        사매가 대장암 세포에 미치는 영향

        Objectives: The aim is to identify any anti-tumor effects of Duchesnea indica(Andr.) Focke on colon cancer cells. Materials & Methods: Colo201 human adenocarcinoma cells were obtained from American Type Culture Collection. The boiled extract of Duchesnea indica(Andr.) Focke was added (10 and 20 microliters) to cultures and observed at 0, 6, and 12 hours, and at 12-hour intervals thereafter. Morphological changes in colon cancer cells were observed through an inverted microscope, Destruction of colon cancer cells was measured through Trypan blue exclusion testing. Suppression of the viability of colon cancer cells were measured via MTT assay. Anti-cancer mechanisms in the cell cycle of colon cancer cells were analysed via flow cytometry. Results: After introduction of Duchesnea indica(Andr.) Focke to cultures several changes were seen. Significant atrophy of the nucleus and cytoplasm of colon cancer cells was observed, indicating cell injury. Destruction of colon cancer cells was observed in direct proportion to dosage and duration. Suppression of viability of colon cancer cells for each test group was greater than that of the control group increasingly over time(36h, 48h, 60h, 72h), which was statistical significant (p<0.05). Cell numbers of the mitosis phase of the colon cancer cell cycle reduced. Conclusions: Statistcally significant anti-tumor effects of Duchesnea indica(Andr.) Focke were observed in this in vitro experiment. Results support a role for Duchesnea indica(Andr.) Focke in treatment of colon cancer. though it will required progressive research to develop a practical treatment.

        정상안과 펨토초레이저 라식수술안에서 서로 다른 4가지 각막두께 측정계들의 중심각막두께 비교

        Purpose: To compare corneal pachymetry assessment using four measurement methods in normal and post-femtosecond laserassisted LASIK eyes. Methods: Central corneal thickness was measured sequentially using Orbscan II, Pentacam, Galilei and ultrasonic pachymetry in 30 normal, non-surgical eyes (Group I), 30 eyes one to six months after femtosecond laser-assisted LASIK (Group II), and 30 eyes six months or longer after femtosecond laser-assisted LASIK (Group III). Results: In Group I, corneal thickness measurements were similar for all four methods (P=0.202, one way ANOVA). In Groups II and III, corneal thickness measurements were significantly different (P=0.000, respectively, one way ANOVA). Compared to the Pentacam, Galilei and ultrasonic pachymetry, Orbscan significantly underestimated the corneal thicknesses in Groups II and III (P<0.005, respectively, one way ANOVA). Conclusions: Central corneal thicknesses of normal eyes were similar for all four measurements, therefore corneal thickness measurements before refractive surgery using all four measurements is suitable. However measurements obtained with the Orbscan II were thinner than those obtained with the Pentacam, Galilei or ultrasonic pachymetry in post femtosecond laserassisted LASIK eyes. Further studies are needed to determine which instrument is more accurate in measuring central corneal thickness before and after refractive surgery. J Korean Ophthalmol Soc 2010;51(3):320-327

        노안에서 조절미세파동의 고 진동수 영역 및 조절래그와 조절력의 관계

        Purpose: To determine the relationship between the high-frequency component of accommodative microfluctuation (HFC) and accommodative lag in presbyopic eyes and how they affect the subjective accommodation power with other ophthalmic factors, such as pupil diameter and corneal multifocality. Methods: Forty subjects (80 eyes) were included in this study, and the non-corrected distance and near visual acuity was measured and then converted to log MAR vision. The addition power for 33 cm near visual work was also measured and the accommodative power was measured by the push-up method. The HFC, accommodative lag and pupil size according to accommodative stimuli (-1.0 to -3.0 D) was measured by Speedy-i Autorefractor (Right Mfg Co., Tokyo). The corneal multifocality and corneal astigmatism was measured by corneal topography (TMS-4; Tomey, Erlangen, Germany). Results: The mean age was 60.0 ± 4.66 years (51-68) and there were 23 male and 17 female subjects (1.35:1). The HFC and pupil size showed no significant change according to accommodation demand, however, the accommodative lag showed a statistically significant increase (<EM>p</EM> < 0.001 by one-way ANOVA). Accommodative lag (-2.0 D) showed a statistically significant negative correlation with the subjective accommodative power, and the pupil size (0 D) showed a positive correlation with the subjective accommodative power. Conclusions: In presbyopia patients, the objective accommodative lag and initial pupil size showed a statistically significant relationship with subjective accommodative power. On the other hand, HFC, pupil size and corneal multifocality, alone may not be indicators to predict accommodative power, but these are important components to explain the dynamic aspect of accommodation.         인트라라식 후 빛간섭단층촬영계로 측정한 각막절편 두께의 재현성 평가

        Purpose: To investigate the relationship among optical coherence tomography (OCT), ultrasound pachymetry, and Orbscan in central corneal thickness measurement and to evaluate the reproducibility of flap thickness using an IntraLase femtosecond laser. Methods: Central corneal thickness was measured by OCT, ultrasound pachymetry, and Orbscan in 59 eyes of 30 patients before LASIK. After IntraLASIK, the corneal flap thickness measured using OCT was compared with the intended corneal flap thickness. Results: Central corneal thickness measured by OCT was thinner than that measured by other instruments preoperatively, but there was no significant difference among these methods (p>0.01), and corneal thickness values obtained by ultrasound pachymetry and Orbscan correlated well with those obtained by OCT (r ranged from 0.804 to 0.889, p<0.01). After IntraLASIK, there was no significant difference between the mean measured flap thickness and the intended flap thickness (p>0.01). Conclusions: OCT is a relatively accurate instrument for measuring corneal thickness and can easily measure the corneal flap thickness after LASIK. Compared with the results of a previous study, the mean measured flap thickness in this study was more reproducible with the IntraLase femtosecond laser.

        Pulmonary Tuberculosis, Tuberculous Meningitis의 임상치험 3례

        Tuberculosis, one of the oldest diseases known to affect humans, is caused by bacteria belonging to the Mycobacterium luberculosis complex. The diseases usually affects the lungs, although in up to one-third of cases other organs are affected. If properly treated, tuberculosis caused by drug-susceptible strains is curable in virtually all cases. If untreated, the disease may be fatal within 5 years in more than half of cases. Transmission usually takes place through the airborne spread of droplets of nuclei produced by patients with infectious pulmonary tuberculosis. Two patients with Pulmonary Tuberculosis and one with Tuberculous Meningitis were recently examined. The changes in these patient's symptoms through both western medical treatment and oriental medical treatment are reported.

        군날개의 크기에 따른 수술 전후의 각막 고위수차의 변화

        Purpose: To investigate the effects of pterygium size on the change of corneal astigmatism and corneal higher-order aberrations (HOAs) after surgery. Methods: Fifty eyes of 46 patients who underwent pterygium surgery were included in the present study and divided into 2 groups according to pterygium size (small group: size ≤ 2.0 mm, large group: size > 2.0 mm). Corneal HOAs using wavefront aberrometer (KR-1W?) and corneal astigmatism using topography (Orbscan II?) were evaluated before surgery and their changes monitored after surgery at postoperative 1 week, 1 month, and 3 months. Results: The small group was comprised of 16 eyes and 34 eyes were included in the large group. In the small group, uncorrected visual acuity (UCVA), best corrected visual acuity (BVCA), and spherical equivalent (SE) were not statistically significantly different after surgery at postoperative 3 months. In the large group, BVCA was not statistically significantly different, but UCVA increase and SE decrease were statistically significant after surgery at postoperative 3 months. Preoperative simulated keratometric (Sim K) astigmatism and corneal HOAs increase were statistically significant according to the pterygium size. Preoperative corneal HOAs of the large group were significantly higher than the small group except for spherical aberration in the central 4 mm optical zone (p = 0.501). In the small group, there was no statistically significant difference in corneal HOAs before and after surgery. In the large group, corneal total HOAs, trefoil and coma in the central 4 mm and 6 mm zone were statistically significantly decreased at postoperative 1 week, 1 month, and 3 months, respectively (p < 0.05). Conclusions: The corneal HOAs and the corneal astigmatism were increased according to pterygium size. In cases with pterygium greater than 2.0 mm in size, the corneal HOAs and the corneal astigmatism were decreased after pterygium surgery which can improve visual acuity and quality. Therefore, the pterygium size can be a useful factor in deciding when to perform pterygium surgery. J Korean Ophthalmol Soc 2014;55(1):32-39

        타카야수동맥염 환자에서 발생한 포도막염 및 각막염 1예

        Purpose: To report a rare case of simultaneous uveitis and keratitis in Takayasu’s arteritis. Case summary: A 22-year-old woman presented with complaints of redness and decrease in vision in both eyes. The patient was diagnosed a decade earlier as having Takayasu’s arteritis. No other systemic or ophthalmic abnormalities were found. She complained of ophthalmic symptom aggravation during an upper respiratory infections and before menstruation. Bilateral corneal edema, conjunctival injection, and anterior uveitis were observed on slit-lamp examination. Concurrent treatments were started in consultation with an internist. Steroid eye drop and steroid systemic administration relieved the ocular pain and symptoms. Conclusions: When a patient presents with recurrent red eyes and visual disturbances with Takayasu’s ateritis, simultaneous uveitis and keratitis due to Takayasu’s arteritis should be considered as a possible diagnosis.         미세각막절개 백내장 수술과 고전적 백내장 수술에서의 각막내피세포의 손상정도 비교

        Purpose: To evaluate the damage to corneal endothelial cells following coaxial phacoemulsification and bimanual microincision cataract surgery (MICS). Methods: We measured and compared the changes in the corneal endothelial mean cell density, cell size variation coefficient, hexagonality, and central corneal thickness in senile cataract patients who had received either coaxial phacoemulsification (Group 1, n=20), MICS using ultrasound (Group 2, n=20), and MICS using laser (Group 3, n=20). The endothelial cell parameters and corneal thickness were evaluated preoperatively and at 1 week, 1 month, and 2 months postoperatively. Results: There was no significant difference among the three groups in terms of the endothelial cell parameters and corneal thickness during two months (p>0.05). Conclusions: MICS is a safe technique that does not appear to be associated with more damage to the corneal endothelium than coaxial phacoemulsification. A longer follow-up study is necessary to investigate its potential benefits for replacing conventional surgery.

        결막이완증 환자에서 백내장 수술 중 통증 경감을 위한 결막 보호막의 효과

        Purpose: To evaluate the effect of conjunctival shield on pain alleviation during cataract surgery in conjunctivochalasis patients for conjunctiva drawn into speculum suction holes during cataract surgery. Methods: Forty patients with conjunctivochalasis undergoing cataract surgery using un aspirating speculum were evaluated in the present study. A thin piece of surgical wipe (conjunctival shield) was inserted between the lid and bulbar conjunctiva to shield the conjunctiva from the suction hole and thereby prevent it from being sucked into the hole. Additionally, patients were asked to rank pain following speculum placement both before and after conjunctiva shield insertion by visual analogue scale. Results: The pain scores were 2.50 ± 0.78 points before conjunctival shield insertion, and 1.25 ± 0.66 points after conjunctival shield insertion, which was a significant decrease (p = 0.01). In addition, the results showed a 70% reduction in pain following shield insertion. Conclusions: Conjunctivochalasis causes pain during cataract surgery when using an aspirating speculum. The conjunctival shield insertion is a safe and simple method for pain-free cataract surgery in conjunctivochalasis patients.
