        앉았다 일어서기 동작 수행 시 발목 관절 각도에 따른 근 활성도 및 역학적 부하량의 비교

        PURPOSE: The purpose of this study was to investigate the comparison of muscle activity and mechanical loading according to the angle of ankle joint during a sit-to-stand (STS) task. METHODS: Thirty-four young participants performed the STS in a randomized trial with the ankle joint at a neutral, 15 degrees dorsiflexion and 15 degrees plantarflexion angle in a fixed sitting posture with the knee in 105 degrees flexion. Muscle activity of the tibialis anterior (TA), rectus femoris (RF), biceps femoris (BF), and gastrocnemius medialis (GCM) was measured, and the parameters calculated in relation to mechanical loading were the STS-time, maximum peak , minimum peak, and total sum of mechanical loading. RESULTS: In the dorsiflexion position, the muscle activity of the TA and GCM showed a significant increase (p<.05), and the STS time, maximum peak and total sum of mechanical loading showed a significant difference compared to that in the neutral position (p<.05). In the plantarflexion position, the muscle activity of the RF and GCM showed a significant increase (p<.05), while that of the TA showed a significant decrease (p<.05) compared to that in the neutral position. And the minimum peak was significantly increased than the neutral position (p<.05), and the maximum peak and total sum of mechanical loading were showed significant difference compared with dorsiflexion position (p<.05). CONCLUSION: These results show that there is a difference in muscle activity and mechanical loading when performing the STS movement according to the change in the ankle joint angle.

        A study on the clinical usefulness, validity, and test-retest reliability of the Spirokit, a device that combines the pulmonary function test and respiratory muscle strength test

        Objective: This study was conducted to identify the clinical usefulness, validity, and reliability of the Spirokit, a device that combines the pulmonary function test (PFT) and respiratory muscle strength (RMS) test. Design: Cross-sectional study. Methods: Forty young adults (male: 23, female: 17) participated in a PFT and a RMS test. The concurrent validity for pulmonary function was assessed by comparing data obtained from MicroQuark and the Spirokit and the agreements between the MicroRPM and the Spirokit for RMS were compared. The test-retest reliability of the Spirokit was determined by comparing data obtained from the first and second sessions. The test and retest were performed at the same time after one day for the PFT and RMS test. Validity was estimated using intraclass correlation coefficients (ICCs), and by calculating 95% limits of agreement (LoA). To estimate interrater reliability, ICCs were calculated. Results: The Spirokit showed a high agreement intra class coefficient (ICC [2, 1]): 0.978-0.999, 95% limits of agreements (95% LOA): −0.798 to 0.847 with MicroQuark. It also showed a high level of concordance ICC (2, 1): 0.992 to 0.993, 95% LOA: −9.313 to 11.169 with MicroRPM. The test-retest reliability of the Spirokit was analyzed using ICC (2, 1), and showed a high level of reliability (ICC [2,1]=0.960 to 0.998). Standard error of measurement % (SEM%) was 0.12% to 3.39%, and minimum detectable change% (MDC%) was 0.02% to 3.79%, indicating high level of reliability. Conclusions: The Spirokit is a device with high validity and reliability that can be used to simultaneously measure PFT and RMS tests.

        양측 리듬청각자극을 이용한 트레드밀 보행이 뇌졸중 환자의 보행에 미치는 효과

        본 연구는 양측 리듬청각 자극을 이용한 트레드밀 보행이 보행 변수와 대칭성에 미치는 효과를 알아보고자 시행하였다. 재활병원에서 입원 치료 중인 24명의 뇌졸중 환자를 대상으로 양측 리듬청각자극의 유무와 트레드밀 속도 변화(느린 속도, 편안한 속도, 빠른 속도)에 따른 조건에서 5분간 트레드밀 보행을 실시하였다. 보행분석기(OptoGait)를 이용하여 보행변수를 측정하였고, 측정한 자료를 토대로 시ㆍ공간적 대칭성을 산출하였다. 연구 결과 트레드밀 보행에 양측 리듬청각자극을 적용하였을 때가 양측 리듬청각자극을 적용하지 않았을 때 보다 모든 속도에서 활보장, 비마비측 보장이 유의하게 증가하였다(p<.05). 여섯 가지 조건 중에 활보장, 비마비측과 마비측의 보장은 양측 리듬청각자극을 적용한 편안한 속도의 트레드밀 보행에서 가장 컸다. 양측 리듬청각자극을 적용하였을 때 공간적 대칭성은 느린 속도와 편안한 속도에서 향상되었으며, 편안한 속도에서 가장 컸다. 시간적 대칭성은 편안한 속도와 빠른 속도에서 향상되었으며, 빠른 속도에서 가장 컸다. 본 연구를 통하여 양측 리듬청각자극을 이용한 트레드밀 보행이 뇌졸중 환자의 보행 변수 조절과 보행의 시ㆍ공간적 대칭성을 향상시키는데 효과적인 방법이 될 수 있음을 제시하였다.

        Effect of High-Intensity Complex Exercise Program Using Whole-Body Vibration and Respiratory Resistance for Low Back Pain Patients with High Obesity

        Objective: The purpose of this study was to investigate the effect of high-intensity complex exercise program using whole-body vibration (WBV) and respiratory resistance on pain and dysfunction, psychosocial level, balance ability, and pulmonary function in low back pain (LBP) patients with high obesity. Design: A randomized controlled trialMethods: A total of 44 LBP patients withhigh obesity (body mass index, BMI≥30kg/m2) were randomly assigned to an experimental group (n=22) and a control group (n=22). Both groups underwent a lumbar stabilization exercise program. In addition, the experimental group implemented the high-intensity complex exercise program combined with WBV and respiratory resistance. In order to compare the effects depending on the intervention methods, numeric pain rating scale (NRPS), Roland-Morris disability questionnaire (RMDQ), fear-avoidance beliefs questionnaire (FABQ), balance ability, and pulmonary function were used for measurement. Results: Both groups showed significant differences in NRPS, RMDQ, FABQ, balance ability before and after intervention (p <0.05). In addition, the experimental groupshowed significant difference in the amount of change in RMDQ, balance ability and pulmonary function values than the control group (p<0.05). Conclusions: High-intensity complex exercise program using WBV and respiratory resistance has been proven to be an effective and clinically useful method to decrease dysfunction, increase balance ablilty, and pulmonary function for LBP patients with high obesity.

        가상현실 게임기반 훈련프로그램을 이용한 체간조절훈련이 아급성기 뇌졸중 환자의 균형 및 상지기능에 미치는 영향

        본 연구는 가상현실 게임기반을 이용한 훈련이 아급성기 뇌졸중환자의 균형 및 상지기능에 미치는 영향에 대하여 알아보기 위하여 실시하였다. 아급성기 뇌졸중환자 30명을 가상현실 게임기반 훈련프로그램을 적용한 실험군(n=15)과 대 조군(n=15)으로 무작위 배정하였다. 중재는 6주간 주 3회, 1회당 30분씩 적용하였다. 대상자의 기능 향상정도를 평가하기 위하여 중재 전·후 균형능력과 상지기능을 평가하였다. 연구결과 실험군에서 균형능력과 상지기능(FMUE), 체간 장애척도 (TIS)에 유의한 향상이 있었으며(p<.05), 두 그룹 간 차이는 버그균형척도(BBS), 일어나 걷기(TUG), 체간 장애척도(TIS)에 서 유의한 차이가 있었다(p<.05). 이러한 결과를 바탕으로 가상현실 게임기반 훈련 프로그램은 아급성기 뇌졸중환자에게 임상적으로 유익한 운동프로그램이라 할 수 있다. The purpose of this study was to investigate the effects of virtual reality game based training on balance and upper limb function in subacute stroke patients. Thirty patients with subacute stroke were randomly assigned to experimental groups(n=15) and control groups(n=15) applying virtual reality game-based training programs. Intervention is applied three times a week for 6 weeks, 30 minutes for 1 time. In the study group, there was a significant improvement in balance ability, upper extremity function, and trunk impairment scale(p<.05), and the difference between the two groups was significant in the BBS, TUG, TIS(p<.05). Based on these results, the virtual reality game based training program is clinically useful exercise program for subacute stroke patients.

        Effect of Taping Technique Applied to Adults with Knee Instability on Landing Error Scoring System, Lower Extremity Joint Angle

        Objective: The purpose of this study was to investigate effects of taping technique applied to knee instability. Design: Cross sectional study. Methods: Twenty-six participants with knee instabilityparticipated in this study. They were randomly assigned to the Kinesio taping (KT) group (n=13) and the dynamic taping (DT) group (n=13). Both groups applied knee stabilization taping techniques. In order to compare the effects of each taping technique, the change in the landing error scoring system (LESS) and lower extremity joint angle wasrecorded before and after the intervention. Results: Both groups significantly decreased in the change before and after the LESS (p<0.05). At the joint angle of the lower extremities, KT group significantly reduced the valgus angle at the max knee flexion (p<0.05). In DT group knee joint flexion and hip joint flexion angles were significantly increased at foot contact (p<0.05). In max knee flexion, the knee joint flexion angle was significantly increased (p<0.05). In foot contact, max knee flexion, the knee joint valgus angle was significantly increased (p<0.05). DT group showed more significant changes in knee joint flexion angle at foot contact and hip joint flexion angle at max knee flexion. Conclusions: Dynamic taping is a clinically applicable intervention method for lowering the risk of non-contact injury in participants with knee instability and for knee stability during rehabilitation exercises.

        다이나믹 테이핑과 키네시오 테이핑 적용에 따른 발바닥 굽힘근의 지구력과 피로도에 미치는 효과 비교

        Background: The purpose of this study is to investigate the effect of plantar flexor on muscle fatigue and endurance when two different sport tapes are applied. Design: Cross-sectional design. Methods: Fifty-one healthy adults were randomly assigned to dynamic taping group (n=17), kinesio taping group (n=17), and control group (n=17). The dynamic taping group used the dynamic tape. The kinesio tape group used the kinesio tape, and the control group doesn't used tape. Every group performed heel rise test and sEMG for every during pre-test and post-test. To evaluate plantar flexor endurance, the number of times was measured during the heel rise test. Results: The number of heel rises increased significantly in the post-test in dynamic taping group compared to the pre-test, and In comparison between groups, the number of heel rises significantly increased in dynamic taping group compared to kinesio taping group and control group. PEF value was significantly increased in the post-test compared to the pre-test in dynamic taping group, and there was a significant difference between the three groups according to the taping application. Conclusion: The results of this study confirmed that dynamic taping was effective in musclular fatigue and endurance on plantar flexor in healthy adults. Based on these results, it is suggested that the application of dynamic tape can be suggested as one of the intervention methods for muscle endurance and muscle fatigue.

        A Study on the Validity and Test-Retest Reliability of the Measurement of the Craniovertebral Angle of the Smart Phone Application ‘Angles Video Goniometer’

        Objective: The purpose of this study was to compare concurrent validity and test-retest reliability based on Craniovertebral angle of ‘Angles video goniometer’, a smart phone application for convenient range of motion measurement, and ‘Image J’, an analysis software with high reliability and validity. This was conducted to find out whether ‘Angle video goniometer’ can be used clinically. Design: Cross-sectional studyMethods: Fifty subjects were imaged laterally, and the angle of the head and spine was measured using Image J and the Angles video goniometer, respectively, in a resting posture and a chin in posture. The level of concurrent validity between the two measurement methods and the level of inter-rater reliability and intra-rater reliability were analyzed. Results: For forty participants, the concurrent validity between Image J and Angles video goniometer showed very high validity with ICC of 0.997(0.995∼0.999) and 0.994(0.994∼0.998), CVME% 0.71∼0.72%, SEM% 0.31∼0.34, MDC% 0.86∼0.94. The test-retest intra-rater reliability showed very high reliability ICC 0.994(0.991∼0.996), CVME% 0.71%, SEM% 0.31∼0.43, MDC% 0.86~1.19%. The test-retest inter-rater showed very high reliability ICC 0.995(0.992∼0.997), CVME% 0.71%, SEM% 0.43∼0.59%, MDC% 1.20∼1.62%Conclusions: Angles video goniometer’, a smartphone application, is a device with very high reliability and validity for craniovertebral angle measurement in healthy adults, and it is a device that can be easily used in clinical practice.

        휴대형 폐기능 검사기 ‘The Spirokit’의 제작 및 ATS 24/26파형을 통한 성능검

        Background: This study aims to examine the usefulness of the portable spirometer "The Spirokit" as a clinical diagnostic device through technology introduction, precision test, and correction. Design: Technical note Methods: "The Spirokit" was developed using a propeller-type flow rate and flow rate measurement method using infrared and light detection sensors. The level of agreement between the Pulmonary Waveform Generator and the measured values was checked to determine the precision of "The Spirokit", and the correction equation was included using the Pulmonary Waveform Generator software to correct the error range. The analysis was requested using the ATS 24/26 waveform recognized by the Ministry of Food and Drug Safety and the American Thoracic Society for the values of Forced Voluntary Capacity (FVC), Forced Expiratory Volume in 1second (FEV1), and Peak Expiratory Flow (PEF), which are used as major indicators for pulmonary function tests. All tests were repeated five times to derive an average value, and FVC and FEV1 presented accuracy and PEF presented accuracy as the result values. Results: FVC and FEV1 of 'The Spirokit' developed in this study showed accuracy within ± 3% of the error level in the ATS 24 waveform. The PEF value of 'The Spirokit' showed accuracy within the error level ± 12% of the ATS 26 waveform. Conclusion: Through the results of this study, the precision of 'The Spirokit' as a clinical diagnosis device was identified, and it was confirmed that it can be used as a portable pulmonary function test that can replace a spirometer.
