        두개골에서 발생한 모세혈관확장성 골육종의 CT와 MR 영상 소견: 증례 보고

        모세혈관확장성 골육종은 심한 골파괴를 동반하는 드문 악성 종양으로, 두개골에 발생하는 경우는 매우 드물다. 저자들은 두개골 종창으로 내원한 17세 남자 환자로 CT와 MRI에서 두개골, 두개골 내측 및 외측을 침범하며, 그 내부에 액체-액체층을 갖는 종괴가 발견되어 조직검사상 모세혈관확장성 골육종으로 진단된 1예를 경험하였기에 이를 보고하고자 한다. Telangiectatic osteosarcoma is rare malignant tumor causing aggressive bone destruction, though the skull is very rarely involved. We report a case in which the condition affected the skull of a 17-year-old male, involving the parietal bone and with intracranial and extracranial extension. CT and MR images depict an osteolytic lesion of the right parietal bone and an enhancing solid mass in the intracranial and extracranial portions. Matrix calcifications, periosteral reaction, and multiple fluid-fluid levels are seen within the masses.

        Efficacy and Safety of Radiofrequency Ablation for Benign Thyroid Nodules: A Prospective Multicenter Study

        Objective: To assess the efficacy and safety of thyroid radiofrequency (RF) ablation for benign thyroid nodules by trained radiologists according to a unified protocol in a multi-center study. Materials and Methods: From 2010 to 2011, 345 nodules from 345 patients (M:F = 43:302; mean age ± SD = 46.0 ± 12.7 years, range = 15–79) who met eligibility criteria were enrolled from five institutions. At pre-ablation, the mean volume was 14.2 ± 13.2 mL (1.1–80.8 mL). For 12 months or longer after treatment, 276 lesions, consisting of 248 solid and 28 predominantly cystic nodules, were followed. All operators performed RF ablation with a cool-tip RF system and two standard techniques (a transisthmic approach and the moving-shot technique). Volume reduction at 12 months after RF ablation (the primary outcome), therapeutic success, improvement of symptoms as well as of cosmetic problems, and complications were evaluated. Multiple linear regression analysis was applied to identify factors that were independently predictive of volume reduction. Results: The mean volume reduction at 12 months was 80.3% (n = 276) and at the 24-, 36-, 48-, and 60-month follow-ups 84.3% (n = 198), 89.2% (n = 128), 91.9% (n = 57), and 95.3% (n = 6), respectively. Our therapeutic success was 97.8%. Both mean symptom and cosmetic scores showed significant improvements (p < 0.001). The rate of major complications was 1.0% (3/276). Solidity and applied energy were independent factors that predicted volume reduction. Conclusion: Radiofrequency ablation performed by trained radiologists from multiple institutions using a unified protocol and similar devices was effective and safe for treating benign thyroid nodules.

        Histopathologic Findings Related to the Indeterminate or Inadequate Results of Fine-Needle Aspiration Biopsy and Correlation with Ultrasonographic Findings in Papillary Thyroid Carcinomas

        Objective: To determine histopathologic findings related to the indeterminate or inadequate result of fine-needle aspiration biopsy (FNAB) in papillary thyroid carcinomas (PTCs) and to correlate histopathological findings with ultrasonographic features of tumors. Materials and Methods: We retrospectively reviewed the medical records of FNAB, histopathologic characteristics, and sonographic findings of the solid portion of 95 PTCs in 95 patients. All cases were pathologically confirmed by surgery. Histopathologic characteristics were analyzed for tumor distribution, microcystic changes, fibrosis, and tumor component. We assumed several histopathologic conditions to be the cause of indeterminate or inadequate results of FNAB, including: 1) an uneven tumor distribution, 2) > 30% microcystic changes, 3) > 30% fibrosis, and 4) < 30% tumor component. Ultrasonographic findings of each PTC were evaluated for echotexture (homogeneous or heterogeneous), echogenicity (markedly hypoechoic, hypoechoic, isoechoic, or hyperechoic), and volume of the nodule. We correlated histopathologic characteristics of the PTC with results of the FNAB and ultrasonographic findings. Results: From 95 FNABs, 71 cases (74%) were confirmed with malignancy or suspicious malignancy (PTCs), 21 (22%) had indeterminate results (atypical cells), and three (4%) were negative for malignancy. None of the assumed variables influenced the diagnostic accuracy of FNAB. Tumor distribution and fibrosis were statistically correlated with ultrasonographic findings of the PTCs (p < 0.05). Uneven tumor distribution was related with small tumor volume, and fibrosis over 30% was correlated with homogeneous echotexture, markedly hypoechoic and hypoechoic echogenicity, and small tumor volume (p < 0.05). Conclusion: No histopathologic component was found to correlate with improper results of FNAB in PTCs. In contrast, two histopathologic characteristics, uneven distribution and fibrosis, were correlated with ultrasonographic findings.

        두개강내 신경초종의 CT 및 MR 소견

        Purpose : To evaluate CT and MR findings of the intracranial schwannomas arising from variable cranial nerves. Materials and Methods : The authors retrospectively analyzed CT (n=21) and MR(n=15) findings of 24 cases in 23 patients(M : 7, F : 16) who had suffered from surgically-proven intracranial schwannomas over the previous five years. Results : Schwannomas arose from the acoustic nerve(n=18), the trigeminal nerve(n=2), the glossopha-ryngeal-vagal-accessory nerve complex (n=2), and the olfactory nerve(n=1). Intracranial schwannomas were well defined, lobulated and inhomogeneously or homogeneously enhancing masses on CT and MR, and were located along the course of the specific cranial nerve. Acoustic schwannomas involved both the internal auditory canal(IAC) and the cerebellopontine angle(CPA) in 14 cases, the IAC in three, and the CPA in two. Two trigeminal schwannomas involved both middle and posterior cranial fossa and were in the shape of a dumbbell. One of the two schwannomas that invelved lower cranial nerve complex(9-11th) was located in the medullary cistern and jugular foramen ; the other was located in the central posterior cranial fossa. A case of olfactory schwannoma was located in the right cribriform plate. The precontrast CT scan showed low density in 13 cases(62%), isodensity in seven(33%) and high density in one(5%). On postcontrast CT scan, enhancement was seen in 20 cases(95%). Of the 15 cases with MR, 12 had low signal intensity on T1 weighted image and 14 had high signal intensity on T2 weighted image. MR imaging after Gd-DTPA infusion showed enhancement in 14 cases. Enhancement was inhomogeneous in 14 cases on CT and in 13 on MR. Of 24 cases, intratumoral necrosis was seen in 19, ring enhancement in five and severe cystic change in one. Other findings were intratumoral calcification(21%), hemorrhage(8%), pressure bony erosion(70.8%), midline shift(58%), peritumoral edema(29%) and hydrocephalus(33%). On MR, there was in all 15 cases a peritumoral low signal intensity rim on T1- and T2- weighted images and on a T1 weighted image following gadolium infusion. A case of olfactory groove schwannoma was associated with neurofibromatosis type I and a case of bilateral acoustic schwannoma with neurofibromatosis II. Conclusion : Schwannomas can be easily diagnosed when a well defined, lobulated and inhomogeneously enhancing mass with intratumoral necrosis, cystic change, calcification or hemorrhage is seen along the course of a cranial nerve. Peritumoral low signal intensity rim on MR may be helpful in differentiating intracranial schwannomas from other tumors.

        새로운 권고안(2021 Korean Thyroid Imaging Reporting and Data System)을 이용한 갑상선 결절의 평가

        The rate of detection of thyroid nodules and carcinomas has increased with the widespread use of ultrasonography (US), which is the primary imaging modality for the detection and risk stratification of thyroid nodules. The appropriate diagnosis and management of these patients is based on the risk factors related to the patients as well as the thyroid nodules. The Korean Society of Thyroid Radiology (KSThR) published consensus recommendations for US-based management of thyroid nodules in 2011 and revised them in 2016. These guidelines have been used as the standard guidelines in Korea. However, recent advances in the diagnosis and management of thyroid nodules have necessitated the revision of the original recommendations. The task force of the KSThR has revised the Korean Thyroid Imaging Reporting and Data System and recommendations for US lexicon, biopsy criteria, US criteria of extrathyroidal extension, optimal thyroid computed tomography protocol, and US follow-up of thyroid nodules before and after biopsy. The biopsy criteria were revised to reduce unnecessary biopsies for benign nodules while maintaining an appropriate sensitivity for the detection of malignant tumors. The goal of these recommendations is to provide the optimal scientific evidence and expert opinion consensus regarding US-based diagnosis and management of thyroid nodules.

        초급성 뇌경색을 일으킨 개에서 Gd-조영제의 주입이 뇌의 확산에 미치는 영향

        목적: 체내에 주입된 Gd-DTPA가 뇌의 확산강조 자기공명영상 신호강도 및 현성확산계수에 미치는 영향을 알아보았다. 대상 및 방법 : 성숙한 잡견 5마리에 대하여 동맥내 도관삽입에 의한 좌측 내경동맥 색전방법을 이용하여 초급성 뇌경색 동물모델을 만들었다. 색전 후 1시간째 확산강조영상을 시행하고, Gd-DTPA를 주입한 다음 다시 90분까지 11회의 추가 확산강조영상을 얻었다. 관심영역을 설정하여 측정한 초급성 뇌경색부위와 반대측 정상부위의 확산강조영상 신호강도 및 현성확산계수를 분석하였다. 결과: 뇌경색은 색전 후 1시간에 시행한 확산강조 자기공명영상에서 잡견 5마리 모두에서 발견되었다. 확산강조영상에서 초급성 뇌경색부위의 신호강도는 Gd-DTPA 주입 여부와 관계없이 시간이 경과함에 따라 증가하였으나, 관류가 유지된 정상부위의 신호강 도는 Gd-DTPA 주입 후 2분에 시행한 첫 검사에서 오히려 저하된 후, 시간경과에 따라 다시 증가하였다. 현성확산계수는 초급성 뇌경색부위에서 Gd-DTPA주입여부에 관계없이 시간 이 경과함에 따라 지속적으로 감소되었으나, 관류가 유지된 반대측 정상부위에서는 변화하지 않았다. 결론: 체내에 주입된 Gd-DTPA는 초급성 뇌경색부위 및 정상부위의 현성확산계수에 영향을 미치지 않으나, 정상부위에서는 조영제 주입 직후 초기의 자화율효과에 의해 확산강조영상의 신호 강도를 저하시켰다. 조영제 주입 후 시행한 확산강조영상 신호 강도의 정량적인 측정이 필요한 연구 혹은 임상 증례에 대하여는 현성확산계수를 측정함으로써 Gd-DTPA의 자화율효과에 의한 영향을 배제하여야 할 것이다. Purpose : To evaluate the effect of Gd-DTPA on signal intensity of diffusion-weighted magnetic resonance(MR) image and apparent diffuse coefficient (ADC) in dog brain with hype racute stroke. Materials and methods : Experimental canine model of hyperacute cerebral infarction was made by selective intraarterial embolization with particulate embolic material. Diffusion-weighted MR imaging was performed in five dogs at 1 hour after the embolization of internal carotid artery. After intravenous bolus injection of Gd- DTPA, additional 11 diffusion-weighted MR images were serially obtained from 2 minutes to 90 minutes after injection in each dog. The author evaluated findings of hyperacute cerebral infarction on diffusion-weighted MR imaging, and calculated mean signal intensity and mean ADC in infarcted region and contralateral normal region. Statistical analysis of mean signal intensity, mean ADC and contrast-noise ratio before and after Gd-DTPA injection was performed. Results : Hyperacute cerebral infarction developed in all five dogs on diffusion-weighted MR images obtained 1 hour after embolization. The area of hyperacute infarction had steady increase in signal intensity on diffusion-weighted MR image and decrease in ADC. In normal perfusion area, decrease in signal intensity was observed at 2 minutes the Gd-DTPA injection, whereas ADC did not changed. Conclusion : Intravenous injection of Gd-DTPA had no influence on ADC in both hyperacute infarction and normally perfused are a, but caused initial transient signal reduction in normally perfused area on diffusion-weighted MR image due to susceptibility effect of Gd-DTPA. It is important to calculate ADC in evaluating the effect of diffusion after injection of Gd-DTPA.

        MR Imaging of Orbital Inflammatory Pseudotumors with Extraorbital Extension

        Objective: To demonstrate a variety of MR imaging findings of orbital inflammatory pseudotumors with extraorbital extension. Materials and Methods: We retrospectively reviewed the MR features of five patients, who were diagnosed clinically and radiologically as having an orbital inflammatory pseudotumor with extraorbital extension. Results: The types of orbital pseudotumors were a mass in the orbital apex (n = 3), diffuse form (n = 2), and myositis (n = 1). The extraorbital extension of the orbital pseudotumor passed through the superior orbital fissure in all cases, through the inferior orbital fissure in two cases, and through the optic canal in one case. The orbital lesions extended into the following areas: the cavernous sinus (n = 4), the middle cranial fossa (n = 4), Meckel’s cave (n = 2), the petrous apex (n = 2), the clivus (n = 2), the pterygopalatine fossa and infratemporal fossa (n = 2), the foramen rotundum (n = 1), the paranasal sinus (n = 1), and the infraorbital foramen (n = 1). On MR imaging, the lesions appeared as an isosignal intensity with gray matter on the T1-weighted images, as a low signal intensity on the T2- weighted images and showed a marked enhancement on the post-gadoliniumdiethylene triamine pentaacetic acid (post-Gd-DTPA) T1-sequences. The symptoms of all of the patients improved when they were given high doses of steroids. Three of the five patients experienced a recurrence. Conclusion: MR imaging is useful for demonstrating the presence of a variety of extraorbital extensions of orbital inflammatory pseudotumors.
