운동 마찰력의 유의한 차이가 없었으나, 3˚에서 자가결찰 브라켓군의 0.016" × 0.022" inch Ni-Ti 호선에서만 코팅된 경우에 마찰력이 유의하게 높았으며 (p < 0.001), 6˚와 9˚의 브라켓-호선 각도에서 원형과 각형 호선은 모두 같은 크기의 코팅된 호선에서 정지, 운동 마찰력이 유의하게 높았다 (p < 0.001). 코팅된 호선은 크기가 커질수록 정지, 운동 마찰력이 유의하게 높아졌다 (p < 0.001). 각형 호선은 원형 호선 보다 정지, 운동 마찰력이 유의하게 높았으나, 9˚의 브라켓-호선 각도에서 0.016" inch coated Ni-Ti 호선만은 0.016" × 0.022" inch Ni-Ti 호선보다 마찰력이 높았다 (p < 0.001). 브라켓-호선 각도가 증가함에 따라 정지, 운동 마찰력도 유의하게 높아졌으나 (p < 0.001), Micro-arch군과 Perpect Clear2군에서 0.016 inch Ni-Ti 호선과 이루는 각도 0˚, 3˚에서는 마찰력의 유의한 차이가 없었다. (대치교정지 2011;41(6):399-410) Objective: The purpose of this study was to evaluate the difference in frictional resistance among metal, ceramic, self-ligation brackets and coated or non-coated Ni-Ti archwires at various bracket-archwire angulations during the sliding movement of an orthodontic archwire, using an orthodontic sliding simulation device. Methods: Four types of bracket (Micro-arch Perpect Clear2 Clippy-C and Damon3 and 5 types of orthodontic archwire (0.014", 0.016", and 0.016" × 0.022" inch coated Ni-Ti, and 0.016" and 0.016" × 0.022" inch Ni-Ti) were used. Further, the bracket-archwire angles were set at 4 different angulations: 0°, 3°, 6°, and 9°. Results: The frictions from all the experimental groups were found to be significantly increased in order of self-ligation brackets, Micro-arch and Perpect Clear2 (p < 0.001). The presence of a coat had no effect on the friction of the same sized archwires at 0° and 3° bracket-archwire angles (p < 0.001). Coated archwires had significantly higher frictions than the same sized non-coated archwires at 6° and 9° bracket-archwire angles (p < 0.001). The frictions increased significantly as the bracket-archwire angles were increased (p < 0.001). Conclusions: The use of self-ligation brackets will be beneficial in clinical situations where a low frictional force is required. Further, in cases where crowding is not severe, the use of coated archwires should not cause problems. However, more additional explanation is required considering the fact that the damage of coated archwire and exposure of the metal portion in case of binding and notching and the effects of saliva were not taken into account.         악교정수술 환자에서 교합기 석고 모형과 측면두부방사선사진의 교합평면에 관한 비교 연구

        The common errors in preoperative treatment plan for the orthognathic surgery can be occurred during cast impression, cast mounting procedure with face-bow transfer, surgical stent fabrication, and so on. One of the most common errors exists during mounting process of the model on the articulator. Accurate mounting of dental casts to articulator should be achieved by transferring the 3-dimensional spatial relationship of the maxillary arch to an articulator. A face-bow is used for transfer this relationship to articulator, usually by relating the face-bow to a plane of reference of maxillary cast. The purpose of this study is evaluation of the accuracy of face-bow transferring of maxillary model to the articulator. The maxillary casts of thirty patients for orthognathic surgery were mounted on articulator with an face-bow instrument. The relationship of occlusal plane angle to Frankfort horizontal plane relations were compared the cephalogram with the cast-mounted articulator. As a result of this study, the significant difference between the maxillary occlusal planes angle in the cephalogram and articulator were found . The results were followed, 1. The mean occlusal plane angle in cast-mounted articulator was 13.5􀆆(SD±5.4). 2. The mean occlusal plane angle in cephalogram was 10.4􀆆(SD±4.3). 3. The mean difference of occlusal plane angle between cast-mounted articulator and cephalogram was 3.3􀆆(SD±4.6). According to the result, we should suggest that the occlusal plane angle to Frankfort plane in cast-mounted articulator is more steeper than that of cephalogram. And then, maxillofacial surgeon should try to get a more predictable result by suggesting the proper correction method and mounting the cast accurately.
