        경추부 이상 환자의 경⋅요추부 운동이 척주만곡과 체중심 이동에 미치는 영향

Therefore, we aimed to solve the problems and restore the body balance through applying cervico-lumbar exercise program to patients with cervical curvature deformity. Method: Twenty-seven patients with more than 5° from the normal curve were randomly assigned to the cervical exercise group or the cervico-lumbar group. In both groups, the patient's vertebral curvature is measured by X-ray and the body alignment value is measured by balance examination. After 8 weeks, the pre- and post-results are compared by remeasurement. We used the paired t test and the covariance analysis(ANCOVA) for examining mean value difference between the groups and comparative analysis in time period. Results: In both groups, there was a significant difference in cervical vertebrae angle before and after the experiment(p<.05). Thoracic kyphotic angle and lumbar lordosis appeared little difference in the control group while the experimental group showed a meaningful disparity(p<.05). The experimental group also brought a noteworthy distinction(p<.05) in body alignment value out of ears, acromion, femoral trochanter major compared to the minor difference of the control group. There was no statistical significance in the lateral meniscus of knee joint and lateral malleolus in both groups. Conclusion: In an experiment to patients with Cervical spine, the application of Cervico-Lumbar exercise program was more effective than cervical spine exercises.

        다발성 국소 골경화증을 동반한 다발성골수종 1예

        Osteosclerotic myeloma is a rare entity, characterized by single or multiple osteosclerotic bone lesions and usually accompanied by a polyneuropathy syndrome (POEMS). Multiple myeloma with osteosclerotic lesions without polyneuropathy is exceedingly rare. We report a case of multiple myeloma associated with multifocal osteosclerotic lesions without any evidence of POEMS. A 48-year-old woman presented with incidentally found osteosclerosis of 8th thoracic vertebra on a plain chest film. Bone survey, CT scan, MR scan, and radioisotope scintigraphy revealed multiple localized osteoclerosis; serum protein immunofixation showed IgG, lambda monoclonal gammopathy. A biopsy of T8 vertebral body disclosed plasma cell myeloma. Given that there was no organ or tissue damage other than multifocal osteosclerosis, the patient was placed on close observation with regular examination. This case indicates that although rare, multiple myeloma should be included in the differential diagnosis of sclerotic bone lesions.

        심장에서 CR6-interacting Factor 1의 미토콘드리아 활성조절

        Background: The major cause of metabolic syndrome and diabetes is reduced cellular performances in fuel metabolism, but the underlying pathways and mechanisms are not completely understood. Dysregulation of energy homeostasis can lead to metabolic disturbances and it predisposes diabetes, cardiovascular disease, aging, and cancer. CR6-interacting factor 1(CRIF1) contacts coiled-coil domain that is required for both genomic stability and mitochondrial integrity. We performed this study to determine the role of CRIF1 on the mice hearts. Methods: CRIF1-deficient mouse was embryonic lethal and we made heart specific CRIF1-deficient mouse using Cre-loxP system. We made thoracotomy and directly injected adeno-Cre virus into the heart of CRIF1-loxP mice. Beta-gal virus was used as a control. Results: Serial echocardiography showed decreased left ventricular ejection fraction and fractional shortening in the CRIF1-deficient mice at four and seven weeks later compared to wild type mice (p < 0.05). H&E showed increased myocardial inflammation in the CRIF1-deficient mice. Terminal deoxynucleotidyl transferase biotin-dUTP nick end labeling staining and LC3 staining showed increased apoptosis and autophage in CRIF1-deficient mice compared with wild type (p < 0.01). Electron microscopy revealed that the mitochondria in CRIF1-deficient cardiomyocytes showed abnormal morphogenesis. For example, the cells showed excessively fragmented mitochondria, intracristal swelling, and thinning of myocardial fiber. The stability of mitochondrial complexes in CRIF1-deficient cells showed marked derangements. Conclusions: CRIF1 is required for maintenance of normal mitochondrial function and modulate apoptosis and autophagy in the heart. 되었다. 또한 심근의 세포사멸 및 세포사멸 외 자식작용을 보기 위하여 시행한 terminal deoxynucleotidyl transferase biotin-dUTP nick end labeling 염색과 LC3염색에서 세포사멸의 증가 및 자식작용의 증가가 확인되었다. 결론: 심장 특이적 CRIF1의 결핍은 생쥐에서 심장 수축기 기능의 감소를 일으켰으며 이는 심근 내의 미토콘드리아의 변형으로 인한 심근세포사멸 및 자식작용의 증가와관련이 있다. 연구배경: 미토콘드리아의 수와 기능 이상은 비만, 인슐린 저항성, 당뇨병, 고지혈증, 고혈압 및 동맥경화증과 같은 만성 질환의 발생과 관련되어 있다고 알려져 있으며,CR6-interacting factor 1 (CRIF1) 단백질의 이상은 이러한 미토콘드리아의 수적, 기능적 이상을 유발한다고 한다. 본 연구에서는 심근에서 CRIF1의 활성을 조절함으로써 심근의 미토콘드리아에 미치는 영향 및 나아가 심장기능에 미치는 영향을 평가하고자 하였다. 방법: Cre-loxP system을 이용하여 heart specific CRIF1deficient mice를 만들기 위해 케타민 마취 후에 개흉술을시행 후 adeno-Cre virus 혹은 beta-gal virus(정상 대조군)를 심근에 주입한 뒤 심초음파검사와 전자현미경, 면역조직학적 검사를 시행하였다. 심초음파검사는 9마리 생쥐에서 0, 3, 7주에 시행하였다. 또한 2‑8주 차에 3군의 심장조직을 얻어 미토콘드리아의 관찰을 위한 전자현미경검사 및 세포사멸을 보기 위한 면역조직학적 검사를 하였다. 결과: 심장 특이적 CRIF1 결핍에 따른 심기능의 영향을알아보기 위해 시행한 연속적인 심초음파검사에서 heart specific CRIF1 deficient mice는 1주 후부터 fractional shortening과 ejection fraction이 감소하기 시작하여 7주후까지 지속된 감소 소견을 보였다. 전자현미경검사에서는 CRIF1 결핍 시 심근의 myofibrillar architecture와 미토콘드리아의 분열 및 해체로 인한 무질서한 형태가 관찰

        Successful Percutaneous Coronary Intervention in an Anomalous Origin of the Right Coronary Artery From the Ascending Aorta Above the Left Sinus of the Valsalva

        The anomalous origin of the right coronary artery (RCA) is a rare condition. Most RCA anomalies are usually found incidentally, but these findings have clinical significance because many patients, particularly young ones, present with sudden death, myocardial ischemia and syncope without other symptoms. We describe a case of a 39-year-old male patient that presented with effort chest pain and was diag-nosed with anomalous RCA that originated from the ascending aorta with prior history of repairing ruptured sinus valsalva and ventricular septal defect. The anomalous origin of RCA was identified by multidetector computed tomography (MDCT). Successful percutaneous coro -nary intervention was performed guided by MDCT coronary images and intravascular ultrasound.

        The Curves Exercise Suppresses Endotoxemia in Korean Women with Obesity

        Obesity and metabolic syndrome is a worldwide pandemic and associated with high cardiovascular risk. Metabolic endotoxemia (ME) is thought to be an underlying molecular mechanism. It triggers toll-like receptor 4-mediated inflammatory adipokines and causes a chronic low grade inflammatory status, which results in cardiovascular risk increase. Exercise is the best nonpharmacological treatment to improve prognosis. In this study, we examined the circulating endotoxin level in Korean obese women and investigated effects of exercise on it. Women over body mass index (BMI) 25 kg/m2 participated in a resistance training exercise, Curves. At baseline and after 12 weeks exercise, tests including blood samples were taken. In Korean obese women, the fasting endotoxin was 1.45 ± 0.11 EU/mL. Ingestion of a high calorie meal led to a peak level after 2 hours (postprandial 2 hours [PP2]) and a significant rise over the 4 hours (postprandial 4 hours [PP4]) in it (1.78 ± 0.15 and 1.75 ± 0.14 EU/mL for PP2 and PP4, P < 0.05 vs. fasting). After exercise, BMI and hip circumference were reduced significantly. The total cholesterol (TC) at fasting, PP2 and PP4 were decreased significantly. All levels of circulating endotoxin at fasting, PP2 and PP4 showed reduction. But, the peak change was only significant (baseline vs. 12 weeks for PP2; 1.78 ± 0.15 vs. 1.48 ± 0.06 EU/mL, P < 0.05). We report the circulating endotoxin level in Korean obese women for the first time. Also, we establish that energy intake leads to endotoxemia and exercise suppresses the peak endotoxemia after meal. It suggests an impact for a better prognosis in obese women who follow regular exercise.

        Cases showing memory disturbances with isolated retrograde memory without any definitive brain lesions are classified as dissociative amnesia. Improvements in imaging techniques have allowed the organic causes of this disease to be identified in several recent cases. Several studies have diagnosed cases of memory disorders as pure retrograde amnesia (PRA). The patient reported here had no psychiatric disorder and had normal MRI results, but showed hypometabolism in PET that met the criteria for PRA.

        간질발작과 기억장애를 보인 신경개회충증

        Toxocariasis has a broad clinical spectrum, from asymptomatic to systemic illness. However, cerebral toxocariasis has rarely been reported. We experienced a patient who presented with seizure and memory disturbance after treatment for visceral larva migrans of toxocariasis. Laboratory findings included eosinophilia, cerebrospinal fluid (CSF) pleocytosis, and positive results for toxocariasis in the serum and CSF. The patient showed marked improvement after combined albendazole and steroid treatment. This case indicates that cerebral toxocariasis is an unusual cause of cognitive dysfunction and seizure.
