        Real-world Adverse Events Associated with Fluconazole and Itraconazole: Analysis of Nationwide Data Using a Spontaneous Reporting System Database

        Objective: This study aimed to investigate the occurrence and types of the adverse events (AEs) associated with oral fluconazole and itraconazole and factors associated with specific types of AEs. Methods: We analyzed AEs reported by community pharmacies nationwide over 10 years using the Korea Adverse Event Reporting System database. Various AE terms were categorized into 18 types, and concomitant medications were classified by drug-drug interaction (DDI) severity. The relationship between the specific type of AE and age, sex, and number of concomitant medications was investigated using multiple logistic regression analysis. Results: A total of 879 AE reports of fluconazole and 401 reports of itraconazole were analyzed; of these reports, 321 and 83 reports of fluconazole and itraconazole, respectively, described concomitant drug administration categorized as DDI severity of contraindicated or major. Women had a higher risk of psychiatric AEs associated with fluconazole use (OR, 1.587; p=0.042). Polypharmacy increased the risk for psychiatric AEs (OR, 3.598; p<0.001 for fluconazole and OR, 2.308; p=0.046 for itraconazole). In dermatologic AEs, the mean age of patients who received itraconazole was lower than that of patients who received fluconazole (46.3±16.8 vs. 54.9±15.4; p<0.001). Co-administration of fluconazole with 1-3 drugs increased the risk of neurological AEs (OR, 1.764; p=0.028). Conclusion: When using fluconazole and itraconazole, psychiatric AEs should be noted, particularly in women and in case of polypharmacy; moreover, when fluconazole is co-administered with other drugs, attention should be paid to the occurrence of neurological AEs.

      • 수소 에너지 저장 효율 향상을 위한 액상유기수소저장체 BN-bicyclohexyl의 탈수소화 메커니즘 규명

        국내 수소 경제 활성화를 위해 안정적인 수소 공급은 매우 중요한 사안이다. 현재 수소 에너지는 해외에서 생산된 수소를 수입하는 방법이 경제성 측면에서 가장 큰 장점을 가진다. 따라서, 대용량의 수소를 저장 및 수송하기 위한 안정적이며 높은 효율을 가진 운반체의 개발이 필요하다. 다양한 수소 저장 기술 중, 액상 유기물에 고밀도의 수소를 저장하는 방법인 액상유기수소운반체(liquid organic hydrogen carries, LOHC) 기술이 주목 받고 있다. LOHC는 상온에서 액상으로 안정적인 수소 저장이 가능하며 기존의 가솔린 기반 시설을 활용할 수 있다는 장점을 가진다. 기존 실험 연구에서 biphenyl과 diphenylmethane의 혼합 시 6.9 wt% H<sub>2</sub>의 수소저장용량을 가진다고 보고하였다. 그러나, 이에 대한 탈수소화 및 수소화 반응의 정확한 반응경로와 메커니즘이 명확하게 규명되지 않은 실정이다. 본 연구에서는 밀도범함수이론(density functional theory, DFT) 계산을 활용하여 탈수소화 반응엔탈피(ΔH)와 반응자유에너지(ΔG)를 계산하였으며, 이를 통해 bicyclohexyl의 탈수소화 메커니즘을 분석하였다. 또한, 탈수소화/수소화 효율을 향상시키기 위해 헤테로원자(B, N)가 치환된 새로운 LOHC 물질인 BN-bicyclohexyl을 제안하였고, 이들로 인한 효율 향상에 대한 원인을 전자구조분석 방법을 통해 규명하였다. 또한, 탈수소화/수소화 효율을 더욱 향상시키기 위해 팔라듐 및 루테늄 촉매를 활용하였으며, 활성화 에너지(E<sub>a</sub>) 계산을 수행하여 탈수소화/수소화에 대한 최적 반응 경로와 반응제한단계를 제안하였다.

        분석심리학적 신화읽기 -의식의 발달사로 보는 신화의 세 단계

        N/A From the standpoint of analytical psychology, myth, that has been lasting since beginning of human being, is considered as a spontaneous product of psyche . Myth, basically originated from the action of the ‘archetype of the collective unconscious`, has influenced on the human conscious life directly or indirectly. Myth is not a causal product of psyche, but provides us upcoming norm for human life. Psychic product such as myth has nutured human being to gain humanity and to develop further. C. G. Jung`s early studies on mythology were mainly about hero myth that had been dealt with in the relationship of mother-image and father-image. In late era, he mentioned that mythology was dealing with dual structure of human psyche of consciousness and unconscious directing toward unifying one. This phenomenon of whole psyche were for `Individuation` of human psyche, he emphasized simultaneously. Therefore, analytical psychological understanding of mythology can divide its processes into three stages; at first `Creation myth` on the birth of consciousness, secondarily `Hero myth` dealing with the self-identification, differentiation and growing-up processes of human psyche acquired the consciousness, and thirdly `Hierogamos myth` on reunification process from conflict and fighting between the consciousness and the unconscious. `Creation myth`, the first stage, may deal with the psychic event of the birth of consciousness, not show us the origin of mankind or world as a real. Man can reflect on the representation of the object `World` after his acquisition of the consciousness from pre-conscious state. ‘Creation myth` shows us the dawn of enlightening human consciousness, symbolized as `World Egg`-the beginning of mankind as well as `flood` the destructive one. It symbolizes the unstable state of consciousness before established self-identification, even returning to the subconscious state before the birth. Diversity of each nation`s creation myth may be originated from the different processes of differentiation and growing up of consciousness by each original collective motive. Even in the `Creation myth` of later era, collective change in consciousness such as founding a new country was meant by new acquisition of consciousness. In ‘Hero myth`, the second stage, libido, the constituents of cosmos and world, transform into the personified being as `hero`. We usually project our ego consciousness into it. However, hero has transpersonal characteristics in the view of analytical psychology. In ‘Hero myth`, the birth of hero was usually written like a birth from egg or virgin pregnancy because it means the birth of psyche, not that of a person. Abandonment of the hero just after birth and his danger, the usual theme in ‘Hero myth` reflects the psychic events or experience of the psyche acquired the consciousness. Typical story unfolds that he had been in danger repeatedly and fight against `monster` like dragon, symbolizing the fundamental inner psyche of mother-image and the uterus of hero himself. Human psyche acquired consciousness had to confront the fundamental psyche that had given birth to him, overcome and grow up. Mankind can be a master of world through this processes. ‘Hierogamus myth` reflects on the self-identification, differentiation and growing up processes after acquisition of consciousness, that might be signifying essential adulthood reunification of psyche that had divided into dual structure of consciousness and the unconscious. At this time, libido regresses or draws back into fundamental maternity. Usual theme of incestuous relationship in it was to show us `Coniunctio oppositorium` Marriage of the King and Queen in ‘Hierogamos myth` unfolds that King passed out and pregnant Queen got her son, rebirth of the dead king and renunciation.       • 중학생의 심리적 안녕감과 자기주도학습 능력의 관계에서 내재적 학습동기와 진로성숙도의 매개효과

        본 연구는 중학생의 심리적 안녕감이 자기주도학습 능력에 정적인 영향을 미치는 관계에서 진로성숙도와 내재적 학습동기의 매개효과를 살펴보았다. 이를 위해 본 연구는 K지역 내 W시에 거주하는 남·여 중학교 3학년 학생 373명을 대상으로 심리적 안녕감 척도, 자기주도학습 능력 척도, 진로성숙도 척도와 내재적 학습동기 척도를 측정하였다. 수집된 자료를 활용하여, 기술통계, 상관분석을 실시했으며, Hayes(2013)의 PROCESS Macro model 4를 이용하여 회귀분석을 통해 매개효과를 검증하였다. 연구결과 첫째, 심리적 안녕감, 진로성숙도, 내재적 학습동기와 자기주도학습 능력의 주요 변인들 간 유의한 정적상관이 나타났다. 둘째, 심리적 안녕감이 자기주도학습 능력에 정적인 영향을 미칠 때, 내재적 학습동기는 부분 매개효과가 있는 것으로 나타나 심리적 안녕감이 자기주도 학습능력을 향상시키는 과정에서 내재적 동기가 부분적으로 그 원인이 될 수 있음을 확인하였다. 이러한 연구결과를 바탕으로 중학생의 인성과 학업 및 진로 발달을 촉진하기 위한 학교상담과 생활지도에서 진로성숙도와 내재적 학습동기의 향상을 위한 개입에 대한 시사점을 제공하였다. This study examined the mediating effect of career maturity and intrinsic learning motivation in a relationship in which the psychological well-being of middle school students positively affects self-directed learning ability. To this end, this study measured psychological well-being scale, self-directed learning ability scale, career maturity scale, and intrinsic learning motivation scale for 373 third-year male and female middle school students living in W city in K region. Descriptive statistics and correlation analysis were conducted using the collected data, and the mediating effect was verified through regression analysis using Hayes (2013) PROCESS Macro model 4. As a result of the study, first, there was a significant positive correlation between the main variables of psychological well-being, career maturity, intrinsic learning motivation, and self-directed learning ability. Second, when psychological well-being has a positive effect on self-directed learning ability, intrinsic learning motivation appears to have a partial mediating effect, confirming that intrinsic motivation can partially be the cause in the process of improving self-directed learning attitude. Based on these research results, implications for intervention to improve career maturity and intrinsic learning motivation in school counseling and life guidance to promote personality, academic and career development of middle school students were provided.

        혈액투석 환자에서 위장관 출혈로 발현된 위결핵

        The incidence of Tuberculosis among the patients with end-stage renal disease(ESRD) has increased up to 16 times of that in the general population. The impairment of the cellular immunity in the ESRD patients may have a role in the pathogenesis. Extrapulmonary tuberculous manifestations such as lymph node, peritoneum, and pleura involvement are more frequent in the ESRD patients than in the general population. However, there has been no case of upper gastrointestinal(UGI) bleeding as a result of a gastric tuberculosis in the ESRD patient on hemodialysis. Here we report an unusual case of a hemodialysis patient with UGI bleeding secondary to a tuberculous gastric ulcer. A 31-year-old female on hemodialysis was admitted with melena. Endoscopy revealed a benign gastric ulcer with a visible bleeding vessel at the base, located in the anterior wall of the antrum. An exploratory laparotomy showed multiple, round, small and yellow nodules on the visceral peritoneum as well as a 1-cm sized gastric ulcer. After gastric resection, a histological examination including peritoneal nodules demonstrated chronic granulomatous inflamation with caseous necrosis and giant cells. The patient has been on antituberculosis medication and followed up in the outpatient department without any event for 8 months.

        태블릿 PC 기반 보완대체의사소통의 어휘 선택 경로가 일반아동과 지적장애아동의 AAC 어휘표현 정확도 및 선호도에 미치는 영향

        배경 및 목적: 본 연구에서는 최근 보급이 증가하고 있는 태블릿 PC의 역동적 디스플레이를 사용하여 AAC 어휘 경로 유형이 일반아동과 지적장애아동의 어휘 선택에 미치는 영향을 비교 분석하고자 한다. 방법: 본 연구의 대상은 생활연령이 만 4;0-6;8세인 경도 지적장애아동 13명을 실험집단으로, 지적장애아동과 생활연령을 일치시킨 일반아동 13명, 언어연령을 일치시킨 일반아동 13명을 통제집단으로 하였다. 실험방법은 각각의 어휘 경로 유형(범주적 배열 및 공간적 배열)에서 정확도 평가를 실시한 후, 동시에 제시된 어휘 경로 유형(범주적 배열 및 공간적 배열)에서 선호도 평가를 실시하였다. 결과: 지적장애아동(ID)집단은 생활연령을 일치시킨 일반아동(CA-N)집단에 비해 AAC 어휘표현 정확도가 통계적으로 유의하게 낮았으나, 언어연령을 일치시킨 일반아동(LA-N)집단과는 유의한 차이가 나타나지 않았다. 어휘 선택 경로에 따른 배열 선호도에서는 세 집단 간에 유의한 차이가 없었다. 논의 및 결론: 결과를 통해 역동적 디스플레이를 사용한 AAC의 어휘 선택 경로를 장애군에 따라 특정한 어휘 선택 경로로 한정하여 지정하기보다는 아동의 개인적인 특성을 고려하여 다양한 어휘표현을 할 수 있도록 어휘 선택 경로를 유연하게 배열할 필요가 있다는 것을 시사한다고 하겠다. Objectives: The purpose of the present study was to investigate the performance of Ko-rean children with normal and intellectual disabilities on augmentative and alternative communication (AAC) systems with different language organizations. Methods: Thirty-nine children participated in this study: 13 children with intellectual disabilities (ID) age 4 to 6, 13 chronological age-matched normally developing children (CA-N), and 13 language age-matched normally developing children (LA-N). Participants were asked to locate AAC vocabulary items using either category-based or place-based language organization. They performed two tasks: an accuracy testing task and a preference testing task. The 27 items (18 items for accuracy test and 9 items for preference test) were selected from lists of words frequently used by young children. All vocabulary items were concrete nouns. Results: There were significant interaction effects in the groups (ID, CA-N, and LA-N) and types of AAC language organization (categorical vs. placed-related). The ID and LA-N groups showed significantly higher effects in the placed-related organization than categorical, while the CA-N group did not. There was no interaction effect among the three groups in the lan-guage organization preference test. Conclusion: The results of this study suggest consider-ing the performance of ID children on different types of AAC vocabulary navigation using dynamic display AAC technologies.
