In this paper, we consider the parameter space of the rational plane curves with uni-branched singularity. We show that such a parameter space is decomposable into irreducible components which are rational varieties. Rational parametrizations of the irreducible components are given in a constructive way, by a repeated use of Abhyankar-Moh Epimorphism Theorem. We compute an enumerative invariant of this parameter space, and include explicit computational examples to recover some classically-known invariants. Anti-holomorphic twistor and symplectic structure

It is well known that the twistor, section of twistorspace, classify the orthogonal almost complex structure on evendimensional Riemannian manifold ( X;g). We will show that exis-tence of a harmonic and anti-holomorphic twistor is equivalent tohaving a symplectic structure on (X;g).

교통성 뇌수종에서 수술 전 자기공명 영화촬영술의 임상적 의의

Objective:To evaluate the changes of intracranial cerebrospinal fluid(CSF) dynamics in communicating hydrocephalus, the authors present an analysis of various parameters of cine magnetic resonance(MR) CSF flow images in case of progressive communicating hydrocephalus. Methods:The MR images were obtained with 1.5T(GE Signa, GE Medical Systems, Milwaukee, USA) unit using the 2 dimensional cine phase contrast sequence with cardiac gating and gradient recalled echo imaging in 10 communicating hydrocephalus and 10 controls. Various parameters of the aqueduct and cervicomedullary subarachnoid space were plotted as wave forms. The wave forms were analyzed for configurations, amplitude parameters(Vmax, Vmin, Vdif), and temporal parameters(R-MSV, R-D, R-MDV, R-S). The statistical significance of each parameter examined with paired t-test. Results:Distinct reproducible configuration features were obtained at both ROIs. We could determine the statistically significant differences between control and communicating hydrocephalus in temporal parameters at the aqueduct level. There was no significant differences at the cervicomedullary subarachnoid space level. In communicating hydrocephalus, the graph showed R-MDV(p=0.005) and R-S(p=0.001) shortening at the aqueduct. Conclusion:The analysis of cine MR CSF flow study may be helpful for diagnosing the communicating hydrocephalus and even deciding the necessity of shunting procedures. The temporal parameters are more important than amplitude parameters for diagnosing communicating hydrocephalus. Key words:Hydrocephalus;Cine MRI;Cerebrospinal Fluid.

Three cases operated on with anterior micorforaminotomy because of chronic symptoms after whiplash injury were analyzed. The indications for surgery were medically intractable, definite cervical radicular symptoms with trivial cervical pathology on preoperative usual radiological studies. On operative findings, the minor disc protrusion and thickening of posterior longitudinal ligament were the representative pathologies. At follow-up, all patients showed disappearance of pre-operative symptoms. Based on Robinson's criteria for the surgical results, all 3 patients had good results. We suggest the trivial cervical pathologies, proved by the target-oriented, anterior microforaminotomy, can be the origin of cervical radiculopathy after low-velocity, rear-car collision injury in our series.

Clinical Analysis for Stereotatic Aspiration and Thrombolysis of Spontaneous Intracerebral Hemorrhage

목적 : 저자들은 자발성 뇌출혈 환자에서 뇌 정위적 혈종 배액술 및 용해술의 경험을 분석하여 이 방법의 유용성, 안전성 및 예후인자 등을 평가하고자 한다. 방법 : 기질적인 원인 혹은 혈액 응고성 질환이 없으면서 천막상부 자발성 뇌출혈 용량이 25ml이상 되는 11명의 환자를 대상으로 하였다. 이 중 글라스고우 혼수계수가 5이하인 환자는 대상에서 제외하였다. 컴퓨터 전산촬영 유도하에 카테터를 자발성 뇌출혈에 정위적으로 유치하고 유로키나제를 주입한 후 현종을 흡인하여 제거하였다. 이 과정은 처음 혈종 용량의 절반 이하가 될 때까지 6시간 간격으로 반복하였다. 예후인자를 분석하기 위하여 환자를 두 개의 집단 즉, 글라스고우 결과계수가 4이상의 양호한 집단과 글라스고우 결과계수가 4이하의 불량한 집단으로 나누어 서로 비교분석 하였다. 결과 : 환자들의 평균연령은 59.8세이고, 초기 혈종의 용량의 범위는 15에서 72ml였다. 혈종 용량의 감소비율은 평균 74.2%였다. 시술 후 6개월 추적관찰결과, 56(50.9%)명의 환자는 양호한 회복을 보였고, 29(26.3%)명의 환자는 의존적인 상태였으며, 10(9.0%)명의 환자는 식물인간상태로 유지되었다. 그리고, 15(13.6%)명의 치료도중 사망하였다. 주요한 중요 예후인자는 젊은 나이, 적은 혈종용량, 높은 글라스고우 혼수계수 및 재출혈, 기존질환, 합병증이 없는 경우였다. 결론 : 자발성 뇌출혈 환자에서 뇌 정위적 혈종 배액술 및 용해술이 혈종의 용량을 감소에는 안전하고 효율적인 방법임을 알 수 있었다. 그리고, 불량한 예후인자를 가지고 있는 자발성 뇌출혈 환자는 주기적인방사선학적 검사와 더욱 세심한 시술이 필요할 것으로 사료된다 그리고, 앞으로 연구는 적절한 용해제 용량의 평가와 치사율과 장해율을 감안한 정밀한 비교분석이 있어야 한다고 생각된다.

포진후 통증환자에 대한 신경차단술의 효능 : 예비적 보고

Objectives : Of all the chronic pain syndromes postherpetic neuralgia ranks the most refractory to treatment. The eight postherpetic neuralgia patients have been treated with nerve blocks or neurolysis and obtained good result. We evaluate the therapeutic effect of nerve blocks for postherpetic neuralgia and reviewed the pathology, clinical consideration, treatments and prevention. Materials and Methods : Eight patients with established postherpetic neuralgia enrolled in pain clinic of Mokdong Hospital of Ewha Womans University from March,2004 to December. Average age at 58 and about 63% of patients were over 68 years. Affected area of forehead was two patients. They have been treated with stellate ganglion block (SCB), two times supraorbital and supratrochlear nerves blocks and neurolysis with absolute alcohol. Face affected patient has was one and has been treated with 5 times SGB, 2 times supraorbital and supratrochlear nerves blocks, infraorbital and mental nerves blocks, and then 2times maxillary and mandibular nerves blocks. Chest affected two patients were treated with intercostal nerve blocks. Thoracic wall affected one patient received 2 times thoracic epidural blocks and thoracic nerve block. Scapalur and thoracic wall affected patient has been treated with SGB and 2 times thoracic epidural block. The chest and thoracic wall affected patient received 10 times intercostal blocks,2 times thoracic epidural blocks,3 times thoracic root block and neurolysis of intercostal nerves with absolute alcohol. The injected agents were 1% mepibacaine or 0.25% mercain heavy and methylprednisolone succinated, and at weekly intervals the agents were injected. Results : All patient having been treated with nerve blocks were free of pain at 3-9 months. Conclusion : Although no controlled trial has been done of nerve blocks to treat postherpetic neuralgia, this technique is safe in experienced hands and, if effected, may be repeated. It suggested that local anesthetics and steroid are the effective components in nerve blocks. Data on the relief of acute pain with nerve blocks, however cannot be extrapolated to predict the prevention of postherpetic neuralgia. The best way to prevent herpes zoster becasuse of post herpetic neuralgia is so difficult to treat.

지난 30년 동안 수학계는 순수수학과 응용수학, 두 분야 모두에서 괄목할 만한 발전을 이룩하였다. 순수수학 분야에서는 무엇보다도 먼저 정수론, 더 나아가서는 수학의 모든 분야에서 가장 오래된(360년) 미해결 문제의 하나인 페르마의 마지막 정리가 증명되었다. 또 위상수학분야의 가장 큰 미해결문제인 푸앙카레 예상이 2006년도에 증명되었다. 응용수학 분야에서는 CT 스캔 등 여러 의료 분야에도 수학이 깊숙이 관계되고 있지만. 특히 암호학에서 괄목할 만한 발전을 이루어 요즘 전자상거래, 특히 신용카드를 사용할 때 이용되는 암호기술, 공개키 RSA 방법이 중요하게 쓰이고 있다.
