        Effects of Isthmus Preservation on Postoperative Hypothyroidism after Lobectomy

        Background and Objectives: Many patients experience hypothyroidism after lobectomy. The purpose of this study was to investigate the effects of the extent of surgery (isthmus-saving lobectomy vs. isthmus-removing lobectomy) on postoperative hypothyroidism after lobectomy. Materials and Methods: In total, 144 consecutive benign or low-risk papillary thyroid cancer patients with euthyroid status were retrospectively enrolled from January 2016 to June 2018; 100 patients underwent isthmus-saving lobectomy (IS group), and 44 patients underwent isthmus-removing lobectomy (IR group). Thyroid function tests (TSH) were performed at 3, 6, 12, and 24 months after surgery. Levothyroxine was administered to patients with overt hypothyroidism and those with subclinical hypothyroidism who complained of obvious symptoms of hypothyroidism. Results: The TSH level was significantly lower in the IS group at 3 and 6 months after surgery (p=0.049 and 0.026, respectively). Among patients with TSH <2 μIU/mL preoperatively, 17 patients (17/66=25.8%) in the IS group had significantly maintained TSH levels at <2 μIU/mL compared with 2 patients (2/21=9.5%) in the IR group even after surgery (p=0.039). Postoperative overt hypothyroidism was found in 5 patients (5.0%) in the IS group and 6 patients (13.6%) in the IR group (p=0.048), and the numbers of patients who took medication after the surgery, 10 (10%) and 14 (31.8%), were significantly lower in the IS group (p=0.001). Conclusion: Isthmus-saving lobectomy in patients with euthyroid status reduced the incidence of postoperative hypothyroidism and thyroid hormone replacement and helped in maintaining a low TSH level when compared with isthmus-removing lobectomy.

        There are about 200 lymph nodes in the neck, and lymph node enlargement can occur due to various diseases. Most cases of lymph node enlargement can be attributed to reactive lymph nodes due to an upper respiratory tract infection or inflammation of the head-neck area. However, lymph node enlargement can also occur due to viral or bacterial acute lymphadenitis, tuberculous lymphadenitis, histiocytic necrotizing lymphadenitis, malignant lymphoma, metastasis from other organs, etc. Therefore, for an accurate differential diagnosis, a comprehensive judgment must be made through the patient's past medical history, clinical symptoms, hematological findings, and imaging studies. Among imaging studies, ultrasonography is a widely used and valuable imaging modality in the evaluation of cervical lymph nodes due to its noninvasive nature, low cost, and high accuracy in detecting changes in the morphology and blood flow of the lymph nodes. It is an essential tool for the diagnosis and differential diagnosis of cervical lymphadenopathy. Therefore, in this article, the ultrasonographic findings of normal cervical lymph nodes, inflammatory cervical lymph node enlargement, and cervical lymph node metastasis due to thyroid cancer are reviewed.

        레이저포인터와 단일카메라를 이용한 거리측정 시스템

        최근에 소형무인기(small UAV)에 대한 관심이 증대되고 있는데, 이는 소형무인기가 비용대비 효율적이고 사람의 접근이 어려운 재난 환경 등에 적합하기 때문이다. 이러한 소형무인기에서 거리측정을 통한 매핑(mapping)은 필수적인 기술이다. 기존의 무인시스템 연구에서 거리 측정 센서는 주로 레이저 센서와 스테레오 비전 센서를 많이 사용하였다. 레이저 센서는 정확도와 신뢰성이 우수하지만 대부분 고가의 장비이고 스테레오 비전 센서는 구현이 용이하지만 무게 측면에서 소형무인기에 탑재하여 사용하기에는 적합하지 않다. 본 논문에서는 레이저 포인터와 단일 카메라를 사용하여 저가의 거리측정기를 구성하는 방안을 소개한다. 제안한 시스템을 이용하여 거리를 측정하고 이로부터 맵을 구성하는 실험을 수행하였고 실제 데이터와 비교 분석하여 시스템의 신뢰성을 검증하였다. Recently, many unmanned aerial vehicle (UAV) studies have focused on small UAVs, because they are cost effective and suitable in dangerous indoor environments where human entry is limited. Map building through distance measurement is a key technology for the autonomous flight of small UAVs. In many researches for unmanned systems, distance could be measured by using laser range finders or stereo vision sensors. Even though a laser range finder provides accurate distance measurements, it has a disadvantage of high cost. Calculating the distance using a stereo vision sensor is straightforward. However, the sensor is large and heavy, which is not suitable for small UAVs with limited payload. This paper suggests a low-cost distance measurement system using a laser pointer and a monocular vision sensor. A method to measure distance using the suggested system is explained and some experiments on map building are conducted with these distance measurements. The experimental results are compared to the actual data and the reliability of the suggested system is verified.

        가우시안 프로세스를 이용한 실내 환경에서 소형무인기에 적합한 SLAM 시스템 개발

        Localization of aerial vehicles and map building of flight environments are key technologies for the autonomous flight of small UAVs. In outdoor environments, an unmanned aircraft can easily use a GPS (Global Positioning System) for its localization with acceptable accuracy. However, as the GPS is not available for use in indoor environments, the development of a SLAM (Simultaneous Localization and Mapping) system that is suitable for small UAVs is therefore needed. In this paper, we suggest a vision-based SLAM system that uses vision sensors and an AHRS (Attitude Heading Reference System) sensor. Feature points in images captured from the vision sensor are obtained by using GPU (Graphics Process Unit) based SIFT (Scale-invariant Feature Transform) algorithm. Those feature points are then combined with attitude information obtained from the AHRS to estimate the position of the small UAV. Based on the location information and color distribution, a Gaussian process model is generated, which could be a map. The experimental results show that the position of a small unmanned aircraft is estimated properly and the map of the environment is constructed by using the proposed method. Finally, the reliability of the proposed method is verified by comparing the difference between the estimated values and the actual values.

        유방암의 항암화학요법으로 Docetaxel 치료 중 발생한 심한 체액정체 1예

        Among many adverse effects of docetaxel, fluid retention is a well recognized, cumulative side effect, but severe fluid retention is rare. We report here on a case of docetaxelinduced severe fluid retention with peripheral edema, pleural effusion, severe ascites and pericardial effusion in a 41-yearold woman. She had been treated with 3 cycles of docetaxel 9 days previously and she was admitted to our hospital due to abdominal distention and mild dyspnea. Radiologic studies revealed pleural effusion, severe ascites and a small pericardial effusion. Diuretics were given for 21 days. The pleural effusion was resolved after treatment with diuretics for 2 days, but the ascites wasn’t resolved until 14 days of diuretics. After treatment with diuretics for 21 days, all the symptoms of the patient were completely resolved. Early detection is mandatory and diuretics are very effective for patient suffering with docetaxel-induced severe fluid retention.

        유방암 환자의 액와부 수술 중 상지에서 배액되는 림프 경로의 보존

        Purpose: The aim of this study was to confirm the feasibility of the technique, the so called ‘‘axillary reverse mapping (ARM)’’, and to test the hypothesis that the arm lymphatics are never involved by the metastatic process of breast cancer. Methods: We reviewed the prospectively maintained database of 129 patients who underwent an operation for breast cancer. Blue dye was injected in the upper inner arm to identify the draining lymphatics or lymph nodes from the arm. During the axillary procedure, we found the ‘‘blue’’ ARM node and the ‘‘hot’’ sentinel lymph node (SLN). The histological results of the ARM nodes were compared with those of the other nodes harvested in SLN biopsy (SLNB) or axillary lymph node dissection (ALND). Results: The ARM nodes were identified in 78.3% (101/129) of the patients. The ARM nodes were identified in 71.6% (58/81) of the patients with SLNB and in 88.4% (38/43) of the patients with SLNB followed by ALND and in all cases with ALND. In 19 of the 96 SLNB cases, the ARM nodes were hot or the SLNs were blue (concordant case), yielding an 18.9% concordant rate between the ARM node and the SLN. Among these 19 concordant cases, 7 ARM nodes contained metastasis (36.8%). But in the 77 non-concordant cases, only one ARM node was positive for metastasis (1.3%) in a heavily metastasized axilla. The ARM nodes were identified in all five ALND cases, and one ARM node was positive for metastasis. Conclusion: It is thought that lymphatic drainage from the arm can share common lymphatic channels in the axilla with lymphatic flow from breast, and these concordant ARM nodes may be involved by the metastatic process of breast cancer. However in the non-concordant cases, ARM nodes will be free from the danger of axillary dissection.
