Despite of the fact that in recent years the term borderline, initially confined to Psychoanalytic use, has achieved wider application within the general mental health community, the term border-line personality remains a controversial and confusing one in mental health today. The purpose of this paper is to clarify the reason why a group of patients and/or clients initially appearing neurotic and analyzable, proved highly resistant to the psychoanalytic process. As psychotherapists treated this group of patients and/or dents. they wrote descriptive and psychodynamic accounts that detailed the difficulties these patients and/on clients presented. Different diagnotic lables were assigned, with varying descriptions and theoretical conceptualizations. This diversity made it difficult for the mental health community to reach a consensus on a precise definition for the borderline personality, the work of Kernberg, beginning in 1967, provided an integration and synthesis of earliers writings, plus a unified and compehensive framework for descrpition, definition, and understanding. This paper is to provide an ego psychological approach to borderline conditions focusing on diagnostic, psychodynanic and therapeutic issues. The evolution of the term borderline is presented to clarify its current uses. A major goal of the paper is to transmit an in-depth yet simplified understanding of the different uses of the term borderline conditions. To provide general background material on ego psychology, it focuses on ego functions thought to be most relevant to the conception of the borderline individual.
境界線의 自我所有量에 準한 刺戟障壁과 認知能力 : 대상중심이론을 배경으로

본 연구는 학업부진과 등교거부 그리고 본드흡입 등의 심각한 문제는 물론 난폭한 행동을 거리낌 없이 자행하는 한 청소년(중2)을 위해서 상담을 의뢰해 온 가족을 대상으로 대상중심 가족치료를 시도하여 경 계선에 억류된 문제 청소년의 병리적 증후군을 제거하므로써 가족의 기능을 긍정적으로 강화시켜 줄 수 있는 가족응집력의 형성과정과 이를 위한 치료적 효율성을 예증하려 하였다. 이를 위해서 치료자는 대상에 의해서 양도되지 않고 있는 자기(문제 청소년)의 영유아 시절의 부족한 자아 소유량을 중심으로 자극장벽의 형성과정과 인지능력 결함과의 관계를 치료의 핵심 문제로 설정하고 이들의 붕괴와 만회를 위한 치료를 1993년 7월 7일부터 1994년 6월 23일까지 총 50회에 걸쳐 집중적으로 실시하였다. 치료적 접근은 지지적, 탐험적, 그리고 표현적 기법을 단계별로 동원 활용하였으며 치료효과에 대한 측정은 자기(문제 청소년)의 학업성취도에 준하였다. 그 결과 문제 행동의 조절이 가능하게 되고 학업성취도에 커다란 긍정적인 변화가 도출되어 치료를 종결하였다.
A Rare Case of Undifferentiated Carcinoma of the Colon Directly Invading the Duodenum

Colorectal cancers often invade adjacent organs; however, direct duodenal invasion is rare. Adenocarcinoma is the most common type of colorectal cancer, but an undifferentiated carcinomatype is unusual. Herein, we present a case of undifferentiated carcinoma of the colon that directly invaded the duodenum and metastasized to distant lymph nodes. An 85-year-old femalepatient was admitted with a 7-cm-sized colonic mass invading the duodenum, detected by computed tomography. Positron emission tomography revealed fluorodeoxyglucose uptake in thecolon, duodenum, and aortocaval lymph nodes. A large encircling mass in the ascending colonand an ulcerated mass in the duodenum were revealed by colonoscopy and esophagogastroduodenoscopy, respectively. Pathologic examinations of the colon and the duodenum revealednonglandular, diffusely infiltrating atypical round cells, confirming undifferentiated carcinomaof the colon. The histologic type of this tumor was distinguished using immunohistochemical(IHC) markers. Finally, microscopic characteristics and IHC markers aided in identifying thehistologic type of colorectal cancer.
