증례 : 감염 ; 진균과 세균 복합 감염의 척추체-추간판염 1예

내원 3개월 전 척추 수술을 시행 받았던 66세 여자 환자가 요통 및 하지 동통으로 내원하였다. Magnetic resonance image상 척추체-추간판염 소견이 보였으며 조직 검사를 통한 세균 배양 검사에서 Enterococcus faecium과 Candida parapsilosis가 동정되어 진균과 세균의 복합 감염으로 확인되었다. 저자는 적극적인 조직 배양 검사를 통해 진균과 세균의 복합 감염을 정확히 진단함으로써 이에 따른 적절한 치료를 시행하여 문헌고찰과 함께 보고하는 바이다. 또한 본 사례를 통해 진단율을 높이는 적극적인 조직 검사를 독려하고자 한다. A 66-year-old female who had undergone surgery for a herniated disc at the L5-S1 level at another hospital 3 months earlier was admitted with persistent back and leg pain. She was diagnosed with spondylodiscitis at the L5-S1 level by magnetic resonance imaging. A biopsy was performed, and Candida parapsilosis and Enterococcus faecium were isolated from the excised material. We report herein a case of successful treatment of polymicrobial spondylodiscitis in accordance with accurate microbiological diagnosis. Based on this case, we hope to encourage physicians to perform biopsies more aggressively or repeatedly to improve the diagnostic yield.

타자윤리적 그림책을 활용한 감성의 도덕교육 프로그램이 유아의 공감과 친사회적 행동에 미치는 영향

본 연구의 목적은 타자윤리적 그림책을 활용한 감성의 도덕교육 프로그램이 유아의 공감과 친사회적 행동에 미치는 영향을 검증하고자 하는 것이다. D시에 위치한 I유치원의 만 5세반 유아 20명을 실험집단에, W유치원의 만 5세반 유아 20명을 통제집단에 임의 할당하였다. 실험집단에는 감성의 도덕교육 프로그램을 8주간 총 32회에 실시하였다. 공감검사와 친사회적 행동검사를 사용하여 수집된 자료를 공분산분석을 하였다. 본 연구의 결과를 살펴보면, 본 프로그램을 실시한 실험집단은 그렇지 않은 통제집단보다 공감과 친사회적 행동의 전체와 하위요인 모두에서 유의미하게 점수가 높았다. 이러한 연구결과는 본 프로그램이 유아의 공감과 친사회적 행동을 함양하는데 있어 유용한 프로그램임을 시사한다. The purpose of this study was to investigate the effects of affective moral education program using picture books based on ethics of others on 5-year-old children’s empathy and prosocial behavior. The subjects of this study consisted of 20 5-year-old children for the experimental group at I kindergarten and 20 5-year-old children for the control group at W kindergarten in D city. The experimental group participated in the program during a span of 32 times for 8 weeks. Data were analyzed using ANCOVA which was applied for the data collected from both the empathy test and the prosocial behavior test. The results of this study were as follows. First, the experimental group showed higher scores of empathy and its subordinate factors than those of control group. Second, the experimental group showed higher scores of prosocial behavior and its subordinate factors than those of control group. The results of this study suggest that the affective moral education program using picture books based on ethics of others provide useful effects on improving young children’s empathy and prosocial behavior.

그림책에 나타난 놀이성 탐구 : 데이빗 셰논의 ‘데이빗 삼부작’ 중심으로

본 연구의 목적은 David Shannon의 연작그림책 ‘데이빗 삼부작’에 나타난 데이빗의 놀이성을 분석하는 것이다. 이를 위해 ‘안돼 데이빗’, ‘말썽꾸러기 데이빗’, ‘유치원에 간 데이빗’ 그림책 3권을 선정하였다. 연구 결과 ‘데이빗 삼부작’에 나타난 데이빗의 놀이성은 첫째, 신체움직임 기술로 구현되는 신체적 자발성, 상징놀이로 발현되는 인지적 자발성, 성취의 절정인 즐거움의 표현, 익살스런 유머감각, 낮은 수준의 사회적 자발성으로 나타났다. 둘째, ‘데이빗 삼부작’에 나타난 데이빗의 놀이성에 영향을 주는 주변인물의 양육태도의 특성은 엄마의 통제적 양육행동과 교사의 과도한 행동주의적 기법이다. 본 연구결과를 통하여 유아기의 놀이성은 개인적 기질, 양육환경에 많은 영향을 받음을 알 수 있으며 유아의 놀이성 발달을 위한 모색 방향의 함의를 가진다. The purpose of this study is to analyze David s playfulness in David Shannon s ‘David Trilogy.’ To this end, three picture books were selected, ‘No, David,’ ‘David Gets In Trouble’ and ‘David Goes To School’ As a result of the study, David s playfulness in the David Trilogy is firstly physical spontaneity embodied in body movement techniques, cognitive spontaneity expressed as symbolic play, expressions of joy at the peak of achievement, a humorous sense of humor, and a low level of social spontaneity. Second, the behavioral characteristics of the surrounding person that affect David s playfulness in The David Trilogy are his mother s controlling parenting behavior and the teacher s excessive activism. The results of this study show that playfulness in infancy is greatly influenced by personal temperament and parenting environment, and have implications for the development of playfulness in infants.

레오 리오니 그림책에 나타난 타자윤리와 배려적사고의 실천: 레비나스 관점을 중심으로

본 연구의 목적은 레오 리오니(Leo Lionni)그림책에 나타난 타자윤리와 윤리적 호소 및 요청의 유형과 도덕적 명령에 따른 배려적 사고의 실천 양상을 레비나스(Emmanuei Levinas)의 관점에서 살펴보는 것이다. 이를 위해 레오 리오니 그림책 10편을 분석대상으로 하여 타자윤리의 윤리적 호소 및 요청의 유형과 도덕적 명령에 따른 배려적 사고의 실천 양상을 살펴보았다. 연구결과는 다음과 같다. 첫째, 레오 리오니 그림책에 나타난 타자윤리의 윤리적 호소 및 요청의 유형은 현실에 대한 어려움에 대한 요청과 인정과 지지를 호소하는 두 가지로 나타났다. 둘째, 타자윤리에 의거한 도덕적 명령에 따른 배려적 사고의 실천 양상은 타자의 어려운 현실극복을 위한 행동적 사고의 실천, 가치부여적 사고, 정서적 사고로 나타났다. 본 연구를 토대로 레오 리오니 그림책은 타자윤리를 체계적이고 풍부하게 내포하고 있음을 알 수 있었다. 즉, 그림책 속에서 타자는 얼굴을 통해 인간 본연의 도덕성에 대한 윤리적 호소와 요청을 하고 있을 뿐 아니라 타자에 대한 책임이 적극적인 도덕적 명령과 실천에 따라 배려, 인정, 나눔, 환영, 응답, 연대성 등 배려적 사고의 실천의 유형으로 표상 되어 있었다. The purpose of this study was to investigate the ethics for the other, types of ethical appeal and requests and the practice of caring thinking as a result of moral imperatives shown in Leo Lionni’s picture books from the perspective of Levinas. For this, based on ten of L. Lionni’s picture books, which revealed ethical appeal and requests in accordance to the ethics for the other at their best, it was analyzed how moral imperatives derived from ethical appeal and requests affect the practice of considerate thinking. The results were as follows. First, the ethical appeal and requests from his books were expressed in two ways: Requests caused by real difficulties and an appeal for recognition and support. Second, the practice of caring thinking as a result of moral imperatives in accordance with the ethics for the other was done in two ways: The practice of behavior-oriented thinking to overcome the other’s practical difficulties and the practice of value-oriented thinking to respect the other. It was verified from the study that the ethics for the other were expressed systematically and fluently in L. Lionni’s books. That is, responsibility of the other was represented in the form of caring thinking such as consideration, recognition, sharing, welcoming, and solidarity according to moral imperatives, and their practice as well as the other makes an ethical appeal and requests for people’s inherent morality through face.

장기간 면역억제제를 사용하는 경우 감염 등의 합병증뿐만 아니 장기간의 면역억제로 인한 림프종의 발생위험도 고려해야 한다. 저자들은 류마티스관절염으로 MTX를 15년간복용한 병력이 있는 환자에서 간과 비장을 침범한 광범위큰 B세포림프종이 발생한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Prolonged inflammation or immunosuppression may cause lymphoproliferative disorders (LPD). Rheumatoid arthritis (RA) itself causes LPD. The immunosuppressive drug methotrexate (MTX) is used widely to treat RA, and is also related to the development of lymphoma. Although the detailed pathogenesis and association are still controversial, care is needed when managing patients with RA treated with MTX. Many reports indicate that discontinuing MTX can induce the regression of LPD. Here, we describe a case of diffuse large B-cell lymphoma that presented with liver involvement in a 67-year-old woman who had used MTX for RA for 15 years. (Korean J Med 2015;88:715-718)
