        인간 유두종 바이러스로 인해 보웬 모양 변화를 보인 국한성 수장 저각화증 1예

        Circumscribed palmar or plantar hypokeratosis is a rare condition characterized by the occurrence of an erythematous, well-circumscribed, and depressed macule or patch on the palms or soles. Histopathologically, it is characterized by sharp, stair-like abrupt thinning of the horny layer between the affected and unaffected skin. The pathogenesis of this condition remains unclear. Recently, the human papilloma virus has been implicated as a possible etiological contributor. Circumscribed palmar or plantar hypokeratosis usually shows a benign course. Previous reports have not described malignant changes in these lesions. However, its association with actinic keratosis has been reported in a previous case. We report a case of circumscribed palmar hypokeratosis showing bowenoid epidermal change with expression of the human papilloma virus types 6 and 16 in a patient who was successfully treated with ingenol mebutate gel and cryotherapy.         주차장 환경에서의 차량 사고 검출

        본 논문에서는 주차장 환경에서의 차량 사고 감지방법을 제안한다. 제안하는 방법은 차량 검출, 차량 추적, 사고 감지의 3단계로 구성된다. 차량 검출 단계에서는 픽셀 기반의 전경 검출 방법과 모션맵을 이용하여 차량을 검출하고, 차량 추적 단계에서는 검출된 차량 정보를 바탕으로 차량의 이동을 추적한다. 마지막 단계인 차량 사고 감지 단계에서는 차량의 이동 방향에 맞추어 사고 감지 영역을 지정하고 사고 감지 영역에서 발생하는 움직임의 변화량을 분석하여 차량 사고를 감지한다. 실험을 통해 제안하는 방법은 주차장 환경에서 발생하는 차량 사고를 효과적으로 검출함을 보였다. We propose a detecting method for a car accident in parking lots. The proposed method consists of 3 parts : car detection, car tracking, and accident detection. In the car detection part, we detect the car using the pixel based foreground extraction method and the motion map. From the result of the car detection, the moving car is tracked. In the accident detection part, we set the accident detecting region in front of car, and then the car accident is detected using the difference of the motion. Experimental results show that the proposed method effectively detects the car accident in the parking lots.

        Hepatocellular carcinoma (HCC) is well known malignancy with poor prognosis, even after resection of the primary tumor. Sorafenib is the first-line treatment in advanced HCC, but the disease control rate of sorafenib is only 43%. Pulmonary metastasectomy in patients with pulmonary metastasis from HCC has been reported to increase long-term survival compared with systemic chemotherapy. Video-assisted thoracic surgery is considered a reliable approach to the diagnosis and treatment of pulmonary diseases with low complication rate. Pulmonary metastasectomy is not universally accepted because of frequent local recurrence, an uncontrollable primary tumor, and frequent multiple pulmonary metastases in HCC, but outcome of pulmonary metastasectomy and adjuvant sorafenib therapy has not been studied. We experienced a patient who had advanced HCC with pulmonary oligometastasis and received surgical resection of the metastatic pulmonary nodule and sorafenib chemotherapy. In advanced HCC with pulmonary oligometastasis, surgical resection of pulmonary metastasis and sorafenib chemotherapy should be considered. (J Liver Cancer 2016;16:145-150)

        대장내시경 시행 전 맑은 유동식이 시행 여부에 따른 대장정결도 비교

        Background/Aims: Adequate bowel preparation is essential for full visualization of colonic mucosa because detection of small polyps and neoplasms depends on the quality of bowel cleansing. The aims of this study were to compare the efficacy, tolerability of preparation and side effect between two groups: clear-liquid diet with polyethylene glycol (PEG) solution versus no diet restriction with PEG solution. Methods: This was a randomized single-blind prospective study. A total of 330 patients were randomly assigned to receive either 2 L PEG solution with a clear-liquid diet on the day before colonoscopy and another 2 L PEG solution on the day of the procedure (group 1) or 2 L PEG solution with a general diet on the day before colonoscopy and another 2 L PEG solution on the day of the procedure (group 2). Results: 162 patients were assigned to group 1 and 168 patients to group 2. The satisfactory quality of bowel preparation was not significantly different between the two groups (80.2%, 78.6%, P=0.707). Patient`s compliance of the clear-liquid diet in group 1 was 50%. The satisfactory quality of bowel preparation was weakly better when the clear-liquid diet was given 2 or 3 times a day (group 1A) than 0 or once a day (group 1B) (74.1%, 86.4%, P=0.048). The tolerability of the PEG solution and side effects of preparation were not significantly different in the two groups (P=0.573, 0.686). Conclusions: Bowel preparation with no diet restriction and split-dose PEG solution was similar to preparation with a clear-liquid diet in efficacy, tolerability and side effect. Therefore, the use of the clear-liquid diet protocol should improve patient`s compliance. (Intest Res 2012;10:272-279)

        초고속주행에 적합한 변속차로 길이 연구

        본 논문은 기존 국외의 설계기준을 바탕으로 현재의 차량특성과 도로환경, 안전성 및 운전자 행태 변화를 고려한 초고속도로(설계속도 140km/h) 변속차로 설계기준을 제시하였다. 본 논문의 방법론으로 기존 논문을 참고하여 변속차로 영향요소 검토를 통해 적합한 영향요소를 선정하였다. 또한 국외 초고속주행에 따른 변속차로 길이 산정방법으로 보외법을 통한 본선에서 가·감속을 시작하는 경우와 연결로에서의 가·감속을 시작하는 경우의 두 가지 방법을 이용하였다. 그 결과 “본선에서 가·감속을 시작하는 경우” 연결로설계속도 70km/h일 때 감속차로는 178m, 가속차로는 507m로 제시하였다. “연결로 진입 후 가·감속을 시작하는 경우” 최적길이 산출, 연결로 설계속도 70km/h일 때 감속차로는 247m, 가속차로는 710m로 제시하였다. This paper indicated different criteria needed for present highway designs based on the different type of transmission, vehicle characteristics, road environments, safety and a driver's behaviour changes. From the chosen influence effect, design different avenues of speed-change lane were investigated using method that starting case of acceleration and deceleration in mainline and ramp according to domestic speed highway based on formula in foreign length of lane changing. As the results, in starting case of acceleration and deceleration in mainline, acceleration and deceleration on mainline, acceleration lane length was proposed 507m and deceleration was 178m when RDS was 70km/h, And also in startiong case of acceleration and deceleration in ramp, acceleration lane length was suggested 710m and deceleration was 247m when RDS was 70km/h.         통상적인 치료에 충분히 반응하지 않는 심한 급성 두드러기 환자에서 저용량 에피네프린의 지속적 정맥 내 투여 치료의 효과와 안전성에 대한 연구

        Background: Acute urticaria sometimes accompanies severe systemic reactions that can be potentially life-threatening. Some patients do not achieve sufficient responses to conventional treatments. There has been no previous study on the effect of continuous intravenous infusion of epinephrine in patients with severe acute urticaria. Objective: This study investigated the efficacy and safety of continuous intravenous infusion of low-dose epinephrine in patients with severe acute urticaria who did not achieve a sufficient response to conventional treatments. Methods: We retrospectively reviewed the medical records of 74 patients with severe acute urticaria who were treated with continuous intravenous infusion of low-dose epinephrine between November 2008 and December 2016. One milligram (1 mL) of 1:1000 epinephrine was diluted in 1 L of saline to yield a concentration of 1 μg/mL. The solution was infused at 0.67 μg/min (40 mL/h). Vital signs were checked at 0, 30, 60, and 90 minutes after infusion of epinephrine. Epinephrine was discontinued after one symptom-free day. Results: Clinical symptoms initially resolved within 24.8 hours on average and symptoms were completely resolved within 73.4 hours on average. Twenty-four adverse events, including palpitation, chest discomfort, hand tremor, increased blood pressure, and elevated cardiac markers, were observed in 19 patients (25.7%). Most adverse events were mild and regressed spontaneously without further management. Four patients (5.4%) stopped the infusion due to adverse events, but all events regressed spontaneously after stopping epinephrine. Six weeks after completion of intravenous infusion of epinephrine, 68 patients (91.9%) were symptom-free and six patients required antihistamines. Conclusion: This study suggests that continuous intravenous infusion of low-dose epinephrine is a safe and effective treatment in patients with severe acute urticaria who do not achieve a sufficient response to conventional treatments.         해상보안관리 분석모델 개발에 관한 연구

        해적과 테러공격에 의한 해상보안사고는 해상운송이 발생한 이후에 지속적으로 증가하고 있다. 하지만 기존의 해양사고 조사방법으로 해상보안사고를 조사하고 문제점을 파악하는데 한계가 있으므로 해상보안관리를 위한 분석모델을 개발할 필요가 있다. 이러한 분석모델을 개발하기 위해 해상운송에서 해상보안관리 위협대상을 선박, 화물의 종류, 항만시스템, 인적요소, 정보흐름으로 구분하고, 이에 대한 위험평가기준을 마련하여 해상보안사고 발생가능성 4등급으로 구분하였다. 또한 해상보안관리 관련 이해관계자를 단계별로 구분하여, 국제기구, 각국정부, 해운회사, 선박에서 이루어지고 있는 각종 보안제도들의 동향을 통해 분석모델의 기본틀을 마련하였다. 해상보안관리 관련 단계주체별 이해관계자들이 시행하고 있는 각종 보안제도를 상향식 및 하향식의 의사결정 및 이행방식을 토대로 하여 해상보안관리 분석모델을 개 발하였으며, 실제 Petro Ranger호에서 발생한 해상보안사고 사례를 적용하여 유효성을 입증하였다. Maritime security incidents by pirates and by terrorists increase, but maritime incidents investigation models are limited to figure out the maritime security incidents. This paper provides the analysis model for maritime security incidents. To develop this analysis model, this categorizes five threat factors, the ship, the cargo type, port system, human factor, information flow system, makes the risk assessment matrix to quantify the risk related to threat factors and classifies four priority categories of risk assessment matrix. Also, this model makes from the frameworks which include a variety of security initiatives implementing in stakeholder levels like international organizations, individual governments, shipping companies, and the ship. Therefore, this paper develops the Analysis for Maritime Security Management model based on various security initiatives responding to the stakeholder levels of maritime security management and top-bottom/bottom-up decision trees, and shows the validity through verifying the real maritime security incident of M/V Petro Ranger.

        자율운항선박의 등장으로 인한 해양사고 예방을 위해서는 자율운항선박 비상상황에 대한 정립과 육상원격제어 업무 프로세스의 수립, 이를 기반으로 한 비상상황 시나리오 개발을 통해 비상대응체계를 구축할 필요가 있다. It is necessary to establish an emergency response system by redefining an emergency situation for MASS, configuring a process for shore remote control, and developing an emergency scenario based on it, in order to prevent maritime accidents caused by the emergence of MASS.
