        Nd : YAG laser 조사가 치근면 상아세관의 노출에 미치는 효과에 관한 주사전자현미경적 연구

        The purpose of this study was to evaluate the effects of Nd:YAG laser treatment on removal of smear layer and exposure of dentinal tubules. The experimental specimens were obtained from root planed surface of 30 human teeth which were extracted due to severe periodontal disease. The specimens were assigned randomly of three groups: root planed group, Tetracycline HCl(100mg/ml, 5min) group, and Nd:YAG laser ($Laser-35^{TM}$, U.S.A. 5 seconds )group. Nd:YAG laser group was divided into 4 subgroups according to 3.0W, 3.5W, 4.0W, 4.5W of power. The surface change of specimens were evaluated by scanning electron microscopy. The number of exposed dentinal tubules and percentage of area occupied by dentinal tubule orifices per unit area between each group was statistically analyzed by paired Student t-test. The results were as follows: 1. By root planing only, dentinal tubule was not exposed, but scale-like smear layer and parallel instrument tracks were resulted. 2. Tetracycline HCl treated surfaces exhibited the small number of partially exposed dentinal tubules with long orifices. The number($3.80{\pm}0.79$) of exposed dentinal tubules on Tetracycline HCl group was significantly less than that in laser groups above 3.5W of power(P<0.001), and the percentage ($0.68{\pm}0.19$) of area occufied by dentinal tubule orifices per unit area($192\;{\mu}m^2$) was significantly lower than that of any laser group(P<0.001). 3. The laser group irradiated with 4.5W of power showed both the most number($10.60{\pm}0.97$) of exposed dentinal tubules among the experimental groups, and the highest percentage($3.75{\pm}0.55\;%$) of area occupied by dentinal tubule orifices. 4. Energy Surge during laser pulsing and overlapping passes of the fiber resulted in melted and resolidified surface textures with lava-like appearance.

        The Result of Radiation Therapy of Superior Vena Cava Syndrome

        1978년 1월부터 1985년 12월까지 경희의대 부속병원 치료방사선과에서 상행정맥 증후군으로 방사선치료를 받은 환자 21명을 대상으로, 분할조사방법과 치료 총선량에 따른 초기종상의 완화와 치료에 대한 반응 및 생존율을 관찰하여 다음과 같은 결과를 얻었다. 1) 고 선량 치료군에서는 $54.5\%(6/11)$에서 치료 $1\~2$일 후에 증상의 판화를 보였으며 $81.7\%(9/11)$에서 치료 $3\~4$일내에 증상의 완화를 나타내었다. 2) 일반분할조사가 각각 $29\%,\;50\%$를 나타낸 것보다는 월등히 많은 환자에서 조기에 증상완화를 보였으나 두 군간의 생존율에는 유의한 차이가 없었다. 3) 치료후의 반응 판정에서는 총 선량 3,000c0y이상 치료한 군에서는 대부분이 반응을 나타냈으나, 3,000cGy이하를 치료한 군의 5예에서는 모두 반응을 나타내지 않았다. To access the result of radiation therapy for 8 years experiences, 21 patients who were treated with superior vena cava syndrome had been analysed according to dose fractionation and total dose. The results are as follows; 1. In high fractionate dose group, six of eleven patients $(54.5\%)$ exhibited relief of symptoms in 1-2 days, and additional three patients of nine $(81.7\%)$ within 34 days, while standard fractionated dose treatment is not effective to achieve initial relief of symptoms. 2. Graded response by total dose was correlated with total dose rather than dose fractionation. 3. Overall one year survival rate with superior vena cava syndrome was $9.1\%$ and mean survival was 4.2 months.

        테트라사이클린계 약물이 치은섬유아세포내 MMP-3의 활성도에 미치는 영향

        Extracellular matrix component is degraded by enzymes of thematrix metalloproteinases(MMPs). MMPs are produced by both hemopoietic and structural cells. Increased activity of MMP-3 in periodontium is strongly associated with inflammatory periodontal disease. The purpose of the present study was to determine the effect of tetracycline analogues on the activity of MMP-3. Tetracycline-HCl, doxycycline-HCl, and minocycline-HCl were applied to huamn gingival fibroblasts at various concentrations of 10, 25, 50, 100, 200${\mu}g$/ml, and 1 hour later IL-$1{\beta}$ of 25ng/ml was added. After incubation for 24 hours the cells were reacted by enzyme-linked immunosorbent assay using proMMP-3 ELISA kit. The optical density was measured by microwell plate reader at 450nm. The relative activity of MMP-3 was calculated as the percentage of the optical density of each experimental group to that of the control. The difference of the optical density and the relative activity of MMP-3 between the experimental groups and the control wasstatistically analyzed by one way ANOVA. The results were as follows: 1. Tetracycline-HCl showed the tendency to inhibit the activity of MMP-3 at the concentration lower than 25${\mu}g$/ml, but increased significantly the activity of MMP-3 at the concentration of 200${\mu}g$/ml(p<0.05). 2. Doxycycline-HCl inhibited significantly the activity of MMP-3 at the concentration lower than 100${\mu}g$/ml, but increased significantly the activity of MMP-3 at the concentration of 200${\mu}g$/ml(p<0.05). 3. Minocycline-HCl inhibited the activity of MMP-3 at the concentration in the range of 10 to 200${\mu}g$/ml. Within the limit of the present study, the above results suggested that the low concentration of tetracycline analogues could inhibit the activity of MMP-3 induced by IL-$1{\beta}$ in human gingival fibroblasts.

        치주질환에서 matrix metalloproteinases의 역할 : Literature Review

        Extracellular matrix component is degraded by enzymes of the matrix metalloproteinases (MMPs). MMPs are produced by both hernopoietic and structural cells of the periodontium. MMPs(MMP-8) in inflamed gingival tissue of adult periodontitis patients originate primarily from infiltrating neutrophils, in contrast, MMP-1 in localized juvenile periodontitis(mainly fibroblast type collagenase). Generally, high levels of MMPs indicate diseased condition. MMPs can be inhibited by the scaling and root planing, and tetracycline family of drugs to produce clinical benefits. In near future, the diagnostic and therapeutic potential of MMps and their key role in the pathogenesis of periodontal disease will bring the development of their inhibitors.

        야구 투수의 투구 동작 시 왼발과 오른발의 하지관절모멘트 분석

        The purpose of this sturdy were to provide objective base data about effective lower extremity joint movement by analysing it. Three dimensional motion analysis and ground reaction force data were conducted on middle school baseball players. As a result of measuring and analysing lower extremity joint moment on pitching motion, the following conclusions were drawn out. First, at the early cocking stage, the right leg, which was the supporting one at the wind-up stage of pitching, taked a stabilized posture by lowering the center of mass by slightly bending down the knee, putting the foot nearby the line of gravity. Second, at the beginning of late cocking stage, moved the body toward pitching direction pushing the ground firmly while extending ankle, knee and hip joint as a reaction of extensor and abductor. At this time, left hip joint stretches far as the leg being extended high up by flexor continuously acting, and therefore, gets a strong momentum by lengthening travel range. Third, at the stage of acceleration, when setting left leg out on the ground, ankle and hip joint act flexor, lower body and upper body accelerated forward more faster, and knee joint support strong momentum toward the front with the reaction of extensor and adductor. Last, at the stage of deceleration and follow through after the ball being released, all the flexor and adductor of an ankle, knee and hip joint decreased the speed and the impulsive force on the body. 이 연구의 목적은 중학교 야구 선수들을 대상으로 야구 투구 동작의 3차원 영상분석과 지면반력 데이터를 이용하여, 하지 관절모멘트의 변화 양상을 분석함으로써, 하지관절의 효율적인 움직임에 대한 객관적인 근거자료를 제시하는데 있다. 이러한 연구목적을 달성하기 위해서 중학교 야구 선수 3명을 대상으로 3차원 영상분석장비와 지면반력 측정 장비를 이용하여 야구 투구 동작에 대한 왼발과 오른발의 하지 관절모멘트를 측정 분석한 결과, 다음과 같은 결론을 도출하였다. 투구 동작의 와인드업(wind-up)단계에서 지지발이 되는 오른쪽 다리는 초기 코킹(early cocking)단계에서 무릎을 약간 굽혀 중심을 낮추고 신체 중심선 가까이에 발을 위치하여 안정된 자세를 취하며, 후기 코킹(late cocking)단계로 접어들면서 하지의 신전근과 외전근의 작용으로 고관절, 무릎 관절, 발목 관절을 순차적으로 빠르게 신전하여 지면을 강하게 밀어내며 신체를 투구 방향으로 전진시켰다. 이때, 왼쪽 다리의 고관절은 굴곡근이 지속적으로 작용하여 다리를 높이 들어 올린 상태로 홈플레이트를 향해 멀리 뻗게 되며, 이로 인하여 신체 중심의 수평방향 이동거리가 길어지면서 큰 추진력을 얻게 되었다. 가속(acceleration)단계에서는 왼쪽 다리를 지면에 내딛으면서 발목과 고관절은 굴곡근이 작용하여 하체와 상체가 더욱 빠르게 전방을 향하여 가속하게 되고 무릎관절은 신전근과 내전근이 강하게 작용하여 전방을 향한 강한 추진력을 지탱하였다. 공이 릴리스 된 후 감속(deceleration)과 폴로스루(follow through)단계에서는 신체에 가해지는 충격을 완화시키기 위히여 발목, 무릎, 고관절이 모두 굴곡근과 내전근이 주도적으로 작용하였다.
