        비중격성형술;최근의 개념과 술기

        Septoplasty is one of the most commonly performed operations in Otolaryngology. It is often considered an easy operation. Yet it is still accompanied by a relatively high rate of failure and complications. It is definitely not an operation in which one surgical technique can suffice for all circumstances. Rather it requires a variety of techniques in accordance to the various septal pathologies in order to achieve a favorable outcome. This study provides a comprehensive review of septoplasty including current concepts, historical background, classification, updated surgical techniques, and management of complications. In particular, the surgical techniques that ensure a successful correction and minimal complications are highlighted. Septoplasty is one of the most commonly performed operations in Otolaryngology.       • Angiomatous Nasal Polyp Presenting Recurrent Epistaxis

        Angiomatous nasal polyp is an unusual, nonneoplastic lesion of which a few cases have been reported in the literature. It may be clinically, radiologically and pathologically confused confused with angiofibroma or other vascular neoplasms. The author reports a case of angiomatous polyp developed in a 53-year-old woman, who presented with recurrent epistaxis and was cured by laser excision.

        코성형술에 사용된 고어텍스의 안정성에 관한 다기관 평가

        Background and Objectives: Materials and Method:A retrospective multicenter study was caried out on 15 surgeons from 1 general hospitals and 4 private practice clinics regarding the safety of the Gore-Tex as a nasal implant. The study involved 853 patients, of whom 656 received primary surgery and 197 revision surgery. Gore-Tex was mainly used as a dorsal implant in a form of sheet or as a reinforced nasal implant. Results:The overall complication rate asociated with Gore-Tex was 2.5% (21 cases). Infection was the most com-mon complication (18 cases;2.1%) folowed by 2 cases of seroma and 1 case of persistent nasal swelling. In 19 out of 21 (91% removal rate). Nine cases of infection developed in both primary cases (1.37%) and in revision cases (4.57%), which suggests a higher asociation rate between in-fection and revision cases (p=0.0062). Infection developed within 1 month in 5 cases while 9 cases developed infection after 6 months of operation. Other complications such as aesthetic problems (malpositioning of the implant or dorsal irregularities) were found in 15 cases (1.8%) and hematoma in 1 case. Conclusion:The infection rate of Gore-Tex used in rhinoplasty was efore, we suggest judicious use of Gore-Tex in rhinoplasty.

      • A Case of an Orbital Abscess Managed by Endonasal Endoscopic Surgery

        An orbital abscess is a serious complication of sinusitis that can lead to blindness and sometimes death. Immediate surgicaldrainage via an endonasal endoscopic approach or through a traditional Lynch-type incision is required for successful treatment. We describe a case of an orbital abscess with a medial subperiosteal abscess complicating acute sinusitis in a five-month-old infant. The patient was successfully treated with endonasal endoscopic surgery. Following an anterior ethmoidectomy and an antrostomy,the medial subperiosteal abscess was drained by removing the involved lamina papyracea and the orbital abscess wasdrained by incising the periorbita.

        이중 상악동에서 발견된 치과 이물 1예

        We report a rare case of a foreign body occurring in a completely isolated, accessory maxillary sinus in a 29-year-old man. A dental Nu gauze accidently left in the accessory sinus after 3rd molar extraction resulted in an oroantral fistula with continuous pus drainage leading to his initial consultation. A complete healing of the oroantral fistula occurred after removal of the dental Nu gauze and the septum of the isolated accessory sinus with proper intranasal drainage by transantral approach.

      • 5 Cases of Major Complications after Rhinologic Surgery: Avoidable and Unavoidable Cases

        Otolaryngologists are sometimes confronted with various complications. They may be minor or sometimes they can bemajor, and rarely fatal. Some complications can be prevented, but some are unavoidable. Five cases of major complications,which occurred during or after routine rhinologic surgery, are presented. Two patients died of toxic shock syndrome: one patientafter endoscopic sinus surgery (ESS) and the other patient after closed nasal bone reduction. One patient died of acutemyocardial infarction during the recovery of anesthesia after uneventful ESS. One patient had a lidocaine shock, which requiredtreatment in the intensive care unit. One patient had a transient unilateral blindness after intranasal steroid injection. Though the 3rd and 4th cases of complications were unavoidable, the others could have been prevented if precautions hadtaken place. The presumed pathophysiology, possible prevention measures, and their implications in routine otolaryngologicsurgery are discussed.

        안와내벽 순수외향골절의 지연 정복

        The timing of surgery is an important consideration point in the management of blowout fracture (BOF) of the orbit. This study aimed to compare the surgical outcome of early and delayed reduction and to suggest the best timing of surgery in the pure medial BOF of the orbit. Subjects and Method:Fifty-two patients who had endoscopic endonasal reduction of medial BOF were studied retrospectively. Patients were divided into an early group (N=42) who had surgery within one month after the trauma and a delayed group (N=10) who had surgery after one month from the trauma. Surgical indications, results of the surgery, and complications were compared between the two groups. Results:The surgical indications in the early group were persistent diplopia, large defect expecting later enophthalmos and limitation of EOM, whereas the delayed group complained mainly of enophthalmos and diplopia. There were no significant differences in surgical outcomes and the rate of complications between the two groups. Conclusion:When patients with pure medial BOF have large defects without any persistent diplopia and limitation of EOM, surgery would be safely deferred until significant enophthalmos occurs.         동적 비밸브 허탈의 수술적 치료

        Collapse of the nasal valve is a rare cause of nasal obstruction in Asians, though it is relatively common in Caucasians. The thick skin and wide internal nasal valve angle were considered to contribute to the rarity of this entity. However, it does exist in Asians, especially who have thin skin and weak, collapsible nasal cartilages. The authors treated 2 patients with dynamic nasal valve collapse with cartilage grafts reinforcing the epicenter of valve collapse. This paper presents 2 cases of dynamic nasal valve collapse and introduces surgical maneuvers which were successfully applied to these cases.
