        Childhood Cancer Mortality and Birth Characteristics in Korea: A National Population-based Birth Cohort Study

        To examine the relationship between birth characteristics and childhood cancer mortality,a retrospective cohort study of Korean children was conducted using data collected by the national birth register between 1995 and 2006, which were then individually linked to death data. A cohort of 6,479,406 children was followed from birth until their death or until December 31, 2006. Poisson regression analyses were used to calculate rate ratios of childhood cancer deaths according to birth characteristics. A total of 1,469 cancer deaths were noted and the childhood cancer mortality rate was found to be 3.43 per 100,000person-years in Korea during the period of 1995-2006. The birth characteristics examined in this study (i.e. , birth weight, gestational age, multiple births, parental ages, and number of siblings) were generally found to be not significantly associated with childhood cancer mortality, and the associations did not vary meaningfully with gender nor with cancer sites. However, among children aged 5-11 yr, higher birth weight was associated with elevated childhood cancer mortality (rate ratio = 1.28, 95% confidence interval 1.04-1.58). Our results offer no overall associations between childhood cancer mortality and birth characteristics, but suggest that the association may be specific to age group.

        Disease Prevalence and Mortality among Agricultural Workers in Korea

        The aim of this paper was to provide an overview of mortality and disease prevalence related to occupational diseases among agricultural workers in Korea. We evaluated the age-standardized mortality rates and the prevalence of chronic diseases and compared them with those of other populations using death registration data from 2004 through 2008 and the 2005 Korean National Health and Nutrition Examination Survey. In addition,we conducted a literature review on published articles examining the health status of farmers in Korea. Agricultural workers have a significantly higher mortality of cancer,tuberculosis, chronic respiratory diseases, liver diseases, suicide, motor and non-motor vehicle accidents. Compared to other populations, farmers have higher prevalence rates of arthritis and intervertebral disc disorders. The literature review revealed a number of workrelated diseases among farmers, such as musculoskeletal diseases, pesticide poisoning,infections, and respiratory and neurologic diseases. Korean farmers demonstrate a distinct pattern of mortality and disease prevalence compared to other populations. Although lifestyle factors remain important contributors to those deaths and diseases, our study suggests that occupation is a major determinant as well. Intensive programs such as surveillance systems, therefore, should be developed in order to identify and prevent workrelated diseases among agricultural workers in Korea.

        Exposure to Dichlorodiphenyltrichloroethane and the Risk of Breast Cancer: A Systematic Review and Meta-analysis

        Objectives This study extended and updated a meta-analysis of the association between exposure to dichlorodiphenyltrichloroethane (DDT) and the risk of breast cancer. Methods We reviewed the published literature on exposure to DDE and breast cancer risk to update a meta-analysis from 2004. The total of 35 studies included 16 hospital-based case–control studies, 11 population-based case–control studies, and 10 nested case–control studies identified through keyword searches in the PubMed and EMBASE databases. Results The summary odds ratio (OR) for the identified studies was 1.03 (95% confidence interval 0.95–1.12) and the overall heterogeneity in the OR was observed (I2 = 40.9; p = 0.006). Subgroup meta-analyses indicated no significant association between exposure to DDE and breast cancer risk by the type of design, study years, biological specimen, and geographical region of the study, except from population-based case–control studies with estimated DDE levels in serum published in 1990s. Conclusion Existing studies do not support the view that DDE increases the risk of breast cancer in humans. However, further studies incorporating more detailed information on DDT exposure and other potential risk factors for breast cancer are needed.

        구강질환과 사회경제적요인 및 보건의식행태와의 관련성 - 2005년도 국민건강영양조사를 바탕으로 -

        The purpose of this study was to evaluate the role of socioeconomic factors and health behaviors on the prevalence of oral diseases(dental caries and periodontal disease) among Korean adults. Data from the 2005 National Health and Nutrition Examination Survey was analyzed for adults aged over 19 years (n=25,215). Oral disease was defined as disease diagnosed by a dentist for the previous 12 months. Chi-square test and logistic regression analysis were used to conduct statistical analysis. The population without dental caries and periodontal diseases were used as the reference category for all analysis. The overall prevalence of annual dentist-diagnosed dental caries and periodontal diseases were 23.5% and 10.6% respectively. The relative risk of developing dental caries for graduates of middle school and lower were 1.53 times (95% CI: 1.24-1.89) more likely to develop dental caries comparing to college graduates. Low income earners were 1.23 times(95% CI: 1.01-1.49) more likely to develop dental caries than high income earners. Persons covered by national medical insurance were 1.45 times(95% CI: 1.08-1.95) more likely to develop dental caries comparing to persons covered by employee medical insurance. Compared with persons not eating snacks, the risk of developing caries in persons eating snacks once a day was 1.16(95% CI: 1.01-1.32), while those who snacked twice or more a day were 1.19(95% CI: 1.01-1.41). The relative risk of developing periodontal disease was 3.71(95% CI: 2.38-5.80) higher in older individuals than younger. In terms of education level, middle school graduates and lower were 1.54 times more likely to develop periodontal disease than college graduates. Low income earners were 1.47 times more likely to develop periodontal diseases than high income earners. Using data from a large, nationally representative sample of Korean populations, we support the hypothesis that the prevalence of oral diseases is related with socioeconomic factors and health behaviors. More intensive intervention efforts to reduce the prevalence of dental diseases are warranted in Korea.

        한국인의 농산물 농약 오염 불안도 연구 -2008년도 사회조사를 바탕으로-

        Objective: This study aimed to examine the level of risk perception related to pesticide contamination of food products and associated sociodemographic factors, as well as investigate the association with environmental pollution preventive behaviors. Methods: Based on data from Social Survey conducted in 2008 among a nationally representative sample of the population 15 years or older, we estimated proportions and 95% confidence intervals of the risk perception of pesticide contamination of food products. Logistic regression analyses were used to analyze the effects of sociodemographic status on risk perception. Results: Among respondents, those who feel anxious about the risks of pesticide contamination of domestic food products accounted for 40.4%, whereas the proportion rose to 87.0% with imported food products. These numbers showed that levels of anxiety about the pesticide contamination of imported food products were ranked at the top, and those for domestic products were at the bottom of the list of all environmental pollutants examined in the survey. Significantly related factors to anxiety about pesticide contamination of food products included women, the 40-50 age group, university graduates, married, non-manual employment, having school-aged children, and owning one's own home. The population attempting to prevent environmental pollution showed higher levels of anxiety about pesticide contamination of food products than did those who were not taking such steps. Conclusion: Compared to other forms of environmental pollution, Koreans show the highest levels of anxiety about pesticide contamination of imported food products. Therefore, it is necessary to establish appropriate regulations and transparent risk communication with the public about the safety of imported food.
