        Clinical Effects and Radiological Results of Vertebroplasty: Over a 2-year Follow-Up Period

        Objective: We investigated the association between clinical and radiological results and assessed the radiological changes according to the distribution pattern and amount of injected cement after vertebroplasty. Methods: Two hundred and one patients underwent vertebroplasty; of these, 15 were follow up for more than 2 years. For radiological analysis, we grouped the patients according to cement distribution as follows: group 1, unilateral, unilateral distribution of cement; group 2, bilateral-uneven, bilateral distribution of cement but separated mass; and group 3, bilateral-even, bilateral single mass of cement. To compare radiologic with clinical results, we assessed the visual analogue scale (VAS) score, amount of injected cement, bone mineral density (BMD), postoperative and follow-up vertebral body compression ratios, and postoperative and follow-up kyphotic angles. Results: There were 4 (26.7%) patients in group 1, 6 (40.0%) in group 2, and 5 (33.3%) in group 3. The mean VAS score was 5.2 preoperatively, 1.8 postoperatively, and 3.2 at 2-year follow-up. The 2-year follow-up compression ratio was better in patients with even distribution of injected cement (group 2 and 3) than group 1. However, it was not statistically insignificant (p>0.05). The follow-up kyphotic angle was more aggravated in the group 1 than in the other groups (p<0.05). Conclusion: Our study showed that vertebroplasty had a beneficial effect on pain relief, particularly in the immediate postoperative stage. The augmented spine tended to be more stable in the cases with increased amount and more even distribution of injected cement.

        Wnt3a-Producing Fibroblasts in Ovariectomy-Induced Osteoporosis in a Rat Model

        We investigated the effect of in vitro cultured Wnt3a-producing fibroblasts on positive bone balance and regeneration of the osteopenic skeleton. Thirty-six female Wistar rats (250–300 g, aged 12 weeks) were randomized into three groups: a control group (sham-operated), ovariectomy (OVX) group, and OVX with Wnt-cell injection (OVX with Wnt) group. Wnt3a-producing fibroblasts were injected into the lateral tail vein at 2 and 4 weeks after OVX. Both tibias were removed at 5, 6, 7, and 8 weeks after OVX, and pathological and micro- CT evaluations were performed. We also evaluated β-catenin expression by immunohistochemical staining. For bone metabolism detection, we evaluated the expression of bone alkaline phosphatase (BALP), osteocalcin, and C-telopeptide of collagen type I. Bone mineral density, trabecular bone volume, trabecular number, trabecular thickness were higher, while trabecular seperation was lower, in the OVX with Wnt group than in the OVX group. BALP and osteocalcin levels were significantly higher in the OVX with Wnt group compared to the control and OVX groups. β-catenin expression was significantly lower in the OVX group than in the other two groups. Based on these results, we hypothesized that Wnt3a-producing fibroblasts may be effective for the induction of bone formation.

        Adipose Tissue-Derived Stem Cells in the Ovariectomy-Induced Postmenopausal Osteoporosis Rat Model

        This study investigated the efficacy of adult adipose tissue-derived stem cells in restoring bone using anosteoporotic rat model. Thirty-six female Wistar rats (250 - 300 g, 12 weeks) were randomized into three equalgroups: SHAM group (sham-operated), ovariectomy-induced (OVX) group and OVX with stem cell injection group(OVX with stem). Femur extraction and blood sampling were performed at 5, 6, 7 and 8 weeks. The proximal femoralmetaphysis was scanned by micro-CT and evaluated for changes in various histomorphometric parameters. β-catenin expression was determined using H&E and immunohistochemistry. Bone metabolism was assessed usingmeasurements of C-telopeptide of collagen type I (CTX), osteocalcin, and bone alkaline phosphatase (BALP). Therewas a trend for an increase in the bone mineral density (BMD), bone volume/trabecular bone volume (BV/TV) andtrabecular number (Tb.N) in the OVX with stem group as compared with the OVX group, and decrease in the OVXwith stem group as compared with SHAM group. The trabecular thickness (Tb.Th) and trabecular separation(Tb.Sp) of the OVX group were increased compared with those values in the SHAM and OVX with stem group. BALP levels were higher in the OVX group as compared with that in the other two groups and osteocalcin levelswere highest in the SHAM group and slightly increased in the OVX with stem group as compared with the OVXgroup. This study may help us gain an understanding of the role of MSCs in the pathophysiology and treatment ofosteoporosis.

        대학에 대한 사회의 요구가 매우 다양하다. 우선 우리 사회의 산업 구조에 급격한 변화가 있으니 대학에서의 인력 양성 방식도 변해야한다는 주장이 있다. 대학 졸업생들이 교육과 산업 현장에서의 요구 사이에 차이가 너무 커서 산업현장에서 바로 활용하기 어렵다는 비판이 뒤따른다. 저성장에 따른 경기 침체와 노동 시장의 경직성으로 인한 청년 실업 문제는 분명히 사회 경제적인 요인에 따른 것이지만 청년 실업에 대한 해결책도 대학에서 찾으려는 경향이 있다. IMF 이후 우리 사회가 겪고 있는 저성장의 원인까지도 대학 교육의 비효율성 때문이라는 주장이 있다. 그동안 대학에서 행해진 연구가 산업에 곧바로 적용하기 어려운 것이었다는 비판도 있다. 내년에 시작하는 제2단계 BK21 사업은 대학원 인력 양성 사업인데, '산학협력'과 연관된 실적이 사업단 선정과 실적 평가에 주요 항목이 될 것이라 한다. '스탠포드 대학과 칭화 대학의 산학 협력 성과를 벤치마킹해야 한다'는 목소리가 대단히 높다. 대학에 대한 사회의 기대가 대단히 높다는 점과 '산학협력'이 오늘날 우리 대학의 화두라는 점을 염두에 두고, 이 글에서 산학협력재단의 현황과 문제점, 향후 과제에 대한 개인적인 견해를 말하려고 한다.

        Greetings from the New Editor-in-Chief: I Would Like to Begin by Expressing My Heartfelt Thanks

        I am honored to be appointed as the new Editor-in-Chief of the Korean Journal of Neurotrauma (KJNT). I am especially thankful to the former editor Prof. Sun-Chul Hwang for his dedication to the journal during the last five years. I am also pleased to inform you that Prof. Suk-Hyung Kang and Prof. Il Choi as deputy editors-in-chief, and Prof. Hyuk-Jin Oh as secretary of the publication, will assist me with manuscript management and journal production. KJNT was first published in 2005 as the official journal of the Korean Neurotraumatology Society. The KJNT has been published in English since 2014 and is indexed in PubMed, PubMed Central, KoreaMed, Synapse, SCOPUS, MedRIC, MEDLIS, KCI, and Google Scholar. It can surely be said that a lot of effort and sacrifice were put in by the previous Editors-in-Chief, the members of the editorial committee, and the reviewers. I would like to express special thanks to them. The Korean Neurosurgical Society is positioned as a leading neurosurgery society in Asia and the world. As a reflection of this, many journals related to neurosurgery in Korea have shown outstanding results in various fields. Recently, Neurospine, (renamed after the March issue, 2018) which is the official journal of ASIA SPINE, the Neurospinal Society of Japan, Taiwan Neurosurgical Spine Society, and the Korean Spinal Neurosurgery Society, was registered in the SCI(E). I think this is truly remarkable and deserves hearty congratulations, and I hope our journal will achieve the same. As we already know, the main articles submitted to journals of the divisional societies of neurosurgery are on neurosurgery residents' training. However, the number of journals for residents' training has decreased and is expected to decrease further. Furthermore, the number of journals of the divisional societies of neurosurgery in our country is increasing rapidly. This condition is the same for KJNT. To make matters worse, KJNT has already been registered in PubMed and PubMed Central, Scopus. Therefore, it should satisfy the standards required of domestic journals, and the published articles should be of the quality and quantity that meet international standards. For an editor who has just taken over, this fact is a huge challenge that cannot be ignored, and must be taken up. As a small start, we would like to publish the April issue as a special issue on “Clinical Guidelines” and the first, October issue as special issue on “Trauma Data Bank.” For each issue, we will make preparations and seek to recruit authors for editorials, review articles, and clinical articles. Second, with the efforts of Prof. Sun-Chul Hwang, KJNT has already activated joint publication as the official journal of the Korean Spinal Cord Society since 2017. This joint publication of related academic societies will help to strengthen the quality and quantity of the published articles. Therefore, we plan to contact other divisional societies of neurosurgery and expand by the joint publication of the journal with these divisional societies of neurosurgery. I think that it will take a considerable time for the results of these efforts to bear fruit. Although I am a newly appointed editor, our editorial team and I will strive to achieve these results. Please submit your research and opinions. We will always welcome them.

        후순환계 뇌동맥류의 임상양상과 치료예후 - 전순환계 동맥류와의 비교분석을 중심으로 -

        Object : With the recent variable treatment modalities and the development of microsurgical techniques, outcomes of surgical and medical management of aneurysm have shown much progress in the last 10 years. However, the management of posterior circulation aneurysm is still a debatable due to its difficulty in limited surgical approach, complicated anatomical structure and many small perforators to vital structure. The purpose of this study is to compare the results of clinical manifestation and outcome of surgery with respect to anterior and posterior circulation aneurysms. Material and Methods : We evaluated the 33 patients with PCAs(posterior circulation aneurysm) and 359 patients with ACAs(anterior circulation aneurysm) treated between 1994 and 1999, retrospectively. Results : Posterior circulation aneurysms showed higher tendency(5 cases, 14.7%) to have unusual shapes, such as dissecting or fusiform compared with anterior circulation aneurysm(15 cases, 4.2%). There were more multiple aneurysms in posterior circulation aneurysm(8 cases, 26.5%) than anterior circulation aneurysm(59 cases, 16.2%). The number of patients with Hunt-Hess grade III or IV on admission were 91(25.3%) in anterior circulation aneurysms, and 14(42.4%) in posterior circulation aneurysms. There were higher incidences of vasospasm and acute hydrocephalus in patients with posterior circulation aneurysm. In cases of anterior circulation aneurysm, neck clipping was possible in 97%. But, in posterior circulation aneurysm, neck clipping was possible only in 67.7% of each. Two hundred forty four cases(85.0%) of all anterior circulation aneurysms and 22 cases(78.6%) of all posterior circulation aneurysms showed good recovery(GR) or moderate disability(MD). The postoperative mortality rates of anterior and posterior circulation aneurysms were 4.9% and 10.7%, respectively. Conclusion : These results indicate that there exist substantial differences with respect to that there were few difference in the aspect of surgery and management outcome between posterior circulation aneurysms and anterior circulation aneurysms.

      • Immobilization of Rat Brain Arylsulfatase A on Sepharose 4B

        브롬화 시안으로 활성화시킨 Sepharose 4B에 쥐 뇌로부터 정제한 arylsulfatase A단량체를 고정화시켰다. 효소의 99%까지 고정화 시킬 수 있었으며 본래의 효소와 고정화된 효소의 일반적 성질을 비교하였다. 고정후에도 효소활성이 잘 보존되었으며, Km 값의 증가 및 pH와 열에 대한 효소안정도의 향상이외에는 효소의 일반적 성질은 거의 변하지 않았다. 이는 Sepharose 4B에 공유결합으로 고정되는 효소의 활성부위가거의 영향을 받지 않고 있음을 나타낸다. 특히 고정화된 효소도 수소이온 농도변화에 따른 최적점이 두 개로 나타나는데, 이는 단량체-이량체 결속현상으로 만은 설명되지 못함을 암시해주고 있다. Rat brain arylsulfatase A monomer was immobilized to CNBr-activated Sepharose 4B. The coupling of the enzyme could be accomplished up to the degree of 99%. The general properties of the immobilized arylsulfatase A were compared with those of the soluble enzyme. The enzymatic activity of the enzyme was conserved well by immobilization and the catalytic properties of the immobilized enzyme were similar to the soluble one except the increased Km value and the improvement of stabilities toward heat and pH. Thus the covalent coupling of the enzyme to Sepharose 4B did not affect vitally the functional groups of the active site of the enzyme. The two-pH optima profile of the immobilized enzyme imply that the multiple pH optima of the arylsulfatase A could not be explained solely by the monomer-dimer association.

        Validation of the European Quality of Life-5 Dimensions, 3-Level Version (EQ-5D-3L) in Patients With Osteoporotic Vertebral Fracture and Comparing Their Health-Related Quality of Life with That of the General Population in Korea

        Objective: This study examined the validity of the European Quality of Life-5 dimensions, 3-Level version (EQ-5D-3L) in patients with osteoporotic vertebral compression fractures (OVCF) and compared the health-related quality of life (HRQOL) of these patients with that of the general population in Korea. Methods: Study participants completed several questionnaires, such as the EQ-5D-3L, 12-item short-form health survey version 2.0 (SF-12v2), Roland-Morris disability questionnaire (RMQ), and Pain Numeric Rating Scale (Pain NRS). Spearman’s correlation analyses of the questionnaire results were conducted to examine the discriminant and convergent validity of the EQ-5D-3L. Finally, EQ-5D-3L results from study participants were compared with those from the comparison group of the general population that were match-sampled according to age and sex from the 2013 Korean National Health and Nutritional Examination Survey. Results: Spearman’s correlation coefficients between EQ-5D-3L and pain NRS, SF-12v2, and RMQ were statistically significant. The coefficients between the relevant dimensions, such as mobility in the EQ-5D-3L and physical functioning in the SF-12v2, were higher than those between irrelevant ones. Problems reported by study participants in each dimension of EQ-5D-3L showed statistically significant lower scores in pain NRS, PCS, and MCS from the SF-12v2 and RMQ scores. In comparison with the general population, the problems reported in the patient group were statistically higher than those reported by the general population. Conclusion: This study showed that EQ-5D-3L will be useful for patients with OVCF in Korea because it could be valid. The HRQOL of patients with OVCF was very low in all dimensions of the EQ-5D-3L.
