        Baseline Characteristics of the Korean Registry of Pulmonary Arterial Hypertension

        Despite recent advances in understanding of the pathobiology and targeted treatments of pulmonary arterial hypertension (PAH), epidemiologic data from large populations have been limited to western countries. The aim of the Korean Registry of Pulmonary Arterial Hypertension (KORPAH) was to examine the epidemiology and prognosis of Korean patients with PAH. KORPAH was designed as a nationwide, multicenter, prospective data collection using an internet webserver from September 2008 to December 2011. A total of 625 patients were enrolled. The patients’ mean age was 47.6 ± 15.7 yr, and 503 (80.5%) were women. The diagnostic methods included right heart catheterization (n = 249, 39.8%) and Doppler echocardiography (n = 376, 60.2%). The etiologies, in order of frequency, were connective tissue disease (CTD), congenital heart disease, and idiopathic PAH (IPAH) (49.8%, 25.4%, and 23.2%, respectively). Patients with WHO functional class III or IV at diagnosis were 43.4%. In total, 380 (60.8%) patients received a single PAHspecific treatment at the time of enrollment, but only 72 (18.9%) patients received combination therapy. Incident cases during the registry represented 297 patients; therefore, the incidence rate of PAH was 1.9 patients/yr/million people. The 1st-, 2nd-, and 3rd-yr estimated survival rates were 90.8%, 87.8%, and 84.4%, respectively. Although Korean PAH patients exhibited similar age, gender, and survival rate compared with western registries, they showed relatively more CTD-PAH in the etiology and also systemic lupus erythematosus among CTD-PAH. The data suggest that earlier diagnosis and more specialized therapies should be needed to improve the survival of PAH patients.

      • CFD-DEM 연동을 통한 함정용 채프운의 시공간 분포 해석

        Warships widely spread numerous chaffs using a blast, which form chaff clouds that create false radar cross-sections to deceive enemy radars. In this study, we established a numerical framework based on a one-way coupling of computational fluid dynamics and discrete element method to simulate the spatiotemporal distribution of chaff clouds for warships in the air. Using the framework, we investigated the effects of wind, initial chaff cartridge angle, and blast pressure on the distribution of chaff clouds. We observed three phases for the chaff cloud diffusion: radial diffusion by the explosion, omnidirectional diffusion by turbulence and collision, and gravity-induced diffusion by the difference in the fall speed. The wind moved the average position of the chaff clouds, and the diffusion due to drag force did not occur. The direction of radial diffusion by the explosion depended on the initial angle of the cartridge, and a more vertical angle led to a wider distribution of the chaffs. As the blast pressure increased, the chaff clouds spread out more widely, but the distribution difference in the direction of gravity was not significant.

        CFD-DEM 연동을 통한 함정용 채프운의 시공간 분포 해석

        수많은 함정용 채프들은 폭발에 의해 확산되어 채프운을 형성하며, 채프운은 허위 레이더 반사 단면적을 생성하여 적의 레이더를 기만한다. 본 논문에서는 전산유체역학-이산요소법 단방향 연동 기법을 기반으로 공기 중에 분포하는 함정용 채프운의 시공간 분포를 해석하는 수치적 프레임워크를 구축하고 바람의 방향과 속도, 채프 카트리지의 초기 각도와 폭발 압력이 채프운 분포에 미치는 영향을 분석하였다. 채프운의 확산은 폭발에 의한 방사형 확산, 난류와 충돌에 의한 전 방향 확산, 낙하 속도 차이에 의한 중력 방향 확산과 같이 세 단계로 구분되는 것을 확인하였다. 바람은 채프운의 평균 위치를 이동시켰으며, 항력에 의한 확산 효과는 나타나지 않았다. 카트리지 초기 각도에 따라 폭발에 의한 방사형 확산 방향이 달라졌으며, 각도가 지면과 수직에 가까울수록 더 넓게 확산되었다. 폭발압력이 증가할수록 채프운은 더 넓게 확산되었으나 중력 방향으로는 분포 차이가 작았다.

        경부 CT 검사 프로토콜 변화에 따른 화질 및 선량에 관한 연구

        본 연구는 환자 피폭선량의 감소를 위한 목적으로 일개 병원에서 시행되는 경부 ct 검사의 촬영조건에서 관 전압과 영상처리 알고리즘의 적절한 적용으로 기존 영상 화질과 유사한 촬영조건과 방사선량을 평가하였다. 2021년 6월부터 11월까지 내원한 여성 환자 150명을 대상으로 경부 CT 검사를 시행하였다. 촬영조건은 80kV, 100kV, 120kV 자동 노출 조절 방법으로 영상을 획득하였다. 쵤영 후 영상 재구성 알고리즘인 SAFIRE를 단계적 0, 1, 3, 5 로 적용하였고, 경부 주변 조직에 관심 영역을 설정하여 신호대잡음비(Signal to noise, SNR)와 대조도비(Contrast to noise ratio, CNR)로 영상을 정량적 분석하였다. 그룹 간 비교에서 SAFIRE의 강도가 증가할수록 노이즈가 감소 하였으며 신호대잡음비, 대조도비가 증가하는 양상을 보였다. 또한 120kV의 자동 노출 조절 방법과 SAFIRE 강도 3과 비교적 유사한 영상의 화질을 가지는 조건은 100kV의 자동 노출 조절 방법과 SAFIRE 강도 3으로 나타났다. 120kV 자동 노출 조절 방법에서 100kV 자동 노출 조절 방법으로 낮추었더니 방사선량이 13.73%로 감소하였다. This study evaluated the imaging conditions and radiation dose similar to the existing image quality by appropriate application of tube voltage and image processing algorithm in the imaging conditions of a cervical ct test performed at a hospital for the purpose of reducing the patient's exposure dose. Cervical CT scans were performed on 150 female patients who visited the hospital from June to November 2021. Images were acquired by automatic exposure control method of 80kV, 100kV, and 120kV under the shooting conditions. SAFIRE, a post-image reconstruction algorithm, was applied in steps of 0, 1, 3, and 5, and the region of interest was set in the surrounding tissues of the neck to obtain images with Signal to Noise (SNR) and Contrast to Noise Ratio (CNR). was quantitatively analyzed. In comparison between groups, as the intensity of SAFIRE increased, the noise decreased, and the signal-to-noise ratio and contrast ratio increased. In addition, the conditions of the automatic exposure control method of 120kV and the image quality relatively similar to SAFIRE intensity 3 were the automatic exposure control method of 100kV and SAFIRE intensity 3. When it was lowered from the 120kV automatic exposure control method to the 100kV automatic exposure control method, the radiation dose was reduced to 13.73%.

        About the Tradition of Writing the “Fu on the Terrace for Viewing Fish” in the Early Joseon Period

        This paper examines how Joseon literati understood wen 文 and how different perspectives on the wen were reflected in their literary writings especially in fu 賦 works. In Korea, from the fifteenth century onward, the elite class was divided into two groups: the Hungu 勳舊 faction and the Sarim 士林 faction. The former focused on literary craft while the latter focused on incorporating Confucian thoughts in their literary works. I argue that factional differences in terms of understanding the wen reflected the different social statuses of Joseon intellectuals. Scholars from the Hungu faction and capital-based candidates became highly confident in their literary skills. In contrast, Sarim scholars and provincial scholars strategically chose neo-Confucianism as the central ideology of their literary works. To prove this, I compare the three “Gwaneodae bu” 觀魚臺賦 written by Korean fu writers of late Goryeo to early Joseon. The focus is on their adoption of the features of dafu 大賦, or grand fu forms. I analyze how their different views of literature influenced them in writing their fu works in different styles.
