레쉬니한 증후군(Lesch-Nyhan syndrome) 환자의 치아와 연관된 자해행동의 관리: 증례보고

저자는 입술 및 혀에 대해 자해증상을 보이는 2명의 Lesch-Nyhan 증후군 환아에서 구강조직에 대한 자해행동을 예방하기 위해 soft mouth guard를 이용한 보존적 치료법과 전악발거의 침습적 방법을 이용하여 치료한 증례를 보고하는 바이다. Lesch-Nyhan 증후군 환자의 자해행동을 예방하기 위한 치료법은 아직 정립된 기준이 존재하지 않아 각 환자의 자해양상 및 정도에 따라 치료방법은 달라질 수 있다. 가능하다면 치아를 보존할 수 있는 장치 또는 BTX-A 제재 등을 이용한 보존적 방법을 먼저 시도해 보는 것이 좋다. 이 때 치료의 성공률을 높이기 위해서는 치과적 접근 이외에도 소아정신과와의 협진을 통한 정신심리학적 접근, 필요시 약물치료를 고려하는 것 등 여러 방향으로 접근하는 것이 더욱 바람직한 결과를 가져올 것으로 사료된다. 보존적 접근이 실패할 경우 더 이상의 조직상실과 전신적 감염 예방을 위해 극단적 방법이지만 전악발거를 고려해야 할 것이다. Lesch-Nyhan syndrome is a rare X-linked recessive genetic disorder. During purine metabolism, the hypoxanthine guanine phosphribosyl transferase (HGPRT) enzyme is deficient causing phosphoribosyl transferase to accumulate and resulting in excessive uric acid. Clinical symptoms include hypercalcemia, choreoathetosis, spasticity, mental retardation, and self-injury to lips, tongue or fingers. This results not only in pain caused by the self-injury but also secondary infection of the wound site and the esthetic damage of the soft tissue defect. Dental treatments include conservative methods using intraoral appliances such as soft mouthguards, fixed lip bumpers, and occlusal guards, and invasive methods such as extraction of all teeth or forming an artificial anterior open bite. We report two cases of Lesch-Nyhan syndrome patients with self-mutilation behavior; one was treated with a preservative method using a soft mouthguard, and the other was treated with extraction of all teeth.

Concern for beauty is an aesthetic instinct, and beauty is what everybody seeks after. The standard of beauty varies from generation to generation, and it is a basic human desire to change one`s own image to meet the standard. Nowadays there is a gradual increase in the number of women who make a further attempt to play up their own beauty by undergoing cosmetic surgery. The purpose of this study is to examine the plastic surgery expriences, satisfaction level and side-effects of the female residents in and around Seoul. This study is in an effort to suggest how to resolve problems caused by indiscreet cosmetic surgery. It is also meant to provide the right knowledge and information on cosmetic surgery in order to raise the right awareness for it. This research surveyed 402 females residing in Seoul and the provinces, who were aged 20 and up. Data was analyzed using SPSS (Statistical Package for the Social Science) WIN 21.0 program. Frequency and percentage were used to calculate the general characteristics of participants. χ<sup>2</sup>-test and Analysis of Frequency were used to answer satisfaction level, status of plastic surgery and side-effetcs. In the survey, most of the women answered that they were not satisfied with their appearances. The women who were in their 30s and over thought that it would be possible to change one`s personality by undergoing cosmetic surgery. The main reason for undergoing cosmetic surgery was because they were unsatisfied with their out appearances. Double eye-lid surgery was the most popular, and nose surgery followed. Many women thought that their image became better after having cosmetic surgery and they were satisfied with the results. Some women answered that they experienced side-effects after plastic surgery; mostly bruise and swelling. As the above, the plastic surgery is now very popular in the broad range of generations, and it should be carefully considered before making decisions. Therefore the ideal environment that can further the acquisition of the right information, right type of cosmetic surgery and hospital, should be created.

청소년참여위원회의 효과적 참여증진방안에 관한 면담조사 연구

목적: 청소년 정책의 패러다임이 청소년 중심의 정책으로 변화됨에 따라, 청소년 참여의 효과적인 증진 방안을 모색하여 관련 기초자료를 마련하는 데 목적이 있다. 방법: 분석틀을 통해 분석대상 자료를 선정하여 내용분석을 수행하여 그 내용을 파악한 후, 그 결과를 기반으로 실제 청소년참여위원회 담당자 10명을 대상으로 면담을 통해 효과적인 청소년 참여증진을 위한 방안과 개선방안을 도출한다. 결과: 자료 분석결과 위원회 사업목적에 따른 주요활동의 차별성을 확인할 수 있었으나, 지자체와의 소통이 어렵고, 참여수준이나 의견수렴 정도가 낮음을 알 수 있었다. 청소년 참여증진을 위한 방안으로는 일시적이 아닌, 지속적ㆍ심화 교육지원, 제안한 정책과제의 실효성 획득이 필요하다. 결론: 청소년참여위원회의 목적 지향성을 확보하는 한편, 교육의 중요성에 대한 인식을 높여 보다 더 다양한 교육과정을 개발할 필요가 있다. 제안한 정책과제의 후속 사항을 지속해서 관리할 필요가 있으며, 지자체ㆍ학교와의 유기적인 협력 관계 구축이 필요하다. Purpose: As the paradigm of youth policy changes to a youth-oriented policy, the goal is to find effective ways to promote youth participation and prepare related basic data. Methodology: After selecting the data to be analyzed through the analysis frame and analyzing the contents, we will interview 10 people in charge of the actual youth participation committee based on the results and draw up measures and improvement plans for effective promotion of youth participation. Findings: The data analysis showed that it was difficult to communicate with local governments and that the level of participation or collection of opinions was low, although it was possible to differentiate major activities depending on the purpose of the committee's project. Measures to promote youth participation need to support continuous and deepening education, not temporarily, and to obtain the effectiveness of the proposed policy tasks. Conclusions: It is necessary to develop more diverse curriculums by securing the purpose orientation of the Youth Participation Committee and raising awareness of the importance of education. It is necessary to continuously manage the follow-up of the proposed policy tasks and to establish an organic cooperation relationship with local governments and schools.

Until 1993, Nanji landfill has been the main solid waste treatment facility for Seoul, which is the capital of Korea. Since the landfill has not been properly lined, soil and ground water around the landfill has been contaminated. Due to the proximity, it has been speculated that the water quality of the Han River would have been also influenced by the leachate. In order to illuminate this, HELP model to estimate the quantity of the leachate from Nanji landfill and RMA model to simulate the change of water quality in Han River were applied in this research. The result indicates that the quantity of leachate from Nanji landfill has been overestimated by previous works arid it was due to the failure to consider the water storage capacity of the landfill. The effect from Nanji landfill leachate on the water quality, in terms of COD input, proves to be smaller than those from other tributaries and influents to the River and the effect is larger when the flow of the River is reversed due to the tide.
