This paper describes a process to access voice of customer (VoC) and improve quality of the product so that customers feel satisfaction by using the products. The authors were involved in a case study project in a Company, Menipuni. The company is undergoing some design change and implementing a product quality improvement project. To assist decision-making on both redesign and quality improvement of the products, we underlook an application of methodologies, quality function deployment (QFD) and Analytic Hierarchy Process (AHP).
제조업에서의 상황변수에 따른 TQM요인의 비교분석 - ISO 9001 인증 받은 한ㆍ일 기업의 차별적 특성을 중심으로

한국과 일본기업 간에는 품질경영수준이 여러 여건에 따라 다를 것이다. 품질경영은 상품의 품질확보를 통해 고객만족을 달성하기 위한 경영혁신의 도구로서 기업의 자원과 역량에 따라 이행 정도가 다를 것이다. 품질경영을 시행하고 있는 기업(ISO 9001 인증받은 기업)을 대상으로 한·일기업간의 품질에 대한 인식과 성공요인으로서 TQM요인에 대한 실증적 연구를 하였다. 한국과 일본의 지역적 특성, 기업의 차별적 특성에 따른 TQM요인(품질프로세스, 품질지원프로세스)에서의 차이를 분석하였다.<BR> 연구결과, 차별적 특성(경영혁신 추진조직유무, 경영혁신활동기간)에 따라 TQM요인에 차이가 있는 것으로 분석되었다. 한국기업이 경영혁신추진 관리조직을 두고, 지속적으로 경영혁신관련 활동을 해나감으로서 TQM의 활동요인을 통해 제품의 품질을 확보하여 고객만족을 거둘 수 있을 것으로 판단된다. It was analyzed about the differences in Total Quality Management(TQM) factors by regional, discriminational circumstances of Korean and Japanese companies. This paper presents TQM as an appropriate management tool for the competitive advantage for the medium and small company.<BR> It was surveyed that most of Korean samples were from small manufacturing company in electrical and electronic field that has not plentiful of human resources and ability. it was analyzed that there were differences in the quality-process and the quality-infra factor in Korean and Japanese companies by discriminational circumstances(management renovation implementing organization, management renovation activity duration). It can be judged that Korean CEO who is implementing or want to implement TQM needs to allocate workforce properly. So it can be judged If growing medium and small companies of Korea set TQM implementing organization and do continuous quality improvement for management, Customer Satisfaction would be accomplished through the effectiveness of TQM activity factors.
관동맥질환에서 디피리다몰부하 99mTc - MIBI 심근 SPECT 극성지도와 관동맥 조영소견의 비교

N/A We have anlayzed the polar maps of dipyridamole stress/rest Tc-99m-MIBI SPECT and compared the quantitated perfusion defects of dipyridamole stress polar map and the findings of coronary angiograraphy in 56 pateints with coronary artery diseases. We performed the same day dipyridamole stress-rest myocardial SPECT, reconstructed the polar maps according to Cedars-Sinai method and quantitated perfusion defects of total myocardium and the territory of each artery, comparing the polar maps of patients with normal files. Stenosis more than 50 percent was considered significant and myocardial ischemic score was calculated as summed score of percents of main coronary arteries. Positive concordance of myocardial SPECT with coronary angiography were 82.6% with left anterior descending artery(LAD), 85.7% with left circumflex artery(LCx) and 78.6% with right coronary artery(RCA). Perfusion defect of SPECT polar map and the stenosis of coronary artery showed the contingency phi of 0.55(p〈0.0001) with total atreries, 0.38(p=0.016) with LAD, 0.50(p〈0.0001) with LCx and 0.40 (p=0.007) with RCA. Dipyridamole stress percent defect of polar map was correlated with myocardial ischemic score with Spearman's rho of 0.47(p=0.001) in total arteries, 0.48(p=0.001) in LAD, 0.56 (p〈0.001) in LCx and 0.38(p=0.002) in RCA. These findings revealed that defect of the dipyridamole stress myocardial Tc-99m-MIBI SPECT and the percent extent of this defect were related with significant artery stenosis of individual arteries and the degree of stenosis. We thought that we could use the defects in the polar map of dipyridamole stress Tc-99m-MIBI SPECT for the quantification of myocardial perfusion decrease.

당뇨병성 케톤산증 환자는 QT 간격 연장, 그리고 이에 따른 Torsades de Pointes 발생 위험의 위험요소를 많이 내포하고 있으며 실제로 치명적인 부정맥인 Torsades de Pointes, 그리고 그에 따른 심정지를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Diabetic ketoacidosis (DKA) is an acute complication related to severe hyperglycemia. While the mortality rate for DKA is low with appropriate therapy, several complications may lead to deterioration of the clinical course. Here, we report a case of a 23-year-old patient with DKA who suffered from a rare but hemodynamically unstable cardiac arrhythmia, polymorphic ventricular tachycardia with prolonged QT interval, or Torsades de Pointes. During the recovery phase of DKA, three episodes of Torsades de Pointes suddenly occurred, and were recovered by immediate defibrillation. The patient did not have structural heart disease or a genetic predisposition. To the best of our knowledge, this is the first report of an adult with DKA complicated with QT prolongation related to Torsades de Points after correction of ketosis. To manage DKA, more attention may be needed on changes in the QT interval as well as risk factors for Torsades de Points. (Korean J Med 2021;96:432-437)
고혈압 환자에서 혈압변동성 및 심장박동수와 hsCRP와의 연관성

Background/Aims: Blood pressure (BP) variability and heart rate (HR) are associated with target organ damage and cardiovascular complications; however, the exact mechanisms are uncertain. In this study, we examined the association of an inflammatory marker with BP variability and HR. Methods: A total of 151 patients diagnosed recently with hypertension were subjected to 24-h ambulatory BP monitoring. BP variability was assessed as the standard deviation of the BP recordings. The average HR and HR variability were calculated from concomitantly recorded HR values. Plasma high-sensitivity C-reactive protein (hsCRP) was used as a marker of inflammation. Results: The mean age of the study population was 44 ± 11.3 years, and 74.2% of the patients were male. The plasma hsCRP level was higher in male patients (0.131 ± 0.014 vs. 0.06 ± 0.023, p = 0.001) and patients with a history of smoking (0.136 ± 0.017 vs. 0.101 ± 0.017, p = 0.003). A correlation analysis showed that the variability in diastolic BP during 24-h monitoring was associated with hsCRP (p = 0.002, r = 0.258). The 24-h (p = 0.004, r = 0.236), daytime (p = 0.003, r = 0.239), and nighttime (p = 0.020, r = 0.190) average HRs were related to the hsCRP level. The 24-h HR variability (p = 0.025, r = 0.182) was also associated with hsCRP. After adjusting for the effect of related variables, the 24-h diastolic BP variability (β = 0.286, p = 0.011) and daytime average HR (β = 0.169, p = 0.049) were positively related to hsCRP. Conclusions: Plasma hsCRP is related to diastolic BP variability in recently diagnosed hypertensive patients. Moreover, HR measured with BP is associated with hsCRP. These findings suggest that inflammation mediates adverse cardiovascular outcomes of BP variability and an elevated HR. (Korean J Med 2013;85:50-57)
한국인 심부전증 환자 심근에서의 인형 거대 세포 바이러스 감염

Objectives: In order to evaluated the prevalence and the site of infection of cytomegalovirus in terminally failing heart, cytomegaloviral DNA was detected in the explanted hearts of transplantation recipients. Methods: DNA extractions were performed from explanted failing hearts(N=22) and normal hearts(N=5) and polymerase cain reactions(PCRs) were done for detection of late gene sequence coding pp150 glycoprotein. The products were confirmed by electrophoresis on the 1% agarose gel. n order to improve the detectability of cytomegaloviral genome, nested PCRs were executed with the primers designed for the original 607 bp products. In situ PCRs also were done with the samples which were confirmed as positive for CMV viral genome by nested PCRs. Results: All patients had IgG anti-cytomegalovirus antibody and did not have IgM anti-cytomegalovirus antibody. Cytomegaloviral genomes in myocardium were detected by polymerase chain reaction. The 607bp products by PCRs were found in both explanted failing heart(3 cases/22. 13.5%) and normal hearts( 1 case/5, 20.0%). In nested PCRs, 186bp products were found in both failing hearts(LV 4/22, LA 3/2, RV 4/22, RA 0/17) and normal hearts(LV 2/5, LA 1/4, RV 2/5, RA 2/5). These was no significant change in positivity of cytomegaloviral DNA genome between failing and normal hearts. Total positivity of cytomegaloviral genome in explanted hearts was 44.4% according to the nested PCR results. The positivity of cytomegaloviral DNA by n situ PCR was 33.3%(4/12), and the site of positive reaction was the nuclei of the myocardial cells. Conclusion: Cytomegalovirus was rarely observed in explanted hearts of terminal heart failure and nested PCR could enhance the sensitivity of cytomegaloviral genome detection. The result of the in situ PCR showed the site of the cytomegaloviral infection was nuclei of the myocardial cells. Cytomegalovirus, however, might have no direct causal relationship in the development of terminal heart failure.

A hemodialysis arteriovenous fistula may cause high output cardiac state by volume overload and decreased systemic vascular resistance. Arteriovenous fistulas for hemodialysis consist usually of either a radiocephalic fistula at the wrist or a more proximal brachiocephalic fistula at the antecubital fossa. In this study the contribution of arteriovenous fistula to cardiac function has been evaluated by comparing cardiac performance before and after the point occlusion of the anastomotic site. The results suggest that the occlusion of arteriovenous fistula may decrease end-diastolic left ventricular volume (VOLd), increase end-systolic left ventricular volume (VOLs), decrease stroke volume (SV), decrease ejection fraction (EF) and decrease cardiac output (CO). This study shows that arteriovenous fistula influences directly to cardiac performance, so careful determination of dialysis option is required for the end stage renal failure patients with marginal heart function.
