        노인에서의 다른 3가지 방법의 척추 경막외 병용마취 및 척수마취 방법의 비교 연구

        Background: Combined spinal epidural anesthesia (CSE) combines the rapid onset and intensity of a subarachnoid blockade with the flexibility of an epidural catheter, thereby allowing the intraoperative extension of anesthesia and postoperative epidural analgesia. This study investigated the effects of hyperbaric bupivacaine during CSE for lower limb orthopedic surgery in geriatric patients. Methods: Group 1 received a subarachnoid injection of 8 mg of bupivacaine and an epidural catheter. Group 2 received a subarachnoid injection of 5 mg of bupivacaine and fentanyl, 10μg and an epidural top-up with 3 ml saline. Group 3 received a subarachnoid injection of 5 mg of bupivacaine and fentanyl 10μg with an epidural top-up of 6 ml saline. Group 4 received a subarachnoid injection of 6 mg of bupivacaine and an epidural top-up with 6 ml saline. The maximal level of the sensory blockade, the time of onset to the maximal sensory blockade, the two segment regression time, the time for sensory regression to the tenth thoracic dermatome (T10) and the motor recovery to modified Bromage Scale 0 were compared. After recording the baseline systolic blood pressure and heart rate, the systolic blood pressure and heart rate were recorded at 5 min intervals. Results: There were significant differences in the two-segment regression times, the times to motor recovery between the groups. However, the intraoperative blood pressure and heart rate were similar. Conclusions: In terms of the peri-operative side effects and recovery from the blocks, CSE was better for lower limb orthopedic surgery in geriatric patients than spinal anesthesia.         The cardiovascular effects of midazolam co-induction to propofol for induction in aged patients

        Background: The aim of this study was to investigate whether a small dose of midazolam and lessening the propofol dosage could prevent cardiovascular change at tracheal intubation for induction in aged patients. Methods: Eighty patients over 65 years (ASA physical status 1, 2) scheduled for elective surgery received general anesthesia with remifentanil and propofol or midazolam. Patients in group P (n = 40) were induced with 0.9% NaCl 0.03 ml/kg, propofol 1.2 mg/kg and remifentanil. Patients in group MP (n = 40) were induced with midazolam 0.03 mg/kg, propofol 0.8 mg/kg and remifentanil. The time taken to reach loss of consciousness (LOC) and the value of bispectral index score (BIS) at LOC were recorded. After LOC, 0.8 mg/kg of rocuronium was given and tracheal intubation was performed. The mean blood pressure (MBP) and heart rate (HR) were recorded before induction as the base value, before intubation, immediately post-intubation and 3 minutes after intubation. Results: Compared with the base values, MBP at before intubation and 3 minutes after intubation was significantly decreased in group P and group MP (P < 0.05). Compared with group P, the decrease of MBP was significantly less at before intubation, immediately after intubation and 3 minutes after intubation in group MP (P < 0.05). The time taken to reach LOC was significantly decreased in group MP compared with that in group P (P < 0.05). There were no significant differences of HR at any time between the two groups. Conclusions: Co-induction with midazolam and propofol could prevent a marked BP decrease at tracheal intubation for induction in aged patients.         Iterative Fourier Transform Algorithm Based on the Segmentation of Target Image for a High-Speed Binary Spatial Light Modulator

        A digital micro-mirror device (DMD) has the potential to modulate an incident wave with high speed,and the application for holographic display has been studied by many researchers. However, the qualityof reconstructed image isn’t good in comparison with that from a gray-scale amplitude-only hologram sinceit is a binary amplitude-only spatial light modulator (SLM). In this paper, we suggest a method generatinga set of binary holograms to improve the quality of the reconstructed image. Here, we are concerned withthe case for which the object plane is positioned at the Fourier domain of the plane of the SLM. In thiscase, any point in the Fourier plane is related to all points in the hologram. So there is a chance to generatea set of binary holograms illuminated by incident wave with constant optical power. Moreover, we findan interesting fact that the quality of reconstructed image is improved when the spatial frequency bandwidthof the binary hologram is limited. Therefore, we propose an iterative segmentation algorithm generatinga set of binary holograms that are designed to be illuminated by the wave with constant optical power. The feasibility of our method is experimentally confirmed with a DMD

        산화물 환원공정에 의해 제작된 n-형 Bi2Te2.7Se0.3 화합물의 열전특성연구

        We report the thermoelectric transport properties of n-type Bi2Te2.7Se0.3 compounds prepared by oxide-reduction process. The oxide-reduction process is a recently developed method that can synthesize Bi2Te3-based compounds using only oxide starting materials, which is advantageous in terms of the diversity of the starting materials and cost efficiency. In this study, starting materials of Bi2O3,TeO2 and SeO2 were ball-milled for homogenous mixing, and then oxidized to prepare mixed oxide powders. The mixed oxide powders were reduced at different reduction temperatures (573, 603, 633 and 663 K) for 7 h under hydrogen atmosphere. A single phase of Bi2Te2.7Se0.3 could be achieved when the reduction temperature was higher than 603 K, and the shape of the particles changed from granules to flakes with increasing reduction temperature. Furthermore, the formation of antisite defects of BiSe was promoted by increasing reduction temperature, which strongly affected the electrical and thermal transport properties of the Bi2Te2.7Se0.3 compounds, which were prepared by spark plasma sintering of the reduced powders. The anisotropic microstructures in the sintered bodies were significantly influenced by the shape of the reduced powders, which was also controlled by the reduction temperature. The thermoelectric transport properties were characterized at room temperature, and the detailed effects of the reduction temperature on these properties are discussed in terms of the anisotropic microstructure and antisite defects.

        협근 근점막 피판을 이용한 구강내 재건

        Since the buccinator myomucosal flap was first described, it has been modified, with the study of anatomical model. Therefore, buccinator myomucosal flap can be elevated in various direction according to defect with its sufficient arc of rotation. With the buccinator myomucosal flap, intraoral reconstruction was performed in 3 patients. Two adult patients were suffered from complete cleft palete. Partial tongue reconstruction was performed in another patient following partial glossectomy due to tongue cancer. Satisfied results were obtained in all three patients. In these cases, the author designed flaps in island patterns. It would be more helpful to close donor site primarily and obtaining free rotation arc. The pedicle was located in retromolar trigone, flap detachment was not needed. Considering the disadvantages of other reconstruction methods, such as contracture, hair growth and poor oral hygiene caused by skin graft or conventional free flap, the buccinator myomucosal flap would be another good option for intraoral reconstruction with “like tissue”.

        Effects of SiO2 Interlayer on Electrical Properties of Al-Doped ZnO Films under Bending Stress

        We report the effects of an SiO2 interlayer on the structural and electrical properties of Al-doped ZnO (AZO) films grown on polyimide (PI) substrate. By inserting a SiO2 interlayer between the AZO film and the PI substrate, we could control the microstructure of the AZO films. The granule size of the SiO2interlayer was determined strate temperature, and the column width in the AZO film was strongly affected by the initial granule size in the interlayer. In-situ Hall measurement of the AZO films under bending stress was performed for the first time, and flexibility-related electrical properties of the AZO films were discussed.
